Fabrication of palatal lift prosthesis for a patient with palatal defect (구개결손이 있는 환자에서 연구개거상장치를 제작한 증례)

  • This report is a case of 38-year old female patient who had a chief complaint about having pronounciation disorder after going under mass excision due to mucoepidermoid carcinoma. She was referred to department of prosthodontics, Yonsei university. Palatal lift prosthesis was fabricated which was then relined with tissue conditioner. Evaluations on phonetics and swallowing were done and improvements were shown. Tissue conditioner was replaced with heat polymerizing resin and the patient was satisfied with the results of treatment.

Factors Affecting Job Satisfaction of Biomedical Engineers Working in General Hospital in Korea (종합병원에 근무하는 의공기사의 직무만족에 미치는 영향요인)

  • The effective administration of hospital with innovation and human resource practices is a matter of grave concern because hospitals are becoming bigger and more specialized. Biomedical engineers who manage medical machineries and tools used to deliver healthcare services in a hospital setting play an important role in providing customers good quality services. Maintaining job satisfaction of biomedical engineers is, thus, important in the delivery of quality care. This is a descriptive cross-sectional study aiming to determine factors affecting job satisfaction of biomedical engineers working in general hospitals. The study population consisted of biomedical engineers at 79 general hospitals of 26 regions based on the registry of the Korea Medical Engineering Association (KMEA). The data were collected using a self-administerd questionnaire between May and July of 2009. Job satisfaction was assessed with 19 items covering 3 dimensions of work-external, work-internal, and organizational aspects (Cronbach's ${\alpha}=0.884$), resulting in an average summary score. Statistical analysis was conducted with SPSS for Windows version 15.0. The mean score of job satisfaction was 3.50 (${\pm}0.04$). There were statistically significant differences in job satisfaction according to age, health status, job position, duration of work as a biomedical engineer, years in the current workplace, difficulty at work, intent to change job, and the amount of support from superiors and colleagues. In multiple regression analysis, the factors affecting job satisfaction of biomedical engineers were salary, health status, and support of superiors and colleagues ($r^2=0.512$). Effective motivation-plans, taking into account organizational characteristics and the working environment of the hospital, may help to improve the job satisfaction of biomedical engineers.

Development of Open Clinical Training Program to Improve Radiology-Major Students' Clinical Competency (방사선(학)과 재학생의 임상적응 향상을 위한 개방형 임상실습 프로그램의 제안)

  • This study aimed to develop an open clinical training program to improve radiology graduates' clinical competency in the hospital setting to raise quality of patient-centered medical service. Development of the training program is similar to that of an instructional system design model. The program was developed according to the ADDIE model. Elements of each phase were collected. Draft of the program was constructed from literature review and clinical demands. Preliminary training program was implemented to sample students with the draft. After consultation with related professionals, the program was modified and completed. Professional groups assessed content validity of the program. Five continuous phases of the program - analysis, design, development, implementation, and evaluation - accommodate changes in clinical environment and demands. They also provide adequate learning needs. Effectiveness of the program and appropriateness of contents in each phase achieved a satisfactory level of significance. This program reflected demands from medical fields and effective learning programs. The purpose of the open clinical training program is to train radiological technologists in clinical setting to actively engage in patient-centered medical service and to help utilize their professional knowledge.

Interrupted Single-layer Suture Technique in Esophageal Anastomosis Using Monofilament Polypropylene Suture (Monofilament Polypropylene사를 이용한 단속단층 식도문합술)

  • Background: Although various anastomotic techniques and suture materials have been used in esophageal anastomosis, anastomotic leakage and stenosis are still somewhat frequent and serious complications when compared to other intestinal anastomoses. We have used interrupted single-layer suture technique using monofilament polypropylene suture in various esophageal anastomoses, including repair of the esophageal atresia, since 1990. Methods and method: We retrospectively evaluated the efficacy of this technique on postoperative leakage and stenosis in several esophageal reconstructions. The esophageal reconstructions using this technique were performed in 90 patients at Dong-A University Hospital from April 1990 through December 1996. Results: Anastomotic leakage occurred in 5 patients(5.6%) with one operative death. Stenosis at the anastomotic site occurred in 15 patients(n=86, 17.4%), which was most common in esophagogastrostomy(22%) and least common in esophagocolostomy (5%). This result was comparable to other methods including the autosuture technique. Conclusions: We concluded that this suture technique in esophageal anastomosis can be used with reasonable results in various esophageal reconstructions including correction of the esophageal atresia.

Influencing Factors on the Satisfaction of the Paramedic Students in Clinical Training (응급구조학전공 학생의 병원 임상실습 만족도에 영향을 미치는 요인)

  • Purpose: This study aims to assess the influencing factors on the level of satisfaction with clinical training and to provide basic data for more efficient clinical training. Method: The study was conducted on 402 paramedic students who have experienced clinical training from September 6 until October 12 in 2011. The questionnaires consisted of 40 questions. We used SPSS 18.0 frequency analysis, technical statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression analysis. Result: 1. The level of satisfaction with clinical training showed significant difference between the frequency (F=8.837, p=.000) and clinical training managers (F=5.418, p=.001). 2. The level of satisfaction with clinical training showed the strongest positive correlation with the satisfaction of clinical training hospitals (r=.694, p=.000). 3. Multiple regression analysis revealed the most powerful predictor for satisfaction with clinical training was the satisfaction level of clinical training hospitals(48.2%) and the frequency of clinical training experiences(.8%), the preparation before the clinical training(5.4%), the total duration of clinical training(.7%), and the satisfaction of emergency department education(1.0%). These five variables accounted for 56.1% of the satisfaction of clinical training among paramedic students. Conclusion: As student's satisfaction of the clinical training increases, the quality of paramedics is expected to improve in the future. As a result, the future paramedics can be nurtured to be highly skilled in on-the-scene emergency situations after graduation.

The Gender Difference between Diet Therapy Satisfaction and Awareness of Nutrition Education of Inpatients in Some Small and Medium-Sized Hospitals in Southern Gyeonggi Area (경기남부지역 일부 중소병원 치료식 환자의 성별 급식만족도와 영양교육에 대한 인식 비교)

  • This study was carried out on hospitalized patients receiving diet therapy satisfaction and awareness of nutrition education in small and medium-sized hospitals in southern Gyeonggi area. By the face-to-face interview with questionnaire method, the survey was carried out on 150 subjects (99 males and 51 females). The patients (64.7% of males, 52.9% of females) heard the description of diet therapy without the aid of written manual. Patients were moderately satisfied by food that was rated based on these indicators: tastiness (2.45), saltiness (2.23), smelliness (2.23), and appetizing appearance (2.39) as a 4 point scale. Most of the patients (80.8% of males, 69.8% of females) wanted to continue diet therapy. The importance of diet was correlated with the tastiness, saltiness, and appetizing appearance of diet. And the help of dietitians' explanation was correlated with appetite. 55.6% of males and 35.4% of females received nutrition education before (p<0.05). The number of times nutrition education was received was once for male (33.3%) and three times for female (44.4%). Most nutrition education satisfaction that was calculated on a 4 point scale was well understood (3.03), recognized importance (3.44 for males, 2.78 for females) and help for disease management (p<0.01). Also they were satisfied with the explanation of disease (3.20). Nutrition education satisfaction was correlated positively with explanation about food related to disease, the current dietary treatment, how to prepare diet recipes, and dietitian's kindness, but negatively with satisfaction with explanation of diet therapy, and the description and help for disease management. In conclusion, the most important factor in practicing diet therapy was meeting with the dietitian. Intensive nutrition education after the patient's discharge is necessary for patients to continue diet therapy.

The Study of Patients Satisfaction and Expectation of Hospital Foodservice (병원급식 서비스에 대한 환자만족도와 요구도에 관한 조사)

  • The purpose of this study was to measure patients satisfaction and expectation with hospital foodservices, and thereby identify areas for improvement and provide basic data for the introduction of total quality management with hospital foodservices. This survey was carried out on 383 hospitalized patients of 7 hospitals in Deagu, Busan, Changwon with 350 beds to determine the quality satisfaction with foodservices. The subjects were 50.5% male and 49.5% female. 62.6% of the subjects were over 40 age, 31.4% were only educated to middle school or below, 28.3% were hospitalized for 7-14 days. The mean score for taste of diet was 2.79, temperature 3.23, appearances 2.96. Most subjects agreed with following foodservice characteristics that meals of movement (4.03), dress of employees (3.84), kindness of employees and meals arrived exactly the same time every day (3.47) and cleanliness of foods (3.34) and dishes (3.33). The unsatisfied quality attributies were information provide (2.82), variety of the meals (2.91), mixing of meals (2.95), the opportunity to meet with a dietitian (2.97) and prompt dealings with meal complaints (3.01). Most subjects expectation that the decrease the multiple of menus, increase provide of fruits in hospital meals and selective menus in hospital foodservices operations. In conclusion, it would seem to be desirable that hospital foodservices departments introduce selective menus, quality assurance, and increase the meal rounding of dietitians in the patient foodservice.

  • 우리 나라 축사는 생산효율 제고를 위하여 대형화, 밀폐화, 고밀도화, 자동화 경향이 뚜렷하다. 대형의 밀폐된 고밀도 축사는 쾌적한 실내환경을 전제로 하기 때문에 기계적으로 실내환경을 적절히 제어하지 않으면 안된다. 제한된 공간에 먼지, 병원성 미생물, 유해기체, 수분이나 열의 과도한 집적은 생산과 재생산효율에 심각한 영향을 미친다. 그러므로 축사내 생산주체인 가축과 작업인이 쾌적한 실내환경에서 생산활동을 할 수 있도록 열적, 화학적/생물학적 환경을 물리적으로 제어하지 않으면 안된다. 본 연구는 실험축사내 가축이 일정한 열을 발생할 때 실내공기의 유동형태를 예측하기 위해서 수행하였다. 이 연구의 결과를 실내환경제어를 위한 환기시스템 책략 개발의 기초자료로 활용할 수 있다. 실험축사내의 공기유동을 예측하기 위해 Body-Fitted Coordinate(BFC)의 격자배열과 k-$\varepsilon$ 난류모형 및 SIMPLE계열 solution scheme을 사용하였으며, 예측의 유효성 검정은 Boon(1978)의 실험결과를 이용하였다. 예측한 공기유동의 형태와 실험한 공기유동의 형태를 비교한 결과 대체로 만족할만한 결과를 얻었다. 그러나 유입공기의 온도가 1$0^{\circ}C$인 경우의 공기유동은 실험유동형태와 약간의 차이가 있었다. 즉, 실험에서는 수평슬롯으로 유입 된 공기가 바로 아래로 굴절되어 유동하였으나, 예측의 결과는 일정 거리로 수평방향으로 유동하다가 아래로 굴절하였다. 이런 유동의 차이는 k-$\varepsilon$ 난류모형 자체가 경험적으로 부력에 민감하게 반응않는 결함이 원인이 될 수도 있으며, 실험의 부적절한 수행이 원인이 될 수도 있다. 이 유동의 경우 Reynolds 수가 3,000정도의 난류이며, 완전발달유동 (fully-developed flow)이므로 관성력 (inertia force)이 부력 (buoyancy force)보다 커, 일정거리 수평으로 유동하다가 아래로 굴절할 수도 있기 때문이다. 앞으로 이를 규명하기 위한 보다 깊이 있는 연구가 이루어져야 할 것이다.

The Association between Perceived Job Performance of Hospital Coordinators and the Satisfaction of Medical Workers (의료기관 종사자가 인지한 병원코디네이터의 업무수행도와 종사자의 직무만족도와의 관련성)

  • Objectives: This study was conducted to investigate general characteristics, job characteristics, characteristics of hospitals, and hospital coordinators, and to investigate the factors affecting the job satisfaction of medical workers. Methods: The data was collected through a self-reported questionnaire among 435 workers in plastic surgery and dermatology clinics in metropolitan city G. We investigated general characteristics, job-related characteristics, characteristics of medical institutions and hospital coordinators, job performance of hospital coordinators, and job satisfaction of medical workers. T-tests, ANOVA, correlation, and multiple regression analyses were conducted to investigate the factors related with job satisfaction. Results: The job satisfaction of the medical workers and the perceived job performance of hospital coordinators were $3.55{\pm}0.40$ and $3.74{\pm}0.43$, respectively. In multiple regression analysis, drinking frequency, number of night shifts, number of work days, and hospital coordinator introduction years were associated with the job satisfaction of medical workers. The more the overall job performance of hospital coordinators increased, the higher was job satisfaction(B=0.458, p<0.001). In detail, the more job roles(B=0.218, p<0.001), core competency(B=0.145, p=0.005), and leadership(B=0.099, p=0.037) increased, the higher was the job satisfaction of medical workers. Conclusions: The job satisfaction of the medical institution workers was associated with the perceived job performance of the hospital coordinator. In order to increase the job satisfaction of medical institution workers, a hospital coordinator with specialized job performance will be able to improve job satisfaction by carrying out efficient work in the medical institution.

The Effect of Empowerment on Job Satisfaction for Physical Therapists in General Hospitals (종합병원 물리치료사의 임파워먼트가 직무만족에 미치는 영향)

  • PURPOSE: This study aimed to investigate the effect of empowerment on job satisfaction for physical therapists in general hospitals. METHODS: A total of 297 physical therapists working in general hospitals throughout the country participated in the study survey using self-administered questionnaires. The questionnaires comprised three major parts, i.e., questions about general characteristics, empowerment, and job satisfaction, which were rated using a 5-point Likert scale. Statistical analysis was performed to analyze the frequency and percentage of participants, and the independent t-test and analysis of variance were performed to determine the difference in job satisfaction according to the general characteristics. Stepwise multiple regression was used to evaluate the factors that affect job satisfaction. RESULTS: The average of total empowerment, including two subordinate concepts, was 3.29 points, that of psychological empowerment was 3.53 points, and that of structural empowerment was 3.05 points. The average job satisfaction of the participants was 3.43 points, and it showed a significant difference in age, overall work experience, current work experience, position, annual salary, and daily work hours. Moreover, job satisfaction increased as the subordinate factors for empowerment, i.e., support, resources, chance, and competence, increased. Other factors that affect job satisfaction were daily work hours, number of colleagues, type of work, annual salary, and therapeutic assistance. CONCLUSION: This study demonstrated that job satisfaction of physical therapists working in general hospitals was related to empowerment, thereby indicating that job satisfaction and work performance can be increased by improving their empowerment.
