  • A beautiful smile has a great impact on the quality of life. It gives you confidence, and makes you function with a positive attitude, which will eventually bring the joy and happiness to daily lives. As dental practitioners, our mission is to bring back the bright smile of the people who come to see us. Restoring masticatory function is one thing but one should always remember that the esthetics should always come first in any kind of restorative procedures. When treating complex rehabilitation cases, treatment planning is probably the most important step before you start any dental procedures. Obviously, esthetics should be the ultimate goal of the treatment and one should approach a case step by step to achieve an optimal result. For the past few years, "digital smile design" has been introduced in the field of dentistry. It can be applied by using various methods and tools. In this article, a very basic form of 2-dimensional digital smile designing was utilized to treat a complete mouth rehabilitation case. We will discuss how the DSD concept can be applied to your daily practice with a simple photograph and photoshop.

Satisfaction practiceof the practice conditions characteristic for Dental Hygiene(department) major (치위생(학)과 전공에 대한 실습여건 특성에 따른 실습만족도 -대구, 경북지역을 중심으로-)

  • In order to examine the characteristics having influence on the satisfaction with clinical practice for the effective clinical practice, this study analyzed the satisfaction with clinical practice in accordance with the characteristics of major practice conditions, targeting 228 students of Dental Hygienics with experience in clinical practice in Daegu and Gyeongbuk region. In case of 4-year major practical conditions, high satisfaction with major, when students could choose the hospital for practice, and when there were lots of dental hygienists in the hospital for practice, the satisfaction with clinical practice was high(p<.05). In case of 4-year course, high satisfaction with major, and when there were lots of dental hygienists in the hospital for practice, the satisfaction with clinical practice evaluation was high(p<.05). In case of 4-year major practical conditions, high satisfaction with major, dental hospital, when students could choose the hospital for practice, and when there were lots of dental hygienists in the hospital for practice, the satisfaction with interpersonal relation was high(p<.05). In case of 4-year course, high satisfaction with major, and when students and school could choose the hospital for practice, the satisfaction after practice was high(p<.05).

Influence of finish line design on the marginal fit of nonprecious metal alloy coping fabricated by 3D printing, milling and casting using CAD-CAM (CAD-CAM을 이용한 3D printing, milling, casting 방법의 비귀금속 코핑의 지대치 변연 적합도 연구)

  • Purpose. The purpose of this study was to examine the correlation between the finish line designs and the marginal adaptation of nonprecious metal alloy coping produced by different digital manufacturing methods. Materials and methods. Nonprecious metal alloy copings were made respectively from each master model with three different methods; SLS, milling and casting by computer aided design and computer aided manufacturing (CAD-CAM). Twelve copings were made by each method resulting in 72 copings in total. The measurement was conducted at 40 determined reference points along the circumferential margin with the confocal laser scanning microscope at magnification ×150. Results. Mean values of marginal gap of laser sintered copings were 11.8 ± 7.4 ㎛ for deep chamfer margin and 6.3 ± 3.5 ㎛ for rounded shoulder margin and the difference between them was statistically significant (P < .0001). Mean values of marginal gap of casted copings were 18.8 ± 20.2 ㎛ for deep chamfer margin and 33 ± 20.5 ㎛ for rounded shoulder margin and the difference between them was significant (P = .0004). Conclusion. Within the limitation of this study, the following conclusions were drawn. 1. The variation of finish line design influences the marginal adaptation of laser sintered metal coping and casted metal coping. 2. Laser sintered copings with rounded shoulder margin had better marginal fit than deep chamfer margin. 3. Casted copings with deep chamfer margin had better marginal fit than rounded shoulder margin. 4. According to the manufacturing method, SLS system showed the best marginal fit among three different methods. Casting and milling method followed that in order.

  • 최근 심미적인 보철 치료에 대한 사람들의 관심이 높아짐에 따라 자연치와 더욱 조화롭게 보철 치료를 하는 것이 요구되고 있다. 이는 구조적인 심미성과 함께 자연스러운 색조 재현을 통하여 이루어지는데, 자연치의 색조는 매우 다양하고 부위별로 달라질 수 있기 때문에 단순히 shade guide상의 비교만으로는 정확한 색조를 얻기가 어렵다. 이러한 색조 선택의 부정확성 때문에 객관적으로 색조를 분석하는 많은 기계적 장치들이 고안되었고, 최근에 ShadeScan system(CYNOVAD, Canada), Vita Easyshade system(Vident, Canada) 등 디지털로 색조를 평가하는 시스템이 각광을 받고 있다. ShadeScan system은 한 번의 클릭으로 치아 이미지를 채득할 수 있고, 바로 컴퓨터를 통해서 이를 분석하여 원하는 shade map, translucency map 등을 얻을 수 있다. 또한 세 방향에서 조사되는 빛은 자체 calibration을 거쳤기 때문에 주변 광원이나 환경의 영향 없이 객관적으로 치아의 이미지를 채득할 수 있다고 한다. 사실 그 동안 이미지 채득이 객관적이라고 주장하는 많은 system이 있었지만 임상의들은 그 신뢰성에대해 의구심을 보였던 것이 사실이다. 이에 본고에서는 ShadeScan system이 반복 촬영 시 같은 색조재현을 할 수 있는지 실험하여 그 객관성에 대해 살펴보았다.

Computer-aided design and manufacturing-based full mouth rehabilitation for a patient with excessive attrition and restricted vertical dimension: A case report (심한 치아 마모와 수복 공간 부족을 보이는 환자에서 CAD/CAM 기술을 활용한 완전 구강 회복: 증례 보고)

  • This study reported the treatment of a patient with excessive worn dentition and limited maxillo-mandibular space for restoration, utilizing the computer-aided design and computer-aided manufacturing (CAD/CAM) technology. After the thorough examination of the patient's occlusal vertical dimension (OVD), full mouth rehabilitation was planned with increase of the OVD. The patient was satisfied with the provisional restorations establishing the increased OVD. The horizontal and vertical data of the patient's jaw relation that the provisional restorations contained were transferred to the definitive metal ceramic fixed prostheses by double scanning and three-dimensional printing. After the fixed restorations were cemented to the abutments, electronic surveying and three-dimensional printing were used to fabricate metal frameworks for the patient's removable partial dentures. The mandibular definitive removable prostheses were delivered to the patient's mouth and the full mouth rehabilitation procedures were completed. The digital technologies used for this case produced fixed and removable restorations satisfactory in masticatory, phonetic and aesthetic functions to both the patient and the dental clinician.

  • Computer-aided design-computer-aided manufacturing technology has been widely used in the manufacture of fixed prostheses including implants, but in the case of removable dentures, the analog method is still being used due to the errors such as a lack of fusion and over-fusion in selective laser meting process. With the recent development of CAD software, virtual surveying and framework design are made possible, and the designed file can be manufactured by milling or 3D printing. It replace the analog method of waxing and denture curing process and also can reduce the production time and cost. Therefore, this case is reported because good clinical results were obtained by digitally surveying on CAD software to produce a surveyed metal restoration and framework on maxillary and mandibular removable dentures.

  • There are still many limitations on fabricating dentures using digital method while computerized production of fixed prostheses utilizing intraoral scanner and CAD/CAM technology has propagated rapidly. Recently the digital solution of fabricating removable partial denture by applying haptic input device, electronic surveying, and rapid prototyping was introduced. In this case presentation, five patients were treated with surveyed crown and removable partial dentures by this digital solution. Fit of dentures was excellent except for one case which showed morphological difference between the actual teeth and that of master cast by the erroneous impression process. There also was not any problem of stability and retention after adaptation in the clinical setting.

Comparison of the accuracy of digital impressions and traditional impressions: Systematic review (디지털 인상법과 전통적 인상법의 정확도 비교: 체계적 고찰)

  • Purpose: This study systematically examines literatures on the suitability of prostheses and accuracy of obtained impressions to see if digital impressions using intraoral scanners can replace traditional impressions. Materials and methods: A MEDLINE/PubMed search and manual search was performed for studies written in English about accuracy of digital impression published in dental journals from August 1, 1997, to July 31, 2017. Depending on criteria, the data for the selected articles were independently organized into standardized spreadsheets by 2 reviewers. Results: Among the total 35 studies met the inclusion criteria, there were 26 studies comparing the suitability of prostheses, and 9 studies comparing the accuracy of impressions through scan data without prostheses. Most studies used prostheses to compare the accuracy of impression techniques. Conclusion: This review suggests that making single crown or mesio-distally short prostheses with digital impressions is clinically reliable in natural teeth. However, there is still a limit to making mesio-distally long prostheses with digital impressions from the lack of related studies. Digital impression cannot fully replace traditional impressions in implant prostheses yet.

A Study on the Evaluation of Repeated Measurement Stability of 3D Tooth Model Obtained by Several Dental Scanners (수종의 치과용 스캐너로 채득된 3차원 치아 모형의 반복측정 안정성 평가 연구)

  • The purpose of this study is to evaluate the reliability of repeated measurements of several dental scanners. Blue-lighted scanners, white-light scanners and optical-type scanners are used in the study of repeatability in this study. The measurement results were calculated as root mean square (RMS) and the significance level was confirmed by applying the 1-way ANOVA statistical technique (𝛼=.05). According to the statistical analysis, the scanner with the largest RMS value was Z-opt group (38.2 ㎛. Next, D-white was 35.2 ㎛ and the group with the lowest RMS value was I-blue (34.1 ㎛). The comparison of RMS means between each group was not significant (p>.05). From this result, the blue light had the lowest error in repeatability of dental scanners, but no statistical significance. The conclusion of this study is that the study results are clinically acceptable.
