  • 폐결핵의 진단은 주로 환자의 증상, 흉부 X-선, 객담의 균도말 및 배양검사에 의존하며 이는 과거와 크게 차이가 없으나 최근에는 좀더 효율적인 객담배출을 위한 방법, 분자생물학적 기법을 동원한 결핵균의 진단 및 결핵균항원 또는 항체를 혈액 등에서 측정하는 방법 등이 개발되어 결핵의 진단 율을 높이는데 기여하고 있다.

  • 바이오칩의 대표 주자인 DNA 칩은 점차 분자생물학의 주요 도구로 인식되고 있다. 쓰임새 또한 다양해져 기초 생물학, 기능 유전체학 연구뿐만 아니라 임상 현장에서의 적용을 위한 연구가 활발히 진행되고 있다. 임상분야에서 최근 주목 받고 있는 분야가 DNA 칩을. 이용한 질병진단 및 예후 측정이다. 개별 환자 세포의 분자유전학적 상태는 DNA 칩의 유전체 프로파일링(genome-wide profiling)으로 상세히 파악될 수 있으므로, DNA 칩은 질병의 세부아형 진단, 약물에 대한 개인 민감도 측정, 정확한 예후 측정을 통한 환자의 세심한 관리 등 미래 의료의 핵심이라 할 수 있는 개인별 맞춤 치료(personalized medicare)를 가능하게 하는데 지대한 역할을 할 것으로 기대되고 있다. 특히 수많은 질병 중에서 현대인의 난치병으로 손꼽히는 암은 DNA 칩 분석의 주요 적용 대상이다. 암에 연관된 복잡한 메커니즘을 기존의 단일 표지자로 진단하는 데는 한계가 있기 때문에, DNA 칩을 이용해 질병의 특정 phenotype과 관련 있는 암의 특이 패턴을 전사체 수준에서 분석하여 새로운 형태의 분자유전학적 표지자(transcriptional molecular signature)를 발굴하는 것이다 본 발표에서는 이러한 연구에 쓰이는 DNA 칩 분석 방법들과 실제 위암 데이터에 적용한 사례에 대해 논의하고자 한다. 연세의대 암전이 연구센터의 17K cDNA 칩을 이용하였으며, 진단 및 예후 측정을 위한 여러 분석 방법을 수행하였다.

Preimplantation Genetic Diagnosis for Single Gene Disorders (단일 유전자 질환에 대한 착상전 유전진단)

  • Preimplantation genetic diagnosis (PGD) has become an assisted reproductive technique for couples who are at risk that enables them to have unaffected baby without facing the risk of pregnancy termination after invasive prenatal diagnosis. The molecular biology and technology for single-cell genetics has reached an extremely high level of accuracy, and has enabled the possibility of performing multiple diagnoses on one cell using whole genome amplification. These technological advances have contributed to the avoidance of misdiagnosis in PGD for single gene disorders. Polymerase chain reaction (PCR)-based PGD will lead to a significant increase in the number of disorders diagnosed and will find more widespread use, benefiting many more couples who are at risk of transmitting an inherited disease to their baby. In this review, we will focus on the molecular biological techniques that are currently in use in the most advanced centers for PGD for single gene disorders, including biopsy procedure, multiplex PCR and post-PCR diagnostic methods, and multiple displacement amplification (MDA) and the problems in the single cell genetic analysis.

  • There has been an enormous progress in understanding of how genes contribute to both normal and abnormal development. Also many laboratory works are exploring the intricacies of how to develop in the human central nervous system. Understanding the mechanisms of cortical development gives essential insight into the pathogenesis of many genetic and acqured developmental psychiatric disorders, including autism, schizophrenia, and teaming disorder. Genes have been implicated in an ever-increasing number of disorders. Advance in genetics have begun to clarify the molecular basis of not only single-gene disorders, but also more complex phenotypes.

Molecular Detection of Harmful Dinoflagellates (Dinophyceae) in Ballast Water (선박평형 수 내 유해 와편모조류(Dinophyceae)의 분자생물학적 검출)

  • Ballast water has been known as a major vector for global dispersal of toxic dinoflagellates and other microalgae. In this study, biodiversity in ships’ ballast water was examined using a dinoflagellate-oriented PCR primer set and species-specific real-time PCR. While motile dinoflagellates could be observe at very low cell densities by light microscopy,a wide range of dinoflagellate taxa including parasitic and phototrophic pico-dinoflagellates as well as harmful species to marine fish/shellfish was detected when techniques for cloning/sequencing of SSU rDNA of sample cells were used. Present result suggests that molecular methods including species-specific PCR primers may offer rapid and accurate detection of invasive species in ballast water.

  • From the concepts of cellular pathology and of exfoliative cytology, as elucidated by Virchow and Papanicolaou respectively in the late 19th and early 20th century, have evolved the primary methods for the diagnosis of cancer today. From Papanicolaou's concept of exfoliative cytology developed fine needle aspiration biopsy in the early 1960's, this has become a major diagnostic procedure and has contributed to a significant reduction in open biopsies and, therefore, to medical cost-effectiveness immunobiochemical techniques provided us with a supplement to cancer diagnosis in the 1980's. The immunoperoxidase method, using monoclonal antibodies, is applied primarily as an ancillary measure to elucidate the nature of cancers The availability of specific monoclonal antibodies has greatly facilitated the identification of cell products or surface markers. For example, antibodies directed against intermediate filaments have proved to be of value in determining the histogenesis oi poorly differentiated neoplasms. Tumor markers may serve as biochemical indicators of the presence of a neoplasm. They can be detected In plasma and other body fluids. Their concentration can be applied as a diagnostic test, for monitoring the clinical course of known cancer, and as a screening measure to detect certain cancers in a population at risk. Flow cytometry is a useful tool for distinguishing several cell characteristics, such as the immunophenotype of leukemia-lymphoma cells, the DNA content of neoplastic cells, and cell proliferation rate. Molecular biologic techniques provided a giant step for the management of cancer patients encompassing diagnosis, prognostic evaluation, and therapy. Nucleic acid hybridization techniques are utilized as Southern, Northern, and dot blots and in situ hybridization. Molecular biology and its techniques may bring a blight new horizon for understanding cancer biology and in designing therapy on the basis of gene manipulation.

A Comprehensive Review of Congenital Disorders of Glycosylation (선천성 당화 장애에 대한 전반적 고찰)

  • Congenital Disorders of Glycosylation (CDG) represent a complex group of inherited metabolic disorders resulting from defects in multiple pathways of glycosylation, a critical biochemical process for protein functionality and cellular communication. This review provides a comprehensive overview of CDG, including its history, epidemiology, classification, diagnostic complexities, and therapeutic developments. Despite advancements in understanding CDG and identifying over 160 subtypes, challenges remain due to the diverse clinical manifestations and multi-systemic involvement. Targeted therapy is available for only a few CDGs, but promising treatments are being investigated. Ongoing research is vital to developing targeted treatments and improving patient outcomes.

  • The diagnosis of tuberculosis is usually established using staining and culturing techniques. Fluorescent stains have improved the sensitivity of direct microscopy. Improved culture media coupled with radiometric means of detecting early mycobacterial growth have shortened the time needed for cultural diagnosis. Rapid immunodiagnostic techniques based on the detection of mycobacterial antigen or of antibodies to theses antigens have not, however, come into widespread clinical use. The DNA or RNA hybridization tests with labeled specific probes which have been described so far are not sensitive enough to be used for clinical speicimens without prior culturing. The advent of the polymerase chain reaction (PCR) has opened new possibilities for diagnosis of microbial infections. This technique has already been applied to a number of microorganisms. In the field of mycobacteria the PCR has been used to identify and to detect DNAs extracted from various mycobacteria. However, despite the extraordinary enthusiasm surrounding this technique and the considerable investiment, PCR has not emerged from the developmental "trenches" in the passed several years. It may be a considerable lenth of time before clinical microbiology laboratories become PCR playgrounds because many details remain to be worked out.
