Evaluation of the Liver Cancer Diagnosis Function of PET-MRI Based on Decision Matrix Analysis (판정행렬분석을 통한 PET-MRI의 간암 진단성능 평가)

  • To evaluate the capability of integrated PET-MRI, which has recently been utilized in the clinical practices, on the diagnosis of liver cancer, its utility was assessed by $2{\times}2$ decision matrix. The numbers of abnormal and normal decisions on the liver cancer were 98 and 51 cases, respectively, upon PET-MRI scan results of the subjects, and the numbers of positive and negative decisions were 103 and 62, respectively, upon cytopathologic results. Out of the two tests, 95 cases were shown as true-positive and 3 were false positive, while 62 were true negative and 5 were false negative. Upon the results of PET-MRI test, its sensitivity, specificity, false negative rate, and false positive rate were 95.00%, 95.38%, 0.05%, and 95.15%, respectively. Therefore, it is considered to have the high potential to use the determination of the stage before the surgery, detections of recurrence and remote metastasis, assessment of uncertain remote lymph node metastasis, and so on in the diagnosis of the liver cancer, and also for the clinical utility of PET-MRI to be sufficient by integrated diagnosis and follow up scan with pathological studies.

Clinical Significance of Tumor Markers in Gastric Cancer Patients after Curative Resection (근치적 절제술을 시행한 위암 환자에서 종양 표지자의 임상적 의미)

  • Purpose: This clinical study was conducted to evaluate the predictive value of tumor markers for recurrence and the clinical significance of false positive findings after curative gastrectomy in patients with gastric cancer. Materials and Methods: Two hundred ninety patients with gastric cancer who underwent gastrectomy with curative intent were evaluated retrospectively. We analyzed the correlations between changes in tumor markers (CEA, CA 19-9, AFP, and CA-125) and clinicopathologic data, and basis for changes in tumor markers without recurrence during the follow-up period. Results: The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of tumor markers for recurrence were 75.0%, 64.6%, 23.1%, 94.8%, and 65.9% respectively. Among 36 patients with recurrences, 10 patients (27.8%) had elevated tumor markers prior to positive findings on imaging studies, while 13 patients (36.1%) had concomitant elevation in tumor markers. At least 1 of the 4 tumor markers increased in 90 of 290 patients during the follow-up period; however, there was no evidence of tumor recurrence. Twenty patients had persistently elevated tumor markers, while the tumor marker levels in 70 patients returned to normal level within $9.08\pm7.2$ months. The patients with pulmonary disease, hepatobiliary disease, diabetes, hypertension, or herbal medication users had elevated tumor markers more frequently than patients without disease (P<0.001). Conclusion: Although detecting recurrence of gastric cancer with tumor markers may be useful, false positive findings of tumor markers are common, so surgeons should consider other chronic benign diseases and medical conditions when tumor markers increase without evidence of recurrence.

  • In this paper, we used the electrical resistivity survey as a simple and reasonable technique to evaluate the effectiveness of grouting. And, a mechanism was also analyzed to understand how the electrical resistivity is affected by the loss of fine material in the core of a dam. From this mechanism, it was confirmed that electrical resistivity value of the damage section may be up or down depending on the survey conditions. This result seems to be contrary to the previous study that electrical resistivity of the damage zone become alway low. To clarify what was investigated, the electrical resistivity before and after grouting was compared and analyzed. As the result, it was concluded that grouting effectiveness was successfully assessed by electrical resistivity survey.

Clinical & Radiologic Result of Arthroscopic Bankart Repair Using Knotless Suture Anchor (비매듭 금속 봉합 나사못을 이용한 관절경적 방카트 복원술: 임상적 및 방사선학적 결과의 비교)

  • Purpose: To study the clinical and radiologic results with arthroscopic Bankart repair using knotless metal suture anchor. Materials and Methods: From February 2001 to January 2005, 68 patients, who underwent arthroscopic Bankart repair using knotless suture anchor and were followed up more than 12 months, were evaluated. A mean follow-up period was 34 months. All shoulders were evaluated by Rowe scoring system, range of motion of the shoulder, pain degree of VAS, and This was compared by radiologic findings after surgery. Results: The Rowe scoring system was 43.30 preoperatively, which improved to 95.55 postoperatively. At last follow-up, there was no significance difference between operated shoulder and non-operated shoulder in range of motion. The degree of VAS was measured from 3.3 preoperatively to 0.5 postoperatively. The radiolucent line was shown around suture anchor in 15 shoulders. 2 shoulders of 15 shoulders were reoperated due to redislocation and anchor arthropathy. In Odds ratio, this group (15 patients) had more 2.6 times the subjective instability than other group (53 patients). Conclusion: Arthroscopic Bankart repair using knotless anchor suture is very effective operative technique. But we have to be careful because the radiolucent line around anchor showed up during a follow -up period may indicate poor prognosis.

Clinical Significance of Coagulation Screening Tests and Platelet Counts in Children Undergoing Endoscopy (소아에서 내시경 검사의 출혈 위험 예측 인자로서 혈액 응고 검사 및 혈소판 검사의 의의)

  • Purpose: The aim of this study was to assess the benefits of routine pre-endoscopy coagulation screening tests and platelet counts in Korean children. Methods: Between March 2004 and December 2009, children who underwent gastrointestinal endoscopy for the evaluation of various gastrointestinal symptoms were included. All of the subjects included in the study also underwent routine coagulation screening and platelet count determinations prior to endoscopy and biopsy. The clinical records and laboratory tests were retrospectively reviewed in all patients. Results: One hundred sixty-two of 1,476 (11%) patients who underwent endoscopy had abnormal results on pre-screening coagulation tests. Fourteen patients underwent coagulation factor assays due to abnormal clotting results in consecutive tests or due to clinical evidence of a bleeding tendency. Seven patients were diagnosed with factor XII deficiency, one patient was diagnosed with von Willebrand disease, one patient had von Willebrand disease and factor XII deficiency, and one patient was presumed to have mild hemophilia. The remaining 4 patients had normal results with the factor assays. The results of platelet counts were normal with the exception of 1 patient. No patient had significant bleeding during the endoscopic procedures, despite abnormal pre-endoscopic coagulation tests. Conclusion: Routine coagulation screening tests and platelet counts revealed abnormal results in some patients. Most of the patients with abnormal clotting were shown to have a factor XII deficiency, which had no significant associated bleeding tendencies; the other patients were diagnosed with hemophilia or von Willebrand disease. Therefore, although abnormal pre-endoscopic coagulation is not always related to significant bleeding complications, pre-endoscopic coagulation screening may be useful in some children in predicting the risk of bleeding tendency during endoscopic procedures.

  • In this paper we propose the automated generation algorithm of penetration scenario using association mining technique. Until now known intrusion detections are classified into anomaly detection and misuse detection. The former uses statistical method, features selection, neural network method in order to decide intrusion, the latter uses conditional probability, expert system, state transition analysis, pattern matching for deciding intrusion. In proposed many intrusion detection algorithms unknown penetrations are created and updated by security experts. Our algorithm automatically generates penetration scenarios applying association mining technique to state transition technique. Association mining technique discovers efficient and useful unknown information in existing data. In this paper the algorithm we propose can automatically generate penetration scenarios to have been produced by security experts and is easy to cope with intrusions when it is compared to existing intrusion algorithms. Also It has advantage that maintenance cost is not high.

Wind Loads of 5 MW Horizontal-Axis Wind Turbine Rotor in Parked Condition (운전정지 조건에서 5 MW 수평축 풍력터빈 로터의 풍하중 해석)

  • In this study, wind loads exerted on the offshore wind turbine rotor in parked condition were predicted with variations of wind speeds, yaw angles, azimuth angle, pitch angles, and power of the atmospheric boundary layer profile. The calculated wind loads using blade element theorem were compared with those of estimated aerodynamic loads for the simplified blade shape. Wind loads for an NREL's 5 MW scaled offshore wind turbine rotor were also compared with those of NREL's FAST results for more verification. All of the 6-component wind loads including forces and moments along the three axis were represented on a non-rotating coordinate system fixed at the apex of rotor hub. The calculated wind loads are applicable for the dynamic analysis of the wind turbine system, or obtaining the over-turning moment at the foundation of support structure for wind turbine system.

Effects of Rotor-Stator Blade Count Ratio on the Unsteady Aerodynamic Characteristics of a Cascade (동익과 정익의 블레이드 개수 비가 익렬의 비정상 공기역학적 특성에 미치는 영향에 대한 수치해석적 연구)

  • Effects of rotor-stator blade count ratio on the unsteady aerodynamic characteristics of a cascade was studied by using a Navier-Stokes code. Present Navier-Stokes code is a parallel code and works on a multi-cpu machine. It is based on the SIMPLE algorithm and uses QUICK scheme for convection terms and second order back difference for all temporal derivatives. Computations were carried out for two cases : case 1 is for 3 stator cascade passages subjected to two upstream wakes while case 2 is for 2 stator cascade passages subjected to three upstream wakes. Numerical solutions show that rotor-stator blade count ratio plays a significant role in the unsteady aerodynamic characteristics of the stator cascade. Case 2 shows smaller unsteady fluctuation than case 1, even if they show the same time averaged value. The smaller fluctuation of case 2 is believed due to strong interaction between unsteady vortices. The unsteady lift variation of case 2 is shown to have many high frequency fluctuations as more unsteady vortices travel around the cascade. The unsteady turbulent kinetic energy due to the upstream wake is also shown to decay faster through the cascade passage than in the free stream.

The Behavior of Silt due to Volume Deformation Tendency (체적변형 경향에 따른 실트의 거동)

  • The behavior of pure silt was investigated by using an automated triaxial testing device. The stress-strain behavior of silt due to the volume deformation tendency was compared with the behavior of clay prior to failure and behavior at failure under monotonic undrained compression and extension conditions. A pure silica flour was chosen to form samples. The isotropically normally-consolidated samples with 450 kPa of effective mean confining pressure and overconsolidated samples through unloading were tested. Based on the experimental results, it was qualitatively identified that the undrained strength of normally-consolidated silt increases due to its dilatant nature which is not seen in clay. Also the overconsolidated silt shows a significantly different behavior under the monotonic loadings due to the volume deformation tendency.

  • The drained and undrained behavior of pure silt was investigated experimentally. Special attention was given to the stress-strain behavior of silt prior to failure and behavior at failure under monotonic and cyclic loading. A pure silica flour was chosen to form samples with two different densities of D,=80%, eo=0.68 and D,=35%, eo=0.9. The isotropically consolidated samples were tested in the triaxial testing device under monotonic undrained, drained compression and extension conditions. Also samples were tested under cyclic undrained condition. Based on the experimental results. it was qualitively identified that the overall behavior of silt is similar to that of sand. When compared with clay, silt shows a significantly different behavior due to its dilatant nature under both the monotonic and cyclic shear loadings.
