CORRECTION OF DENTAL MIDLINE DEVIATION BY MEANS OF SPACE SUPERVISION (Space supervision을 이용한 치성 정중선 변위의 교정)

  • A midline deviation is divided into three types, such as a skeletal midline deviation, a dental midline deviation, and a combination. Among them, the midline deviation which appear in the early mixed dentition without any skeletal problem can be managed differently by the stage of dental development. The location of the permanent incisors can be improved without using artificial force in consideration of the condition of the space, the eruption timing or the eruption path of the adjacent permanent teeth in early mixed dentition, especially when the permanent incisors are erupting. Therefore, the space supervision technique, which can have the teeth align themselves utilizing physiologic events such as sequence or timing of the eruption of permanent teeth, can be an alternative to orthodontic movement. This case report describes midline deviation improvement by space supervision.

A STUDY FOR OCCLUSAL FEATURES OF FIRST PERMANENT MOLAR AND SECOND PRIMARY MOLAR (제 1대구치와 제 2유구치의 교합면 양상에 관한 연구)

  • The purpose of this study was to analyze the morphometrics of primary second molar and permanent first molar. Samples were consisted of normal occlusion in the primary dentition(50 males and 50 females) and permanent dentition(43 males and 43 females). Their upper and lower plaster casts were used and their measuring points were decided, through 3-dimensional laser scanning(3D Scanner, DS4060, LDI, U.S.A.), fitting standard horizontal plane were made for measuring the intercuspal distance, volume of intercuspal area and section curve. The results were as follows; 1. Average distance from the fit plane to the cusp tips of mandibular primary second molar was smaller than any other tooth. (0.05-0.09 mm in male and 0.04-0.09 mm in female). 2. Intercuspal distances of mandibular primary second molar and permanent first molar were larger in male than in female. Especially, there was statistical significance in primary second molar(p<0.05). 3. Intercuspal distance between distobuccal and distolingual cusp was larger in maxillary primary second molar, except cross intercuspal distances. And distances between distal and distolingual cusp, in mandibular primary second molar, between mesiolingual and mesiobuccal cusp, in maxillary first molar, and between distolingual and mesiolingual cusp, in mandibular first molar were larger than any other intercuspal distance. 4. Volume of intercuspal area of primary second molar and permanent first molar was larger in mandible than in maxilla and that of permanent first molar was 1.40-1.75 times of primary second molar (p<0.05). Also it was larger in male than in female, but there was no statistical significance. 5. In most cases, section curves were wider and deeper in permanent dentition than in primary dentition. Except cross intercuspal distances, in maxilla, section curve between mesiobuccal and mesiolingual cusp was the deepest in both dentition. In mandible, section curve between distobuccal and distal cusp was the deepest in permanent dentition and between distolingual and distal cusp was the deepest in primary dentition.

BILATERAL SUPERNUMERARY TEETH IN THE MANDIBULAR INCISOR REGION; A CASE REPORT (하악 전치부에 발생한 과잉치)

  • Supernumerary tooth describes an excess of tooth number, which are found in primary dentition with 0.3-0.8%, permanent dentition with 1.0-3.5% prevalence. Their frequency is about 2:1 (male vs female) and 9:1 (maxilla vs mandible). However, occurrence is very rare in the incisor region of the mandible. We need a early diagnosis and appropriate treatment plan because of possibility of diastema, eruption failure, displacement, rotation of the associated permanent teeth, root resorption, dentigerous cyst with presence of the supernumerary teeth. This is a case report about two impacted supernumerary teeth found in madibular anterior region of 6 years old girl. One was extracted and another was retained because of fusion with permanent central incisor on the labial surface.

  • Fusion is defined as union of two separate tooth buds at some stage in their development with confluence of dentin and characterized by separate root canal and large single crown, while gemination is defined as an attempt of the single tooth bud to incompletely divide and usually result in a single root with one root canal and two completely or incompletely separated crowns. It is sometimes difficult to decide whether an abnormally large tooth is the result of fusion of a normal and a supernumerary tooth, or of gemination; use of the term 'Double tooth' may make the clinicians avoid this difficulty(Brook & Winter). Commonly there are no symptoms, but the problems associated with these anomalies include esthetics, possible loss of arch length and delayed or ectopic eruption of the permanent teeth, caries along the line of demarcation, and periodontal disease. Commonly, it dose not need to be treated in primary dentition but in case of permanent dentition, it may be requested to be treated due to esthetics and other problems. In our case, a 8 years old girl showed a Double tooth, we attained the favorable results by performing hemisection with apexification.

  • An eruption failure can be observed for child and adolescent periods when the primary dentition is changed to the permanent dentition through the mixed dentition frequently. The eruption failure can lead to miss erupting times of the tooth, then it will cause a lot of problems including root resorption, esthetic problem, transposition of adjacent tooth, malocclusoin and etc. Especially, the maxillary first molar is importantly concerned with occlusion and growth and is an essential tooth for development and maintenance of occlusion. So, it is a momentous part of more proper occlusal management to find these abnormal cases at the early stage and solve the problems. The sorts of eruption failures of the maxillary first molars can be divided into delayed eruption, impaction and the primary retention and the secondary retention. When physical obstacles cause impaction, first of all they must be removed then we can treat the impaction with observation after removal, surgical exposure or orthodontic traction. If the source of impaction is an ectopic eruption, the treatment can be a brasswire, a pendulum appliance, a space maintainer or space regainer after the extraction of the second deciduous tooth and etc. These cases are made a diagnosis of eruption failures of the maxillary first molars in mixed dentition period and have good prognosises after my treatments. So I reported them.

A clinical study of the width of attached gingiva in the deciduous, mixed and permanent dentitions (성장기 아동의 연령에 따른 부착치은 폭경에 대한 연구)

  • A certain width of attached gingiva is required to maintain gingival health The purpose of this study was to examine the dimensional changes in the width of attached gingiva and the depth of gingival sulcus among the deciduous, mixed and permanent dentitions and establish baseline information on the width of attached gingiva in Korean children. Eighty-eight children aged 4 to 14, who visited the Department of Pediatric Dentistry at Samsung Medical Center, were selected and divided into 3 groups according to the periods of dentition: deciduous, mixed and permanent dentitions. The width of keratinized gingiva and the depth of gingival sulcus were measured in each group with a periodontal probe and the width of attached gingiva was determined. The width of attached gingiva in maxillary and mandibular first molars increased significantly with age after eruption in the permanent dentition (p<0.05). The sulcus depth significantly increased in newly erupted permanent teeth with narrower width of attached gingiva (p<0.05) in all of the experimented teeth with the exception of the mandibular central incisor during the transition period. The results suggest that the mean width of attached gingiva does not increase steadily from the deciduous to the permanent dentition.

  • The supernumerary premolars account for 4%-9% of all supernumeraries and reported prevalence is 0.01%-1.00%. Multiple supernumerary teeth can be seen in syndromic phenotypes. But, only 2% of supernumerary premolars exhibited any pathological changes. Most of them cause interference with normal occlusal development that makes maintaining the disabled patients's oral hygiene difficult. It is recommended to delay the timing of extraction until the full permanent dentition is mature. If there is no complication, leaving the supernumerary premolars impacted is recommended. This case is about mental retardation patients with multiple supernumerary premolars.

  • Hypohidrotic ectodermal dysplasia refers to a group of disorders with the following common features : thin, sparse or absent hair, missing or peg-shaped teeth and inability to sweat adequately. Both the primary and secondary dentition are affected. Teeth may be absent (anodontia) or reduced in number (oligodontia) and abnormally shaped. In case, A 5-year-old man presented with hypohidrosis and dry skin. He had no teeth in mouth. We report a case of hypohidrotic ectodermal dysplasia with anodontia.

ESTHETIC RESTORATION OF FRACTURED IMMATURE PERMANENT INCISORS (파절된 미성숙 영구 전치의 수복)

  • Injuries of permanent teeth by trauma occur frequently in childhood and adolescence. Crown fractures are as frequent as 25-76% and especially maxillary incisors are prone to fractures. There have been numerous efforts to achieve both aesthetically and functionally satisfying restoration. When a mature tooth is fractured, porcelain crown or laminate veneer could be a choice of prosthodontic treatment. However, in a case of immature permanent incisor fracture, prosthodontic treatment is more complicated due to the immaturity of the tooth. Moreover, if endodontic treatment is accompanied with the prosthodontic treatment, the treatment period is prolonged. In the past, restoration using an orthodontic band, a ready-made crown, or glass ionomer cement did not exhibit esthetically satisfying result. As restorational materials have been improved, now more esthetic restoration is possible by reattaching fractured fragments or light-curing composite resin restoration. We reports cases of patients with fractured maxillary incisors and their successful treatment results through reattachment of fractured fragments and composite resin restoration.

  • Supernumerary tooth is one of the abnormalities in tooth number. Supernumerary tooth result from excessive proliferation of dental lamina and incidence reports identify a range of $0.3\sim0.8%$ in primary dentition, $1.0\sim3.5%$ in permanent dentition with males being affected twice as frequently as females, maxilla nine times as frequently as mandible The incidence is more in anterior tooth region than in posterior region. The most common supernumerary tooth is the mesiodens, which located between maxillary central incisors. The occurrence is very rare in the incisor region of mandible and the reports on incidence is 2%. In this case, there were two supernumerary teeth in the mandibular region and we could acquire normal alignment of mandibular incisors by extraction and orthodontic treatment.
