  • Background : The mortality from acute respiratory distress syndrome(ARDS) is >40-50%. Although some prospective trials have failed to demonstrate a survival benefit of steroids in the early stages of ARDS, there are some reports showing some success with steroids in the later stages. This study observed the changes in the physiologic parameters with time in late ARDS patients who were treated with steroids. Methods : The medical charts of 28 intensive care unit patients(male:female=24:4; mean age 64 years), who had been diagnosed with refractory late ARDS ($PaO_2/FIO_2$ <200) and were treated with corticosteroids from December 1999 to July 2002, were retrospectively reviewed. The patients were divided into two groups: the weaned group(n=14), which included the patients who had been successfully weaned from a ventilator after corticosteroid therapy, and the failed group(n=14), which included the patients who had failed weaning. The physiologic parameters included the $PaO_2/FIO_2$ ratio, the positive end-expiratory pressure(PEEP) level, the $PaCO_2$, compliance, the sequential organ failure assessment(SOFA) score, the acute physiologic and the chronic health evaluation(APACHE) II score, and the Murray Lung Injury Score(LIS) in the two groups were compared from the day of mechanical ventilation(Dmv) to 7 days after the corticosteroid therapy. Results : There was no significant difference in the clinical characteristics and the physiologic parameters between the two groups prior to the corticosteroid therapy except for the SOFA score at Dmv(weaned group : $6.6{\pm}2.5$ vs failed group : $8.8{\pm}2.9$, p=0.047). However, within 7 days after corticosteroid therapy, there was significant improvement in the $PaO_2/FIO_2$ ratio, the PEEP level, the $PaCO_2$, the SOFA score, the APACHE II score, and the LIS of the weaned group compared to the failed group. Conclusions : During corticosteroid therapy in late ARDS, the continuation of corticosteroid therapy should be determined carefully in patients who do not show improvement in their physiologic parameters by day 7.

  • Effect of Quartz porphyry (QP) on clinical chmical parameters in male common finch by feeding the basal diet (Control group) containing 3.0% QP (QP group) for 14 days was studied. Body weight gain, feed intake, feed efficiency, the relative weight (mg/10 g body wight) of liver and heart were not significantly different in the both groups. However, the relative weights of kidney and gizzard in the QP group were higher and lower than in the control group, respectively. The contents of triglyceride, cholesterol, total protein, glucose and albumin, and the activity of glutamic pyruvic transaminase (GPT) in serum were not significantly different in the both groups. The activities of alkaline phosphatase (ALP) and glutamic oxaloacetic transaminase (GOT) in serum were lower and higher in the QP group than in the control group.

Retention Effects of Dietary Education Program on Diet Knowledge, Diet Self-Care Compliance, Physiologic Indices for Hemodialysis Patients (혈액투석 환자의 식이 교육프로그램이 식이지식, 식이 자가간호 이행 및 생리적 지표에 미치는 지속효과)

  • Purpose: The purpose of this study was to identify the retention effects of an individualized dietary education program for hemodialysis patients on diet knowledge, diet self-care compliance, and physiological indices, thus to find the most effective time period for re-education. Method: This study utilized one-group repeated pretest-posttest design. The participants were 52 hemodialysis patients in C hospital, Gyeonggi-Do. Data were collected at 4 and 12 weeks after the education from January through April 2016. Results: There was significant increases in diet knowledge even 12 weeks after the education (p= .007). Diet self-care compliance showed a significant increase at 4 weeks (p= .001), but a decrease at 12 weeks after the education. The level of blood natrium was significantly decreased between 4 and 12 weeks after the education (p= .006). The weight was significantly decreased at 12 weeks after the education. Conclusion: It has been identified that re-education for hemodialysis patients should be implemented between 4 and 12 weeks after education in order to maintain patients' diet self-care compliance, an ultimate aim of diet education. By helping them with their self-care compliance, the patients would maintain their physical and psychological function optimally, thus contributing to a better quality of life among hemodialysis patients.

Exploratory Study of the Prevalence of Major Chronic Disease According to Daily Water Intake and Physiological Parameters related to Dehydration in Korean Elderly: Based on the Sixth Korea National Health and Nutrition Examination Survey (2015) (한국 노인의 일일 수분섭취 상태에 따른 주요 만성질환 유병률 및 탈수 예측 생리적 지표 탐색 연구: 2015년 국민건강영양조사 결과를 바탕으로)

  • Purpose: The aim of this study was to explore the prevalence of major chronic diseases related to daily oral water intake and to identify the physiological parameters related to dehydration in Korean elderly. Methods: The data were collected from the sixth Korea Health and Nutrition Examination Survey (KHANES), which was a nationwide and cross-sectional survey in 2015. We analyzed 1,392 participants using t-test and logistic regression. All participants were divided into the adequate water intake (AWI) group and the non-adequate water intake (NAWI) group based on the dietary reference intakes for Koreans. Results: There was a significant difference in the water intake between the AWI (6.8 cups in a day) and NAWI (2.8 cups) groups (p< .001). There was no statistically significant association between the level of water intake and any of the major chronic diseases. Blood urea nitrogen (BUN) and BUN/Creatinine (Cr) ratio were significantly higher in the NAWI group. Especially, BUN/Cr ratio shows that the NAWI group reached dehydration status. Older age (adjusted odd ratio, OR= 1.07, 95% confidence interval, CI [1.04-1.10]), female gender (adjusted OR= 1.56, 95% CI [1.05-2.33]), lower body mass index (BMI) (adjusted OR = 1.00, 95% CI [0.92-1.00]), higher BUN (adjusted OR = 1.04, 95% CI [1.01-1.08]), and higher urine specific gravity (USG) (adjusted OR= 1.56, 95% CI [1.19-2.05]) were factors associated with the NAWI group. Conclusion: The findings suggest that the level of water intake needs to be considered in relation to age, gender, BMI, BUN, and USG. These are sensitive physiological parameters used for predicting dehydration of the elderly according to their daily oral water intake. It would be helpful to develop strategies to prevent dehydration in elderly individuals and enhance their water intake.

A Study on Physiological Parameters, Health Perception, and Health Behavior of Adults in Kyungi Province Area (경기지역 일부성인의 생리적 지표와 건강지각, 건강행위에 관한 연구)

  • Purpose: The purpose was to investigate the physiological parameters and health perception of adults in Kyungi province area, and the correlation between these variables. The subjects were 95 adults who participated voluntarily in the health examination program. Methods: The instruments for this study were physiological parameters, which were composed of blood pressure, blood sugar, and body fat. The health perception scale developed by Ware (1976), and the health practice performance scale developed by Chang et al. (1999). Results: The average blood pressure was 137/85mmHg, the average blood sugar was 108.56mg/dl, and the average body fat arm sunt was 27.08% in males, and 30.07% in females. There were significant negative correlation between health perception and blood sugar (r=-2.36, p=.01), and body fat (r= -2.77, p=.001). There were significant positive correlations between blood pressure and health behavior (r=2.76, p=.001). However a significant correlation between health perception and health behavior was not found. Of the sociodemographic characteristics, sex (F=5.42, p=.01), and marital status (F=3.29, p=.04) were related to blood pressure. Also sex (F=4.17, p=.04) was related to body fat, and sex (t=4.04, p=.03) was related to health perception and disease status (t=4.33, p=.040). Thus having a relationship with health behavior. Conclusion: It is suggested that health perception provide important information about health status. However, this study has not shown a correlation between health perception and health behavior. Therefore further research is needed to find the other variables related to health behavior.

Effects of an Individualized Educational Program on Knowledge, Compliance and Physiologic Parameters in Non-Compliant Hemodialysis Patients (개별교육프로그램이 역할불이행 혈액투석환자의 투석관련 지식, 이행 및 생리적 지표에 미치는 효과)

  • Purpose: This study was done to provide effective nursing interventions using an individualized educational program designed to contribute to the knowledge, compliance and physiologic parameters (serum potassium, phosphorus and interdialytic weight gain) of non-compliant hemodialysis patients. Methods: There were 22 participants in the experimental group and 19 in the control group. Nurses with rapport with patients provided education to non-compliant hemodialysis patients for 20 minutes three times a week for six weeks. This education program consisted of individual consulting, telephones conversations regarding hemodialysis, and requests for patient support from patients' families and colleagues. Results: Hypothesis 1; "Knowledge about hemodialysis in the experimental group will be higher than the control. group" was not supported. Hypothesis 2; "Compliance will be higher in the experimental group than in the control group", and Hypothesis 3; "Physiologic parameters of experimental group will improve" were supported as serum potassium and interdialytic weight gain decreased, but results were not consistent for phosphorous. Conclusion: This program for non-compliant patients increased compliance and improved physiologic parameters. Therefore, this educational program should be effective as a nursing intervention.

Self-care, Social Support, and Biological Markers in Liver Transplant Recipients (간이식 수혜자의 자가간호이행, 사회적 지지, 생리학적 지표)

  • Purpose: To examine the relationships between self-care, social support, and biological markers in liver transplant recipients. Methods: The participants included 118 liver transplant recipients who visited outpatient clinic at Y University Hospital in Seoul from April to May, 2013. Questionnaires consisted of self-care and social support scales. The biological markers were collected by reviewing electronic medical records. Data were analyzed with descriptive statistics, t-test, ANOVA with Scheff$\acute{e}$ post-hoc test, and Pearson's correlation. Results: The self-care score was significantly higher in a patient group within 6 months post-transplant when compared to a patient group post-transplant 3 to 5 years (F=3.10, p=.018). The self-care showed positive correlation with social support with statistical significance (r=.36, p<.001). Conclusion: As the self-care in liver transplant recipients had a positive correlation with social support from family and healthcare providers, the development of comprehensive long-term nursing intervention systems including counseling, education, and support in consideration of progress of time period after transplantation is necessary to enhance self-care behaviors among this population.

Sexual Differences of Cocoon weight, Cocoon Shell Weight, and Cocoon Shell Percentage in the Sex- limited Silkworm Strains, Bombyx mori L., (한성계통의 전견중, 견층중, 견층비율에 대한 성차)

  • Using sexual differences between female and male as the indication of the degree of the physiological distrubance in the sex-limited larval marking strains or sex-limited egg colour strains, it was investigated whether the physiological distrubance caused y the translocated autosome fragment on the W-sex determination chromosome, have an effect on expression of the three quantitative characters; cocoon weight, cocoon shell weight, and cocoon shell percentage in the female silkworm. The mean values of the ratios of female to male the above two experimental groups were 127%, 107%, 85%, in cocoon shell weight, and cocoon shell percentage, respectively. On the other hand, those from the three normal stains, namely the basic stock strains, white egg strains, and hybrids were 129% for cocoon weight, 110% for cocoon shell weight, and 85% for cocoon shell percentage, respectively. From the results, it comes to the conclusion that the translocated autosome fragment on the W-chromosome has no influences on the expression of the quantitative characters of the female silkworm because sexual differences of the sex-limited strains were very similar to those of the three normal strains.

Psychophysiological Responses Evoked by Fear and Disgust Emotion Using Audiovisual Film Clips in Children (공포와 혐오 정서에 대한 아동의 심리생리반응)

  • The study is to examine the psychophysiological responses evoked by negative emotions(fear and disgust) in children. 47 children(11-13 years old, 23 boys) participated in the study. While the children were experiencing fear or disgust emotion induced by audio-visual film clips, ECG, EDA, PPG and SKT are measured. Emotion assessment scale was used to confirm that emotions elicited by the film clips were significantly noticeable, which was measured self-report. The results turned out to be 100% and 89.4% of appropriate for fear and disgust emotions, respectively. Emotional intensity the children had experienced was rated as 4.05, 4.07 on 1-5 scale based on effectiveness of measurement of fear and disgust emotion. ANS reponses by fear and disgust were significantly between the resting state and emotional state induced. The result obtained from the fear emotion showed significant increases in SCL, NSCR, HR, RSA, RESP and HF. There was a significant difference in SCL and NSCR between the two emotions.
