Nurses' Experience of Middle East Respiratory Syndrome Patients Care (메르스 환자 간호에 대한 간호사의 경험)

  • This study was conducted to investigate the practical experience of nurses in MERS patients care. Nine nurses in one general hospital in S city underwent in-depth interviews from August 2015 to March 2016. The collected data were analyzed according to the procedure of the phenomenological method of Colaizzi. The following five categories were derived: '1. Anxiety and burden due to the risk of transmission of new infectious diseases', '2. Social isolation because of being the nurse in charge of MERS patients', '3. Faced with problems due to an unprepared treatment environment', '4. Overcome the burdensome MERS patient care process', '5. Reflect on the steps for preparing against new infectious diseases'. These results suggest that it is important to draw positive experiences as well as the negative effects of nurses who are involved in treatment of various infectious diseases, including MERS. Based on this, it can be used as a basis for reducing negative psychological and social impacts and improving positive adaptation. Additionally, it is necessary for healthcare workers including nurses to establish the necessary workforces, departments and guidelines for nursing new infectious disease patients.

A Rapid Detection of Methicillin-Resistant Staphylococci by Polymerase Chain Reaction (Polymerase Chain Reaction을 이용한 Methicillin-resistant Staphylococci의 신속 검출)

  • PCR of the mecA gene for the rapid detection of methicillin-resistant staphylococci was perfomed and compared with the antibiotic sensitivity test. A total of 43 strains of staphylococi from clinical specimens were used in this study. An antibiotic sensitivity test by the agar dilution method of NCCLS (The National Commitee for Clinical Laboratory Standard) was performed for the strains. Among them, 39 isolates were methicillin-resistant (MRS), and 4 isolates were methicillin-susceptible (MSS). With the exception for one strain (Staphylococcus cohnii, HRC2-4), all MRS strains amplified the expected 533 bp fragments of the mecA gene by PCR, However, one strain (Staphylococcus aureus, HSA1-10) that was classified as a sensitive strain by the antibiotic sensitivity test was mecA positive by PCR. All 35 methicillin-resistant Staphylococcus aureus (MRSA) strains were mecA positive, but overall, concordance between the results of the mecA PCR and antibiotic sensitivity test was 95.6%.

Radiation safety management for diagnostic radiation generators and employees in animal hospitals in Korea (동물병원의 진단용 방사선 발생장치 및 방사선종사자 안전관리 실태 조사)

  • A nationwide survey on radiation safety management in Korean animal hospitals was conducted. By 2013, 53 radiation generators were registered as veterinary medical devices (41 X-ray generators and 12 computed tomography scanners). Additionally there were six approved laboratories for radiation equipment and protection facility, and five approved laboratories for radiation exposure of employees, respectively. By March 2013, 2,030 out of 3,829 animal hospitals operated radiation-generating devices. Among these devices, 389 (19.2%) out of 2,030 were not labeled with the model name and 746 (36.7%) were not labeled with production dates. Thus, most veterinary X-ray generators were outdated (42.6%) and needed replacements. When periodic inspections of 2,018 animal hospitals were performed after revision of the Veterinarians Act in 2011, the hospitals were found to be equipped with appropriate radiation generators and protection facilities. Among 2,545 employees exposed to radiation at the hospitals, 93.9% were veterinarians, 4.3% were animal nurse technicians, and 18% held other positions. Among 169 employees supervised by administrators, none of those had a weekly maximum operating load that exceeded $10mA{\cdot}min$. This study suggests that the radiation safety management system of animal hospitals was general good.

방사성옥소 치료를 위한 표준 지침서 개발과 치료병실 환경 개선 연구

  • 목적: 방사성옥소 치료 환자들은 주의 사항이나 식이요법 등을 지키는 것에 대한 어려움과 3~4일간의 격리 치료기간 동안의 막연한 불안감을 느끼는 등의 심리적, 신체적 부담감을 가지고 있다. 본 과에서는 방사성옥소 치료의 예약과 치료병실에 대한 표준지침서를 개발하여 환자들에게는 정확하고 쉬운 정보를 제공하여 막연한 불안감을 해소하고 치료 업무에 종사하는 종사자들에게는 통일된 정보를 제공하여 업무효율을 향상시키는 효과와 치료 병실의 환경개선을 통한 환자의 심리적 안정감을 찾는데 주력하고자 본 연구를 시작하였다. 대상 및 방법: 2006년 9월부터 2007년 2월까지 치료병실에 입원한 환자를 대상으로 치료환자의 치료 일정, 주의사항, 저옥소 식단과 치료병실의 표준 진료 지침서를 개발하고 치료 병동의 간호사와 협력하여 치료병실의 환경을 개선하여 환자에게 적용하였다. 결과: 방사성옥소 치료의 특성상 환자는 치료가 결정되는 순간부터 치료과정에 대한 이해도 부족과 치료병실 생활에 대한 불안감을 느끼게 되므로 치료 시 사용되는 일정 및 주의사항, 저옥소 식단의 표준지침서를 개발하여 환자 치료병실 개선과 치료병실에서의 표준지침서를 개발하고 사용하였다. 결론: 치료에 따른 진료지침서와 병실 환경을 개선 후 치료에 대한 환자의 이해도 및 치료병실에서의 생활이 예전과 비교해서 많이 향상 되었고 치료의 과정을 설명하는 예약 시에도 많은 도움이 되고 있다. 이러한 치료과정의 규격화 하는 작업들은 본원뿐 아니라 치료병실이 있는 병원의 치료환경을 개선하는데 도움이 될 것이라고 사료된다.

Recognition on Hospital Evaluation Result, Influences and Task of Mid Sized Hospital Employee Having Experience of the Hospital Evaluation (중소병원 종사자의 의료기관평가 수검 후 평가결과 및 영향.과제에 대한 인식)

  • The purpose of this study is to provide basic material about the improvement method of the hospital evaluation system by grasping recognition, corresponding behavior to evaluation, influence and problems about the hospital evaluation system with respect to the employee of mid sized hospitals having relatively poorer environment. In this study were used 518 questionnaires to hospital employee working at 20 general hospitals in Pusan, Kyungnam with less than 500 beds and having experience of the hospital evaluation during 2005~2007. As a result of the research, they felt the investigation tool used for the hospital evaluation has too many items and it could be difficult to reflect characteristics of each hospital. It appeared that utilization of the report result after the hospital evaluation was helpful for grasping a part necessary for hospital improvement and the present level of hospitals and increase of interest about medical service quality improvement of the a hospital director or hospital employee. They trusted the result of the hospital evaluation to some degree, and recognized it helps the patients to select a hospital and increase rights-to-know. Hospital employee recognized the necessity and showed high interest and participation, but they didn't sympathize in the propriety and the feasibility of the hospital evaluation items. High degree evaluated hospital employee receive hospital evaluation system positively and think hospital services to be improve after taking hospital evaluation. The hospital evaluation should make the hospital to effort to achieve medical quality improvement to its purpose. Also, systematic reformation will be necessary to reflect characteristics of the hospital that has relatively small scale or locates in poor environment.

Leakage and Scattered Radiation from X-ray Unit in Radiography (영상의학과 엑스선 발생장치의 누설 및 산란선량 측정)

  • The purposed of this study were measured the radiation exposure of patients and workers by generators, and the protection state for radiation facilities. The subject of the study by X-ray generators in university hospitals of capital area, we measured the maximum irradiation condition of 80 kVp, 200 mA, 0.1 second in the control entrance, control room window, entrance of radiography, adjacent site. The leakage dose per week was which the control entrance was 0.11 mR/week, control room window was 0.15 mR/week, entrance of radiography was 0.12 mR/week and adjacent site was 0.06 mR/week with X-ray unit the mean And the leakage mean dose was 0.11 mR/week. Diagnostic X-ray tubes must ensure that the leakage radiation in the maximum leakage dose in week emitted by the tube outside the useful beam does not exceed certain levels provided by standards.

Ergonomic evaluation and improving measures of tasksperformed in a general hospital (종합 병원 종사자 업무의 인간공학적 평가 및 개선 방안)

  • The objectives of this study were to ergonomically evaluate varying tasks performed in a general hospital and to propose their improving measures based on the evaluation results. The tasks found in the hospital were largely classified into two groups of manual materials handling and awkward posture related tasks. Ergonomic tools of NLE, 3-D SSPP and RULA were used for evaluating workload of the tasks. The major findings are: 1) L5/S1 compressive force of patient transferring by one person exceeded the maximum permissible limit(6,400N) by NIOSH. The L5/S1 compressive forces for most of the patient transferring tasks by 2-4 persons were larger than the action limit (3,400N), and the tasks by five persons were analyzed to be safe in the view of L5/S1 compressive force; 2) patient repositioning tasks by 2-3 persons were hazardous on the basis of L5/S1 compressive force, while most of the tasks by 4-5 persons were safe; 3) many tasks performed in wards were found to be stressful, most of which resulted from improper heights of their working tables or working points. Of varying tasks in general hospitals, patient transferring was the most stressful. Based on the results of this study and high prevalence of musculoskeletal disorders from other studies, it is recommended that the ergonomics program be introduced and enforced for doing improving activities systematically

The Level of Emotional Labor among Workers in One University Hospital (한 대학병원 종사자의 감정노동 수준과 그에 영향을 미치는 요인)

  • Purpose : The purpose of this study was to investigate the level of emotional labor and to identify affecting factors on emotional labor among one university hospital employees. Material and Method : This study was based on cross-sectional, self-administered, and Internet-based survey. The survey was conducted from Mar. 2 to Mar. 28. 2011. Total subjects were 812 employees working in one university hospital. Total response rate was 61.5%. Modified evaluation tool was used, which was originally developed by Morris and Feldman, to measure the level of emotional labor among hospital personnel. In order to identify the affecting factors on high level of emotional labor, we conducted logistic regression. The SPSS statistical software package was used to perform the statistical analysis. All statistical tests were 2-sided and a p-value<0.05 was considered statistically significant. Results : Average score of emotional labor was 3.06. Employees(3.39) who are working at the emergency department and hemodialysis room indicated the highest level of emotional labor, followed by wards(3.14), department of administration(3.14), department of ambulatory cares(3.06). The factors affecting on the high level of emotional labor were the highest level of schooling, types of department, and types of personality(p<0.05). Conclusion : The survey results showed that there was significant level of emotional labors among hospital employees. Therefore, the efforts to reduce the level of emotional labors are needed.

Organizational Commitment and Job characteristics of Hospital Foodservice Employees (병원급식 종사자의 조직헌신성 분석)

  • For the purpose of disclosing the relationship between job characteristics and satisfaction and organizational commitment for foodservice employees, questionnaire survey was carried out on 427 subjects of 14 general hospitals in Seoul, Korea. Questionnaire items consisted of general characteristics, organizational commitment, job satisfaction and characteristics. Data were analyzed by ANOVA, Duncan's multiple range test and Pearson correlation using SPSS PC package. The results were as follows 1. Mean score of value commitment and commitment to stay were 3.57 and 3.67. 2. There were significant differences between value commitment and age, marital status department, period and management, and between commitment to stay and experience and management. 3. Mean score of job satisfaction was the highest in co-workers(3.37) and work itself (3.37) and followed by in supervision(3.25), wage(2.43) and promotion(2.01). There were significant differences between job satisfaction for work itself and age, educational status and job department and management, between job satisfaction for wage and position, department, period and management, between job satisfaction for supervision and age, educational status and department, between job satisfaction for promotion and age, marital status, position, period, and management, between job satisfaction for co-workers and sex and educational status. 4. Mean score of job characteristics was the highest in dealing with others(4.13) and followed by in feedback(3.51), autonomy(3.29), task identify(3.07), variety(2.71) and friendship(2.47). 5. Job satisfaction for work itself, supervision and co-workers were significantly increased with increasing value commitment. Job satisfaction for work itself, supervision were significantly increased with increasing commitment to stay. Job satisfaction for promotion had negative correlation with organizational commitment in all job position. 6. Value commitment had significantly positive correlations with variety, autonomy, identity, feedback and dealing with others, and significantly negative correlation with friendship. Commitment to stay had significantly positive correlations with dealing with others, and significantly negative correlations with friendship. In all job position organizational commitment had significantly negative correlations with friendship.

Development and Validation of an Instrument to Assess Hospital Workers' Perception for Protection of Personal Health Information (의료기관 근로자의 환자 개인정보 보호 인식에 대한 평가도구 개발 및 검증)

  • The purpose of this study was to develop and examine a validity and reliability of an instrument to assess hospital workers' perception for protection of personal health information. The process included construction of a conceptual framework, generation of initial items, verification of content validity, preliminary study, and extraction of final items. Content validity was verified by 3 experts from nursing and health information management areas, and the construct validity was evaluated by item analysis, exploratory factor analysis and confirmatory factor analysis. The reliability was examined by Cronbach's alpha. The participants were 279 hospital workers including nurses, administrative officers, and other medical health professionals. Using a confirmatory factor analysis, a four-factor structure was validated with construct validity. The Cronbach's alpha coefficient of a total of 13 items categorized into 4 factors was .83. Finally, the instrument is a valid and reliable to assess hospital workers' perception for protection of personal health information.
