The Effects of Acupuncture Treatment on Hwa-byung patient's Insomnia: Patient-assessor blind, Randomized, Placebo-controlled Clinical trial (화병환자의 불면증상에 대한 침치료 유효성 평가연구: 환자-평가자 눈가림, 무작위배정, 일반대조군 임상시험)

  • Objectives: The purpose of this research is to examine the effects of acupuncture treatment on Hwa-byung patient's insomnia. Methods: The study was performed through a patient-assessor blind, randomized, placebo-controlled trial in which the volunteers, data collectors and analysts were unaware of individuals who were receiving the treatment. A total of thirty-seven volunteers were divided into 2 groups. 18 subjects were placed into a trial group and 19 subjects into a control group by a randomization table. The trial group was treated on bilateral Shigu, Ahnmyun, B62 (Shinmaek), and K6 (Chohae), while the control group was not given any other treatment. The ISI(Insomnia Severity Scale) was measured as the 1st evaluative instrument, and then a comparative analysis was conducted by comparing the results with those measured by a Likert scale for major symptoms of Hwa-byung, PSQI, Hwa-byung scale, BDI, STAI, and STAXI. Results: In the ISI, PSQI, and the Likert scale regarding major symptoms of Hwa-byung, Hwa-byung scale, BDI, and STAI, more significant decreases were found in those concerning the trial group compared to those of the control group from a statistical perspective. However, in the STAXI, regarding the trial group, there was no significant decrease compared to the control group. Conclusions: The results suggest that acupuncture treatment is effective in the treatment regarding Hwa-byung patients who suffer from insomnia.

  • Systemic lupus erythematosus(SLE) is an autoimmune disease which may affect many different organs and disclose various clinical manifestations. Recently central nervous system(CNS) involvement has been recognized as an increasingly significant contributor to morbidity and mortality of SLE. The clinical manifestations of CNS-lupus are highly variable and range from mild cognitive dysfunction, movement disorder, headache, psychosis to life-threatening stroke and coma. Among the neuropsychiatric disorders encountered in patients with SLE, cerebrovascular disease has been a relatively rare complication. The diagnosis and management of CNS-lupus is difficult because of the lack of useful diagnostic methods. If cerebrovascular involvement is suspected, aggressive treatment such as high dose steroid, immunosuppressive therapy, plasma exchange may be required to reduce high mortality rate. We experienced 2 cases cerebrovascular dis eases occurring in SLE patients which presented with various neuropsychiatric manifestations. They were diagnosed as CNS-lupus by neuropsychiatric symptoms, brain MRI, and BEG, and showed good response to high dose steroid pulse therapy.

A Case of Combination Therapy of Cognitive - Behavioral Therapy and Oriental Medical Treatment On Panic Disorder (한방치료와 인지행동치료를 병행한 공황장애 환자 1례에 대한 임상적 고찰)

  • Panic disorder is a frequent anxiety disorder. Recently many studies raised that the course of panic disorder is the interaction of biological and psychological factor. So we used Oriental Medicine Treatment to control biological factor and Cognitive - Behavioral Therapy to control psychological factor of panic disorder, obtained good results. Practicing Cognitive - Behavioral Therapy, we were able to destroy catastrophic misunderstanding. And then in this case, patient is diagnosed Deficiency of Kidney(腎虛) with Fire from stagnation of Liver(肝鬱化火). So we used herbal medicine and acupuncture according to oriental medical theory and these efforts helped the case of disease.

  • Paraneoplastic limbic encephalitis, a remote effect of cancer without nervous system metastasis, is rare, especially in childhood. Here, we report a case of paraneoplastic limbic encephalitis associated with an ovarian mature teratoma in an adolescent girl. The 15-year-old girl developed neuropsychiatric symptoms, memory loss, seizures, and unconsciousness. Cerebrospinal fluid analysis and brain magnetic resonance imaging (MRI) findings were normal, while single photon emission computed tomography imaging showed hypoperfusion in both temporal lobes. Ultrasound and MRI of the abdomen revealed a left ovarian cystic mass. The patient experienced a significant recovery of cognitive function after surgical resection of the tumor, which was pathologically identified as a mature ovarian teratoma, and treatment with intravenous immunoglobulin.

  • The Common Symptoms of insomnia are to be hard to sleep, often wake up at night, be easy not to fall asleep again after waking up. In serious conditions, an insomniac can never get to sleep overnight. This is the case that we diagnosed a 49-year-old patient who demonstrates insomnia, palpitation, anxiety, dizziness, distress in the stormach as Deficiency of Yang(陽虛) and treated her. At 1st stage we diagnosed the patient as Insufficiency of both the heart and the spleen(心脾兩虛) and prescribed Gwibi-tang(歸脾湯), and next stage we dispensed bogol-dan(補骨丹) to cure Deficiency of Kidney Yang. As the result of treatment, the patient made a good improvement. We considered that this case report to be a good prcedent of Insomia due to deficiency of Yang.

  • Objective: This clinical study was aimed to search the therapeutic effects of Oriental Medicine in Submaxillary Tremor. In Oriental Medicine, the tremor of the elder is caused by Gansinyinheo and Qihyeolyangheo. Besides, it has relation to liver, kidney, heart, and spleen according to Jangbu theory. On the basis of this theory, we performed various treatment on Submaxillary Tremor. Method: We carried out three people cases who are elder female-patients with Submaxillary Tremor. We treated them with Herbal medicine(Jihwangyeumjagami, Guibiondamtanggami), acupuncture(Ganjeonggyeok), and moxibustion on Baekhoi(GV20). Result and Conclusion: As the result of our treatment, Submaxillary Tremor was disappeared and some other symptoms were improved.

A Clinical Survey Study on Clinical Symptoms of Dementia Patients (치매 환자의 임상증상에 대한 조사 연구)

  • Objectives : The aim of this survey study is to analysis clinical symptoms and patterns of dementia patients. The results of this study will be used to support development of the oriental herbal medicine for dementia. Methods : Clinical data were collected from 41 dementia patients and 20 aged persons who have no cognitive impairment in 3 sites including 2 hospitals and 1 sanatorium. 'The instrument of oriental medical evaluation for dementia' is used to evaluate clinical symptoms and patterns of all subjects. Results : 1. Subjects of deficiency syndrome patterns were nearly three times more than subjects of excess syndrome patterns. 2. In dementia patient group, the average rate for 4 clinical symptoms, related with treatment principle of clear heat, was 26.9%. 3. The average rate for 4 clinical symptoms, related with condition of urine and feces, was 15.6%. 4. The average rate for 5 clinical symptoms, related with anger, irritation, anxiety and restlessness, was 40.0%. Conclusions : 1. It is needed to develop new herbal medicine for dementia focuing on clear heat, anger, irritation, anxiety, restlessness, and condition of urine and feces. 2. 'The instrument of oriental medical evaluation for dementia' has strong tendency that excessively reflects general geriatric symptoms, related to deficiency syndrome patterns. so, more clinical symptoms of excess syndrome should be added in this tool.

  • Objectives : This case report presents a 33-year-old male patient who complained chest pain and anxiety. diagnosed with panic disorder with agoraphobia improved by Herbal Extracts. SRP(Stress Reduction Program) and Acupuncture treatment. Methods : In early treatment period. we evaluated his general psychological state through interview and self-questionnaires(STAI, SCL-90-R, STAXI, BDI) for treatment planing. And then we had prescribed herbal extracts for 20 weeks with applying SRP and acupuncture treatment. After 20 weeks we assessed his psychological symptoms with same evaluation tools. Results : After 20 weeks. We found that patient's somatic and psychological symptoms(anxiety, depression, anger) were alleviated. Frequency and severity of panic attacks were reduced also. Conclusions : Herbal extracts. especially Gyejigayonggolmoryo-tang(桂枝加龍骨牡蠣湯) and Jakyakgamcho-tang(芍藥甘草湯), may be useful therapeutic tool for control of panic attack symptoms and anxiety.

Interim Report about The Comparative Clinical Study of Efficacy of Gamisoyo-san(Jiaweixiaoyaosan) on Anxiety of Generalized Anxiety disorder (범불안장애의 불안증상에 대한 가미소요산의 제형별 효능 비교 임상연구의 중간보고)

  • Objectives : The purpose of this research is to examine the efficacy of Gamisoyosan on anxiety of generalized anxiety disorder according to dosage form. Methods : In this randomized, double blinded, placebo-controlled study, we planned to give Gamisoyosan simple extract mixture or Gamisoyosan compound extract or controlled medication for major symptoms of generalized anxiety disorder. As preparatory research, Hamilton rating Scale for Anxiety(HAM-A) was measured as the 1st evaluative instrument, and Korean State-Trait Anxiety Inventory(STAI-K), Penn State Worry Questionnaire(PSWQ) Korean Beck's Depression Inventory (BDI-K), Symptom Checklist-90-Revised(SCL-90R), WHO Quality of Life Abbreviated(WHOQOL-BREF) and Heart Rate Variability(HRV) were also measured as the 2nd evaluative instrument at the before treatment. Results : Demographic characteristics showed that there are Clinical characteristics-vital signs are within the normal range. The characteristics of disease-chief complaint, pattern Identification and etiological factor of the highest frequency number were worry, heart deficiency with timidity(心膽虛怯), family matters. The average period of disease in subjects is 6.31years. The results of Chest PA, EKG and clinicopathologic examination are within the reference range. The Scores of HAM-A, STAI-K, PSWQ were measured above the cutoff point. There are significant positive correlations among HAM-A, STAI-K, BDI-K and among HAM-A, STAI-K, PSWQ. There are no significant correlations between PSWQ and BDI-K. Conclusions : We considered that selection of subjects in this research is appropriately accomplished And this methodology is expected to be applied to the subsequent research. And also, we hope to make up for this study through various study and discussion.

Preliminary Study on the Usefulness of Psychological Scale Measurements in Depression Symptoms (우울증상에 대한 심리척도의 유용성에 대한 기초연구 -비우울증, 화병과의 비교를 중심으로-)

  • Objectives: The purpose of this study is to investigate the usefulness of psychological measurements in depression symptoms as compared with non-depressions and hwabyung. Methods: A total of 21 patients, who were diagnosed with depression and hwabyung, were evaluated by using the SCL-90-R, BDI, STAI, STAXI, ISI, HRSD, 20-Item Alexithymia Scale, BSDS and NEO-PI. Thereafter, they were statistically analyzed. Results: 1) In the results of SCL-90-R, the PST score group of hwa-byung group without depression was significantly higher when compared to the score from the depression group without hwa-byung. 2) In the results of NEO-PI, the NEO-PI-O score of hwa-byung group without depression was significantly higher when compared to the score from the depression group without hwa-byung. 3) In the results of 20-Item Alexithymia Scale, the total score and score of difficulty for identifying feelings scale from the depression group was significantly higher when compared to the score of non depression group. Conclusions: The results found are expected to serve as a useful resource for future studies when developing the instruments of oriental medical evaluations for hwa-byung.
