Along with the wide use of radiotherapy in cancer treatment, there is growing interest in beneficial effect of low-dose irradiation (LDI) in cancer therapy. Therefore, we investigate how LDI affects immune responses in mice model. Total body irradiation (TBI) on C57BL/6 mice was given at low-dose rate of $1mGy{\cdot}min^{-1}$ using $^{137}Cs$ source at three times for consecutive three days. Hematological examination, total cell numbers of spleen, populations and characteristics of splenocytes were determined. Total numbers of RBC or platelet in irradiated mice showed no significant changes. WBC counts were decreased in a dose-dependent manner 2 days after TBI, however, these differences are gradually waned until 28 days. Dose-dependent decrease in the number of splenocytes of TBI mice at day 2 was also improved as time progressed. While the level of Foxp3 mRNA was decreased, the frequency of $CD4^+$ T cells and $CD69^+$ cells in spleen was increased at day 2 and 14. Fractionated low-dose TBI on mice exhibited normal body weight with no distinguishable behavior during whole experimental periods. These results suggest that some parameters of immune system could be altered and evaluated by fractionated low-dose TBI and be used to broaden boundary of low dose radiation research.
Purpose: This aim of this study is to analyze the dosimetric difference between intensity-modulated radiation therapy (IMRT) using 3 or 5 beams and MSF in the radiotherapy of the left breast. Materials and Methods: We performed a comparative analysis of two radiotherapy modalities that can achieve improved dose homogeneity. First is the multistatic fields technique that simultaneously uses both major and minor irradiation fields. The other is IMRT, which employs 3 or 5 beams using a fixed multileaf collimator. We designed treatment plans for 16 early left breast cancer patients who had taken breast conservation surgery and radiotherapy, and analyzed them from a dosimetric standpoint. Results: For the mean values of $V_{95}$ and dose homogeneity index, no statistically significant difference was observed among the three therapies. Extreme hot spots receiving over 110% of the prescribed dose were not found in any of the three methods. A Tukey test performed on IMRT showed a significantly larger increase in exposure dose to the ipsilateral lung and heart than multistatic fields technique (MSF) in the low-dose area, but in the high-dose area, MSF showed a slight increase. Conclusion: In order to improve dose homogeneity, the application of MSF, which can be easily planned and applied more widely, is considered an optimal alternative to IMRT for radiotherapy of early left breast cancer.
The goal of a radiation treatment plan is to deliver a homogeneous dose to a target with minimal irradiation of the adjacent normal tissues. Dose uniformity is especially important for stereotactic radiosurgery using a linear accelerator. The dose uniformity and high dose delivery of a single spherical dose distribution exceed 70%. This also results with a similar stereotactic radiosurgical plan using a Gamma Knife. The dose distribution produced in a stereotactic radiosurgical plan using a Gamma Knife and Linear accelerator is spherical, and the application of the sphere packing arrangement in a real radiosurgical plan requires much time and skill. In this study, we found a characteristic of dose distribution with transformation of beam parameters that must be considered in a radiosurgical plan for effective radiosurgery. First, we assumed a cylinder type tumor model and a cube type tumor model. Secondly, the results of the tumor models were compared and analyzed with dose profiles and DVH_(Dose Volume Histogram) representative dose distribution. We found the optimal composition of beam parameters_(i.e. collimator size, number of isocenter, gap of isocenters etc.), which allowed the tumor models to be involved in the isodose curve at a high level. In conclusion, the characteristics found in this study are helpful for improving the effectiveness and speed of a radiosurgical plan for stereotactic radiosurgery.
Recently a variety of high technologies for radiation therapy (IMRT, SRS,. 3D-RT, etc.) has been developed. For the cervical and rectal cancer, 3field or 4field radiotherapy have been applied to the patients. In the case of two-dimensional treatment, one of the most typical side-effects is skin burn due to the radiation irradiation. In general the skin dose is evaluated by only a single measurement during the whole treatment period. In this study, however, skin dose was measured in each radiation treatment and the total skin dose was accumulated in a glass dosimeter through all the cases. After simulating the skin dose from treatment planning system, the results were compared with the actual skin doses. The results showed a good agreement between two data sets. Even though there are certain amount of errors caused by the patient movement along the treatment, the difference between actual dose and simulated dose was within the accepted range of error.
Dose limits are not applied to medical radiation exposure therefore justification and optimization should be essential for protecting radiation. This study explores methods to reduce exposure dose undergoing general radiation exam by bolus(tissue equivalent material) with keeping image quality. Hand PA projection with 50 kVp, 5 mAs, SID 100 cm, and 8×10 inch is referred by covered bolus of thickness 0, 3, 5, 8, and 10 mm for evaluation entrance dose and SNR. The entrance dose (μGy) to the hand by bolus thickness was 125.41±0.288, 106.85±0.255, 104.97±0.221, 91.68±0.299, and 90.94±0.106 showing a significant reduction in radiation exposure depending on if the bolus was used and bolus thickness. The SNR of the image was 13.997, 13.906, 12.240, 12.538, and 12.548 at each bolus thickness, showing no significant difference. It was confirmed that if appropriate thickness and size of bolus is used depending on the type of radiological imaging exam and the body site, a significant radiation dose reduction effect can be achieved without deteriorating image quality.
In its recent recommendation No. 60(1990), ICRP has newly introduced several terminology which had not existed in its prior recommendation No. 26(1977). Of these, a newly defined quantity 'Equivalent Dose' replacing the 'Dose Equivalent' of the ICRU concept has been recommended to be adopted in the radiation protection programme. However, since the committee still uses the 'Dose Equivalent' and 'Equivalent Dose' in its several publications, it is likely to provoke unnecessary confusions and misuses in applying these two quantities. In this paper were described the definition and difference between these two quantities to help in understanding of these two quantitites among the person involved in the radiation protection activities.
Recently, There has been a growing interests in exposure dose to the patient who take a examination using radiation. The radiological technologists should be concerned about the exposure dose to patients and make an efforts to reduce the patient dose without decreasing the image quality. In the case of foreign, the exposure dose of general X-ray examination have been managed by standard value of exposure dose using dose area product (DAP) and entrance surface dose (ESD) dosimeter. This study is to compare DAP and ESD in skull anterior posterior (AP), chest posterior anterior (PA), and abdomen AP projections of phantom by using DAP and ESD dosimeter. In the results, there were no differences between DAP and ESD dosimeter.
Radiosurgery treatment in the brain requires detailed information on three-dimensional dose distribution. A three-dimensional treatment planning is a prerequisite for treatment plan optimization. It must cover 3-D methods for representing the patient, the dose distributions, and beam settings. Three-dimensional dose models for non-coplanar moving arcs were developed using measured single beam data and efficient 3-D dose algorithms for circular fields. The implementation of three dimensional dose algorithms with stereotactic radiosurgery and the application of the algorithms to several cases are discussed.
