  • 본 연구는 임상에서 사용되어지는 두 가지의 불소 바니쉬를 비교연구 하였다. 또한 각각 치아표면에 10mg의 불소 바니쉬를 도포하였다. 각 대상자들의 타액의 불소이온유리농도는 1,3,12,24시간으로 채취하여 분석하였다. 각각의 불소 바니쉬를 도포한 타액 내 불소이온의 유리 누적량은 차이를 보였다. 본 연구는 현재 사용되어지는 불소 바니쉬를 선택함에 있어 기초자료로 사용될 수 있을 것이라 사료되어진다.

ANTICARIOGENIC EFFECT OF FLUORIDE VARNISHES AND CHLORHEXIDINE VARNISHES (불소 바니쉬와 클로르헥시딘 바니쉬의 항우식 효과)

  • Dental caries which is one of the most common chronic disease complexly developed by the action of oral bacteria, diet, and host factor. Various prevention program enhance resistance of demineralization and reduce the acidogenecity of oral bacteria have been introduced, representative material is fluoride and chlorhexidine. The purpose of the study was to evaluate and compare effectiveness of fluoride varnish and chlorhexidine varnish in vivo. Bovine tooth specimens were implanted in the lower space maintainers and applied with fluoride varnish and chlorhexidine varnish. After seven days in oral environment, metal mesh was covered to make similar condition of plaque accumulation and induce caries. All specimens were analysed by EPMA to evaluate quantitative change of Ca, P and by polarized microscope to identify histological changes. The results were as follows : After initial artificial caries induction in the mouth, there were remarkable enamel caries lesion in the control group under polarized light microscopy. The highest amount of mineral decrease were showed in control group. No statistically significant mineral decrease were showed in fluoride varnish group, while chlorhexidine varnish group showed only significant decrease of P(P<0.05). In conclusion both fluoride varnish and chlorhexidine varnish seemed to be effective for protecting enamel surface from caries activity, although fluoride varnish has more anticariogenic effect than chlorhexidine varnish.

A Convergence Study on the Remineralizing Effect of Domestically Sold Fluoride Varnish Using QLF-D (QLF-D를 활용한 국내 시판 불소 바니쉬의 재광화 효과에 대한 융합적 연구)

  • This study is to analyze seven fluoride varnishes remineralizing effects on early carious lesions in the clinical field. While CS, MI, CW, VV, EP, FP and FN were used for permanent teeth, CS, MI, CW and FP were used for primary teeth. The specimens were demineralized for 4 days after forming a $3{\times}3mm^2$ window with nail varnish, applied with varnishes separately, then were stirred in artificial saliva for 1 week. QLF-D analysis of carious lesion size showed that permanent teeth's ${\Delta}F$ value decreased after applying CS(4,64), MI(4.86), CW(4.50), and EP(5.50), while for primary teeth, FP(3.00) and MI(3.00) displayed a remineralizing effect. Thus, MI can be used effectively for preventing caries for both adults and children in the clinical field.

THE ANALYSIS OF REMINERALIZATION EFFECT IN FLUORIDE VARNISH USING CONFOCAL LASER SCANNING MICROSCOPE (공초점 레이저 주사 현미경을 이용한 불소 바니쉬 재광화 효과의 분석)

  • It is well established that fluoride products play an important role in the prevention and remineralization of carious lesion. Fluoride varnish is a concentrated topical fluoride and varnishes adhere to tooth surface, permitting prolonged fluoride exposure and uptake. In this study, the artificial initial enamel caries was caused on the sound human enamel and divided 60 specimens into three groups. Group 1 and group 2 were treated with the topical application of fluoride varnish and stored in artificial saliva for 1 and 2 weeks. Group 3 was stored in artificial saliva for 2 weeks, which acted as control group. Changes in mineral contents were analysed with the confocal laser scanning microscope. The following results were obtained: 1. In group 1 and group 2, the total fluorescence of the lesion(TFL) was reduced in remineralized area compared to in demineralized area(p<0.05). 2. The total fluorescence of the lesion of remineralized area was more reduced in group 2 than in group 1(p<0.05). 3. The total fluorescence of the lesion was more reduced in group 2 than in control group(p<0.05). 4. Confocal laser scanning microscope can be used in quantitative analysis of remineralization by fluoride varnish.

COMPARATIVE STUDY OF FLUORIDE RELEASE AND RE-UPTAKE OF SEVERAL RESTORATIVE MATERIALS (수종 수복재에 불소 도포제 적용 후 불소유리에 관한 비교연구)

  • In the child, it is very important that he/she will have the ability to suppress aesthetic restorative materials of secondary caries. With the representative preventive material against caries, the importance of fluoride is more emphasized. This study examined the differences in fluoride release and re-uptake among some restorative materials, following a treatment of APF gel and fluoride varnish. The surface roughness was observed under scanning electron microscope. Studying this will provide for the research to find effective restorative materials and fluoride type in tooth caries prevention. It is applied from presence at a clinic that restorative materials are resin, flowable resin, compomer and glass ionomer. Fluoride release was measured at 24-hour intervals for 7 days, 3-day intervals from 8th to 38th day using an ion-selective electrode and analyzer. Then, the materials were treated with the fluoride gel and fluoride varnish respectively, fluoride release was measured and specimens were evaluated under scanning electron microscope for 4 weeks. It was concluded that 1. Fluoride was released for 38 days from restorative materials under 1 ppm in case of flowable resin, 1-2 ppm in compomer and 2-8 ppm in glass ionomer, a few of fluoride was released after 45 days 2. Fluoride has more releasing after application of APF gel than fluoride varnish. Fluoride re-uptake was observed under 0.6-0.2 ppm in fluoride varnish and 0.6-2.6 ppm in APF gel after starting the procedure one day(p<0.05). For the remaining 4 weeks, they demonstrated a similar release. 3. Specimens were evaluated under scanning electron microscope. Applied fluoride in the experimental group surface was rougher than the control group that did not receive fluoride application. Fluoride varnish group had a smoother surface than both the APF gel group and the varnish APF gel group that received a fluoride application.

Elemental analysis of the fluoride varnish effects on root caries initiation (불소 바니쉬 도포 후 초기 치근 우식 발현에 대한 정량원소분석)

  • Objectives: The usage of fluoride varnish for a moderate to low caries-risk group has not been well validated. This study aimed to evaluate the preventive and therapeutic efficacies of fluoride varnish on the initiated root caries. Materials and Methods: Ten premolars were sectioned into quarters, further divided into two windows, one of which was painted with Fluor Protector (1,000 ppm fluoride, Ivoclar Vivadent). An initial lesion with a well-preserved surface layer was produced by pH cycling. Scanned line analysis using energy dispersive spectrometry determined the weight percentages of Ca and P in the demineralized layer. Scanning Electron microscopy and confocal laser scanning microscopy (CLSM) evaluated the varnish-applied root surfaces. Results: The mean lesion depth (SD) was 12.3 (2.6) ${\mu}m$ (single cycling) and 19.6 (3.8) ${\mu}m$ (double cycling). Double cycling extended the lesion depth, but induced no more mineral loss than single cycling (p < 0.05). The mean weight percentages of Ca and P between groups with and without varnish were not significantly different (p < 0.05). A CLSM showed varnish remained within 15 ${\mu}m$ of the surface layer. Conclusions: When a mild acid challenge initiated root tissue demineralization, the application of low-concentration fluoride varnish did not influence the lesion depth or the mineral composition of the subsurface lesion.

PREVENTIVE EFFECT OF FLUORIDE-CONTAINING ADHESIVE FILM MADE BY NANO (나노기술을 이용해 제작한 불소함유 접착필름의 치아우식증 예방효과)

  • The purpose of this in vitro study was to evaluate the effect of fluoride containing adhesive film on prevention of enamel demineralization. Eighty bovine enamel blocks were divided randomly into 4 groups of 25: (1) APF gel applied ; (2) CavityShield$^{TM}$ applied; (3) 3% fluoride film applied; (4) 5% fluoride film applied; Early caries lesions were produced by placing each specimen into demineralization solution at pH 4.0 for 72 hours. Then lesion of the surface microhardness were measured by the Vicke's hardness test and the lesions depth were measured by polarizing light microscope. The results of the present study are as follows: 1. Difference of microhardness value ($M{\pm}SD$) between control and experimental side was the highest in group II, followed by group IV, III, I but, no significant difference was between group II, III and IV. 2. Difference of mean lesion depth ($M{\pm}SD$) between control and experimental side was the highest in group II, followed by group III, IV and I but no significant difference was between group III and IV. The results of the present study indicate that the fluoride film application is more effective than APF gel and is similar to fluoride varnish application for prevention of dental caries.

IN VITRO EVALUATION OF EXPERIMENTAL FLUORIDE TAPE IN INHIBITION OF ENAMEL DEMINERALIZATION (불소 테이프의 법랑질 탈회 억제 효과에 관한 실험적 평가)

  • The aim of this in vitro study was to evaluate the effectiveness of experimental 2.26% fluoride-polyvinyl alcohol (F-PVA) tape in inhibition of enamel demineralization using enamel surface microhardness (SMH) analysis and scanning electron microscopy (SEM) examination. Enamel specimens (n=60) randomly assigned to four groups: control group, F-PVA tape group, fluoride varinish (F-varnish) group, casein phosphopeptide-amorphous calcium phosphate (CPP-ACFP) group. After topical application, pH-cycling was processed. Then, SMH was measured and the percentage loss of surface microhardness (%SML) was calculated. For the SEM examination, five sample specimens in each group were treated and the morphologic character was evaluated. After pH-cycling, the SMH values of the enamel specimens of F-PVA tape and F-varnish group were significantly higher than that of CPP-ACFP group, there was no significant difference between F-PVA tape and Fvarnish group. With SEM examination, enamel surfaces in the F-PVA tape group and F-varnish group showed numerous spherical and ovoid crystals formed on the enamel surface were also observed. The density of crystals was higher than that of both control group and CPP-ACFP group. F-PVA tape is effective in inhibition of enamel demineralization. Also, F-PVA tape's inhibition of enamel demineralization is comparable to that of F-vanish and greater than that of CPP-ACFP.

THE THERAPEUTIC EFFECT OF FLUORIDE-CONTAINING ADHESIVE TAPE ON DENTIN HYPERSENSITIVITY (불소함유 접착 테이프의 상아질 지각과민증 치료효과)

  • In this clinical research, the fluoride tape(SCMC-T-5) using fluoride(NaF) was developed and manufactured and the treatment effect of the fluoride tape in dentin hypersensitivity patients was evaluated and compared with the effect of existing fluoride varnish($CavityShield^{TM}$). Twenty two healthy adult patients(88 teeth) having dentin hypersensitivity participated in this clinical research and they were divided into two groups. The fluoride product was applied according to the manufacturer's instruction and the level of pain in the tooth after giving irritation using compressed air and ice stick was measured just after the application, after 3 days, after a week and after 4 weeks each using visual analog scale(VAS). In the experimental group, compared with the early VAS scores, all other VAS scores showed the significant decreases statistically. In the control group, all VAS scores except the VAS score of 34.091(air) measured 3 days after(using the irritation examination by the compressed air) showed the significant decreases statistically when compared with the early VAS scores. The fluoride tape and fluoride varnish used in this clinical research were able to treat the dentin hypersensitivity effectively.

EFFECT OF A FLUORIDE VARNISH ON THE ENAMEL DEMINERALIZATION (불소바니쉬가 법랑질 탈회에 미치는 영향)

  • The aim of this study was to evaluate the effect of fluoride varnish application on enamel decalcification. Eighty bovine enamel blocks divided randomly into 4 groups. Group I is the control group. Group II was treated with the APF gel and washed after 4 minutes. Group III and IV was treated with Fluor $Protector^{(R)}$ and $CavityShield^{TM}$ and washed after 1 minutes. Decalcification were created by placing all specimen into artificial acidic solution(pH 4.0). Then the optical density of the lesions were measured by visible light fluorescence and the lesion depths were measured. The results were : 1. The optical density of group II was higher than group I but lower than group III, IV(p<0.05) and there was no difference between group III, IV(p>0.05) at 48 hours. 2. The optical density of group IV was highest at 72 hours(p<0.05). 3. Mean lesion depths were $205.36{\pm}42.85{\mu}m$ and $210.81{\pm}44.60{\mu}m$ in group I, II but no significant difference between two groups(p>0.05). 4. Mean lesion depths were $80.03{\pm}21.66{\mu}m$ and $77.46{\pm}27.72{\mu}m$ in group III, IV but no significant difference between two groups(p>0.05). Fluoride varnish treatment resulted in a significant reduction in lesion depth compared with APF gel. Fluor $Protector^{(R)}$ and $CavityShield^{TM}$ provided the similar effect.
