A Study on Health Statue of the Elderly in Rural Community according to Drinking Pattern (일부 농촌지역 노인의 음주양상에 따른 건강상태)

  • Objectives: This study was conducted to investigate the trend of health statue of the elderly living in rural area according to drinking patterns. Methods: This study was conducted with 2,421 elderly people (male 1,273 and female 1,148) residing in the selected 25 villages, with exclusion of a few elderly people who were in hospital, out for a long time or had an unknown address. This study were carried out, face-to-face interviews with the subjects were made from January to March 2002. Results: The investigation of drinking state showed that for male subjects, drinkers accounted for 48.8%, nondrinkers 35.1% and abstainers from drinking 16.1%, whereas for female subjects, drinkers accounted 15.3%, nondrinkers 80.2% and abstainers from drinking 4.5%. The health status was analyzed according to drinking pattern. For elderly men, abstainers from drinking showed worse health state than nondrinkers and drinkers. Elderly women showed the same result. It is widely known that drinking are the important causes of chronic diseases. Therefore, it is needed to provide the elderly with education on control of preventable health risk factors and effect of living state on health, in order to prevent aggravation of health level of the elder population aged 65 and over. This will also help them promote their health. It will be desirable that for the elderly, the objective will focus on health promotion rather than treatment of diseases. Conclusions: Carry out health plan for rural communities and health maintenance programs and health promotion of the elderly in those communities shall be developed. In addition, preventive education and health examination shall be conducted more frequently with the elderly who drink but are still healthy.

The Role of Pulmonary Capillary Pressure in the Oxygen Free Radical-Induced Acute Lung Injury (산소기에 의한 급성 폐손상에서 폐모세혈관압의 역할에 관한 연구)

  • Background: Regardless of its causes, acute lung injury is characterized pathophysiologically by increased pulmonary arterial pressure and the protein-rich edema. Many inflammatory mediators are known to be involved in the pathogenesis of acute lung injury, including oxygen free radicals (OFR). But the changes in pulmonary capillary pressure in the OFR-induced acute lung injury is not clear. While the pulmonary edema characterized by the movement of fluid and solutes is dependent on the pressure gradient and the alveolar-capillary permeability, the role of pulmonary capillary pressure in the development of pulmonary edema is also not well understood. Method: Male Sprague-Dawley rats were divided into 5 groups: normal control (n=5), xanthine/xanthine oxidase (X/XO)-treated group (n=7), catalase-pretreated group (n=5), papaverine-pretreated group (n=7), and indomethacin-pretreated group (n=5). In isolated perfused rat lungs, the sequential changes in pulmonary arterial pressure, pulmonary capillary pressure by double occlusion method, and lung weight as a parameter of pulmonary edema were determined. Results: Pulmonary arterial pressure and pulmonary capillary pressure were increased by X/XO. This increase was significantly attenuated by catalase and papaverine, but indomethacin did not prevent the X/XO-induced increase. Lung weight gain was also observed by X/XO perfusion. It was prevented by catalase. Papaverine did not completely block the increase, but significantly delayed the onset. Indomethacin had no effect on the increase in lung weight. Conclusion: These data suggest that increased pulmonary capillary pressure by OFR may aggravate pulmonary edema in the presence of increased alveolar-capillary permeability and this may not be mediated by cyclooxygenase metabolites.

Conjugal transfer and fate of the genetically engineered $Km^{r}$ gene in freshwater environments (유전자 조작기법으로 변형시킨 $Km^{r}$ 유전자의 담수 환경에서의 전이 및 행방)

  • A kanamycin resistance($Km^r$) gene was studied for its transfer in natural freshwater environments by using the natural bacterial isolate(M1) of DK1 and the DKC601 strain, $Km^r$ plasmid of which was genetically engineered from the NI strain. The transfer frequency ofthe $Km^r$ gene and rearrangement of the $Km^r$ plasmid were compared between the gnetically engineered microorganism(GEM) and the NI parental strain by conjugation with the same recipient strain. The transfer frequency of the $Km^r$ gene was about $9.1\times 10^{-12}-1.8\times 10^{-11}$ in both the GEM and NI strains at 5 to $10^{\circ}C$, but the frequency of the NI was about 10 times higher than that of the GEM at 20 to $30^{\circ}C$. The $Km^r$ plasmid in the transconjugants obtained by conjugation of the NI with the MY1 strain as a ricipient showed alot of rearrangement, but the $Km^r$ plasmid transferred from the GEM was stable without alteration of its size. When the MT2 strain was used as a recipient, however, such a rearrangement of the $Km^r$ plamid was observed in the transconjugants obtained from the GEM as well as the NI strain. In those transconjugants obtained from different mating pairs and water environments, the plasmid were appeared to decrease in their number as the period of conjugation time was prolonged, but only the $Km^r$ plasmid transferred from the GEM kept having its size of 52kb. Therefore, the $Km^r$ gene was transferred at the same rate from the GEM and NI strains in natural freshwater environment, but the gene of the GEM strain was more stable than the NIduring conjugation and the $Km^r$ plasmid was rearranged by changing the recipient strain for conjugation in any water environments.

Studies on Cryo-preservation of Registered Strains of Lentinula edodes (표고 등록균주의 초저온 보존에 관한 연구)

  • New strain needs to maintain desirable characteristics for long term when it was bred, but in lapse of time it degenerates into a bad condition. Therefore the influence of temperature on the viability and survival rates of Lentinula edodes strains were examined after cryopreservation. Also, liquid nitrogen preservation for L. edodes has been proved to be one of the most reliable method. However, a mechanical damage of strain is inevitable during cryopreservation of the fungus because the fungus is very sensitive to stress of cooling rate in the freezing process. So we tried to find out state change of L. edodes with a programmable freezer. L. edodes strains were preserved at $-20^{\circ}C$, $-80^{\circ}C$ and $-196^{\circ}C$ for 50 days. At $-20^{\circ}C$, its mycelial growth became extinct. When thawed, the growth of mycelia which were preserved at $-80^{\circ}C$ was fastest. Attempts were made to investigate viability of L. edodes strains after freezing at $-80^{\circ}C$ and $-196^{\circ}C$, respectively. As the result, more than 90% showed high survival rate of strains tested at $-80^{\circ}C$ and $-196^{\circ}C$. Mycelial growth between apical and basal parts of colony after freezing preservation for 50 days was compared. At apical and basal parts, the survival rates showed 100% at $-80^{\circ}C$, but 98% and 94% at $-196^{\circ}C$, respectively. We confirmed that the ice crystal formation temperatures of L. edodes strains were $-6.0^{\circ}C$ for Sanlim 1, $-5.5^{\circ}C$ for the Sanlim 2, $-4.0^{\circ}C$ for the Sanlim 3 and $-15.5^{\circ}C$ for the Sanzo 302. These results indicated that L. edodes strains showed completely different responses to the ice crystal formation. We knew the fact that even the same species, especially L. edodes, they displayed completely different responses to the same freezing condition. Also, this has nothing to do with the connection between temperature type and freezing point. And a protocol was tried to minimize state change of L. edodes strains using programmable freezer when they are frozen, but it was not effective on them.

Correlation between the Serum Leptin Level and the Expression of Leptin in Stomach Cancer Patients (위암에서 혈청 렙틴 레벨과 조직 내 렙틴 발현과의 상관관계)

  • Purpose: The adipocyte-derived cytokine leptin plays a major role in the control of stable body weight by suppressing food intake and increasing energy metabolism. Leptin regulates the cell proliferation of various epithelial cells and it may be involved in the promotion of cancer. Leptin and its receptor are highly expressed in gastric adenocarcinoma, but the association between the serum leptin level and the tissue expression of leptin is uncertain. We evaluated the serum leptin level and the expressions of leptin and leptin receptor in gastric cancer, and we explore the possible mechanism and role of leptin in the carcinogenesis of gastric cancer. Materials and Methods: 72 carcinomas that were curatively resected at our hospital from October 2005 to March 2007 were included in this study. By immunoassay and immunohistochemical staining, we evaluated the serum leptin level and the expressions of leptin and its receptor, and we analyzed their relationship together with the clinicopathological variables. Results: The serum leptin level was increased as the patient's BMI increased and it was decreased in H. pylori infected patients. The expression of leptin was increased as the TNM stage increased (P=0.014), and the expression of leptin receptor in the intestinal type gastric adenocarcinoma was higher than that in the diffuse type gastric adenocarcinoma (71.4% vs 28.6%, respectively, P=0.033). Conclusion: There was no significant correlation between the serum leptin level and expression of leptin in gastric cancer patients. The expression of leptin was associated with the TNM stage, but its role in the pathogenesis of gastric cancer has to be elucidated.

The Comparison of Protective Effects of Adenosine Included Cardioplegia According to Adenosine Dosage (심정지액 속에 포함된 아데노신의 용량에 따른 심근보호 효과 비교)

  • Background: Adenosine is secreted by myocardial cells during myocardial ischemia or hypoxia. It has many beneficial effects on arrhythmias, myocardial ischemia, and reperfusion ischemia. Although many investigators have demonstrated that cardioplegia that includes adenosine shows protective effects in myocardial ischemia or reperfusion injury, reports of the optimal dose of adenosine in cardioplegic solutions vary. We reported the results of beneficial effects of single dosage(0.75 mg/Kg/min) adenosine by use of self-made Langendorff system. But it is uncertain that dosage was optimal. The objective of this study is to determine the optimal dose of adenosine in cardioplegic solutions. Material and Method: We used a self-made Langendorff system to evaluate the myocardial protective effect. Isolated rat hearts were subjected to 90 minutes of deep hypothermic arrest(15$^{\circ}C$) with modified St. Thomas' Hospital cardioplegia including adenosine. Myocardial adenosine levels were augmented during ischemia by providing exogenous adenosine in the cardioplegia. Three groups of hearts were studied: (1) group 1 (n=10) : adenosine - 0.5 mg/Kg/min, (2) group 2(n=10): adenosine -0.75 mg/Kg/min, (3) group 3 (n=10) : adenosine -1 mg/Kg/min. Result: Group 3 resulted in a significantly rapid arrest time of the heart beat(p<0.05) but significantly slow recovery time of the heart beat after reperfusion(p<0.05) compared to groups 1 and 2. Group 2 showed a better percentage of recovery(p<0.05) in systolic aortic pressure, aortic overflow volume, coronary flow volume, and cardiac output compared to groups 1 and 3. Group 1 showed a a better percentage of recovery(p<0.05) in the heart rate compared to the others. In biochemical study of drained reperfusates, CPK and lactic acid levels did not show significant differences in all of the groups. Conclusion: We concluded that group 2 [adenosine(0.75 mg/Kg/min) added to cardioplegia] has better recovery effects after reperfusion in myocardial ischemia and is the most appropriate dosage compared to group 1 and 3.

Anther Culture Efficiency affected by Growth Condition and Pre-treatment Methods in Barley (보리 생육환경 및 전처리 방법별 약배양 효율)

  • This experiment was carried out to improve the anther culture efficiency of barley (Hordeum vulgare L.). Callus induction rates from anther cultures of the five domestic naked barley and four unhulled varieties ranged from 0 to 5.6%, and plant regeneration rate to callus was 30.4% in the donor plants grown in a greenhouse during winter, among which the green plant regeneration rates ranged from 0 to 4.4%. Plant regeneration rate was 30.4% in the donor plants grown in a greenhouse during winter, whereas 21.3% in the normal field condition in spring. In addition, callus induction rates were 19.2% in plants grown in a normal field and 7.2% in drought-stressed condition, respectively. Being Considered the anther culture efficiency affected by the sampling time, the optimum sampling stage of anthers was 3~4 days before heading when the length between the 1st and 2nd auricles reaches 5 to 10 cm and at the uninucleate of pollen which the tip of the 2nd auricle aligns with the middle of panicle in the leaf sheath. Best callus induction rates came from the anthers stored at $4^{\circ}C$ for 3 weeks in a 10 to 15 cm diameter polyethylene bag with 5 to 10 panicles and Duwonchapssalbori and Saessalbori showed the higher induction rate of 4.8% and 1.7%, respectively.

Comparative Analysis of Fracture Angulation between Parallel Pinning and Plate Fixation Techniques in the Management of 5th Metacarpal Fractures (제 5 수지 중수골 골절에서 평행 핀 또는 플레이트 고정술 이후 골절각 변화에 대한 비교 연구)

  • Purpose: Metacarpal fractures are common injuries of the hand. They are treated using closed reduction (CR) or open reduction (OR) techniques. The management strategy depends on fracture site characteristic and fixation methods. In this study, we evaluated pre- and postoperative fracture angulation, when metacarpal fractures bad been treated using two different techniques: CR with parallel transverse pinning and OR with plate fixation. Methods: Forty-six patients undergoing anatomic reduction to treat extra-articular metacarpal fractures were recruited. They were included in one of two therapeutic groups: Group 1, CR with parallel transverse pinning (n=21); Group 2, OR with plate fixation (n=25). Fracture angulation values have been measured on pre- and postoperative radiologic images. Values were compared between pre- and postoperative states, and between corresponding measurements of each group. Results: All extra-articular metacarpal fractures were successfully treated without wound related complications or the limit of joint motion. Both groups demonstrated adequate reduction at immediate postoperative period (postoperative angulation of group 1, $20^{\circ}{\pm}7^{\circ}$; group 2, $19^{\circ}{\pm}5^{\circ}$). During the observation at follow-up period, Group 1 exhibited slight recurrence (follow-up angulation of group 1, $24^{\circ}{\pm}10^{\circ}$). Nonetheless, Group 2 showed adequate reduction state in both immediate postoperative and long-term follow-up periods (follow-up angulation of group 2, $18^{\circ}{\pm}6^{\circ}$). Conclusion: Extra-articular metacarpal fractures were successfully restored without functional complications. CR with parallel transverse pinning method exhibited recurrence after pin removal, which necessitates cautious postoperative exercise and monitoring.

Field Assessment of in Situ Remediation of NO3--contaminated Ground Water Using Zero-valent Iron/Bio Composite Media (영가철/바이오 복합처리제를 이용한 질산성 질소 오염 지하수의 현장 지중정화 적용성 평가)

  • In this study, the assessment of field applicability of in-situ remediation of nitrate-contaminated groundwater located in Yesan-gun was performed. Zero-valent iron/bio composite media injected PRB (Permeable Reactive Barrier) and monitoring well were installed in the contaminated groundwater site and monitored main remediation indicators during the PRB operation. Nitrate, nitrite, ammonia, Fe ion, TOC, and turbidity were analyzed and the diversity and population of microorganism in the PRB installed site were investigated for the verification of effect of injected PRB. In the study site where is an agricultural area, a river flows from west to east that forms a river boundary and the southern area has an impermeable sector. It was found that nitrate flows into the river, which is similar as groundwater flow. Simulation result for the fate of nitrate in groundwater showed steady state of nitrate arrived after 3~5 years passed. However, it is just to consider current conditions with no additional input of contaminant source, if additional input of contaminant source occurs contamination dispersion and time for steady state are expected to be increased. The monitoring results showed that Fe ion, TOC and turbidity in groundwater were not clearly changed in concentration after PRB installation, which indicates adaptability of the injected PRB for remediation of groundwater with no additional harmful effect to water quality. The concentration of nitrate maintained less than 5mg/L until 42 days after PRB installation and recovered its initial concentration after 84 days passed and showed termination of reactivity of injected zero-valent iron/bio composite media for removal nitrate. Nitrite and ammonia ions found after installation of PRB indicates reductive removal of nitrate. And the outstanding increase of microorganism diversity and population of Betaproteobacteria Class which includes denitrification microorganism explains biologically reductive removal of nitrate in injected PRB.

Study on health anxiety issues, health-promoting behavior, and quality of life of middle-aged women in Jeonbuk area (전북지역 중년여성의 건강염려, 건강증진행동 및 삶의 질에 대한 연구)

  • Purpose: The purpose of the study was to identify the health anxiety issues of middle-aged women, their health-promoting behavior, and quality of life as well as to examine the relationship between these variables. Methods: The participants were 334 women in Jeonbuk area. Demographic characteristics, the status of health anxiety, health-promoting behavior, and life quality was assessed using a self-administered questionnaire. The data were analyzed using a t-test, analysis of variance, Duncan test, and hierarchical regression analysis with SPSS ver. 24.0. Results: The score for health anxiety was 37.64 points out of a possible score of 60, and the score for health-promoting behavior was 79.18 points out of a possible score of 115. The score for the quality of life was 101.18 points out of a possible score of 150. The health anxiety scores showed significant differences, varying as per body mass index (BMI) (p < 0.05), income (p < 0.05), occupation (p < 0.05), disease (p < 0.05), satisfaction with weight (p < 0.05), and interest in weight control (p < 0.05). The health-promoting behavior showed significant differences according to age (p < 0.01), BMI (p < 0.01), income (p < 0.05), menses (p < 0.05), intake of dietary supplements (p < 0.05), perception of body image (p < 0.05), and satisfaction with weight (p < 0.05). The quality of life showed significant differences according to BMI (p < 0.05), income (p < 0.01), education level (p < 0.05), occupation (p < 0.05), disease (p < 0.05), and satisfaction with weight (p < 0.05). Regression analysis showed that health-promoting behavior was the most influential variable on the quality of life, followed by disease and health anxiety. Conclusion: Based on these results, we conclude that it is necessary to consider educational programs on improving the quality of life of middle-aged women according to the health anxiety levels and health-promoting behavior.
