Anti-platelet Effect of Carvacrol Extracted from Thuja Orientalis L.;A Possible Mechanism Through Arachidonic Acid Pathway (백자인에서 추출된 Carvacrol의 항혈소판 효과)

  • 목적 : 백자인에서 추출된 Carvacrol이 혈소판 활성화와 혈액 응고에 미치는 영향에 대해 알아보고자 하였다. 방법 : Carvacrol의 항혈소판 효과의 기제를 밝히기 위해 토끼 혈소판으로 Arachidonic Acid 유리,TXB2, PGD2, 12-HETE의생성을 방사선 크로마토그래피 분석을 사용하여 측정하였다. 결과 : 1. U46619를 제외하고 Collagen과 AA에 의해 유발된 응고는 Carvacrol 농도에 따라 억제되었다. 2. Collagen으로 인하여 자극된 AA 유리에 대한 Carvacrol의 유의한 억제 효과는 나타나지 않았다. 3. AA로 유발된 TXB2, PGD2와 12-HETE의 생성억제에 대한 실험에서 Carvacrol은 유의한 억제가 있는 것으로 나타났으며,농도의존적으로 억제되었다. 결론 : Carvacrol은 항혈소판 작용이 있는 것으로 볼 수 있다. 이는 한의학에서 활혈거어 작용으로 해석될수 있으며,타박상,윌경곤란증,탈모증 등 어혈질환의 예방 및 치료와 관련된 약침개발에 기초가 될수 있을 것으로 사료된다.

A Case Report of Chronic Insomnia Diagnosed Blood Stasis (어혈로 변증된 불면증 환자의 치험례)

  • Objectives: Insomnia is a common disease in the general population. The purpose of this paper was to suggest that insomnia subtype of blood stasis can be effectively treated with Danggwisu-san and acupuncture. Methods: This is a clinical case report of a patient with chronic insomnia that was diagnosed with blood stasis subtype. I treated him with Danggwisu-san (Dangguixu-san) two to three times daily and with acupuncture and moxibustion two to three times weekly. Results: After short-term treatment, the patient was gradually recovering from deteriorating chronic insomnia and multiple accompanying symptoms such as severe constipation, spontaneous bleeding and edema, darkened face, depressive moods, etc. Conclusions: Results revealed a possibility that chronic insomnia subtype of blood stasis may be successfully treated with Danggwisu-san and acupuncture and moxibustion.

Identification of Antioxidative Component from Stem Bark of Rhus verniciflua (옻나무 껍질에서 분리한 항산화물질의 성분)

  • An antioxidant compound was obtained from the water extract of the stem bark from Rhus verniciflua, which has been used in traditional folk remedies. The compound was purified by HPLC, using DEAE, CN and ODS columns. The chemical structure of the compound was identified as gallic acid (3,4,5-hydroxylbenzoic acid) by spectral data including UV, IR, EI (HR)-MS, $^1$H-NMR, $\^$13/C-NMR and elemental analyzer. This compound was found show cytotoxicity against HeLa cell ( IC$\_$50/ : 8.5$\mu\textrm{g}$/$m\ell$).

중의치료소아효천임상관찰(中醫治療小兒哮喘臨床觀察) -소아천식에 대한 중약치료의 임상관찰-

  • 소아천식은 기도의 만성 염증 및 과민반응을 특징으로 나타내고 있어 염증을 일으키거나 알레르기 반응을 일으키는 물질로부터 차단하는 방법이 효과적임을 현대의학에서 이미 밝히고 있다. 한의학에서는 천증(喘證)에 본디 뿌리 깊은 근원(천유숙근(喘有夙根))이 있다고 간주하여 허약한 체질은 내적인 요인이고, 풍한침습(風寒侵襲)은 유발요인이며, 담음복폐(痰飮伏肺)와 신기휴허(腎氣虧虛)는 재발의 근본이라고 하였다. 본 연구에서는 소아 천식에 구마황(灸麻黃), 은행(銀杏), 건강(乾薑), 오미자(五味子), 백전(白前), 구백부(灸百部), 소자(蘇子) 등을 사용하였고, 경우에 따라 황기(黃?) 혹은 선령비(仙靈脾)를 추가하여 正氣(正氣)를 강화시키는 목적으로 사용하였다. 약리학적 연구에 따르면 마황과 건강, 오미자 등은 모두 기도의 염증세포 침윤(浸潤)를 억제하고 항알레르기 효과가 있으며, 건강과 오미자는 또한 히스타민에 대한 길항효과(拮抗效果)도 있는 것으로 알려져 있다. 은행 추출물은 혈소판 활성화 수용체에 대한 길항작용을 갖고 있으므로 이러한 물질들로 인한 기관지의 경련을 방지하여 천식증상을 왕화시킨다. 구백부(灸百部)는 지해(止咳)효과가 뛰어나다. 소아천식에 대한 치료는 표본동치(標本同治)의 원칙을 지키며, 증상이 완화된 단계에서도 중약 투여를 1-2개월 지속시키는 것이 중요하다. 소아에게 있어서 천식증상이 완화된 후 어혈(瘀血) 상태가 성인에 비해 심하지는 않지만, "구병필어(久病必瘀)"의 결과에 유의할 필요가 있다. 천식이 장기간 지속되면 필히 혈액순환 장애가 수반되므로 천궁(川芎)이나 단삼(丹蔘) 등의 활혈제(活血劑)를 가미하여 사용하면 효과를 높일 수 있다. 결론적으로 소아의 천식은 항염증, 평천(平喘), 화담(火痰), 지해(止咳)와 더불어 보신익기화어(補腎益氣化瘀)의 약물을 병행하여 사용하는 것이 바람직하며, 단순한 중약 투여는 일부 양약에 의한 소아성장에 미치는 영향을 피할 수 있어 더 많은 임상연구가 이어져야 한다.

A study on the Correlation between Primary dysmenorrhea and Blood stasis (원발성(原發性) 월경곤난증(月經困難症)과 어혈(瘀血)의 상관성 연구)

  • Purpose: We intended to observe the correlations between Primary dysmenorrhea severity and Questionnaires for Blood Stasis Pattern. Methods: After initial approval by Kyung-Hee University Oriental Medical Hospital Institutional Review Board of Clinical Trials, volunteers for the clinical trial were recruited. We selected the 52 primary dysmenorrhea patients by the screening tests (clinical examination and inquiry). The severity of dysmenorrhea was evaluated by VAS (Visual Analog Scale), VRS (Verbal Rating Scale) & MVRS (Multidimensional Verbal Rating Scale). The severity of Blood Stasis was evaluated by Questionnaires for Blood Stasis Pattern. For statistics, we used Spearman's rho correlations, SPSS 13.0 for windows. Results: In case of VAS, though two items (眼瞼下靑紫, 便黑) of Questionnaires for Blood Stasis Pattern were correlated, total score & discriminant function score of Questionnaires for Blood Stasis Pattern were not correlated. In case of VRS, though two items (小腹痛, 夜間痛) of Questionnaires for Blood Stasis Pattern were correlated, total score & discriminant function score of Questionnaires for Blood Stasis Pattern were not correlated. In case of MVRS, though one items (久痺症) of Questionnaires for Blood Stasis Pattern were correlated, total score & discriminant function score of Questionnaires for Blood Stasis Pattern were not correlated. Conclusion: Though the results showed partial correlation of Primary dysmenorrhea severity and Questionnaires for Blood Stasis Pattern, we need further study after improvement and complementation of Questionnaires for Blood Stasis Pattern.

Dose dependent effects of Jungsongouhyul Pharmacopuncture on Low Back Pain (요통환자에서 중성어혈 약침 사용 용량에 따른 효과)

  • Objectives : The object of this study is to analyse about Low Back Pain's intensity according to dosage of Jungsongouhyul pharmacopuncture. Method : Three groups were made with 15 patients in Po-Hang Oriental Hospital, which is affiliates to Daegu Haany University. They were observed August 1st, 2010 to September 30th. 2010. Each group was treated by based on acupuncture, herb and other therapy and differential dosage of Jungsongouhyul pharmacopuncture 0,4, 0.8, and 1.2cc. We had measured pain threshold and Visual Analog Scale during first week of their admission. The statistical analysis was performed by using the oneway ANOVA and Tukey's test. Result : Change of VAS was not statistically significant. Change of pain threshold was statistically significant. Multiple comparisons of pain threshold between the group1,2 was not statistically significant. Multiple comparisons of pain threshold between the group1,3 and group2,3 was statistically significant. Conclusions : Jungsongouhyul Pharmacopuncture 1.2cc was more effective than 0.4 and 0.8cc.

Clinical Study on the case of Lumbar compression fracture with Traditional Korean Medicine, Especially the SAAM Acunpture, Carthami-Flos Herbal Acupuncture (사암 어혈방과 홍화약침요법을 병행한 요추압박골절 1례에 관한 고찰)

  • There are little report on treatment of compression fracture with Traditional Korean Acupuncture Therapy. We suggest to treat Lumbar compression fracture with the SAAM(사암) Acunpture, Carthami-Flos Herbal Acupuncture Therapy. The Treatment methods of the SAAM(사암). Acnpucture, Eliminating extravasated Blood, was apllied at SP3, LU9, LI11 and Carthami-Flos Herbal Acupuncture at the waist. We treated the patient who was diagnosed as lumbar compression fracture by MRI. After 15 days of Treatment, SAAM Acupuncture and Carthami-Flos Herbal Acupuncture, a remarkable improvement was made for Lumbar compression fracture. There was reports about SAAM Acupuncture and Carthami-Flos Herbal Acupuncture of Patient with Lumbar Compression Fracture. It is very effective to reduce the pain and shortening the period of therapy.

Revision of the modified Blood Stasis Questionnaire II (어혈 진단 설문지 개정 연구)

  • Objectives : The objective of this study was to revise the modified Blood Stasis Questionnaire II. Methods : This revision focused on refining the Korean literature expression of Blood Stasis Questionnaire II consisting of 30-question questionnaire. Seven external experts and five researchers of Korean Institute of Oriental Medicine reviewed the questionnaire and its protocol, while the addition or deletion of questions and changes in scoring method were not dealt with in this revision. Results : Among thirty questions, four questions were corrected to appropriate expressions. In case of eight questions, explanations in Korean or Chinese were added. Thirteen questions in the phrase were changed in sentence form to unify the whole questionnaire. Conclusions : This study introduces the revised version of the modified Blood Stasis Questionnaire II. It is expected that clinical demand of this questionnaire will increase and it will be used vigorously in blood stasis research.

Case Report of Radial Nerve Palsy Patients Treated with Additional Jungsongouhyul Pharmacopuncture (중성어혈 약침치료를 병행한 요골신경마비 환자 치험 4례)

  • Objectives: This study was to investigate the clinical effect of additional Jungsongouhyul Pharmacopuncture on radial nerve palsy patients. Methods & Results: The patients were hospitalized or outpatients in Department of traditional korean medical hospital, Dongshin University from 28th, Oct. 2009 to 25th, Aug, 2010. All patients were treated by additional Jungsongouhyul Pharmacopuncture. To evaluate the wrist drop and numbness of fingers, coding result, Verbal numerical scale (VNS) were used. As the result, symptoms are improved remarkably according to improvement of coding result, VNS score. Conclusions: Patients were treated during 3.5 weeks in average. All cases were excellent results by additional Jungsongouhyul Pharmacopuncture. But further studies are required to investigate exact effects.

Development of International Versions of Pattern Identification Questionnaires using Cross-cultural Translation Methodology: Seven Emotions, Fatigue and Malaise, Phlegm, Food Retension, and Blood Stasis (칠정, 노권, 담음, 식적, 어혈 변증 설문지의 횡문화적 번역 연구)

  • Objectives This study aimed to perform a cross-cultural translation of 5 kinds of pattern identification questionnaires from Korean to English: questionnaires for seven emotions, fatigue and malaise, phlegm, food retention, and blood stasis. Methods We followed the strict guideline on the cross-cultural translation of healthcare evaluation tool. Total five stages of study were conducted. First, translations of two individual translators. Second, synthesizing of the two results. Third, two back translations from synthesized version to Korean. Fourth, expert committee reviewed with the original version, synthesized version, back translated versions to make a pre-final version. Last, with the pre-final version, 5 Americans evaluated face validity of the pre-final version. We made a final version after the above-mentioned 5 stages. Result and conclusion International versions of the 5 kinds of pattern identification questionnaires were completed. We can expect this versions are widly used for clinical usage and following academical researches.
