고정성 가공의치를 지지하는 임프란트의 길이에 따른 임플란트 주위 골조직에서의 응력분산에 관한 광탄성 연구 (A PHOTOELASTIC STUDY OF STRESS DISTRIBUTIONS AROUND SURROUNDING BONE TISSUES OF IMPLANTS DEPENDING UPON THE LENGTH OF IMPLANT FIXTURES SUPPORTING FIXED BRIDGES)

  • 저자는 두개의 브로네마트 임플란트를 지대치로 하는 가공의치를 제작할 때 임플란트의 길이에 따른 임플란트 주위 조직의 응력분산양상을 평가하기 위하여 임플란트의 길이가 각각 10mm, 7mm, 5mm인 여섯가지의 실험모형을 제작하고 가공의치 중아부에 64kg의 수직하중을 가하여 2차원적 광탄성응력분석 실험을 행하여 비교 분석 해본 바, 다음과 같은 결론을 얻었다. 1. 여섯가지 실험모형 중 응력분산 양상은 실험모형 I (10mm, 10mm)일 경우가 가장 좋았다. 2. 근심과 원심에 같은 길이의 임플란트를 매식하였을 경우, 응력은 대체로 임플란트 표면적의 감소에 비례하여 증가하였다. 3. 근심과 원심에 다른 길이의 임플란트를 매식하였을 경우, 짧은 쪽 임플란트의 경부에 응력이 집중되었다.

PET/CT 영상에서의 치과재료에 의한 인공물에 관한 연구 (Evaluation of Artifacts by Dental Metal Prostheses and Implants on PET/CT Images: Phantom and Clinical Studies)

  • PET/CT 검사는 기술의 발전에 따라 감쇄보정 방법이 $^{68}Ge$이나 $^{137}Cs$등의 동위원소를 사용하지 않고, CT 기반의 감쇄보정 영상을 구현하여 검사 시간의 단축과 해부학적인 영상을 제공한다. 그러나 CT 기반의 금속에 의한 CT 영상의 인공물이 발생하여 감쇄보정된 PET 영상에 영향을 준다. 그러므로 본 연구는 임상 실험과 phantom 실험을 통해 금속 치과 임상 실험은 구강 내질환이 없는 40명의 환자(평균나이: $56{\pm}17$세)를 대상으로 하였으며, 치아에 치과보철을 매식한 환자 20명과 치과 임플란트를 매식한 환자 20명을 대상으로 PET/CT 검사를 시행했다. phantom 실험은 원형 phantom내에 치과보철과 치과 임플란트를 매식한 치아모형을 이용하여 PET/CT 검사를 시행했다. 분석방법은 같은 단층상의 PET/CT 영상에서 CT 영상의 CT 값과 PET 영상의 표준섭취계수 변화를 인공물의 영향이 없는 부분, 어둡게 인공물이 나타난 부분, 밝게 인공물이 나타난 부분, 세 부분의 관심영역을 설정하여 측정했고, 통계분석은 대응표본 t-test를 이용했다. 치아 보철을 매식한 실험에서 환자의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은 약 19.6% (p<0.05) 감소됐고, 밝게 인공물이 나타난 부분은 90.1% (p>0.05) 증가했다. phantom의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은약 18.1% 감소됐고, 밝게 인공물이 나타난 부분은 18.0% 증가했다. 치아 임플란트를 매식한 실험에서 환자의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은 약 19.1% (p<0.05) 증가됐고, 밝게 인공물이 나타난 부분은 96.6% (p>0.05) 증가했다. phantom의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은 약 14.4% 감소됐고, 밝게 인공물이 나타난 부분은 7.0% 증가했다. PET/CT 검사의 CT 기반 보정 영상 구현 시 금속 치과 재료로 인해 CT 값에 영향을 주어 PET 영상에서도 표준섭취계수에 영향이 미치는 것을 볼 수 있다. 그리고 치아 임플란트보다 치아 보철에서 과 보정이 된 것을 볼 수 있다. 특히, 임상 실험에서 치아 보철의 표준섭취계수가 어둡게 인공물이 나타난 부분이 19.6% 감소되어 나타난 것을 확인 할 수 있어 위 음성으로 오인할 가능성이 있다. 그러므로 금속 치아 보철이나 임플란트를 시행한 환자들의 PET/CT 검사 시 구강내 병변이 의심 된다면, CT 영상으로 보정이 되지 않은 무보정(non-attenuation correction) PET 영상을 확인하는 것이 정확한 진단에 도움을 줄 것으로 사료된다.

  • Ridge expansion osteotomy(REO) technique is a simple and more conservation method to widen a narrow alveolar ridge in the maxilla. This method is superior to drilling method in soft and narrow maxillary alveolar ridge and allows the surgeon to widen the ridge in routine office procedure. Therefore, it is the treatment of choice to implant the maxilla with narrow alveolar ridge. This article presents clinical cases and discusses the advantages, rationale and surgical protocol of REO technique.

  • $Medpor^{(R)}$(porous polyethylene) Surgical Implants are used for the augmentation or restoration of bony contour in craniofacial defects. The purpose of this study is to evaluate the ingrowth of soft tissue and bone after application in calvaria of rats. The experiment was carried out in 60 rats. The reflected periosteum was resutured after implantation of $Medpor^{(R)}$ as a experimental site, while in the calvarial bone the reflected periosteum resutured without implantation as a control site. The histologic examination was performed after 1-, 2-, 4-, 8-, 12-, 24-weeks implantation in calvaria of rats. I concluded that there was abundant ingrowth of soft tissue and bone without any adverse tissue response and that it shows good stability.

상악동거상술 및 임플란트매식에 의한 상악 구치부 수복에 관한 임상적 연구 (MAXILLARY SINUS ELEVATION PROCEDURES: A Report of Six Cases)

  • Methods for restoring the posterior portion of the maxilla with delayed and simultaneous sinus lift, composite graft and placement of TPS cylinder and screw type implants are presented. Sinus grafting is needed because of minimal remaining alveolar bone and supporting posterior maxillary portion, The composite graft material uses a combination of autogeneous bone from tuberosity, dimeneralized freeze dried allogenic bone and hydroxylapatite with saline. Since 1991, feb, 1 sinus graft with delayed implantation of 3 cylinder implants and 5 sinus grafts with simultaneous placement of 4HA coated screw typed, 8 TPS cylinder typed implants have been performed for 6 patients. None of the 15 restored implants have been lost, Temporization of prosthesis was done about 2-5 months before final prosthetic work, None of them shows any severe complication.

한국환자에 매식된 ITI 임플란트에 관한 조사 (An analysis of ITI implants placed in korean patients)

  • The aim of this study was to analyze various data of 120 non-submerged ITI implants placed in 64 korean adults. The data were retrieved from patients' charts and registered in the computer and coded for a statistical analysis. The results revealed that the most common type of edentulism was a distal extension case, and the largest number of implants were placed in the mandibular first molar position. Implants were placed more frequently in mandibular, posterior position than maxillary, anterior position, respectively. With respect to the implant diameter and length, an implant with 4.1mm in diameter and more than 10mm in length was the most common. Since a molar position was the most prevalent position for implant placement in korean patients, clinicians should be careful about implant diameter which can resist a strong occlusal force expected in the molar position. In addition, the distance to the mandibular canal and the floor of maxillary sinus should be considered to avoid possible damages, such as nerve injury and sinus perforation in selecting a proper implant length.

Medpor 매식후 치유과정에서 Methylprednisolone이 미치는 영향 (Effect of Methylprednisolone on the Healing Process after $Medpor^{(R)}$ Implantation in Rats)

  • The purpose of this study is to evaluate the difference in healing of $Medpor^{(R)}$ implants of two different thickness (1.5mm and 4.5mm) and the effects of methylprednisolone on the healing process. Light microscopic and scanning electron microscopic examinations, and hardness measurement were made in 100 rats 2, 4, or 8 weeks postoperatively. The 1.5mm thin implants were taken better than the 4.5mm thick implants. The inflammatory responses were reduced after application of methylprednisolone with arrangement and amount of bone matrix deposited being more irregular and reduced in bulk than in the control group. To hasten postoperative healing after $Medpor^{(R)}$ implantation, the thinner material should be used and steroid injection should be avoided.

  • This study was undertaken to access the effect of toothash combined with plaster of Paris in the filling of jaw defect and the substitution as new bone during the follow up period. We used the toothash and plaster after the cyst enucleation, the apicoectomy, the extraction of supenumerary tooth with ratio of 2 : 1 by weigh. 15 consecutive patients were evaluated retrospectively. Complications were swelling, perforation, infection and treated without problems using incision & drainage, aspiration, antibiotic treatment, 2ndary buccal flap. The follow-up period ranged from 28 to 35 months. Based on radiographic and clinical observation, it may be concluded that toothash and dental plaster of Paris($CaSo_4\;{\cdot}\;1/2H_2O$) are useful for bone substitute.

임플란트 매식부의 고경 평가 : 파노라마 촬영법과 나선형 전산화 단층촬영법의 계측 비교 (Bone height measurements of implant sites : Comparison of panoramic radiography and spiral computed tomography)

  • Purpose: To compare the bone height of implant sites measured using panoramic radiography and spiral CT. Materials and Methods : The available bone height was determined for 263 maxillary and mandibular implant sites in 59 patients. Distortion was calculated using the metal bar for the panoramic radiographs. Results: Significant differences in mean bone height between the two imaging modalities were found in maxillary and mandibular anterior regions (p<0.05). The mean difference in bone height recorded by the two techniques was smallest in the maxillary and mandibular molar areas (0.8 mm), and greatest in the mandibular anterior region (1.3 mm). With the exception of the mandibular anterior region, ninety percent of all the sites showed measurement differences within 2 mm. Conclusion: A safety margin of 2 to 3 mm is called for when utilizing panoramic radiography, otherwise additional imaging modality such as computed tomography is necessary to obtain accurate measurements.

  • This paper reports histologic and histomorphometric results concerning bone healing around vent area of 16 titanium plasma sprayed IMZ implants in rabbit femurs. Bone contact around the implants 8 weeks after placement showed a great deal of variability from 14.32% to 65.94% and mean total bone contact was 33.96%. The mean percent amount of bone contact was 43.68% in inner surface of vent and 27.79% in outer surface. Histologic examination of horizontal sections of vent area showed direct contact with living lamellar bone and some woven bone. The implants surface area not in contact with bone was in contact with collagenous connective tissue. But there was no sign of foreign-body reaction.
