Linear Accelerator Radiosurgery for Trigeminal Neuralgia: Case Report (선형가속기를 이용한 삼차신경통의 정위적 방사선수술: 증례보고)

  • Trigeminal neuralgia is defined as an episodic electrical shock-like sensation in a dermatomal distribution of the trigeminal nerve. When medications fail to control pain, various procedures are used to attempt to control refractory pain. Of available procedures, stereotactic radiosurgery is the least invasive procedure and has been demonstrated to produce significant pain relief with minimal side effects. Recently, linear accelerators were introduced as a tool for radiosurgery of trigeminal neuralgia beneath the already accepted gamma unit. Author have experienced one case with trigeminal neuralgia treated with linear accelerator. The patient was treated with 85 Gy by means of 5 mm collimator directed to trigeminal nerve root entry zone. The patient obtained pain free without medication at 20 days after the procedure and remain pain free at 6 months after the procedure. He didn't experience facial numbness or other side effects.

A Case of Nasofrontal Dermoid Cyst Via Transcollumelar Approach (경비주 접근법으로 안면부 손상없이 제거한 비전두 유피낭종 1예)

  • The midline nasal dermoid cysts are rare congenital neoplasms, which are diagnosed frequently in childhood. Masses are often noticeable at birth gaining size over time with recurrent infections and usually arise from the nasal cavity or lower 1/3 of the nasal dorsum. CT scan as the primary investigation is helpful to determine accurately the size and extent of the lesion as well as the integrity of adjacent bony structures. MRI scan is recommended to rule out an intracranial extension or sinus tracts. Treatment of choice is the complete surgical excision preserving the cyst wall. Here in, we present an unusual case of nasofrontal dermoid cyst in a 19-year-old boy without radiographic evidence of transcranial extension. In this case, we surgically removed nasofrontal dermoid cyst via transcolumellar approach. We also corrected saddle nose deformity after mass removal. Therefore, in this case, we experienced a successful case in which the nasofrontal dermoid cyst was totally removed without facial scar and deformity.

  • Pleomorphic adenoma is a circumscribed tumor characterized microscopically by its pleomorphic or mixed appearance, and its clearly recognizable tissue intermingled with areas of mucoid, myxoid, or chondroid appearance. Plemorphic adenoma, the most common tumor of the salivary glands, is now generally accepted as epithelial and benign, and therefore an adenoma. Pleomorphic adenoma occurs about 10 times more often in the parotid gland than in the submandibular gland, and is rare in the sublingual gland of the major salivary gland. And it occurs most often in the parotid and lip, but is very rare in the buccal mucosa of the minor salivary gland. The reported age of occurrence for pleomorphic adenoma ranges from 5 to 84 years, with a peak incidence during the third decade. We experienced a case of pleomorphic adenoma which occurring in buccal mucosa of 13-year-old boy. And so both the site of origin and age of occurrence make this case unusual. This lesion was treated with surgical excision. No specific change has seen in 1 year postoperative follow-up.

Inflammatory pseudotumor of the midfacial area (중안면부에 발생한 염증성 가종양)

  • Inflammatory pseudotumor was originally described in the lung, but recently has been recognized to occur in various sites. A 56-year-old female was referred to our department with a painless swelling of the right midfacial area since 3 months ago. Clinical examination showed non-specific intraoral findings, but asymmetric facial appearance and numbness of the right midfacial area. Plain radiographs and CT images showed aggressive destruction and irregular thickening of the right maxillary sinus wall, increased antral opacification, and destruction of the zygomatic arch. A relatively well-defined soft tissue mass occupied the right maxillary sinus, nasal cavity, zygoma, and infraorbital region. The soft tissue mass showed mild enhancement on CT. Radiographically, this lesion presented a rapidly enlarging mass demonstrating aggressive behavior, mimicking a malignant tumor. Histopathologic examinations showed plasma cells and inflammatory cells in variable fibrotic tissues and demonstrated positive reactivity for vimentin. No malignent changes could be found.

A case of unexpected adjacent tooth extrusion after implant fixed prosthetic treatment, who had undergone mandibular resection and reconstruction due to ameloblastoma (법랑모세포종으로 하악골 절제 및 재건술 시행한 환자에서 임플란트 고정성 보철물 수복 후 원인 미상의 인접 치아 정출이 발생한 증례 및 고찰)

  • Ameloblastoma is a benign odontogenic epithelial tumor with high recurrence rate and requires extensive resection of the surrounding tissue and reconstruction of defect site. Because of the anatomical limitation of the reconstruction site, prosthetic treatment with implants is the first recommendation. This is a case of prosthetic restoration of the reconstruction site with implant fixed prosthesis in patient who underwent mandibular resection and iliac bone reconstruction due to ameloblastoma. However 14 months after completion of implant prosthesis, adjacent natural tooth erupted unexpectedly, resulting in 1mm infra-occlusion occurred including posterior implant prosthesis and anterior natural teeth. In adults, implant infra-occlusion may occur due to residual growth after placement of the maxillary anterior implant. But this case, hypo-occlusion of molar implant and open bite of anterior natural teeth is occurred due to extrusion of adjacent tooth, is rare. Thus we report the treatment process including orthodontic treatment with intrusion of the posterior tooth, and investigate the causes of sudden, unexpected tooth extrusion.

Successful Carboplatin Chemotherapy for Oral Fibrosarcoma in the Buccal Mucosa of a Dog (개의 구강점막에 발생한 구강 섬유육종에서 카보플라틴의 성공적인 항암치료 효과)

  • An 11-year-old castrated male cocker spaniel presented with halitosis, anorexia, and swelling of the left face. Upon physical examination, a firm mass was detected in the left buccal mucosa. Fine needle aspiration cytology revealed a malignant tumor of mesenchymal origin. We performed biopsy, which gave a diagnosis of oral fibrosarcoma. The client refused surgical treatment, and carboplatin chemotherapy (300 $mg/m^2$, IV, q 21 days) was initiated. The mass gradually decreased in size and disappeared about 89 days after the initial chemotherapy. Complete remission was attained, and the tumor did not relapse. This case report shows that a single carboplatin chemotherapy session can achieve macroscopic complete remission of oral fibrosarcoma.

FUNCTIONAL RECONSTRUCTION OF MANDIBULAR DEFECTS WITH FREE BONE GRAFT (유리골 이식을 통한 하악골 결손부의 기능적 재건술)

  • Mandibular discontinuity defect due to benign tumor, malignant tumor, infection, or truma results in major esthetic and biologic compromise. The primary goal of reconstruction is full restitution of function, which secondarily lead to normalization of the cosmetic deformity. The authors make a clinical study of 61 consecutive bone graft cases for mandibular reconstruction of discontinuity defect which were studied retrospectively using clinical data and radiographic findings. The cases were reviewed to evaluate the clinical success in the period from 1981 to 1990 in the Dept. of Oral & maxillofacial Surgery, Seoul National University Hospital. The criteria of the success in bone graft, are no residual infection, graft in with maintain its integrity, and remain over a half of its original size of graft in the radiographic features. The purpose of this clinical survey is to study of the mandibular discontinuity defects and success rate of free bone graft in mandibular defects. To summarize the clinical study of free bone graft, the main type of autogenous bone graft is iliac bone and corticocancellous type. Overall success rate is 80.3% in 61 followup cases over 6 months. Wire fixation and Extraoral approach has realtively better prognosis than other methods. It showed relatively poor prognosis in symphysis defects than other recipient site.

  • Maxillectomy is a treatment option for maxillary cancer, which leaves the patient with a palatal defect. It may cause problems with facial deformation, swallowing, mastication, and speech. These functional problems and changes in appearance may result in psychological problems. To control these deficits after maxillectomy, surgical reconstruction or prosthodontic treatment can be chosen as a treatment option. Obturator prosthesis has been used as a preferred method of rehabilitation for most maxillectomy patients. This case is a patient who was classified Aramany classification II hemi-maxillectomy patient with residual teeth from #11-25, whose teeth had substantial labioversion and clinically lengthened from alveolar bone involution, thus making it hard to select proper framework design and resist to the rotational dislodging force of the obturator. Therefore we selected swing-lock attachment design to remain pre-existing crown and bridges and obtain retention and stability of obturator. The swing-lock RPD is economical than the conventional RPD because we can remain pre-existing crown and bridges. And residual teeth which have mobility and poor prognosis can be successfully retained through properly designed swing-lock RPD as it is functioning as a removable splint on the teeth.

Thoracic Actinomycosis (흉부 방선균증)

  • Thoracic actinomycosis is a rare, chronic debilitating disease and it is difficult to diagnose and treat. Material and Method: Between March 1990 to December 2003, 17 patients were diagnosed and treated for actinomycosis in our center. Except 4 patients (involving cervicofacial, and abdominopelvic area), we reviewed the clinical characteristics, diagnosis method, and treatment in 13 patients. Result: In 8 patients, the operation was required for diagnosis and treatment. 7 of 8 patients had tumor-like lesions in radiological findings and they were not distinguished as lung cancer, and 1 of 8 patients was non-responsive to the antibiotics therapy. Among the 5 medically treated patients, 4 of 5 patients were diagnosed by bronchoscopic biopsy and one by CT-guided biopsy. All of them was well-responsive by the antibiotic treatment. Conclusion: Thoracic actinomycosis is a chronic inflammatory disease that respond well to antibiotics (penicillin therapy). It should be diagnosed by the repeated biopsy (CT-guided or bronchoscopic) before starting treatment. However, if the lesion is not distinguished as the lung cancer, non-responsive to the medical treatment, and the patients experience the recurrent hemoptysis, we can consider the operation for the diagnosis and treatment.

SOLITARY NEUROFIBROMA OF THE CHIN (이부에 발생한 신경섬유종)

  • Tumors originated from peripheral nerve tissues are neurofibroma, neurilemoma, plexiform neurofibroma, malignant schwannoma, and granular cell tumor. Neurofibromas seem to occur in two forms : The first one is circumscribed solitary neurofibroma and the second group is neurofibromatosis or Von Recklinghausen's disease, which is a congenital and familial disease, presenting abnormalities of the skin, nerve system, bones and soft tissue. A solitary neurofibroma is a benign, relatively circumscribed, noncapsulated tumor which often presents in the skin and subcutaneous tissue as a soft sessile or pedunculated mass. It may occur anywhere in the head and neck, but the common site of the occurrence is the tongue, buccal mucosa, palate in frequency. Since solitary neurofibroma is a relatively radioresistant and its recurrence rate seem to be low, the treatment of choice is surgical excision. The author would like to present a case of unusually large solitary neurofibroma occured in the chin, which was successfully treated with surgical excision and reconstructed using deltopectoral flap and tongue flap.
