  • Kawasaki disease is an acute febrile vasculitis that occurs predominantly in young children under 5-years-old. The patients present generally with a high spiking fever that is unresponsive to antibiotics and lasts for more than five days at least. Prolonged fever has been shown to be a risk factor in the development of coronary artery disease. It seems to be certain that infectious agents are associated with the pathogenesis of Kawasaki disease. The differential diagnosis of Kawasaki disease must rule out infectious diseases including scarlet fever, toxic shock syndrome, measles, and so on. This is very important for adequate treatment and prevention of cardiac complications of Kawasaki disease. We experienced a 25-month-old boy who had high fever and pneumonic consolidation in the right middle and lower lobe of the lung that was considered as mycoplasma pneumonia on admission and developed coronary artery aneurysmal dilatation during treatment with roxythromycin.

  • Erythema nodosum (EN) is a painful skin disease characterized by erythematous tender nodules located predominantly over the extensor aspects of the legs. Various etiological factors, including infection, drug administration, and systemic illness have been implicated as causes of EN. Mycoplasma pneumoniae is one of rare infectious agents to cause EN in children. We report a case of a 7-year-old boy with context of respiratory illness and skin lesions with arthralgia. From stepwise approaches, IgM antibody against M. pneumoniae was positive with titers of 12.18, consistent with respiratory infection of M. pneumoniae and histopathology showed findings of septal and lobular inflammation without vasculitis consistent with EN. In addition, we reviewed the pathogenesis of this disease based on our case and the previous reports.

  • Today swine respiratory disease is one of the most important diseases because of its economic losses and severe infection nationwide, and swine society as well as veterinary service are trying to prevent the diseases in Korea. This study would like to obtain some information useful for the control of the diseases. A total of 174 lung specimens with lesion consisted of 3 sorts; 60 were collected from nursey pigs requested for diagnostic service from March of 2006 to October of 2007, 58 finishing pigs and 56 sows were selected from slaughterhouse from September to November 2007. In the detection test of pathogens by PCR, porcine circovirus 2 (PCV2), porcine reproductive and respiratory syndrome virus (PRRSV) and Mycoplasma hyopneumoniae were positive in 95.4%, 31.6%, and 20.1%, respectively. Double infection rate with PCV2 and PRRS was 30.4%, PCV2 and M hyopneumoniae was 19.5%, triple infection with PCV2, PRRS and M hyopneumoniae was 5.7%, respectively.

  • Seasonal incident rates were examined from the 981 pigs slaushtered in southern Kang-won and distribution of lesions and histopathological examination were conducted from 231 Enzootic pneumonia affected lungs. The results obtained were summarized as follows: 1. 39.7∼50.2% of 981 slaughtered pigs showed pneumonia lesions without seasonal variation and the incidence rate of Enzootic pneumonia was 23.5% 2. The distribution of Lesions of Enzootic pneumonia lobes was observed. The right lobes were affected more frequently than the left, and the highest frequency of 79.7% being recorded in the right cardiac lobe followed by the left cardiac, right apical, intermediate, left apical, right diaphrogmatic and diaphrogmatic. 3. In histopatological observation lung lobes were forming Iymphonodulus from perivascular, peribronchiolar Iymphoid hyerplasia and it was remarkable to bronchostenosis.

  • Mycoplasma pneumoniae (M. pneumoniae) is the leading cause of pneumonia in older children and young adults. Normally, it does not progress to a condition requiring hospitalization but improves spontaneously or has a mild clinical course. We report two cases of M. pneumoniae pneumonia with different clinical manifestations from the normal course. The patients were young healthy individuals. The diagnoses were made by serology. However, it could not be determined beforehand that they had M. pneumoniae pneumonia. Based on the empirical treatment strategy of severe community acquired pneumonia, the patients were treated with broad-spectrum antibiotics including cephalosporin, quinolone and macrolide. After administering the antibiotics, they showed a gradually favorable clinical course and recovered without residual complications. A M. pneumoniae infection should be considered as a cause of severe community acquired pneumonia, and empirical treatment targeting this organism might be helpful in treating patients with the severe manifestation.

  • Mycoplasma pneumoniae is a common cause of respiratory tract infections. M. pneumoniae infection frequently manifests with extrapulmonary symptoms such as central nervous system complications, skin or mucosal involvement, and gastrointestinal problems. However, cardiac complications associated with M. pneumoniae are rarely reported. We report the case of a 47-month-old girl who died of fulminant myocarditis associated with M. pneumoniae pneumonia.

C-reactive Protein Level in a Variety of Infectious Diseases (감염성 질환들에서 C-반응 단백 값)

  • Purpose : We evaluated the C-reactive protein(CRP), white blood cell(WBC) and neutrophil levels in the various infectious diseases in a single hospital. Methods : A total of 640 medical records of children with infectious diseases such as bacterial meningitis(19 cases), acute pyelonephritis(55 cases), measles(253 cases), chicken pox (38 cases), mycoplasma pneumonia(160 cases), tsutsugamushi disease(39 cases) and Kawasaki disease(152 cases) admitted to The Catholic University of Korea, Daejeon St. Mary's hospital from 1996 to 2002 were retrospectively analyzed. Results : The mean CRP level was $17.9{\pm}6.4mg/dL$ in bacterial meningitis, $9.1{\pm}5.6mg/dL$ in Kawasaki disease, and $8.1{\pm}3.3mg/dL$ in acute pyelonephritis. In the mycoplasma pneumonia and tsutsugamush disease group(atypical bacterial group), the CRP level was $3.2{\pm}2.5mg/dL$, and $1.0{\pm}0.8mg/dL$ in the viral diseases group(measles and chicken pox). There were also significant differences for the WBC count and neutrophil differential between the 3 infectious groups with higher level in the bacterial infections group($15,600{\pm}6,100/mm^3$, $62{\pm}21%$) than in the atypical bacterial infections and in the viral infections group($9,600{\pm}3,300mm^3$, $57{\pm}11%$ and $7,300{\pm}2,900/mm^3$, $49{\pm}16%$, respectively). The inflammatory indices in Kawasaki disease were like those of bacterial infections. There was a correlation between CRP level and WBC or neutrophil count in the bacterial infections and Kawasaki disease groups. Conclusion : The CRP, WBC and neutrophil levels showed a clear difference between the infectious diseases according to causative agents. The WBC and neutrophil level was different according to age in measles and mycoplasma pneumonia. There was a correlation between CRP level and WBC or neutrophil count in the bacterial infections and Kawasaki disease groups.

  • Mycoplasma pneumoniae produces illness in man ranging from mild upper respiratory tract infection to severe bronchitis and pneumonia. We experienced a case of mycoplasma pneumonia complicated with acute respiratory failure, cold agglutinin hemolytic anemia, pleural effusion, Raynaud's phenominon and hepatitis in 27-year-old female. She was diagnosed as having mycoplasma pneumonia by detecting mycoplasma antibody and cold agglutinin test and treated effectively with erythromycin.

Evaluation of vaccination against Mycoplasma hyopneumoniae in pigs at different vaccination time-points (돼지 마이코플라즈마 폐렴 예방백신의 접종시기별 방어효과에 관한 연구)

  • This study was performed to evaluate the effect of commercial swine mycoplasma vaccine (M+$Pac^{(R)}$, Schering-Plough Animal HealthCo.) at different vaccination time points. Total 139 piglets were moved to experimental farm after weaning and were randomly allocated to 3 treatment groups and 1 control group. Piglets of 3 treatment groups (A, B and C) were vaccinated twice at 1 and 3 weeks (A group), 3 and 5 weeks (B), and 6 and 8 weeks (C), respectively. All vaccinates showed higher antibody titers compared with nonvaccinates, and maintained to finish (P<0.001). Performance and RBC/WBC count showed no significant difference between groups. Vaccinates at 6-8 weeks showed better effect on decrease of clinical sign compared with vaccinates at 1-3 weeks and 3-5 weeks and nonvaccinates. Also, lung lesion assessment showed significant difference between vaccinates at 3-5 weeks and 6-8 weeks, and nonvaccinates and vaccinates at 1-3 weeks (P<0.05). Vaccinates at 3-5 score in comparison with other groups. In conclusion, these results suggested that vaccination at 3-5 weeks or later may be more effective than earlier vaccination.

The Relationships Between Mycoplasmic Pneumonia and Production Traits in Pigs (돼지의 마이코플라즈마성 폐렴과 경제형질간의 상관관계 연구)

  • Pneumonia is one of the important diseases in commercial pig. In order to delineate the relationships with pneumonia and productions traits, five different economic traits including carcass weight(CW), backfat thickness(BF), meat quality(MQ), carcass characteristics(CC) and pork belly thickness(PB) were measured in 6,362 commercial pigs. Significant differences were observed in all the traits with pneumonia(P<0.01). Carcass weight and backfat thickness decreased significantly as the symptom of pneumonia was severed (P<0.01). Also, when the pneumonia was severed, the carcass characteristics and meat quality became worse compared with normal unaffected animals(P<0.01). In case of pork belly thickness, the significant decrease was observed in pneumonia infected animals compared with normal animals(P<0.01). Based on this study, the pneumonia infection affects most of the important economic traits in pigs and it is ultimately needed to pay more attention to prevent this disease.
