Petrological Characteristics and Deterioration State of Standing Buddha Statue in the Gwanchoksa Temple, Nonsan, Korea (논산 관촉사 석조미륵보살입상의 암석학적 특성과 풍화훼손도)

  • The Standing Buddha Statue in the Gwanchoksa temple consists of medium to coarse grained biotite granodiorite with dark grey color, and it has a week gneissosity along the pegmatite veins. The results of magnetic susceptibility and geochemical patterns of the host rock of Standing Buddha Statue and the basement rock suggest that both values are formed from the co-genetic magma with the same differentiation process. The CIAs of the basement rock and the Standing Buddha Statue are calculated to 51.43 and 50.86, and the WPIs are estimated 4.52 and 8.95, respectively. So the weathering potential from the host rock of Standing Buddha Statue and basement rock prove to be high. The Standing Buddha Statue is terribly damaged with physical weathering from deterioration and exfoliation, and are scattered with secondary pollutant and precipitate. Basement rock is also in danger of ground collapse because of irregularly developed discontinuity system. Most surface of Standing Buddha Statue is seriously discolored into yellowish brown and dark gray, or black precipitates are also formed. Moreover, it is heavily covered with crustose lichen, fungi and algae, or moss are also found. In order to control the influential factors with the complex deterioration of Standing Buddha Statue, it is needed to rearrange a site environments, and conservation scientific management is required to protect it from covering lichens, exfoliations and fractures.

Study on Hair Characteristics Analysis and Performance Evaluation of Traditional Brushes (전통 붓의 섬유 특성 분석 및 성능 평가 연구)

  • In this study, the characteristics of various raw hairs used for traditional Korean brushes were examined; further, the characteristics and deterioration patterns of Korean, Chinese, and Japanese brushes were compared, with a quantitative evaluation to assess the brush performance. The tensile strength was generally found to be higher with a greater fiber thickness. Among the hairs examined, the back and flank hair of goat was more damaged than that in other parts, and the tensile strength was low. Higher elasticity of the brush made with hair of high cysteine content was measured. Owing to deterioration by use of the brushes, artificial drying brushes had a higher yellowness index and lower tensile strength than natural drying brushes. Further, it was confirmed that brushes with good absorbency exhibited good consistency, but not good elasticity. Thus, the performance of the brush can be influenced by the kind of material used and the brush usage pattern. In addition, it is possible to identify the material science characteristics of brushes which have been produced only by experience; therefore, the results of this study could provide basic data for manufacturing brushes employed in conservation treatment, in the future.

Repair of Ruptured Papillary Muscle of the Tricuspid Valve as a Cause of Tricuspid Insufficiency Following Blunt Chest Trauma (흉부 둔상후 삼첨판막 폐쇄부전을 유발한 삼첨판막 유두근 파열의 치료)

  • A 47-year-old man presented with complaints of chest pain and dyspnea caused by deceleration injury due to an automobile accident. Systolic cardiac murmur was audible at the right sternal border. An electrocardiogram showed sinus tachycardia. Transthoracic echocardiography revealed a flailing anterior leaflet of the tricuspid valve, papillary muscle rupture, and severe valve insufficiency. Rupture of papillary muscle of the anterior leaflet and chordae tendineae of the posterior leaflet were confirmed by right atrial incision under routine cardiopulmonary bypass. Artificial chordae tendineaes were implanted between the anterior and posterior leaflet and papillary muscles in the right ventricles. De-Vega annuloplasty was also added. This is a very rare case in which a surgery was done for tricuspid valve regurgitation caused by post-traumatic papillary muscle rupture.

  • Hypertermia for the treatment of cancer has been introduced for a long time and the biological effect for the use of hyperthermia to treat malignant tumors has been well established and encouraging clinical results have been obserbed. Unfortunately, however, the engineering or technical aspects of hyperthermia for the deep seated tumors has not been satisfactory. We developed the radiofrequency capactive hyperthermia device (Greenytherm-GY8) in cooperation with Yonsei Cancer Center and Green Cross Medical Corporation. It was composed with $8{\sim}10MHz$ RF generator, capacitive electrode, matching system, cooling system, temperature measuring system and control PC computer. The thermal profile was investigated in agar phantom, animals and in human tumors, heated with capactivie RF device. Deep and homogeneous heating could be achieved in a large phantom of 25cm diameter and 19cm thick when heated with a pair of 23cm diameter electrodes, coupled to both bases of the phantom, when the size of the two electrodes was not the same, the region near the smaller electrode was preferentially heated. It was, therefore, possible to control the depth of heating by choosing proper size of electrodes. Therapeutic temperature $(42^{\circ}C{\sim}43^{\circ}C)$ could be obtained in the living animal experiments. Indications are that deep heating of humn tumors might be achieved with the capacitive method, provided that subcutaenous fat layer is cooled by temperature controlled bolus and large size of electrodes.

Anteroinferior Capsulolabral Complex Repair Using Antegrade Suture Passer - Technical Note - (Antegrade Suture Passer를 이용한 전하방 관절낭-관절와순 복합체의 복원술 - 수술 술기 -)

  • We introduce arthroscopic Bankart repair technique using antegrade suture passer that can effectively restore detached anteroinferior capsulolabral complex for shoulder anterior instability. After diagnostic arthroscopy is performed using posterior, anteroinferior and anterosuperior portals, we confirm Bankart lesion and perform debridement and decortications of anteroinferior glenoid edge and neck. Suture anchor is inserted through anteroinferior portal at 2 mm medial side of glenoid edge (4:30 direction). Scorpion$^{TM}$ loaded suture is directly advanced to detached and retracted anteroinferior capsulolabral complex and the suture is passed at 10~15 mm medial side of detached anteroinferior capsulolabral complex (5:30 direction). The suture is retrieved by Scorpion's hook and then is tied using samsung medical center (SMC) sliding knot technique. Then suture anchors are serially inserted (2:30, 3:30) and capsulolabral complex repair is performed using suture hook and suttle-relay technique. This technique that can obtain anatomical restoration of anteroinferior glenohumeral ligament with proper tension is useful technique to reduce postoperative recurrence and makes it possible for less experienced surgeons.

  • Small cell lung cancer is characterized by an aggressive clinical course and a high tendency for early dissemination in spite of a good chemotherapy response. Topotecan is a topoisomerase I inhibitor, and it is used as second-line treatment for small cell lung cancer. The reported dose-limiting adverse reactions to topotecan are mainly hematologic. Yet pulmonary toxicity associated with topotecan is known to be rare. We report here on a case that showed the development of acute respiratory distress syndrome during the 3rd cycle of topotecan chemotherapy in a patient with small cell lung cancer. He developed dyspnea and respiratory failure, and the chest CT scan revealed diffuse ground-glass opacity that was probably due to chemotherapy-related pulmonary toxicity. He finally died of acute respiratory distress syndrome.

DENTAL TRAUMA MANAGEMENT KNOWLEDGE AMONG A GROUP OF TEACHERS IN GWANG-JU (광주광역시 초중고 교사들의 치아외상에 대한 지식과 태도)

  • The purpose of this study is to evaluate the level of dental trauma management in elementary, middle and high school teachers using questionnaires. 325 elementary school teachers, 101 middle school teachers, and 232 high school teachers, totally 658(males of 362, females of 296)teachers were provided with the questionnaires. Part I in the questionnaires was composed of their gender, age, experience as teachers and whether they received education about dental trauma or not. Part II included questions about appropriate attitudes in case of specific dental trauma. Part III was composed of questions asking knowledges about states of damaged teeth and possible emergency service. The attitude in managing tooth fracture showed significant difference between male and female teachers. The attitude in managing tooth avulsion showed significant difference among elementary, middle and high school teachers. Although it had difference according to questions, insufficient knowledge and awareness about the management of dental trauma were shown in the majority of teachers. It is recommended that educational program for improvement of awareness about immediate management of traumatized teeth is necessary for teachers.

  • Congenital esophagobronchial fistula presented in adult life is rare and usual1y manifests as recurrent pneumonias or other chronic suppurative complications such as bronciectasis. Most of congenital esophagobronchial fistula can be diagnosed by esophagography and can be cured by fistulectomy and/or resection of destroyed pulmonary lobes. We recently experienced 5 cases (2 female and 3 male patients) of congenital esophagobronchial fistula (all of them were Braimbridge type I). Mean age of the patients at the time of diagnosis of esophagobronchial fistula was 53.8 year-old (44-70 year-old) and mean duration of symptoms and complications such as cough after swallowing water, recurrent pneumonias and bronchiectasis was 29 years (12-50 years). 4 patients were treated by fistulectomy and resection of destroyed lobes with abolition of symptoms. So we report 5 cases of congenital esophagobronchial fistula with review of literatures.

  • Serum CA 125 is the most useful marker for monitoring patients with epithelial ovarian cancer. However, it can be elevated above normal level in a variety of conditions other than ovarian cancer such as endometriosis, pelvic inflammation disease, and other malignant or nonmalignant disorders, including pulmonary diseases. Recently, we experienced a case of bronchiectasis in which the serum CA 125 level was elevated, changing with the patient's condition. There was no evidence of underlying malignant disease on positron emission tomography or on gynecologic examination, including transvaginal ultrasonography. During follow-up for 14 months, we could not find any clue of malignant disease that could have been the cause of the elevated levels of serum CA 125. Elevated serum CA 125 level should be interpreted carefully according to the patient's clinical condition. In addition, our case suggests that CA 125 may be used as a surrogate marker for acute inflammatory status for chronic pulmonary diseases.

  • The Inhalation of certain freshly formed metal oxides can cause metal fume fever, which is an acute, self-limiting, flu-like illness. The most common cause of this syndrome is the inhalation of zinc oxide. The inhalation of zinc oxide can lead to tracheobronchiolitis, chemical pulmonary edema or to respiratory failure and acute respiratory distress syndrome(ARDS). We encountered a 43-years-old man who developed severe dyspnea after inhaling of zinc oxide while working for 5 hours in a closed space. He was diagnosed with ARDS and was treated successfully with glucocorticoid. We report a case of ARDS caused by the inhalation of zinc fumes.
