A Case Report of Valproic Acid-Induced Tremor by Korean Traditional Medicine (Valproic Acid 투여로 인한 약인성 떨림 환자의 한방 치료 치험 1례)

  • A 56-year-old male patient with a sudden onset of tremor and involuntary movement of right upper and lower extremities, head, and voice was diagnosed as having a drug-induced tremor, with valproic acid being the culprit drug. The patient had undergone admission treatment at an internal Korean medicine department with herbal medicine according to constitutional diagnosis, acupuncture, moxibustion, cupping, and rehabilitation. The change in the severity of tremor was assessed with the Fahn Tolosa Marine scale. After discharge, the patient took herbal medicine for two more months. After 20 days of admission and 2 months of treatment with herbal medicine, the patient's tremor improved. In this case, the patient with valproic acid-induced tremor showed improvement in symptoms after 20 days of hospitalization and further reduction of tremor and improvement of quality of life were confirmed through follow-up for 2 months.

  • Squamous cell carcinoma is the most common cancer occurring in the oral cavity and oncologic wide cancer excision is a major cause of large lip defects. Large lower lip defect reconstruction with good functional and aesthetic results has always presented a challenge for plastic surgeons. There are various lower lip reconstruction methods depending on its size, location, and surgeon's expertise. This is a case of a large defect spanning more than two-thirds of the lower lip after wide excision due to squamous cell carcinoma. The Karapandzic flap was used to reconstruct the defect with a commissuroplasty carried out in a second operation, which yielded a relatively good functional and aesthetic result. No recurrences or metastases were observed within a one year follow-up period.

  • Most acute patella dislocations can be reduced closely using a spontaneous or simple maneuver, but it may not be possible if accompanied the by anomalies, such as rotation of the patella, osteophyte ridge protruding from the femur, or anatomical variations. This case report outlines irreducible acute patella dislocation with anatomical variations, a notched patella. If the reduction of an acute patella dislocation fails, an additional radiological examination and reduction under general anesthesia may be necessary with the possibility of an anatomical variant in mind.

  • Down syndrome is the most common chromosomal disorder and is accompanied by hip osteoarthritis in approximately 28% of patients. This paper reports a case of hip arthrodesis as a treatment for severe hip osteoarthritis in adolescent Down syndrome patients. When performing surgical treatment of patients with Down syndrome, it is necessary to take a comprehensive approach that considers not only the medical point of view, but also the psychological and economic requirements of the patient and their social environment.

  • Most low-energy pelvic ring fractures in elderly patients are treated conservatively so that an initial evaluation for complications such as vascular injury is usually overlooked. An 81-year-old female, who was taking regular aspirin, visited the emergency room and was diagnosed with a simple non-displaced pubic ramus fracture from a low-energy fall from standing, which was complicated by massive hemorrhage from the overlooked injury of the corona mortis. Elderly patients with pelvic ring fractures can have a delayed presentation of vascular injuries, regardless of the degree of displacement of the fractures, which highlights the need for a careful physical examination and close monitoring.

A Case Report of a Patient with Rapid Weight Loss due to Nausea, Vomiting and Stomachache (오심, 구토, 복통으로 인해 급격한 체중감소를 보인 환자 치험 1례)

  • This patient was 18 year-old man who had severe nausea, vomiting, stomachache but medical examination revealed no abnormality. The symptom lasted for 2 months and he lost 15kilograms. The patient had no underlying diseases. He was treated by korean medicine, containing acupuncture, cupping therapy and herb medicines during 9 days. The severity of nausea was scored by the Numerous Rating Scale and the frequency of vomitng was evaluated by count. After that period, patient's symptom was improved and his other conditions were also better, compared with his admission. This study suggests that Korean medicine could have a therapeutic effect for nausea, vomiting, stomachache and rapid weight loss that the cause has not been diagnosed by Western medicine. It could help to improve patient's symptoms and make general conditions better.

Daoyin Exercise Therapy for Scoliosis : A Systematic Review (측만증에 대한 도인 운동요법의 효과 - 체계적 문헌 고찰 -)

  • Objectives: This study aimed to review the effects of Daoyin exercise on scoliosis in clinical studies. Methods: In this study, we reviewed 9 electronic databases (Pubmed, CAJ, EMBASE, Cochrane Library, CiNii, KISS, KMBASE, OASIS, ScienceON) on May 22, 2021. We included only randomized controlled trials (RCTs) which evaluated the effect of Daoyin exercise on scolisos. The methodological quality of the included studies was checked using the Cochrane risk of bias tool. Results: After screening, eight RCTs fulfilled the inclusion criteria. The results of the meta-analysis showed that Daoyin exercise improves the measurement value of Cobb's angle, SRS-22, pulmoary function, physiological curvature, muscle strength, effective rate in scoliosis patients. Conclusion: We concluded that Daoyin exercise has therapeutic effects in relief of imbalance, improvement of symptoms, improvement of function, muscle strength for scoliosis. However, the quality of the original articles was low and the number of papers included were few. Thus, to confirm these results, further well-designed RCTs should be conducted.

  • The absence of the common carotid artery (CCA) and the common origin of the left external carotid artery (ECA) and the right CCA are rare anomalies of the cervical vascular system. We report here a case involving the coexistence of these vascular anomalies with an aberrant right subclavian artery, which is a common congenital anomaly in the aortic arch, and review the embryologic mechanism and clinical importance of this case.

Knowledge, Attitude and Practice on Maternal Immunization with Tetanus Toxoid, Reduced Diphtheria Toxoid, and Aellular Pertussis (Tdap) among Pregnant Women (임신 중 Tdap 접종에 대한 임부들의 인식, 태도 및 행동)

  • Purpose: Despite effectiveness and safety of maternal tetanus toxoid, reduced diphtheria toxoid, and acellular pertussis (Tdap) vaccination, Tdap vaccination coverage during pregnancy remains quite low. We assessed the knowledge, attitude and practice on maternal Tdap vaccination among pregnant women. Methods: This study was a cross-sectional survey of pregnant women who visited tertiary obstetrics and gynecologic units in Seoul and Gyeonggi province of Korea. Individual questionnaires were administered to assess knowledge, attitude and practice on maternal immunization with Tdap. Results: The questionnaires were completed by 184 pregnant women; 158 (86%) had not received information from doctors about pertussis and Tdap, and 166 (90%) did not know the need for Tdap vaccination. Only 7% of pregnant women unlikely to receive Tdap vaccine during current pregnancy answered 3 or more of the 5 knowledge-based questions correctly. By logistic regression analysis, recommendation by doctor (adjusted odd ratio [OR], 236.2; 95% confidence interval [CI], 12.6-4,432), belief that the vaccine is effective (adjusted OR, 40.21; 95% CI, 2.35-687.7), and belief that the vaccine is safe (adjusted OR, 19.83; 95% CI, 1.54-255.9) were significantly important factors to respondents' intention to be vaccinated. Conclusions: Most pregnant women seem to be neither recommended nor adequately informed about Tdap vaccination. Information given by health care professionals is very important to increase Tdap coverage among pregnant women.
