전국적으로 균형있는 의료공급체계를 확립하기 위하여 정부가 건립 추진하여온 민간지역병원은 계획목표 년도가 2년이나 경과한 1984년 4월 현재 설립인가된 51개 병원중 38개 병원(74%)이 개원하였다. 이중 4개 병원은 심각한 재정난과 부실운영으로 이미 문을 닫은 실정이고 나머지 대부분의 병원들도 운영난에 봉착하였는바 이러한 민간지역 병원의 특성과 운영성과에 관련된 요인을 분석하기 위하여 각 병원별 실태조사표와 설문조사서 및 방문면담으로 개원중인 병원전체를 대상으로 자료를 수집,조사하였다. 각 병원의 운영성과에 미치는 요인중 가장 큰 것은 지역적 여건과 병원의 입지조건이었다. 병원운영이 다소 호전되는 병원일수록 의료수가나 진료미수금과 같은 병원외부요인에 관심을 갖는데 반해 악화되는 병원일수록 병원의 위치나 타병원과의 경쟁등 병원내부요인에서 더욱 많은 문제점을 느끼고 있었다. 또한 자력에 의한 운영자금 및 양질의 타인자본 조달능력이 중요한 것으로 지적되었다. 이러한 요인은 정부의 민간지역병원 건립계획에 대한 불충분한 검토와 병원들의 판단력부족 및 자금동원능력 부족과 관리운영미숙에도 문제가 있었다. 막대하게 소요된 시설자금 및 운영자금으로 인한 높은 부채비율로 인해 이를 감당할 만한 이자부담 능력도 없으며, 기존병원에 비하여 환자절대수마저 부족으로 정상운영을 기대할 수 없었다. 이와 같은 결과는 앞으로 병원신설을 할 때 기획단계부터 면밀한 검토가 필수적이라는 것을 시사해 준다.

급격한 정보기술의 발전에 따라 최근 중 대형병원중심으로 많은 예산을 병원정보시스템 구축에 투자하고 있다. 이렇게 구축된 병원정보시스템들에 대한 사용자 중심의 운영 및 유지보수서비스 수요가 지속적으로 증가되고 있다. 하지만, 병원정보시스템 유지보수는 사용자의 의견이 무시되고 운영자의 의지에 좌우될 수밖에 없었다. 따라서 본 연구에서는 병원정보시스템 운영 및 유지보수 현황을 파악하여 문제점을 도출하였고, 이러한 문제를 해결하기 위해 병원정보시스템의 유지보수를 위한 사용자의 문의 및 요청 사항을 웹상에서 실시간으로 확인 할 수 있는 도구를 소개하여 병원정보시스템 운영 및 유지보수 기능을 사용자 중심으로 개선하기 위한 지침을 제공하여 사용자 중심적인 시스템 운영, 신규 시스템 개발 시 사용자 참여 유도를 할 수 있게 하였다.

우리나라병원에 전산화가 도입된지도 어언 7여년이 넘었다. 그간 많은 비용이 소모됨에도 불구하고 병원전산화는 증대되는 업무의 효율화를 위해 꾸준히 병원관리,정보처리분야에서 운영.개발되어 왔다. 이제 지향하는 국민전체보험화시대에 따라 증대되는 병원관리 효율성 제고는 합리화를 추구하는 시대적 요청이기도 하여 점차 병원인의 많은 관심속에 전산화제도가 도입되어 운영되고 있다. 본 고는 이에따라 병원에서의 전산제도의 효율적 운영을 도모하고자 병원전산화에 관해 이모저모를 살펴보고자 한다.

본 고는 성남병원 최염이사장의 연세대학교보건대학원 학위논문으로 정부에서 국민의료의 균점을 위해 추진중인 민간지역병원운영실태의 전반적인 특성과 근본적 문제점을 중심으로 병원운영의 제도적 발전방향을 제시하고 있다. 본 연구서는 51개 민간지역병원중 현재 개원한 38개병원을 대상으로 실태조사표, 설문서 및 자문을 통한 자료에 의한 것으로 결론적으로 정부가 국민에 평등한 의료혜택을 부개하고자 설립한 민간지역병원은 시행 2차년도를 맞는 현실에 있어 도출된 문제점에 의해 운영난에 봉착하고 있음을 나타내고 있다. 이에 따라 많은 투자를 통해 건립된 민간지역병원의 성공적인 정착과 국민의료의 균점시혜를 위해서는 본계획의 면밀한 재검토와 이에따른 제도적 보완이 뒤따라야 한다며 이에 관한 세세한 문제점을 제시하고 있는데 본 지는 병원운영 발전에 도움을 주고자 이를 전호에 이어 게재하고자 한다.

컴퓨터라는 기계가 우리 생활의 주변에 나타나기 시작한 것이 그리 오래전의 일은 아니다. 처음 컴퓨터가 선을 보였을 때, 이 기계는 일반인들의 것이 아니었다. 일부의 아주 일부의 특별한 직종의 사람들만의 것이었다. 그러나, PC가 보급되면서 컴퓨터는 우리의 생활에 성큼 다가오기 시작했고, 우리의 생활 깊숙이 자리잡았다. 당장 동네 비디오 대여점에만 가도 컴퓨터로 회원 관리를 하고 있을 정도니까 말이다. 필자는 개인적인 컴퓨터 실력이 남들보다 뛰어난 것은 아니지만 컴퓨터를 좋아해서 병원 운영에 여러가지로 응용을 하고 있다. 부족하나마 지금까지 필자가 병원에서 컴퓨터를 활용해 왔던 경험과 그동안 생각해 왔던 동물병원 운영과 컴퓨터의 이용에 대해 이야기해보고자 한다. 글의 내용들이 현실과 다소 동떨어져 있다거나 딴세계의 이야기 같이 들리는 부분이 있을 수 있지만, 현재 가능한 이야기들이고 이미 이렇게 운영하는 병원들도 있다.

본 논문은 전국적으로 병원 전산시스템을 운영하는 근로자의 근무 조건들과 애로사항, 병원 전산시스템의 운영 특징들에 대해 조사하였다. 대전지역 보훈병원 전산 근로자들을 대상으로 설문조사한 결과 기술적, 환경적, 동료관계적 특징들에 대해서 파악하게 되었고, 이를 해결할 수 있는 방법으로 지식관리시스템 기술을 도입함으로써 효율적인 운영이 가능하다. 향후 연구를 확대해 대전지역 보훈병원뿐만 아니라 충남지역 일반병원까지 제안 연구기법을 적용함으로써 지식관리시스템 기술을 도입함으로써 병원 전산시스템의 운영 및 관리에 효율성을 높일 수 있음을 보인다.

The purpose of this study was to compare and analyze the differences before and after certification for internal members of nursing hospitals and Western hospitals that received the certification system of medical institutions after conducting a survey on 297 members of nursing hospitals and Western hospitals, SPSS 26.0 was used, and to understand the effect of improvement of member satisfaction, job stress, and medical service on the hospital operation effect. As for the hospital operation effect of a nursing hospital, it was revealed that the higher the satisfaction of the members and the improvement of the medical service, the higher the effect of the hospital operation. In the case of Western hospitals, the higher the satisfaction of the members and the improvement of the medical service, the lower the job stress, the more the hospital operation effect can be seen to increase. As the basic value system of the medical institution certification system is the improvement of patient safety and quality of care, it is believed that the effect of the certification system was higher than that of Western hospitals due to mandatory certification in nursing hospitals.

The purpose of this study was to classify the internal members of nursing hospitals and Western hospitals that received the certification system of medical institutions, and to examine the effect of certification on member satisfaction, job stress, hospital operation effect, and improvement of medical services. In nursing hospitals, the certification system was higher in member satisfaction, hospital operation effect, and medical service effect than in Western hospitals, and members of Western hospitals were higher in job stress. In both nursing hospitals and Western hospitals, the satisfaction of members, hospital operation effect, and medical service effect according to certification showed positive (+) results. It is believed that the effect of the certification system has been empirically high in nursing hospitals as mandatory certification from 2013. Therefore, this study aims to contribute to the efficiency of hospital management and improvement of patient satisfaction through continuous quality management of medical care by analyzing the effect of certification system on medical service improvement.

This study focused on the domestic hospital libraries, the policies and support projects for them, and the current operation status of them. To this end, a list of hospital libraries was established and the operation status was investigated by conducting telephone interviews with 73 libraries collected. As a result, it was found that hospital libraries were established through support projects of other institutions (foundations, enterprises, etc.) rather than by the parent institutions, most of which were barely maintained except for certain actively operated ones. In terms of space, hospital libraries equipped with independent space were mostly operated on a small scale, and bookshelves were deployed in space for other purposes such as lounge and hallway. In terms of operation, a dedicated department or personnel for the library operation hardly existed, and more than half of them did not purchase additional books, simply maintaining the donated books at the time of the establishment. It was also shown that for services and programs, only booklending services are provided, except for a few libraries. Therefore, national policies and support will be needed to revitalize hospital libraries in the future, and the government needs to seek ways to continuously operate them through links with local representative libraries and public libraries.

The purpose of this study was to grasp which effect the healthcare accreditation has on hospital employees' satisfaction level and hospital management performance by comparatively analyzing between accreditation hospital and non-accreditation hospital. As for difference in hospital employees' satisfaction level according to accreditation system, the incentive payment and pride of own task at the accreditation hospital and the hospital management effect were indicated to have positive(+) influence with statistically significant difference upon the item such as accuracy of task performance owing to the business standardization. It was indicated that hospital employees' satisfaction is high at the accreditation hospital and that the higher satisfaction level leads to having influence upon the effect of hospital management. What a country differentiates medical bill or supports medical equipment for medical institution that acquired accreditation is considered to be a plan for activation that can allow non-accreditation hospitals to be able to participate positively in the voluntary accreditation system.
