Nursing outcomes of inpatient on level of nursing staffing in long term care hospitals (요양병원 간호인력 확보수준에 따른 입원환자의 간호결과)

  • This study was conducted to explore the impact of nursing staffing on inpatient nursing outcomes in long term care hospitals. A secondary analysis was done of national data from the Health Insurance Review and Assessment Services including evaluation of long term care hospitals. Patients per RN was a significant indicator of foley catheter ratio in high risk group and low risk group. Patients per RN&NA was a significant indicator of decline in ADL for patients with dementia, non dementia, urinary incontinence and new pressure ulcer development in the high risk group. The average nursing outcome of inpatient in high grade was higher than that low grade in long care hospital. This higher level of nursing staffing and the higher the grade shown a positive effect on the nursing outcomes of the inpatient. We therefore recommend modifying the above nurse staffing policy so as to make it more effective in improving nursing outcomes.

  • The purpose of this study examine to identify the mediating Organizational citizenship behavior in the relation Authentic leadership and Organizational support. Data were collected by self-report questionnaires targeting 229 caregiver in Daegu city and Gyeongsangbuk-do city. Data were analyzed with SPSS/PC ver. 21.0 programs. There were significantly positive correlations between toward the Organizational citizenship behavior, Authentic leadership and Organizational. As a result of analyzing the main factors the level of care performance for authentic leadership and Organizational support to be significant explanatory variables and this model's explanatory power was 50.0%. Therefore, it is necessary required to improve the Organizational support and leader's authentic leadership for the nursing work performance.

Factors Influencing health Promoting Behaviors for Long-term care Hospital Nurses (요양병원 간호사의 건강증진행위에 미치는 영향요인)

  • This study examined the influence of self efficacy, social support, and health promoting behaviors of long-term care hospital nurese. Data were collected from March 1 to April 30, 2021 for 180 nurses who worked for 6 months at long-term care hospital nurses by D city. Collected data were analyzed by t-test, ANOVA and multiple regression analysis using SPSS/WIN 21.0. Nursing professionalism was positively correlated with the self efficacy(r=.35, p<.001) and social support(r=.34, p<.001). The significant variables explaining self efficacy(β=.290, p=.041) and social support(β=.189, p=.004) These factors explained 17.2% of health promoting behaviors. It is necessary to provide education that consider the self efficacy, social support of long-term care hospital nurse to help them establish an promotion view of health promoting behaviors.

Study on the Efficient Integration of Long-term Care Facilities and Geriatric Hospitals by Using NHIC Survey Data (실태조사를 통한 장기요양시설과 요양병원의 효율적 연계방안)

  • The purpose of this study is to identify how to efficiently integrate long-term care facilities into geriatric hospitals. We conducted a survey on the current operations of facilities and medical services of 2009 of 192 long-term facilities and 168 geriatric hospitals in Korea between October and November. Technical statistics and chi-square test were conducted on the collected data using the SPSS 13.0/Win program. There was a difference between the two facility types in terms of the co-payment levels of the food services. Both types selected the budget deficit as their major management problem. Ease of access and the surrounding environment were critical factors used to select the location of both types of facilities. Facility users benefited from the discounted co-payments of both facility types. However, facility users wanted more frequent visits and support from their family members during their stay at the facilities. It was discovered that users in the long-term care facilities stayed longer, that is until they died, compared to their counterparts in geriatric hospitals. The two types of facilities provided their services totally separately to users. Users of the two types of facilities are poorly supported and cared for by their families. This study suggests that setting reasonable service fees, paying caretakers, introducing an integrated facility, strengthening facility assessment standards, introducing the family doctor system, and introducing the handling of long-term care insurance by geriatric hospitals would allow the integration between long- term care facilities and geriatric hospitals to be beneficial.

A Study on the Shape and Color Analysis of the Symbols of Nursing Hospitals in Korea (국내 요양병원 심벌마크의 형태 및 색채 분석 연구)

  • South Korea is an aging society with 10.9 percent of its population over 65 years of age, and the aging population naturally leads to a surge in elderly care costs. Compared to general hospitals and other hospitals that have systematic management strategies in an environment of intensifying hospital competition, the design and management strategies of nursing hospitals are insufficient due to the hospital's characteristics of long-term hospital admission and treatment. In this study, We conducted a symbol mark color and form analysis of nursing hospitals to establish a design strategy to meet the increasing demand on nursing hospitals. The subjects of the study were 131 hospitals that received the first grade of nursing hospitals in Korea, and each symbol mark was collected to analyze its form and color. Analysis of the form of symbol revealed that the most anthropogenic symbols were made with artificial objects, and among them, the number of symbols with crosses, hearts and human forms were higher. The color analysis showed that more than 50% of symbols consisted of two colors and the main colors were blue and green. It is expected that this research data will be used as a reference in the design of the symbolism of nursing hospitals, which will help consumers choose and develop strategies for hospital management in order to compete with the ever-increasing demand of nursing hospitals and hospital competition.

Experience of the Role Conflict of Nurses in Long-term Care Hospitals (요양병원 간호사의 역할갈등 경험)

  • Purpose: The purpose of this study was to describe the experience of the role conflict among nurses in long-term care hospitals. Method: Data were collected through in-depth interviews with nine nurses in long-term care hospitals. Colaizzi's phenomenological method was used for data analysis. Result: In this study, five categories and twelve theme clusters were generated. The five categories consisted of 'Heavy workload,' 'Nursing job delegation of unclear regulations,' 'Unprotected doctor's job performance,' 'Non-systematic nursing assistants management' and 'Limit of competency.' Conclusion: These results will contribute to the role establishment of nurse in long-term care hospitals and the qualitative improvement of work by providing the grounds for the strategy development which improve working environment and job satisfaction of nurses in long-term care hospitals apposite to the case of Korea.

Status of Infectious Disease Inpatients at Long-Term Care Hospitals in Korea (국내 요양병원의 감염병 입원환자 실태 분석)

  • This study investigated the status of infectious disease inpatients at long-term care hospitals in Korea. A descriptive study was conducted on patients with 14 infectious diseases at 798 long-term care hospitals during 2016-2017. The number of infected patients, total admission days, and total medical expenses were higher in 2017 than in 2016. The most common infectious diseases were enterocolitis due to Clostridium difficile, influenza, and scabies. The number of hospitals with patients who had enterocolitis due to C. difficile and resistance to carbapenem was higher in 2017 than in 2016. Hospitals with 150-299 beds had higher numbers of infectious disease patients than those with under 150 or over 300 beds. Therefore, intensive efforts are needed to control the most common diseases at long-term care hospitals, such as enterocolitis due to C. difficile, influenza, and scabies. It is recommended to apply relevant guidelines related to infection control management as well as implement educational programs. It will also be necessary to develop applicable infection monitoring standards and support the facilities and health workforce of long-term care hospitals under 300 beds through an effective infection surveillance system.

  • This Study will focus the fact that large portion of inpatient treatment cost might incurred in nursing hospital and consider whether policy of allowing inpatient treatment is appropriate or not. Finally This study will suggest alternative way to make improvement based on cases from other countries. This study use data published by Health Insurance Review & Assessment Service. & National Health Insurance Service which is very reliable. This Study found biggest medical spending in allowance of medical care is inpatient treatment cost and large portion of inpatient treatment cost might incurred in nursing hospital. This Study found policy of allowing patient to get inpatient treatment is not clearly determinded. Therefore patient who don't actullay need medical service enter and stay in nursing hospital. Their inpatient treatment cost is paid by allowance of medical care and this cost is unnescessary medical cost. This study suggest policy of allowing patient need to be clear. Government should mandate nursing hospital to check whether patient's condition is appropriate to enter and stay in nursing hospital. This study suggest way to reduce unnecessary inpatient treatment cost incurred in nursing hospital

A Study on the Conceptualization and Architectural Planning Strategy of Elderly Hospitals - Focused on Nursing Hospitals - (노인전문병원의 개념설정 및 건축계획방향에 대한 연구 - 요양병원을 중심으로 -)

  • According to the rapid increase of the elderly population, especially frail older population, many kinds of elderly care facilities have been supplied within a relatively short period. Among them, elderly hospitals and elderly welfare facilities have occupied a major portion. The elderly hospital, which had emerged from 1994, has recorded sharp increase in facility numbers and bed numbers by the support of Korean government together with the increase of care demand. However, the concept and fundamental planning criteria of elderly hospitals have not yet been set up. This paper has derived the concept of the elderly hospital from the Medical Law and Elderly Welfare Law, and prospected the supply of it from domestic and international statistics. Also this paper has explored the fundamental design issues of elderly hospitals by analysing precedent studies and designs, and by surveying some facilities.
