The Effects of Kyongrak Massage in the Elderly with Chronic Pain (경락마사지가 만성 통증 노인에게 미치는 효과)

  • Purpose: this study was implemented to develop new nursing intervention, Kyongrak massage, and to examine its effect on chronic pain in elderly women. Method: Kyongrak massage is a manipulation massaging on Kyonghyul site, and Duboo, Kyongchoo, Chucksoo, Hajee, Sangjee Kyongrak massages were used in this study. The subjects(11persons) took Kyongrak massage on head, neck, spinal cord, upper extremities and back lower extremities for 25minutes daily during 5days. The dependent variables were subjective health state, paid, blood pressure, pulse and body temperature. This study was carried out, at 10-12 o'clock, from August 7th, to 22th, 2000. Data were analyzed using frequency, percentage, mean, standard deviation, paired t-test by SPSS PC. Results: subjective health state((t=-3.560, p=.005), body temperature(t=-2.557, p=.029) were increased, and pain level(t=7.884, p=0.000), systolic blood pressure(t=2.923, p=.015) were decreased significantly by Kyongrak massage. Conclusions: The above results have informed us that this Kyongrak massage program(Duboo, Kyongchoo, Chucksoo, Hajee, sangjee) for 25minutes is a useful nursing intervention to decrease chronic pain in each life styles.

Factors Influencing Suicidal Ideation in Community-dwelling Patients with Chronic Mental Health Problems (지역사회 만성정신질환자의 자살사고에 영향을 미치는 요인)

  • Purpose: The study was performed to identify factors influencing suicidal ideation in patients with chronic mental health problems and who live in the community. Methods: Data were collected using questionnaires and participants were 140 people with mental illness living in Y-si, Gyeonggi-do. The study was carried out from May 15 to September 30, 2014. Data were analyzed using descriptive statistics, Pearson Correlation Coefficients, and Logistic Regression with the SPSS/WIN 18.0 statistical program. Results: The number of persons who attempted suicide was 47(33.6%) and the number who had suicide ideation was 22(15.7%). The mean score for depression was $6.4{\pm}6.45$. The mean score for life satisfaction was $16.5{\pm}8.21$. Depression had a significant negative correlation with life satisfaction. Odds ratio of suicide ideation was 1.19. The significant factor influencing suicidal ideation was depression. Conclusion: The results of this study can be used in the development and practice of programs to decrease suicide in people with mental illness.

The Effect of Headaches and Quality of Life with the Combined Western-Oriental Medicine Massage Program for Chronic Headache Patients (양.한방 혼합 마사지요법이 만성두통 환자의 두통 및 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of combined Western and Oriental medicine headache massage program for chronic headache patients. Methods: This study was designed using the Nonequivalent control group pretest-posttest design. The forty nine patients were divided into an experimental group (25 patients) and a control group (24 patients). Collected data were analyzed by Chi-test, t-test, Fisher's exact test, Repeated measures ANOVA using SPSS/WIN 14.0 program. Results: After treatment in this program, the experimental group had no significan difference in scores for headache intensity and frequency as the length of time in treatment increased compared to the control group. But there were significant decreases in the use of medication (F=4.209, p=.046) and disturbances in the quality of life related to headaches (F=13.097, p=.001) after providing the combined Western-Oriental massage program. Conclusion: This program could have a positive effect on using medication for pain controllers and headache related quality of life. Based on the results of this study, further research focusing on the effectiveness of unique nursing intervention for chronic headache patients is suggested.

Factors Influencing Health related Quality of Life among Patients with Chronic Hepatitis B (만성 B형 간염 환자의 건강 관련 삶의 질과 영향요인)

  • Purpose: The aim of this study was to investigate the factors contributing to health-related quality of life (HRQOL) among patients with chronic hepatitis B (CHB). Methods: A cross-sectional, descriptive design was used. The sample included 114 patients in a gastroenterology outpatient department at one hospital located in G city. Data were collected using a structured questionnaire from June to December 2009. The collected data were analyzed using SPSS/WIN 15.0. Results: Health-related quality of life was most strongly related to age, sex, income, occupation, and the care provider. Old age, female gender, lower income, not having an occupation and having a non-medical person were associated with lower HRQOL ($R^2$=.03~.22, p=.041~<.001). Conclusion: CHB infection had a negative impact on HRQOL among the older, female gender, low socioeconomic status and not having medically knowledgeable care givers. Interventions beyond nursing care may be needed to improve the quality of health for patients with Chronic Hepatitis B.

The Relationships of Illness Intrusiveness and Quality of Life in Chronic Liver Disease Patients (만성 간 질환자의 지각된 질병장애정도와 삶의 질과의 관계)

  • Purpose: This study was performed to identify the illness intrusiveness, quality of life, and their relationships in patients with chronic liver disease. Method: Data was collected using scales of illness intrusiveness developed by Devins et al and Korean health related quality of life (KoQoLS) developed by Shim et al to measure the illness intrusiveness and quality of life from 141 chronic liver disease patients. Result: The total mean score of illness intrusiveness was $38.85{\pm}18.45$. The domain of illness intrusiveness which showed the highest mean score was health ($4.60{\pm}2.02$), and the lowest mean score domain was relationship with spouse ($20.4{\pm}1.69$). The subcategory of KoQoLS which showed the highest mean score was bodily pain ($6.96{\pm}2.98$), and the lowest mean score subcategory was vitality ($2.75{\pm}1.20$). There were negative relationships between illness intrusiveness and KoQoLS in all subcategories. Abovel all, Illness intrusiveness had the strongest negative relationship with role limitation (r=-0.641) among the KoQoLS subcategories. Conclusion: Because the illness intrusiveness had negative influence on the quality of life in patients with chronic liver disease, further research will need to specify detailed illness intrusiveness and to explore influencing factors on quality of life in them.

The Lived Experience of Caregivers of Chronic Renal Failure Patient (만성신부전환자의 주보호자로 살기)

  • Purpose: The research aims to understand the lived experience of the caregivers of chronic renal failure(CRF) patients and its essential meaning. The results of the study can be used as basic materials for developing comprehensive intervention methods of care givers of CRF patients. Method: The research used van Manen's hermeneutic and phenomenological research methods in order to describe the lived experience and to understand its meaning. It concentrates on the understanding the essence of experience and consists of existential survey, hermeneutic and phenomenological reflection and hermeneutic writings. Participants in this research were five women care givers of CRF patients who had hemolysis at C university hospital in a metropolitan city, the period of data collection was from July 27 to Sep. 4, 2004 and major data of results in the following 5 essential themes. "sole responsibility for the patient enduring everything", "creating their own field", "heavy and painful life without hope of their private life", "wishing not to be inherited and consoling each other". Conclusion: The above findings point out that the experience of care givers of CRF patient affected and changed all parts of an individual life and his or her family life. Therefore, it suggests that total family nursing care must be considered in order to provide the holistic caring for CRF patients and their care givers.

A Study on Characteristics of Dyspnea in Patients with Chronic Obstructive Pulmonary Disease in the Emergency Department Visits (응급실에 내원한 만성 폐쇄성 폐질환 환자의 호흡곤란 속성에 관한 연구)

  • Purpose: The purpose of this study was to examine descriptors of dyspnea and clinical characteristics in patients with COPD at ED visit. Method: The study design was a descriptive survey, and 46 patients with COPD complaining of dyspnea were participated during their visits to the ED of C university hospital in G city, from October 1, 2004 to April 8, 2005. Result: The qualitative characteristics of dyspnea at the time of decision to the ED visit were short of breath (71.7%), tight (32.6%), hard to breathe (15.2%), smothering or suffocating (10.9%), gasping (8.7%), couldn't breathe (6.5%), constricted (2.2%), and hunger for air (2.2%). In the item of dyspnea checklists, My chest felt tight was the most common description among subjects. As the result of factor analysis of dyspnea checklists, the first factor was characterized rapid and shallow, the second was suffocating/smothering, the third was hunger for air, and the fourth was constricted. Conclusion: This study suggests that a checklist of dyspnea descriptors based on this findings would be utilized as a tool of initial and ongoing assessment for dyspneic patients with COPD in the ED after identifying the validity and reliability of the checklist.

The Experience of Life Experiences among Patients with Chronic Low Back and Extremity Pain (만성 요$\cdot$하지통 환자의 생활세계 경험)

  • Purpose: The purpose of this study was to understand the meaning and nature of pain experienced among patients with chronic pain. The present study adopted a hermeneutic phenomenological method which was developed by van Manen. Method: The participants for this study were 4 men and 5 women, who were over the age of 20 with chronic pain more than 6 months. Data was collected by using in-depth interviews and observations from September, 2004 to December, 2004. The contents of the interviews were tape-recorded with the consent of the subject. Result: The essential themes that fit into the context of the 4 existential grounds of body, time, space and other people were 'untamed and unremitting pain', 'the body as an obstacle', 'continuity of suffering time as if the moment would never end', 'a narrow radial range of action' and 'separating from other people'. Conclusion: Patients with chronic pain experienced and perceived the world through the filter of their pain. It is necessary for nurses to understand the experiences of chronic pain patients and to provide more empathic, supportive care. Further research is needed on nursing interventions that could help chronic patients cope with and find the meaning in their suffering.

The Study on the Effects of a Respiratory Rehabilitation Program for COPD Patients (만성 폐색성 폐질환자를 위한 호흡재활 프로그램 개발 및 효과에 관한 연구)

  • It is known that a pulmonary rehabilitation program improves dyspnea and exercise tolerence in patients with chronic obstructive pulmonary disease. However, it is also known that although it does not improve pulmonary function. This study was performed to evaluate the effect of a 4 week pulmonary rehabilitation on pulmonary function, gas exchange, and exercise tolerance in patients with chronic obstructive pulmonary disease. The pulmonary rehabilitation programs included breathing exercises, such as pursed-lip breathing and diaphragmatic breathing, upper-limb exercises, and inspiratory muscle training. These activities were performed for 4 weeks in twenty one patients with chronic obstructive pulmonary disease. Pre and post-rehabilitation pulmonary function and exercise capacities were compared after the 4 week period. Results are as follows: 1) Before the rehabilitation, the predicted value of FVC and FEV1 of the patients were 70.3$\pm$16.7% and 41.1$\pm$11.9% respectively. These pulmonary functions did not change after pulmonary rehabilitation. 2) Aloility of walking a 6 minute distance (325.29$\pm$122.24 vs 363.03$\pm$120.01 p=.01) and dyspnea (p=.00) were significantly improved after rehabilitation. Thus showing that pulmonary rehabilitation for 4 weeks can improve exercise performance and dyspnea in patients with chronic obstructive pulmonary disease.

A Structural Model for Symptom Management of the Patients with Chronic Fatigue (만성피로 환자의 증상관리 구조모형 구축)

  • Purpose: This study was designed to construct a structural model for symptom management of life of the patients with chronic fatigue. The hypothetical model was developed based on the literature review and Self-regulating Model. Method: Data were collected by questionnaires from 252 patients with chronic fatigue in the 8 community from December 2002 to April 2003 in Seoul. Data analysis was done with SAS for descriptive statistics and PC-LISREL Program for Covariance structural analysis. Result: The fit of the hypothetical model to the data was moderate, thus it was modified by excluding 4 path and including free parameters and 3 path to it The modified model with path showed a good fitness to the empirical data($x^2$=318.11, p=0.0, GFI=.98, AGFI=.98, NNFI=.95, RMSR=.03, RMSEA=.05). The symptoms of stress, self-efficacy, and present fatigue level were found to have significant direct effect on symptom management of the patients with chronic fatigue. The ways of coping, perceived stress, and fatigue symptom were found to have indirect effects on symptom management of the patients with chronic fatigue. Conclusion: The derived model is considered appropriate in explaining and predicting symptom management of the patients with chronic fatigue. Therefore, it can effectively be used as a reference model for further studies and suggested direction in nursing practice.
