A Study on Priority of Patient's Medicine Task for the Emergency Department in IoT Environment (사물인터넷(IoT) 환경의 응급실에 있어서 진료테스크 선정 지원 알고리즘 개발)

  • With high interest in the patient satisfaction of emergency medical services, there is a lot of effort into improving the process of Emergency Department(ED) utilizing the technology of Internet of Things(IoT). In this study, the core technologies of smart ED are examined and a decision support algorithm for medicine tasks is proposed. The proposed algorithm minimizes the decision risks such as task selection accountability, patient complaints, care delays and longer stay time. It can reduce the nurses burnout and improve the patient care with kindness and consideration. Ultimately, patient satisfaction, job satisfaction and professional identity of nurses can be increased. The comparative study was carried out by simulation in terms of the average length of patient stay in a simplified hypothetical ED system. In all the cases, the proposed algorithm was shown to perform substantially better than the other rule.

  • 최근 폭발적인 스마트폰의 보급률 증가와 이에 따른 사용자 증가로 인해 스마트폰을 이용한 다양한 어플리케이션이 개발되어 사용자들에게 배포 되고 있다. 본 논문은 이러한 스마트폰을 활용하여 언제 발생할지 모르는 응급상황에 대해서 스마트폰 어플리케이션 사용자에게 각 상황에 따른 응급상황에 대한 대처 요령 및 처치 방법을 제공함으로써 긴박한 실제 응급상황에서도 신속하고 유연하게 초기 대응할 수 있도록 하였다. 또한 가까운 응급실 정보를 제공함으로써 빠르게 응급실로 연락 또는 이동할 수 있는 정보를 제공하여 2차 피해를 줄일 수 있는 어플리케이션을 구현하였다.

  • Recently, there will be many changes that revised emergency medical service with prescribe specialist doctor emergency medical center duty. if emergency patients come emergency medical center, in the existing system, emergency patients receive treatment in order emergency room doctor and 1-2 resident and 3-4 resident and specialist doctor. in improved system, emergency patients receive treatment to emergency room doctor and duty specialist doctor. as a result, the procedure was. simplify. but appling such a system, there should be placed duty specialist doctor about all departments in hospital. So, all hospitals be difficult to place duty specialist doctor about all departments in hospital. In this paper, to use mobile device, there design integrated emergency center management system for revision of the emergency medical service to use emergency medical center service near the user's and specialist doctor service in hospital and the hospital's information service and Emergency room usage service.

  • In emergency situations, first aid workers are the main task of trauma evaluation and care. However, the scope of first-aid treatment is small and they are mainly carried out to the emergency room of the hospital. If a specialist who is in charge of an emergency situation is connected and emergency medical treatment through remote medical direction is performed, accurate diagnosis and appropriate care can be helped. This paper has developed an smart application(app) to support remote medical direction for emergency medical services. The developed smart app allows emergency rescuers to call a doctor at a remote location and transmit real-time status of emergency patients to vital sign and video. This will help to diagnose the patient's condition accurately. In addition, emergency care can be instructed and response in the emergency room can be made quickly.

  • Recently, medical technology and IT technology have been trying to converge to overcome limitations of the time and the space in medical technology application. In this study, an app was developed to support remote medical direction for emergency medical services. The developed app allows doctors in remote locations to receive real-time emergency patient situations from emergency paramedics. It can also guide the patient's condition diagnosis and emergency treatment and enable rapid response from the emergency room.

  • FHIR is a protocol that enables easy data exchange from the time the event occurred in health care settings as a standard for next-generation message exchange of HL7. Create a meaningful message from the ECG and medical equipment, and express the messages generated by standardized FHIR message it will be used in various medical institutions to ensure delivery to EMR, such as hospital information systems can query the results via a smartphone. In addition, it thought to be used in the future to expand SMART on FHIR, Cardiology, Cardiovascular Surgery ward in real-time remote monitoring.

Survey on the Educational Needs for Smartphone Based Emergency Situations - Targeting Parents with Elementary School Children (스마트폰 기반 응급상황 대처 교육에 대한 요구도 조사 - 초등학생 자녀를 둔 부모를 대상으로)

  • This study investigates the educational needs of parents of elementary school children, who use smartphones for emergencies. This is a descriptive cross-sectional study, and data of 200 parents of students under the 6th grade was collected from October 17 to 27, 2017. The study results indicate that 67.5% parents visited the emergency department due to a child's emergency accident. Of these, 74% respondents replied that they were unaware of how to deal with the emergency at that instance. Most parents had never received education, and were using smart-phones to acquire information on first aid. Ed. Notes: Do you mean education in general (i.e. Most parents were uneducated) or do you mean 'education for emergencies'? Please revise for more clarity. Parents' demand for education on child emergency response scored high, with an average of 3.40 (±0.42) out of 4 points. Among the educational categories, "the management of foreign body airway obstruction and cardiopulmonary resuscitation (CPR)" scored the highest average for educational needs (3.64±0.52). This research provides baseline information for understanding the educational needs of parents for first aid for the child. Our findings indicate that the smartphone is potentially an effective educational tool for parents.

Smartphone Fundus Photography in an Infant with Abusive Head Trauma (학대뇌손상 영아에서 스마트폰으로 촬영한 안저소견)

  • Purpose: To report fundus photography using a smartphone in an infant with abusive head trauma. Case summary: An 8-month-old male infant presented to the emergency room with decreased consciousness and epileptic seizures that the parents attributed to a fall from a chair. He had no external wounds or fractures to the skull or elsewhere. However, computerized tomography of the brain revealed an acute subdural hematoma in the right cranial convexity and diffuse cerebral edema, leading to a midline shift to the left and effacement of the right lateral ventricle and basal cistern. The attending neurosurgeon promptly administered a decompressive craniectomy. Immediately after the emergency surgery, a fundus examination revealed numerous multi-layered retinal hemorrhages in the posterior pole extending to the periphery in each eye. He also had white retinal ridges with cherry hemorrhages in both eyes. We acquired retinal photographs using the native camera of a smartphone in video mode. The photographer held the smartphone with one hand, facing the patient's eye at 15-20 cm, and held a 20 diopter condensing lens at 5 cm from the eye in the other hand. Our documentation using a smartphone led to a diagnosis of abusive head trauma and to obtain the criminal's confession, because the findings were specific for repetitive acceleration-deceleration forces to an infant's eye with a strong vitreoretinal attachment. Conclusions: This ophthalmic finding had a key role in the diagnosis of abusive head trauma. This case presented the diagnostic use of a smartphone for fundus photography in this important medicolegal case.

A Study on Emergency Medical Information Management Methods for Elderly Patients using QR code and Finger-print Recognition (QR 코드와 지문인식을 이용한 고령 환자의 응급 의료정보 관리 방법에 대한 연구)

  • Recently, as elderly people suffering from chronic diseases have increased and disasters such as traffic accidents have increased, urgent rescue workers and doctors need to respond effectively to such emergencies. For elderly people, the difficulty in communicating their condition especially the health condition during disaster is one of the problems in which delay on immediate handling by rescue team can cause death. Providing a solution for this problem is one of the main issues in our research. In this paper, we use the QR code to communicate the minimum personal information and medical history needed for emergency medical treatment to emergency medical personnel, and to transmit the medical history and treatment data to the doctors of the hospital through QR code and fingerprint recognition, respectively. Especially, by using both QR code of smart phone and fingerprint of individual, we can protects personal information and strengthens privacy and security of medical information by allowing only authorized physicians to check medical records of the old patient.

  • Korea has very easy access to tertiary hospitals, including university hospitals, among OECD countries, and patients can reach the emergency room of a university hospital within 1-2 hours. However, there are many so-called 'essential medical' blind spots where people die because they do not receive surgery in time. In particular, in the case of essential medical care in the surgical field, despite basic surgery, there is a shortage of medical staff to perform outpatient surgery in emergency situations at university hospitals. Although this lack of manpower has a problem with low insurance premiums for surgery, it also has a very large impact on the burden of criminal liability for medical malpractice, which increases the incidence in case of emergencies. Here, we propose crime immunity to solve the manpower shortage of converged smart surgical essential medical (SES) hospitals. Currently, the medical community agrees on the need for crime immunity, but it is an ambiguous scope of immunity that has not reached a national consensus. We would like to present clear standards for essential medical practices (surgery) that require criminal immunity.
