Gastrointestinal Foreign Bodies in Children -Experiences of 60 Cases in Kangwon, Korea- (소아 위장관 이물 -강원지역 소아 60례-)

  • Purpose: Ingested foreign bodies present a common clinical problem. It is well known that most of them pass uninterrupted through the gastrointestinal tract. We evaluated the role of endoscopy and Foley catheter for removal of foreign bodies in the gastrointestinal tract. Methods: We investigated retrospectively 60 cases with foreign bodies in the gastrointestinal tract. They had been treated at Wonju Christian Hospital, Yonsei University of Korea, from January, 1996 through December, 1999. Results: The age of the patients ranged from 7 months to 13 years. Patients under 5 years were 57 cases (97%) and there was no significant difference in sex (M : F=1.07 : 1). 45 cases of the patients had no symptom. The most common foreign bodies were coins (43 cases). The most common location was esophagus (31 cases). The number of foreign body removal using flexible endoscopy and Foley catheter was 22 (36.7%) and 18 (30.0%) cases, respectively. In 18 cases (30.0%), foreign bodies passed spontaneously. Only 1 case (1.7%), curtain pin impaction at ileocecal region, required surgery. Conclusion: Early foreign body removal from esophagus and stomach is recommended to lessen the morbidity and complication. Fluoroscopic foley catheter technique and flexible endoscopy for removal of esophageal foreign bodies in children is safe and effective.

Pose Transformation of a Frontal Face Image by Invertible Meshwarp Algorithm (역전가능 메쉬워프 알고리즘에 의한 정면 얼굴 영상의 포즈 변형)

  • In this paper, we propose a new technique of image based rendering(IBR) for the pose transformation of a face by using only a frontal face image and its mesh without a three-dimensional model. To substitute the 3D geometric model, first, we make up a standard mesh set of a certain person for several face sides ; front. left, right, half-left and half-right sides. For the given person, we compose only the frontal mesh of the frontal face image to be transformed. The other mesh is automatically generated based on the standard mesh set. And then, the frontal face image is geometrically transformed to give different view by using Invertible Meshwarp Algorithm, which is improved to tolerate the overlap or inversion of neighbor vertexes in the mesh. The same warping algorithm is used to generate the opening or closing effect of both eyes and a mouth. To evaluate the transformation performance, we capture dynamic images from 10 persons rotating their heads horizontally. And we measure the location error of 14 main features between the corresponding original and transformed facial images. That is, the average difference is calculated between the distances from the center of both eyes to each feature point for the corresponding original and transformed images. As a result, the average error in feature location is about 7.0% of the distance from the center of both eyes to the center of a mouth.

Comparison of Image Quality and Dose According to the Arm Positioning in the Chest CT (흉부 CT 검사에서 환자 팔의 위치에 따른 영상의 화질과 선량 비교)

  • The aim of this retrospective research was to investigate the influence of the patient's arm position on radiation dose and scanning during CT. Chest CT image created image degradation, artifact and overdose to the patient due to the difference of the chest thickness. Therefore, the patient's arm should up position during the CT chest examination. In 2012, 1,642 patients underwent chest CT examination in Seoul K hospital. 118 chest CT examination performed hands down position. The average DLP value of the CT chest arm up examination was 275 $mGy{\cdot}cm$. The average DLP value of the CT chest arm down examination was 312.46 $mGy{\cdot}cm$. In the retrospective study with same patient, The average DLP value of the CT chest examination arm up vigorously was 267.5 $mGy{\cdot}cm$. The average DLP value of the CT chest arm down examination was 307.5 $mGy{\cdot}cm$. Chest CT scan without raising arm created linear artifact due to the lack of X-ray photons which is the thickest part of the human body of shoulder area. In conclusion, arm positioning patients' arms above the shoulders at CT of the chest increases image quality and substantially reduces effective radiation dose.

The Kinetic Analysis of Arabesque Turn Motion in Modern Dance by Upper Extremity Usage (상지이용유무에 따른 현대무용 아라베스크 턴 동작의 운동역학적 분석)

  • This study puts the purpose in providing the scientific basis of dance motion as an artistic expression by analyzing the kinematic variable and the distribution factor of power affecting the motion, which is connected to the turn, right after the arabesque motion according to the existence and non existence of using the arm in the arabesque motion of modern dance. As a result of this study, arabesque turn motion, not using the upper limbs, used more turning force of head and body than the arabesque turn motion, using the upper limbs, and arabesque turn using the upper limbs obtained the turning force, using the right shoulder. The range of the hip joint on the left and the position change of left tiptoe in the Arabesque turn motion using the upper limbs is largely ascended to the vertical axis, while, the position of tiptoe in the Arabesque turn motion, not using the upper limbs is dropped to the lower part of each event. In the replacement of body center, Arabesque turn motion using the upper limbs is moved more to the turning axis than arabesque turn motion not using the upper limbs. As a result of maximum vertical ground reaction force, Arabesque turn motion using the upper limbs appeared to be a lower value than the Arabesque turn motion not using the upper limbs.

Development of Patient-Immobilizing Device for Total Body Irradiation (TBI) (전신 방사선치료(Total Body Irradiation, TBI)를 위한 한국인에 맞는 환자 고정장치에 관한 연구)

  • A immobilizing device that is essential for correct lung and lens shielding with homogenous dose distribution in fractionated total body irradiation was developed and it's efficiency was evaluated. The main frame was made of stainless steel bar (5 cm in diameter) to withstand up to 230 cm in height and 100 kg in weight to prevent any injury even in unconsciousness condition. The saddle was designed to adjust the body weight and hight of standing patients. Chest and back supporter were made of 1 cm acryl which could fix the lung block and cassette holder. Leather and sponge pedding were used for head rest to keep patients comfortable. The device was strongly fixed by specially designed bolts on the bottom panel which was made of 1 cm stainless steel and 10 cm thick wooden board. Precise manipulation ($\pm$2 mm) was possible by upper two pulleys and side handles. Average four minutes twenty five seconds were needed for exact setting in fractionated TBI. No significant difference of lung block location on repeated verification films was confirmed and relatively homogeneous dose distribution was measured in rando phantom experiments and patient treatments ($\pm$5%). This immobilizing device was very efficient to keep correct position of patients, which is essential for better result and less complication in fractionated TBI.

The Comparison of Wild Birds Movement between Eco-Corridor and Neighboring Crossing Road (생태통로와 주변도로에서 야생조류의 이동 비교)

  • We recorded the movement of wildbirds nine times at Hoam 1st Tunnel, Kkachisan Park, Deungneung pass to compare the movement between eco-corridor and neighboring crossing road from June to September, 2006. Among three areas, birds did not prefer the eco-corridor at pass type such as Kkachisan Park and Deungneung pass, however number of species and individuals were high at the eco-corridor at Hoam 1st Tunnel. Over 90m width and the slope location of eco-corridors can be beneficial for wild birds to enhance the movement along eco-corridors between isolated two patches. Average foliage volume under two meters showed the relation with bush nesters, and that from 7 to 8m had the highest relationship with canopy nesters. In Seoul city, target species should be considered at the construction of eco-corridors according to site characteristics, but Paradoxornis webbianus could be suggested as a main target species at eco-corridors. To enhance the movement of diverse wildbirds, we would implement that eco-corridors could be located at the slope area with the size of 1ha (over 90m width), eco-corridors could be planted and managed with the high foliage volume of shrub layer under twometers and canopy layer over 8 meters.

Spontaneous Passage of Gastrointestinal Foreign Bodies in Children (소아에서 위장관 이물의 자연 배출에 대한 경험적 고찰)

  • Purpose: Ingested foreign bodies are removed by endoscopy, surgery or spontaneous passage, however, the decision of therapeutic modality chosen depends on the type, size, shape and location of the ingested foreign bodies. Therefore, this study was conducted to investigate the rate and characteristics of foreign bodies that were passed spontaneously out of the intestine. Methods: One hundred and sixty patients who visited Samsung Medical Center for treatment of gastrointestinal foreign bodies between January 2001 to July 2007 were enrolled in this study. Related data was colleted by reviewing the medical records of patients with proven foreign bodies retrospectively, as well as by conducting phone interviews with the parents of the patients. All cases were classified based on the nature and location of the ingested foreign body, as well as whether it was treated by spontaneous passage. Results: Of the 160 cases involving the passage of foreign bodies in children were included in this study (95 boys and 65 girls), endoscopic removals, operative removals or spontaneous passages were conducted in 80, 3 and 77 patients, respectively. The spontaneous passage rates for each type of object were as follows; coins (36.5%), bead and baduk stones (83.3%), long and sharp materials (52.6%), magnets (69.2%) and disc batteries (50.0%). In cases involving round-shaped foreign bodies, such as coins, the diverse spontaneous passage rates were more diverse. When foreign bodies were stuck below esophagus, similar sized coins and baduk stones had spontaneous passage rates greater than 80% regardless of age. Conclusion: It is better for clinicians to wait for spontaneous passage to occur in cases involving coins or round-shaped foreign bodies that are located at or below the stomach.

Spawning Behavior and Early Life History of the Elegant Blenny, Omobranchus elegans (Pisces: Blenniidae) (앞동갈베도라치의 산란행동 유도 및 초기생활사)

  • This study was conducted to observe the spawning behavior and early life histroy of elegant blenny, Omobranchus elegans reared in the laboratory. The elegant blenny were caught at Dolsan lsland, Yeosu-si, Jeollanamdo from June in 2012. As a result of observeation, male fish attracted female continuously and guide lead to spawning site and clean the surface of inner pant in oyster shell or barnacle shell. Female left after spawning and male protected their eggs until they had hatched out. The fertilized eggs were spherical in shape (mean diameter: 1.06 mm; mean oil globule diameter: 0.24 mm) and transparent. Larvae hatched at 203 hrs 40 mins after fertilization at $25.5{\sim}28.5^{\circ}C$ (mean $27.0^{\circ}C$). The newly hatched larvae were 3.04~3.09mm(mean 3.06 mm, n=10) in total length and their mouth and anus were already opened. They began to eat rotifer and transformed to postarvae stage. 10 days after hatching postlarvae was measured 6.39~6.45mm(mean 6.42 mm, n=10) in total length. 60 days after hatching juvenile was measured 21.5~22.2 mm (mean 21.8 mm, n=10) in total length with dosal fin rays XI-22; ventral fin rays I-2; anal fin rays II-23; caudal fin rays 21; pectoral fin rays 13.

Skin Damage Sustained During Head-and-Neck and Shoulder Radiotherapy Due to the Curvature of Skin and the Use of Immobilization Mask (머리-목 그리고 어깨의 방사선 치료 시 피부곡면과 고정장치로 인한 피부손상연구)

  • The purpose of this study was to measure curvature contour skin dose using radiochromic film and TLD for a conventional open field. We also attempted to quantify the degradation of skin sparing associated with use of immobilization devices for high energy photon beams and to calculate the skin dose with a help of Monte Carlo (MC) simulation. To simulate head-and-neck and shoulder treatment, a cylindrical solid water phantom 11 cm in diameter was irradiated with 6 MV x-rays using $40{\times}40\;cm^2$ field at 100 cm source axis distance (SAD) to the center of the phantom. Aquaplastic mesh mask was placed on the surface of the cylindrical phantom that mimicked relevant clinical situations. The skin dose profile was obtained by taking measurements from $0^{\circ}$ to $360^{\circ}$ around the circumference of the cylindrical phantom. The skin doses obtained from radiochromic film were found to be 47% of the maximum dose of $D_{max}$ at the $0^{\circ}$ beam entry position and 61% at the $90^{\circ}$ oblique beam position without the mask. Using the mask (1.5 mm), the skin dose received was 59% at $0^{\circ}$ incidence and 78% at $80^{\circ}$ incidence. Skin dose results were also gathered using thin thermoluminescent dosimeters (TLD). With the mask, the skin dose was 66% at $0^{\circ}$ incidence and 80% at $80^{\circ}$ incidence. This method with the mask revealed the similar pattern as film measurement. For the treatments of the head-and-neck and shoulder regions in which immobilization mask was used, skin doses at around tangential angle were nearly the same as the prescription dose. When a sloping skin contour is encountered, skin doses may be abated using thinner and more perforated immoblization devices which should still maintain immoblization.

Achievement of Excavation of Gwiam(Turtle Rock) and Nakseojae Restoration in Bogil-do Yun,Seondo Wonlim (보길도 윤선도원림(명승 제34호) 낙서재지역 원형복원과 귀암(龜巖) 발굴의 성과)

  • This study regards a series of achievement on Wonlim(Garden) Cultural Properties Excavation Project, led by field of landscape architecture. It will mainly describe excavation results of Gwiam(龜巖: Turtle rock) and historical value of it in terms of Nakseojae(樂書齋) restoration in Bogil-do Yun,Seondo Wonlim(Scenic Sites, No.34). Gwiam(Turtle rock) was found 14.6m apart from the north of Nakseojae, and it covered with 10~15m topsoil tilted toward Nakseojae, The size of Gwiam, which was Granite, was 360cm length, 270cm width, 95cm high. The Edge of Gwiam's North west part was of triangular shape forming Turtle head. The back of the Turtle head was form of Tortoise-shell because of wide backboard with both side groove. The southeast part of Gwiam projected was Turtle's tail. This Granite was obvious Turtle shape artificially made, and there are less likely to relocate from place to place. This Turtle-shaped Gwiam is important landmark for Nakseojae, which is one of the four spiritual creatures written in Bogildoji(甫吉島識) and Gosanyugo(孤山遺稿) by Yunwi. According to Bogildoji, it is estimated that Gwiam were on the axis with Soeunbyung(小隱屛), Nakseojae and was buried when Yiguan(Gosan's grandson) reconstructed a building. Also, it was place for enjoying the moon. But, Even after three times excavation in Nakseojae, there was no way to identify further information regarding Gwiam, so it was a matter of mystification. As a result of this study, Gwiam is laid bare to light in at least 260 years, so it is good example for boosting importance of landscape architecture field and restoring Nakseojae. Furthermore, firm base-soil was discovered in 135m high Rock Mass below, so natural ground of Nakseojae can be estimated by this basis. To be conclusion, Preservation Process for Gwiam and Estimation Space through interpretation of four spiritual creatures(四靈) in Gosan's Poetry should be continue.
