Laparoscopic Retrieval of Ectopic Adult Heartworms from the Abdominal Cavity of a Dog with Heartworm Infestation (심장사상충 복강내 이소기생 개에서 복강경적 치료 1예)

  • A 5-year-old, intact male Beagle was presented with chronic abdominal pain. The dog was diagnosed with dirofilariasis by positive heartworm antigen detection via ELISA and concurrent microfilaria. Thoracic radiographs revealed cardiomegaly with dilation of the main pulmonary artery. Echocardiography revealed the adult worms in the main pulmonary arteries, but other abnormalities other than heartworm infection were not present. To find the cause of the abdominal pain, exploratory laparoscopy was performed. Ectopic migrating adult heart worms were visualized through exploratory laparoscopy and the clinical sign resolved after removing the heart worm. This report describes removing the ectopic migrating adult heartworms using exploratory laparoscopy in the abdominal cavity.

Taussig-Bing Anomaly with Coarctation of Aorta (대동맥 축착을 동반한 Taussig-Bing 기형의 수술 치험 1예)

  • The patient was a 30-day-old female infant with symptoms of severe dyspnea and cyanosis, when she was admitted to the ER. The echocardiography revealed DORV with subpulmonary VSD, and the diagnosis of Taussig-Bing anomaly was made. Two days after admission, an urgent operation was performed. The operation consisted of intraventricular tunnel repair and arterial switch operation. She was discharged, and after checking her chest X-ray through OPD, there was no interval change of cardiomegaly. She was then re-admitted, and the angiography revealed coactation of aorta. We performed a resection and end-to-end anastomosis of aorta. She is currently in good condition 11 months postoperatively.

  • Pericardial abscess due to liver abscess is a very rare disease, the result of which is usually fatal when untreated. But a combination of antibiotics therapy and pericardial drainage has broug t good result. A 32-year-old man was admitted to the emergency room because of fever and chilling sensation. Chest X-ray showed cardiomegaly and a mass lesion In the left lobe of liver on ultrasonography. The echocardiography revealed impending cardiac tamponade. Pus drainage of the liver abscess through per- foration of right diaphram and pericardiotomy were performed. The pericardium was thickened and 600 ml of purulent fluid and necrotic debris were drained. Fibrin clots were firmly attached to the undelying myocradium. Histological examination of the pericardium showed acute inflammation with microabsscess and Escherichia soli was cultured from pericardial pus. He was discharged after 4 weeks on antibiotics, but Doppler echocardiograpy revealed a slightly thickened pericardium with a mild constrictive physiology.

  • Pompe disease (glycogen storage disease type II) is an autosomal recessive disorder caused by deficiency of acid-${\alpha}$-glucosidase (GAA) resulting in lysosomal glycogen accumulation in multiple tissue, particularly cardiac and skeletal muscle. The classic infantile form of Pompe disease is characterized by marked cardiomegaly, respiratory failure and severe generalized hypotonia. Most patients die from cardiorespiratory failure or respiratory infection within the first year or two of life without treatment. A "non-classic" phenotype presents with less severe clinical feature and slow progression of disease. We report two patients with non-classic infantile Pompe disease from one family manifested hypertrophic cardiomyopathy and progressive proximal weakness.

Leaf Gall of Rhododendron indicum Caused by Exobasidium japonicum in Korea (Exobasidium japonicum에 의한 영산홍 떡병)

  • Leaf gall was occurred on Rhododendron indicum(L.) Sweet[=Azalea indica L.] in a farmer's garden located in Geumsan-myon, and along the walk way of Gangju pond in Jeongchon-myon, Jinju, Korea. The infected leaves was deformed into very conspicuous, pale, fleshy shapeless galls. Basidia were fusiform, $3{\sim}5$ sterigma, and $8{\sim}35{\times}3{\sim}6{\mu}m$ in size. Basidiospores were musiform, straight or curved and $10{\sim}22{\times}3{\sim}5{\mu}m$ in size. The causal organism was identified as Exobasidium japonicum Shirai based on the mycological characteristics of the fungus and pathogenicity. This is the first report on the leaf gall of R. indicum caused by E. japonicum in Korea.

  • A study on the wood quality from planted Pinus koraiensis Sieb. et Zucc. has been undertaken by the University Forest in order to gain results of the gross structural features of stem. For this study some groups of trees were felled and several characteristics such as the degree of stem, slenderness, eccentric growth, heartwood diameter, height of butt sweep and butt-swell were analyzed. The results of the study are as follows. 1) The relation between diameter of log and its height in sample trees are nearly parallel to each other. But their degrees of slenderness in logs cut from intermediate and codominant trees are some what gentle as compared with that of logs from dominant tree. 2) The eccentric growth at transverse section of sample trees is generally decreased with the increase of tree height on all sample trees. 3) On the eccentric growth at transverse section of tree, a rather distinct tendency was noticed that all of sample trees showed the direction of eccentric growth in $R_2$ side. 4) Heartwood diameter appeares to become larger as tree growth decrease under same diameter of logs and log diameter in which heartwood appeares is larger in rapid growth rate trees. 5) The maximum height of butt-sweep generally shows that the larger the breast height diameter of tree is, the smaller its height becomes. On the contrary it was noticed that the larger the breast height diameter of trees, the higher the maximum height of butt-swell became. The direction of butt-sweep and butt-swell were shown in $R_1$ side in all sample trees.

Transvenous Balloon Valvuloplasty through Femoral Vein in a French Bulldog with Pulmonic Stenosis (대퇴정맥을 통한 풍선확장술로 치료한 프렌치불독의 폐동맥협착증 치료증례)

  • A 9-month-old castrated male French Bulldog (13 kg of body weight) was presented with the primary complaint of loud heart murmur and exercise intolerance. Diagnostic imaging studies found severe pulmonic stenosis (peak velocity > 5 m/s) with right ventricular hypertrophy. Because of higher pressure gradient between right ventricle and right ventricular outflow tract (> 100 mmHg), the dog was underwent balloon valvuloplasty through femoral vein. After procedure, the peak pulmonic velocity was reduced to 2.1 m/s (PG = 18 mmHg). Further follow-up study found no further deterioration, for 6 months to date. This is the first case report of pulmonic stenosis treated by transvenous balloon valvuloplasty through femoral vein in Korea.

  • A patient failed to use conventional complete dentures due to severe mandibular bone resorption and soft tissue hyperplasia on lips, tongue and oral vestibule from vascular malformation. The treatment of choice would be an implant-retained overdenture, however, he refused owing to high cost. The closed mouth impression technique using BPS principle, CR record registration with gothic arch tracing and monoplane occlusion using non-anatomic teeth were used to fabricate complete dentures for this patient. Patient satisfaction, the retention and the stability of complete dentures were highly improved as a result.

A Deep Learning Model for Judging Presence or Absence of Lesions in the Chest X-ray Images (흉부 디지털 영상의 병변 유무 판단을 위한 딥러닝 모델)

  • There are dozens of different types of lesions that can be diagnosed through chest X-ray images, including Atelectasis, Cardiomegaly, Mass, Pneumothorax, and Effusion. Computed tomography(CT) test is generally necessary to determine the exact diagnosis and location and size of thoracic lesions, however computed tomography has disadvantages such as expensive cost and a lot of radiation exposure. Therefore, in this paper, we propose a deep learning algorithm for judging the presence or absence of lesions in chest X-ray images as the primary screening tool for the diagnosis of thoracic lesions. The proposed algorithm was designed by comparing various configuration methods to optimize the judgment of presence of lesions from chest X-ray. As a result, the evaluation rate of lesion presence of the proposed algorithm is about 1% better than the existing algorithm.

Clinical and Histopathological Findings in Mice Heavily infected with Fibricola seoulensis (Fibricolu seoulensis 중감염 마우스의 임상적 및 조직병리학적 소견)

  • To observe the clinical course and intestinal histopathology in heavy infection of Fibricola seoulensis, an experimental study was performed in mice. Clinical, experimental infection with 1, 000 metacercariae. On the 11th day after infection, the mice began to die and all of the infected mice were dead by the 16th day. The infected mice showed gradual weight loss. Occult blood was detected after the 10th day. Diarrhea accurred after the 9th day and was recognized in all of the infected mice on the 12th day. Hemoglobin and mean corpuscular hemoglobin decreased significantily after the 12th day, and mean corpuscular hemoglobin concentration decreased in all infected mice. The histopathological changes were more marked in the duodenum than in the jejunum or ileum. Major changes were villous atrophy and crypt hyperplasia, with decreased villus/crypt ratio, inflammatory cell infiltration and stromal edema. The present results suggest that the cause of death of mice heavily infected with F. seoulensis should be malnutrition and severe fluid loss due to malabsorption, together with intestinal bleeding.
