Study on Research for Reducing Radiation Dose of Head and Neck for Cephalometric Radiography System (두부규격방사선촬영장치의 두경부 피폭 저감에 대한 연구)

  • Recently, the interest in the orthodontic treatment for children is increased by a rise in national income level. The number of cephalometric radiography that could diagnose a malocclusion and malposition between teeth and jawbone increased. It required attention to radiation exposure, because the subject of dental examination is children which are more sensitive to radiation and the head and neck, the object of that include radiation sensitive organ such as the thyroid, bone marrow, eyes, salivary gland, and so on. In this study, we measured two-dimensional dose distribution in cephalometric radiography system (VATEC Pax-400C) using Agfa CP-G Plus film and MagicMax Dosimeter, and calculated radiation organ dose of head and neck through MCNPX simulation. And then we designed a radiation protective device to decrease radiation dose. The dose distribution of the cephalometric radiography system irradiated the head and neck overall as well as the oral and maxillofacial parts. The radiation organ dose calculated that thyroid, oesophagus and eyes are irradiated high, and the radiation organ dose decreased about 70 ~ 80% by the application of the radiation protective device. The results of this study will be used construction of database for dental radiation exposure and research of reducing radiation dose.

The Effect of Middle Schoolers' Oral Health Awareness on Oral Health Condition and Oral Health Behavior (중학생들의 구강보건인식이 구강건강상태와 구강건강행위에 미치는 영향)

  • The purpose of this study is to investigate the effects of awareness of oral health on oral health status and oral health behavior with subjects of students in the middle school which is the important period when lifetime set of teeth is completed with eruption of the second molar tooth and complete set of permanent teeth, and provide the basic data for development of oral health program for adolescents. We examined oral condition and conducted the survey about students' awareness of oral health and behavior in the middle school located in JeonJu. As the results of oral examination, dental caries and malocclusion were higher in female students and periodontal disease was higher in male students (p < 0.038). There was no significant difference in awareness of oral health between male and female students. However, M = 4.08 out of 5 point scale, which was high. For oral health behaviors, female students (M = 4.508) was higher than male students (M = 4.257) in the question of 'I visit the dental office when my gums are bleeding'. Female students (M = 4.547) was higher than male students (M = 4.333) in the question of 'I use the dental floss after brushing my teeth'(p<0.05). Awareness of oral health gave the significant effect on the oral health behavior at 95 % confidence level. Therefore, the education for oral health behavior is required to improve oral health and studies are needed to present the development of program for awareness and knowledge of oral health affecting behavior.

PSEUDOHYPOPARATHYROIDISM : CASE REPORT (가성부갑상선 기능저하증 환아의 구강 증상)

  • The parathyroid hormone plays a major role in the metabolism of calcium and phosphorus. In hypoparathyroidism the parathyroid glands are atrophied or absent associated with autoantibodies against parathyroid tissue. Pseudohypoparathyroidism is a metabolic disease caused by the disturbance in peripheral action of parathormone, but parathormone level is normal. In general, patients with pseudohypoparathyroidism have short stature, round face, brachydactylia, obesity, mental retardation, cataracts & ectopic calcifications on soft tissues. Dental manifestations are enamel hypoplasia, delayed eruption, blunting of root apex, hypodontia, pulp calcification, thickened lamina dura, excessive caries & malocclusion. In this case, intraoral examination showed enamel hypoplasia on the erupted permanent teeth & hypermobility on the remaining deciduous teeth. From the radiographic view severe dental anomalies were observed on canines and shortening and blunting of root apex was observed on mandibular incisors. Pathologic root resorption was also observed on deciduous teeth.

The differences of STO between before and after presurgical orthodontics in skeletal Class III malocclusions (골격성 III급 부정교합자에서 술 전 교정치료 전과 후의 수술계획의 차이)

  • Objective: To evaluate the discrepancies between initial STO and final STO in Class III malocclusions and to find which factors are related to the discrepancies. Methods: Twenty patients were selected for the extraction group and 20 patients for the non-extraction group. They were diagnosed as skeletal Class III and received presurgical orthodontic treatment and mandibular set-back surgery at Pusan National University Hospital. The lateral cephalograms were analyzed for initial STO (T1s) at pretreatment and final STO (T2s) after presurgical orthodontic treatment, and specified the landmarks 3s coordinates of the X and V axes. Results: Differences in hard tissue points (T1s-T2s) in the X coordinates of upper central incisor edge, upper first molar mesial end surface, lower central incisor apex, lower first molar mesial end surface and mesio-buccal cusp and Y coordinates of upper central incisor edge, upper central incisor apex, upper first molar mesio-buccal cusp were statistically significant in the extraction group. Differences in hard tissue points (T1s-T2s) in the X coordinates of upper central incisor edge, lower central incisor apex, lower first molar mesial end surface and Y coordinates of lower central incisor apex were statistically significant in the non-extraction group. In the extraction group, the upper arch length discrepancy (UALD) had a statistically significant effect on maxillary incisor and first molar estimation. Lower arch length discrepancy and IMPA had statistically significant effects on mandibular incisor estimation in both groups. Conclusions: Discrepancies between initial STO and final STO and factors contributing to the accuracy of initial STO must be considered in treatment planning of Class III surgical patients to increase the accuracy of prediction.

A Study on the Status of Recognition and Practical Application of Oral Hygiene Devices : with outpatient as the central figure (치과병·의원 내원환자의 구강위생용품에 대한 인지도 및 사용실태에 관한 연구)

  • This study was carried out to investigate recognition level and practical application status of oral hygiene devices through making a survey of Seoul inhabitants. And survey was implemented in order to propose oral hygiene device for household health care activities. The following results were obtained by analyzing personal interviews of 347 commuting patients at two university hospitals and seven dental clinics. 1) The average length of toothbrush head was estimated as 22.3 mm and average changing cycle was 2.3 months. The user ratio of flat-headed brush was estimated as 51.9% and 46.7% were using fluoridated toothpaste. 2) Generally the ratio of toothpick users was higher than other device users. But the user ratio of dental floss was higher than toothpick in case of patients under orthodontic treatment. 3) The patients under orthodontic treatment were not familiar with handling orthodontic toothbrush. Though 45.8% among orthodontic patients recognized this type of toothbrush, only 25.4% of them knew how exactly to use it. 4) It was shown very low user ratio of oral hygiene devices that the patients who had periodontal problem, hypersensitive trouble, halitosis, implant or denture 5) The patients who had halitosis showed the highest user ratio of toothpick. The patients who had separation of teeth showed the highest user ratio of dental floss. The patients who're under orthodontic treatment showed the highest user ratio of interproximal brush and motorized brush. 6) In response to the most interesting dental disease, it's surveyed as follows; 60.3% of dental caries, 24.0% of periodontal disease, 14.8% of false occlusion and 0.9% of oral cancer. 7) Regarding the motivation of using those oral hygiene devices, 45% responded that it was because of recommendation by dental clinics. Among the negative answers, 38.6% responded that it was because of no selection guidance. 31.3% answered that they didn't use hygiene device because it's inconvenient. 12.0% answered that it's difficult to buy and expensive. 7.8% responded that they didn't feel significant improvement. 4.8% answered that dental hospitals and clinics didn't even introduce those hygiene devices. Therefore efficient campaign for those hygiene devices over all Korea nation should be developed and education program must be prepared for each case of patients in every dental hospitals and clinics.

A comparison study of the effects of hypertrophied adenoid tissue on jaws morphology (정상 교합자와 adenoid가 과식증된 부정교합자의 악골 형태의 비교)

  • To investigate the association between the hypertrophy of adenoid and jaw morphology during growth, this paper was based on children patients with experimental adenoids (male-15 subjects at each bone age group, female-15 subjects at each bone group) and comparing them to data taken from a control group (male-15 subjects at each bone age group, female-15 subjects at each bone group) with normal respiratory function. The comparisons between the groups were done at each growth stage using cervical vertebrae maturation index(CVMI) of Hassel. The obtained results were as follows : 1. The differences in craniofacial morphology between experimental group and control group were appeared from CVMI 3 and CVMI 4 (aroud adolescent period) in males, and from CVMI 1 in females. 2. The mandibular position of experimental group was more inferior than control group. The difference appeared at adolescent period(male : at CVMI 4, female : at CVMI 5). 3. Experimental group had greater anterior facial height than control group. This difference seemed a relation with lower anterior facial height. The difference appears at CVMI 3(11.94 ${\pm}$ 1.38 years old, at adolescent period) in male and at CVMI 1 in female. 4. The adenoid size of control male group was increased until CVMI 2(10.58 ${\pm}$ 1.07 years old, just before adolescent growth peak) and then decreased, but in female the adenoid size was decreased from CVMI 1(6.92 ${\pm}$ 0.53 years old).

A STUDY ON THE IRREGULARITIES OF TEETH IN MALOCCLUSION (부정교합(不正咬合)의 치아부정양상(齒牙不正樣相)에 관(關)한 연구(硏究))

  • The purpose of this study was to investigate the pattern of irregularities of teeth in various malocclusion groups. The subjects consist of 803 out-patients (355 males, and 448 females) in department of Orthodontics of S.N.U. Hospital, Yonsei University, and Kyunghi University Hospital. The results were as follows. 1. The proportions of subjects on the basis of Angle's Classification were 39.2% (42.2% male, and 57.8% fomale) in class I malocclusion, 29.0% (44.6% male, and 55.4% female) in class II. div. 1., 3.5%(46.4% male, and 53.6% female) in class II. div. 2., 28.3%(46.3% male, and 53.7% female) in class III. 2. Considering all the subjects, the percentage of teeth crowding was 67.8% (45.0% male, and 55.0% female). In class I malocclusion, the percentage of Crowding was 70.8%(43.5% male, and 56.5% female) with higher frequency in upper anterior teeth than in lower anterior. 3. The percentage of Maxillary anterior diastema was 25.6% (45.6% male, and 54.4% female) on the whole. In class II. div. 1. malocclusion, the percentage was 28.8% (46.3% male, and 53.7% female) and in class III, the percentage was 19.8% (46.7% male, and 53.3% female). Thus, frequency of maxillary anterior distema, was comparatively higher in class II. div. 1. than in class III. 4. The percentage of high canine was 25.1% (53.2% male, and 46.8% female) on the whole, and was 86.0% male and 76.6% female in right side, 73.0% male and 72.3% female in left side. In calss II. div. 2., the percentage was 53.6% (46.7% male, and 53.3% female ). In class II. div. 1., the percentage was 16.7% (46.2% male, and 53.8%) with higher frequency in class II. div.2. 5. The percentage of deep overbite was 23.0% (43. 2% male, and 56.8% female) on the whole. Ia class 11. div. 2., and in clas sll. div. 1., its were 89.3%(48.0% male and 52.0% female), 54.5% (40.9% male, and 59.1% female) respectively. This result can be considered as one of the characterics of Angle's class 11 malocclusion group. 6. The percentage of spacing was 23.0% (36.8% male, and 63.2% female) on the whole, In class II. div. 1., and in class II. div. 2., its were 26.1% (44.3% male, and 55. 7% female), 7.1% (50.0% male, and 50.0% female) respectively. 7. The percentage of open bite was 14.3% (42.6% male, and 57.4% female) on the whole with higher rate on the anterior part. It rated 17.6%(50. 0% male, and 50.0% female) in class III, but none in class II. div. 2. 8. The percentage of crossbite was 22.5% (55.8% male, and 44.2% female) on the whole, with higher frequency on the anterior part than on the posterior part. In Angle's class III, it rated as much as 55.1% (57.6% male, and 42.4% female). 9. The percentage of edge-to-edge bite was 20.4% (47.6% male, and 52.4% female) with higher frequency on anterior part than on posterior part. 10. The percentage of irregularities of teeth in various malocclusion groups, was 21.5% (24.8% maxillary, and 18.1% mandible) in crowding, 20.8% (23.5% maxillary, and 18.0% mandible) in rotation, 10.7% (10.6% maxillary, and 10.8% mandible) in cross bite, 9.5% (11.8% maxillary, and 7.3% mandible) in spacing, 8.5% (8.5% maxillary, and 8.5% mandible) in edge-to-edge bite, 8.1% (8.3% maxillary, 7.8% mandible) in open bite. Crowding teeth, spacing teeth, and rotating teeh were more prevalent in anterior part than in posterior part. Cross bite teeth and edge-to-edge bite teeth were more prevalent in class III malocclusion than in another.

Hard and soft tissue profile changes following anterior subapical osteotomy in bimaxillary dentoalveolar protrusion patients (양악성 치조 전돌 환자의 근첨하 분절 골절단술 후 경조직 및 연조직 측모 변화)

  • The purpose of this study is to evaluate hard and soft tissue changes following the subapical osteotomy in bimaxillary dentoalveolar protrusion patients requiring maximal retraction adult female patients was selected. Surgical procedures were performed by the same surgeon, anterior subapical osteotomy techniques were employed on the maxilla and cephalometric radiograms were traced and superimposed using the best-fit method and two reference The results were as follows 1. The bodily movement of the maxillary anterior segment was achieved in a posterior moved posteriorly with a slight correction of the lower incisors. 2. The horizontal soft tissue measurements changed significantly after treatment, but Nt and Sn vertical soft tissue measurements indicated that Ls moved inferiorly and Li superiorly. 3. The correlation between hard and soft tissue changes indicated that ${\Delta}HId/{\Delta}HLi,\;{\Delta}HId/{\Delta}LL-Eline,\;{\Delta}Hpt.B/{\Delta}HILS,\;and\;{\Delta}UI-FH/{\Delta}NL$ were significant. 4. More lower lip relative to upper lip retraction was demonstrated in relation to Rickett's E-line. The ratio between upper lip displacement was $50\%$, and between the lower incisor and lower lip displacement was $60\%$. We conclude from the results that the anterior subapical osteotomy is an efficient treatment severe dentoalveolar protrusion and desire rapid results.

A study on Pre-and Post-surgical Patterns of Mandibular Movement and EMG in Skeletal Class III Prognathic Patients who underwent Intraoral Vertical Ramus Osteotomy (하악 전돌증 환자의 구내 하악골 상행지 골절단술전후의 하악골 운동양상 및 저작근 근전도 변화에 관한 연구)

  • Stomatognathic system is a complex one that is composed of TMJ, neuromuscular system, teeth and connective tissue, and all its components are doing their parts to maintain their physiological relationships. Mandible, in particular, performs various functions such as mastication, speech, and deglutition, the muscular activities that determine such functions are signalled by numerous types of proprioceptors that exist in periodontal membrane, TMJ, and muscles to be controlled by complicated pathways and mechanics of peripheral and central nervous system. Orthodontic treatment, especially when accompanied by orthognathic surgery, brings dramatic changes of stornatognat is system such as intraoral proprioceptors and muscle activities and thus, changes in patterns of mandibular function result The author tried to analyze changes in patterns of mandibular movement and physiologic activities of surrounding muscles in Skeletal Class III ortlrognathic surgery patients who presently show a great increase in numbers. The purpose of this study was to draw some objective guidelines in evaluating funclierual aspects of orthognathic surgery patients. Mandibular functional analysis using Biopak was performed for skeletal Class III prognathic patients who underwent IVRO(lntraoral Vertical Ramus Osteotmy), and the following results were obtained: 1. Resting EMG was greater in pre-surgical group than the control group, and it showed gradual decrease after the surgery. Clenching EMG of masseter and anterior temporalis of pre-surgical group was smaller than those of control group, they also increased post-surgically, and significant difference was found between pre-surgical and post-surgical(6 months) groups. 2. Resting EMG of anterior ternporalis was greater than that of all the other muscles, but there was no significant difference. Clenching EMG of anterior temporalis and masseter were greater than those of the other muscles with statistical difference. In swallowing, digastric muscle showed the highest EMG with statistical significance. 3. Limited range of mandibular movement was shown in pre-surgical group. Significant increase in maximum mouth opening was observed six months post-surgically, and significant increase in protrusive movement was observed three months post-surgically.

A Descriptive Study of Oral Health Knowledge & Behaviors in Middle School Students (일부지역 중학생의 구강건강 지식 및 행동에 관한 조사연구)

  • Objectives: This study was designed to understand the oral health knowledge & conduct of middle-school students, search for the learning objective and the educational method in line with the subjects and of utilizing as the basic data for an effective oral health-care program. Methods: The samples to achieve the purpose of this research are composed of 139 students in middle-school, OO county. Chungcheongbuk-do, the number of male students 64, and female students 75. Data were statistically analyzed by frequency analysis, $x^2$-test or Fisher's exact test by using SPSS WIN Ver. 12.0. Results: Among items on oral-health knowledge in middle-school students. the awareness ratio on a cause and preventive method for oral disease was surveyed to be lower than the awareness ratio on symptoms of oral disease. As a result of examining by comparing knowledge and behavior on the time of tooth brush. both awareness and behavior were the level of 50% or less than it. In particular, 46.2% perceived after lunch. but practice just accounted for 33.0%. The frequency of tooth brush a day was the largest in a case(47.5%) of doing twice a day. However. there was also the response (5.8%) with saying of brushing once or not brushing even once. Thus, the practice of tooth brush was surveyed to be very low even if being a minority of students. The frequency of taking a light meal was 68.8% in less than twice a day. However, even students of taking more than five times were surveyed to be 9.8%. Out of the whole-body health in over 50%-59.9%. the oral health was surveyed to be perceived to be very important. Compared to the awareness level on importance of a tooth, the ratio of visiting a dentistry was analyzed to be very low. Conclusions: The study results suggest that the school oral-health project was examined to have the necessity of being expanded and carried out even in middle-and-high schools, by which the specific oral-health promotion program including oral-health education in this period is developed.
