A SURVEY OF DENTAL TREATMENT UNDER OUTPATIENT GENERAL ANESTHESIA IN DEPARTMENT OF PEDIATRIC DENTISTRY AND CLINIC FOR DISABLED AT YONSEI UNIVERSITY DENTAL HOSPITAL (연세대학교 치과대학병원 소아치과 및 장애인 클리닉에서 시행된 외래 전신마취하의 치료에 대한 연구)

  • General anesthesia has been researched and developed in dentistry as on type of management technique to sedate patients who may be uncontrollable or require medical consideration. There has been continuous research into this area, but analysis of large set of patients over a sustained period of time is lacking. Thus, this study analyzes the records of patients who received general anesthesia at the Yonsei University Dental Hospital Department of Pediatric and Clinic for the Disabled. 1. Patient's age ranged from 1 to 66, with under 5 being the largest group with 410 members(38.5%). The study included more men than women, with 695 male members(65.3%). 2. Type of dental procedure performed were as follows(per person) : 5.6 Dental restoration; 2.3 Endodontic treatment of deciduous and primary teeth; 2.5 preformed crowning; and 1.6 extractions. Procedures took an average of 100 minutes. 3. 1022 patients(95.9%) received dental care under general anesthesia once and 43 patients(4.1%) received dental care under general anesthesia two or more times. Dentistry under general anesthesia has the many benefit. However, without appropriate post-treatment care, it is difficult to maintain good oral health. Therefore, it is important to improve the efficiency and safety of general anesthesia through future research.

Long Term Follow Up of Interferon-alpha Treatment in Children with Chronic Hepatitis B (만성 B형간염 환아에 대한 Interferon-alpha 치료결과의 장기 추적관찰)

  • Purpose: We tried to evaluate the long term efficacy and positive predictive factors of interferon-alpha treatment in children with chronic hepatitis B. Methods: The study population included 113 children who received interferon therapy between May 1982 and July 2002 (20 years) for chronic hepatitis B in Department of Pediatrics, Yonsei University College of Medicine. Male to female ratio was 2.3 : 1 and the mean age at diagnosis was $11.1{\pm}4.1$ years old. Response to treatment was defined as normalization of alanine aminotransferase (ALT), disappearance of HBeAg and HBV-DNA Eighty two children responded while 32 did not. Interferon-alpha was given intramuscularly for 6 months at a dosage of $3{\times}10^6$ unit, 3 times weekly. In relapsed cases, lamivudine or interferon retreatment was done. Results: Seroconversion rate was 77.0% in terms of HBeAg, 74.3% in terms of HBV-DNA, and 80.5% in terms of ALT normalization after treatment. Seroconversion rate of both HBeAg and HBV-DNA was 72.6%. Analyzed by life table method, the effect of the treatment had been maintained over 10 years after cessation of therapy. Pre-treatment ALT level was the only significant positive predictive factor of response. Eleven cases (13.4%) relapsed, and 2 out of 3 showed response when treated with lamivudine and 1 out of 3 with interferon retreatment. Conclusion: Interferon-alpha showed significant efficacy in the treatment of chronic hepatitis B in our study. Further studies about the effect of interferon therapy on complications of hepatitis such as hepatocarcinoma, cirrhosis are warranted.

  • A 36-year-old woman with a history of spontaneous abortion and photosensitivity was admitted to our hospital with periumbilical pain and lower extremity edema. The preoperative CT scan revealed massive inferior vena cava thrombus, which extended from the suprarenal portion of the vena cava to the hepatic vein. The laboratory data showed an elevated level of anticardiolipin antibody, which suggested the diagnosis of antiphospholipid syndrome. After medical management, she underwent an operation for removal of an vena cava thrombus by performing cavectomy and thrombectomy. After the operation, she has been taking oral anticoagulation and she is doing quite well at 9 months after her operation.

  • Percutaneous transthoracic fine needle biopsy is a popular technique in the diagnosis ot'pulmonary and pleural diseases and its complication rate is very low. One of the rarest but potential complications is that of implantation of malignant cells along the needle tract and subsequent development of a chest wall mass . We experienced a case of chest wall implantation of lung cancer after percutaneous transthoracic fine needle biopsy. The patient was a 59 year old female who had undergone right upper lobectomy for squamous cell carcinoma of the lung (T,N,Mo). 6 months after operation, a loculated mass was palpated at the right posterior .chest wall where the percutaneous transthoracic fine needle biopsy had been performed before operation. We carried out wide excision of this mass and confirmed squamous cell carcinoma histopathologically.

  • Fenestrated medial patellar plicae are unusual. These fenestrated medial plica may vary in size and shape from being small circular and 5mm in diameter to being large opening 3-4cm long. Arthroscopic resection of the painful medial plica can provide lasting and satisfactory relief of symptoms. There is high percentage of associated medial knee symptoms that are relieved by complete resection. We had performed arthroscopic excision of the symptomatic fenestrated medial plicae present in both knees. The symptoms were dramatically relieved after arthroscopic surgery.

  • The purpose of this study was to analyze the factors of health services that satisfy chronic musculoskeletal patients in the northern part of Gyeong-buk, South Korea. One hundred and seven chronic musculoskeletal patients who had visited a clinic in Gyeong-buk were recruited for this study. The data were collected through a questionnaire survey administered from May 7 to June 6, 2012. The collected data were analyzed by multiple regression analysis. The major findings of this research are as follows. The chronic musculoskeletal patients' satisfaction was determined by the "physical therapist's explanation," "registration process," "home exercise," "respect from staff," and whether the physical therapist and staff "listened to concerns." The chronic musculoskeletal patients' likely to revisit is strongly related to "physical therapist's explanation," "registration process," and "up-to-date equipment." Therefore, it is very important for clinics to improve the quality of the "physical therapist's explanation," "registration process," "respect from staff," "informed consent," "up-to-date equipment," and "home exercise." Furthermore, it is expected that the outcomes of this study will contribute to the customer satisfaction of chronic musculoskeletal patients in clinics.

  • The purpose of this study was introduce peer mentoring which is well used in various rehabilitation areas in overseas country to Korea academically and clinically and help psychological and physical recovery for stroke survivor. It was two group experimental design study, this protocol is designed for stroke survivor, 3-6 months after the onset. Peermentors were consisted of outpatients who is stroke survivor in different recovery levels, at least 2 year after the onset. Peermentors received education about how to support and mentor participants of the program emotionally, appraisally, and informationally and how to run the program. Geriatric Depression Scale Short Form-Korea Version(GDSSF-K) is used to measure psychological factor, Stroke Impact Scale(SIS) is used to measure recovery level of patient, and for physical factor Upper Extremity Function Test for the Elderly(TEMPA) was used. A researcher who is interested in the peer mentor program needs to find more effective applying method based on be offered method in this study for helping recovery after stroke.

  • The Clinicostatical and Bacteriological studies on the otofuruncles were carried out on 297 pts who had visited OPD of ENT depart of St. Benedict Hospital during the 2 years from Jan. 1980, Until Dee, 1981. The results obtained were as follows; 1) The otofuruncle ratio to the whole ENT visiting cases was 27,149:991. (3.6%) 2) The age distribution showed the highest incidence in 0-10 years. (63%) 3) The ratio, male vs female was 180:133 (1.3:1). 4) The seasonal incidence showed the highest incidence in July and August. 5) The side of otofuruncle was left side (56.7%). 6) The site of otofuruncle was posterior (21%), anterior (19%), multiple (13%). 7) Average duration of illness on first visiting was 4.7 days. 8) The numbers of cases who performed I&D was 103 pts. (35%). 9) The most common causative organism was Staph. aureus (88%). 10) Average duration of treatment was 5.3 days.

The Survey on Satisfaction of Outpatient Using Physical Therapy in part of Kyeong-sang Province (경상도 일부 지역에서 물리치료서비스를 이용하는 외래환자의 만족도 조사)

  • Purpose: The purpose of this survey was to assess the satisfaction of patients suffering from musculoskeletal pain with physical therapy (PT) in Busan, Daegu, and Kyeongsang provinces using a Korean-language version of the patient satisfaction questionnaire with physical therapy service (PSPT). Methods: A total of 654 patients, having PT for their musculoskeletal pains, participated. Consenting subjects completed a 20-item PSPT on their $6^{th}$ or last treatment day. Simple descriptive statistics were used for respondent characteristics, and mean of satisfaction score in each item. Regression analysis was performed on each item of the questionnaire. Results: 654 subjects responded during or after their PT services. 297 (45.5%) of them were male and the most common site of pain was the spine. The mean of each item score (1-5) ranged from 2.14 for "my therapist did not listen to my concerns" to 4.12 for "my therapist treated me respectfully". Receptionist's attitude, registration process, service time of the hospital, and answer to the patient's questions greatly affected the general satisfaction of the respondents. Conclusion: The results showed high levels of satisfaction to the general process for obtaining treatment as well as the therapist's attitudes toward their patients. They may provide valuable patients' feedback regarding PT service for musculoskeletal pain. Therefore, it may be used by physical therapists to improve future patient experiences with a view to improving the patient's attendance and compliance with the PT service protocols for patients suffering from musculoskeletal pain.

Incidence and Factors Influencing Neutropenia in Patients with Chemotherapy (항암화학요법을 받은 유방암환자의 호중구 감소증 발생실태와 영향요인)

  • The purpose of this study was to identify the incidence of neutropenia in patients with breast cancer who received chemotherapy and to identify the differences in incidence according to influential factors. We analyzed the medical records of 353 breast cancer patients who received chemotherapy at university hospital in Seoul, Korea from January 2010 to March 2016. The collected data were analyzed by descriptive statistics, $X^2-test$, and logistic regression analysis using SPSS 20.0. Among the 353 subjects, 33.1% had neutropenia, and the factors that showed significant difference according to neutropenia were exercise performance, RT status, and regimen. The results of this study suggest that it is important to predict the prevalence of neutropenia in breast cancer patients receiving chemotherapy and to provide appropriate education and nursing intervention.
