The Effect of Informational Intervention Delivered by Electronic Patient Information Board in The Waiting Room on The Anxiety of Mothers Whose Children Underwent Elective Surgery (수술대기실 환자현황판을 이용한 수술진행 정보제공이 환아 어머니의 불안에 미치는 효과)

  • Purpose: The purpose of this study was to explore the effectiveness of informational intervention delivered by electronic patient information board in the OR waiting room for reduction of anxiety of mothers with child having operation. Method: Nonequivalent control group non-synchronized design was used in this study. The subjects was 52 mothers whose children underwent elective surgery in one Pediatric Hospital. Informational intervention in this study consisted of four critical stages. State anxiety by Spielberger, mean arterial pressure, and heart rates at preoperative and postoperative period were measured. The data were analyzed by using Chi-Square test and t-test with SPSS/PC 10.0 program. Results: 1) There was a significant difference in the state anxiety between the control and the experimental groups. 2) There was no significant difference in the mean arterial pressure between the two groups. 3) There was no significant difference in the heart rate between the two groups. Conclusion: The results of this study indicate that informational intervention delivered by electronic patient information board would be an effective intervention during the operation in reducing anxiety of mothers with child having operation.

Effects of pain, sleep and self-care behavior in patients underwent robotic minimally invasive cardiac surgeries (로봇을 이용한 심장수술이 환자의 통증, 수면 및 자가간호수행에 미치는 영향)

  • The purpose of this study was to know the difference in pain, sleep, self-care behavior in patients performed by sternotomy and robotic minimally invasive cardiac surgeries. The participants were 64 patients with sternotomy and 64 patients with minimal thoracotomy in heart valve surgeries at a tertiary hospital in Seoul. Data were analyzed using descriptive statistics, ${\chi}^2$ test, paired t-test. with SPSS/WIN(22.0). The participants with minimally invasive thoracotomy felt severe pain than the patients with thoracotomy at post operation day 2 & 5. The severe pain site were the surgical site in both groups. The participants with sternotomy had better sleep than minimally invasive thoracotomy patients. Self-care behavior was higher in the minimally invasive thoracotomy group. Therefore, despite the many advantages of robotic surgery such as rapid recovery and shortening of length of stay in hospital, accurate pain assessment and application of differentiated protocols are needed for the management of pain in the patients with robotic minimally invasive cardiac surgeries. In addition, a structured education program intervention is needed to improve comfort by considering gender, age, and method of operation.

Relationships between Specific self-efficacy, Family support, and Self-care performance for Patients with Stomach Cancer after Gastrectomy (위암 수술환자의 구체적 자기효능감, 가족지지와 자가간호수행의 관계)

  • This study was conducted to investigate the relationships among self-efficacy, family support, and self-care performance of gastric cancer surgery patients. The study was conducted from 11 October to 15 November 2013. Data were collected by self-report questionnaires from 121 patients who were diagnosed with stomach cancer and had undergone surgery at D university hospital in B city and were in follow-up care for 3 months to 5 years after surgery. Data were analyzed using SPSS/PC WIN 18.0 to obtain the percentage, average, mean rating, and standard deviation. In addition, t-test, ANOVA, Scheffe's test, Pearson's correlation and stepwise multiple regression were conducted. The degree of self-care performance in research targets showed significant differences by sex (t=-2.25, p=0.027), religion (F=3.67, p=0.028) and profession (F=4.17, p=0.008). Self-care performance was positively correlated (r=0.60, p<0.001) with the degree of specific self-efficacy. There was a significant difference in self-care performance by specific self-efficacy, religion and gender. The total explanatory power was 37.9% and the explanatory power of the degree of specific self-efficacy (${\beta}=0.53$) was greater than that of other factors. Therefore, it is necessary to provide interventions that improve specific self-efficacy to help patients with gastric cancer patients conduct self-care performance, and practical measures should be made with respect to religion and gender.

Stress among Hospital OR Nurses (수술실 간호사의 스트레스와 관련된 요인 분석)

  • To measure the degree of work stress and identify factors influencing the stress among OR nurses, a study was implemented for 131 OR nurses working at four university hospitals in Taegu city through self administered questionnaires. Mean score of total work stress of nurses was 3.71, when maximum score was 5. Relatively high degrees of work stress was evident among OR nurses. When the related factors of the stress specified in the questionnaire were grouped into 11 categories, the respondents turned out to feel the highest stress when they were faced with the categories of non-peculiar duty and personal conflict with doctors. The most stressful events were nursing malpractice during surgical operations and damage to OR equipments. The nurses more susceptible to workstress were married with one child, less educated and worked at OR for 3 to 6 years.

Factors Affecting Intraoperative Body Temperature in Surgical Patients with Laparotomy under General Anesthesia (전신마취하 개복술 환자의 수술중 체온에 영향을 미치는 요인)

  • Purpose: The study was conducted to identify factors affecting the intraoperative core body temperature (CBT) of surgical patients under general anesthesia. Methods: This study was performed through a prospective descriptive research design. The sample consisted of 138 patients who had undergone elective laparotomy surgery. Age, weight, height, the basal preoperative CBT, blood pressure, and heart rate were collected. CBT was again measured at induction of anesthesia, 1 hour, 2 hours, and 3 hours following induction of general anesthesia. Results: Factors affecting intraoperative hypothermia < $36^{\circ}C$ at 1 hour following induction, were CBT at induction and total body fat (TBF) ($R^2=.569$, p<.001); at 2 hours after induction, CBT at induction and TBF ($R^2=.507$, p<.001); at 3 hours after induction, CBT at induction (${\beta}=0.34$), TBF, age and the ambient temperature in the operating room ($R^2=.449$, p<.001). Conclusion: CBT at induction and TBF appear to be factors affecting intraoperative CBT within 2 hours after induction of anesthesia; CBT at induction, TBF, advanced age and the ambient temperature after 3 hours following induction. We recommend keeping surgical patients warm before induction of anesthesia and providing intraoperative warming for surgical patients of advanced age with low TBF and when the duration of general anesthesia will last more than 3 hours.

The Competence and Satisfaction on Inventory Management of the Operating Room Nurses (수술실 간호사의 물품관리 업무역량과 업무만족도 연구)

  • The study verified the work capacity (job performance, perception, knowledge) and satisfaction on managing the inventory of operating room nurses working in a more than 700-bed hospital. The purpose of this study was to provide basic data of education for work capacity and satisfaction and quality insurance by analyzing the scores between the three different operating rooms and the correlation between the work capacity and satisfaction by investigating the related factors. This study presented a structured and self-administered questionnaire to 181 nurses who had been working in the operating room more than six months. The mean and standard deviation of the job performance, perception, knowledge, and satisfaction were 4.2(${\pm}0.56$), 3.4(${\pm}0.76$), 3.5(${\pm}0.40$), and 3.4(${\pm}0.55$), respectively. The work capacity and satisfaction of each group did not show a statistically significant difference. The correlations between the job performance and knowledge, knowledge and perception, perception and satisfaction were positive (r=.627, p<0.01), (r=.663, p<0.01) and (r=.485, p<0.01), respectively. Among the factors related to the general characteristics of operating room nurses, only age significantly affected their job performance. This study provides basic data on the maintenance and improvement of their competence and satisfaction by being served as a resource for sustainable human resources management and training, and efficient management of the communication channel between hospitals.

Analysis of Unmet needs by Elapsed Time in Kidney Transplantation Patients (신장이식 환자의 수술 후 경과기간에 따른 미충족요구 분석)

  • This study was conducted to for the development of nursing education programs suitable for the period by identifying the unmet needs of kidney transplant surgery patients who need continuous self-care according to the elapsed period. The subjects of the study consisted of 84 inpatients and outpatients for up to 1 year after kidney transplant surgery. The data collection period was from February 17 to May 31, 2020, and the collected data were analyzed by Chi-square, independent t-test, one-way ANOVA, and post-test by Scheffé using the IBM SPSS 21.0 program. As a measurement tool, the Comprehensive Needs Assessment Tool in Cancer(CNAT), a tool to assess unmet needs of cancer patients, was modified and supplemented to receive expert validity, and then self-report through a questionnaire consisting of a total of 57 questions. As a result of the study, the average of the total score out of 171points for the unmet needs of kidney transplant patients was 68.28(±38.88), and information and education accounted for the highest score of 17.92(±8.36) out of the eight sub-areas. The unmet needs according to the elapsed period were significantly higher in the 6 months ~1 year group(F=7.57, p=.001) compared to the 3-6 months group. Based on this, it is necessary to develop a nursing intervention program that can provide appropriate information and support at a time when the unmet needs of patients up to increase from 6 months to one year after kidney transplantation increase.

Development of an Instrument to Measure Intra-Operative Caring Behaviors Perceived by Regional Anesthesia Patients (부위마취 환자가 지각한 간호사의 수술 중 돌봄행위 측정도구 개발)

  • Purpose: The purpose of this study was to develop a tool for measuring the intra-operative caring as perceived by regional anesthesia patients. Methods: A preliminary instrument with 54 questions, based on a literature review and semi-structured interviews with 11 regional anesthesia patients, was developed. A group of experts revised individual questions, and 34 questions were finally selected. This tool was tested with 137 regional anesthesia patients admitted to D university hospital in B city from August to October 2008. Results: The validity and reliability of the tool were tested using factor analysis. After item analysis, one question with a correlation coefficient under .30 was discarded After performing factor analysis on the final 33 questions, 7 factors were identified; holistic needs fulfillment, consideration, protective environment, cautiousness, concern, information, physical comfort. The total variance shown in the test was 73.5%. A Cronbach alpha of 0.96 showed the reliability of the instrument. Conclusion: Results of this study indicate that the tool is statistically reliable and valid to measure the intra-operative caring perceived by regional anesthesia patients. This tool can be utilized usefully in assessing the effects of nursing interventions for regional anesthesia patients.

Differences between Critical thinking Disposition and Clinical practice according to Satisfaction for Nursinga Practice (간호학 만족도에 따른 비판적 사고성향과 임상수행능력의 변화)

  • This study examined the effects of stepwise education for operation patient care according to nursing satisfaction on the critical thinking disposition and clinical performance. The participants were 192 junior students who signed a consent form with an explanation of the study. Data were obtained from the students on the first day of junior's first semester and the last day of junior's first and second semester. Data analysis was performed using the number, percentage, mean, standard deviation, Chi-square test, t-test, and repeated measure ANOVA through the SPSS/WIN 23.0 program. Results showed that nursing students with high nursing satisfaction had higher critical thinking disposition than those with lower nursing satisfaction (F=4.57, p=.012). There were no significant differences according to time (F=0.20, p=.821). The clinical performance was not significantly different according to nursing satisfaction (F=1.08, p=.343) and time (F=1.58, p=.210). This means that increasing nursing satisfaction would be a strategy for increasing the critical thinking disposition that was the core competency of nursing students.

Convergence study on Effects of Music Therapy in Patients Undergoing Prostatectomy with Spinal Anesthesia (척추마취 전립선절제술환자의 음악요법효과에 대한 융합연구)

  • The purpose of this convergence study was to identify the effects of favorite music therapy on anxiety, fatigue, and vital signs of patients undergoing prostatectomy with spinal anesthesia. This study used a nonequivalent control group design. A sample of 45 patients was included. The experimental group was given music therapy during operation. The data were collected using a structured questionnaire and monitoring at 30 min before operation, at 20 min and 40min undergoing operation, and at arrival recovery room after operation. Data were analyzed using descriptive statistics, ${\chi}^2-test$, Fisher's exact test, t-test, repeated measures ANOVA. The experimental group reported significantly lower anxiety and lower fatigue than the control group(p=.001; p=.020). However there were no significant differences in the systolic blood pressure, diastolic blood pressure and pulse rate between groups(p=.821; p=.473; p=.782). This findings indicate that the tailored favorite music therapy can be an effective nursing intervention for patient undergoing prostatectomy with spinal anesthesia to reduce anxiety and fatigue related to operation.
