Combined Surgery of Pulmonary Resection and Coronary Artery Bypass Grafting (CABG) (폐절제와 관상동맥 우회술의 동반 수술 치험 3예)

  • The patients with primary lung cancer who have symptoms of angina pectoris commonly have underwent coronary artery bypass grafting (CABG) prior to lung resection. Recently, there are attempts to conduct simultaneous operation of CABG and lung resection to reduce disadvantages of staged operation, such as repeated general anesthesia, two wound incisions and delay of lung operation. These attempts generally report good results. Three patients underwent lung resection and CABG simultaneously, and had good post operative status without recurrence of cancer and angina pectoris. We conceive these as worthy cases to report.

  • A functional upper airway obstruction due to a vocal cord dysfunction(VCD) is characterized by a paradoxical adduction of the vocal cords throughout the respiratory cycle with no obvious organic cause for the obstruction. It commonly occurs paroxysmally and imitates acute asthmatic attacks, often in patients with coexisting asthma. They present with episodes of dyspnea associated with inspiratory wheezing that persists despite conventional asthma treatment and a flattening of the inspiratory limb of the flow-volume curve ; an adduction of the vocal cord during inspiration. Failure to recognize concurrent vocal cord dysfunction and asthma has led not only to the excessive use of bronchodilators and corticosteroids, but also to intubation and tracheostomy. Here, we report a case of coexistent obstructive pulmonary disease and functional upper airway obstruction due to a vocal cord dysfunction where a bronchoscopy showed a paradoxical vocal cord motion and typical features of a variable extrathoracic obstruction and a lower airway obstruction on the Flow-volume loop during a symptomatic period.

  • This study describes a case of sudden sensorineural hearing loss(SSNHL) accompanied with adjunctive symptoms including tinnitus and aural fullness. In this case, we evaluated the effect of Korean medicine treatment for SSNHL accompanied with adjunctive symptoms including tinnitus and aural fullness. We treated 1 SSNHL patient who had adjunctive symptoms of tinnitus and aural fullness. After Korean medical treatment for 9 days, two approaches were used in order to evaluate the effect of the treatment. The hearing recovery was measured with pure tone audiometry, and the decrease of tinnitus and aural fullness was identified by patient's subjective expression. After Korean medical treatments, clinical symptoms of SSNHL accompanied with adjunctive symptoms including tinnitus and aural fullness were considered improved. This study shows that the Korean medical treatment effects on the SSNHL accompanied with adjunctive symptoms including tinnitus and aural fullness.

  • Treating patients with severely worn dentition often requires comprehensive prosthetic treatments. Oral rehabilitation and long-term stability could be achieved by analyzing the cause of the wear and changing vertical dimension. The temporary restoration procedure is executed and patient's adaptation is evaluated by long-term observation when treatment is accompanied with increasing vertical dimension. In this case, 54 year-old male presented with worn dentition and esthetic dissatisfaction. In oral manifestations, severely worn dentition with attrition and erosion is diagnosed. To evaluate change of vertical dimension, interocclusal rest space, facial appearance, pronunciation, clinical crown length, distance from incisor edge to labial vestibule are evaluated. Consequently, 2.5 mm increase of vertical dimension that based on articulator incisal pin is determined. After 8 weeks stabilization period with temporary prostheses, definitive prostheses are fabricated. After 6 months follow up period, satisfactory outcomes were achieved both in functional and esthetic aspects through this procedure.

  • Hyponatremia which is due to excessive sodium loss in the urine and decrease in extracellular fluid volume following an acute or chronic central nervous system injury, has been conjunctively described as cerebral salt wasting syndrome (CSWS). This syndrome is often confused with dilutional hyponatremia due to inappropriate secretion of antidiuretic hormone. Accurate diagnosis and management are mandatory for improvement of the course of the disease. This report describes a case of a 31-year-old male patient with CSWS associated with tuberculous meningitis. The patient exhibited hyponatremia, polyuria, excessive natriuresis, volume depletion, and hypotension. He was diagnosed to manifest CSWS and was treated by administration of fluids, salt, and fludrocortisone. After the respective treatments, symptoms of polyuria and hypotension were gradually resolved and hyponatremia was corrected.

Intramedullary Spinal Cord Metastasis : A Report of Two Cases and a Review of the Literature (척수내 전이암 - 2례 보고 및 문헌고찰 -)

  • Intramedullary spinal cord metastases (ISCM) account for only $3.4\%$ of symptomatic metastases to the spinal cord. The survival of patients with ISCM is characteristically short, often no longer than 2 months, due to a rapid neurologic deterioration and the presence of widespread metastases, Including metastases to the brain. We report two cases of ISCM arising from primary sphenoid sinus carcinoma and primary lung cancer along with a review of the literature. The case of ISCM from the primary sphenoid sinus is the third case of secondary syringomyelia due to ISCM In the world literature, and ISCM from the primary lung cancer is the first case reported in Korea. One case showed a slow progression of symptoms and a longer survival (26 months after the radiotherapy to the spine), and the other showed a rapid deterioration of symptoms with a shorter survival. More effective palliation can be achieved if the disease is diagnosed at an early stage when the neurologic deficits are still reversible.

  • A one-day-old baby was transferred for cyanosis and heart murmur. Echocardiographic evaluation revealed that he had severe neonatal Ebstein's anomaly (Carpentier type C), pulmonary atresia, and pulmonary circulation via patent ductus arteriosus. Because the wall of the atrialized right ventricle was very thin, showed decreased contractility, and the small right ventricle showed pulmonary atresia, we decided that a two-ventricular repair was impossible. When the patient was one-month-old, he underwent right atrium reduction-plasty, a right ventricular exclusion procedure (including atrialized right ventricle resection and functional right ventricle plication), and right modified Blalock-Taussig shunt. He was discharged without specific problems. He received a bidirectional cavopulmonary shunt successfully at 4 months later.

  • Submitral left ventricular aneurysm (SMLVA) is a rare disease entity that exclusively occurs in the black population. We herewith report on a surgical case of SMLVA in a 68-year-old male who presented with ventricular tachycardia. He underwent surgical repair under standard hypothermic cardiopulmonary bypass. In the state of apical elevation, a vertical ventriculotomy was made directly over the aneurysm, which was located at the posterobasal aspect of the left ventricle. Radiofrequency ablation was performed on the endocardium all around the neck of the aneurysm and then patch endoaneurysmorrhaphy was carried out with particular care not to injure the mitral valve and subvalvular structure. His postoperative recovery was uneventful. There has been no dysfunction of the mitral valve or recurrence of the ventricular tachycardia at 2 years' follow-up.

A Case of Angiosarcoma Presenting Pleural Effusion (흉막삼출을 동반한 맥관육종 1예)

  • Angiosarcoma is a very rare malignant tumor of endothelial cell origin. We experienced a case of angiosarcoma presented with massive pleural effusion, which was considered as a metastasis from right kidney. A 44-year-old male patient was admitted due to dyspnea for one month. He had a history of transient hematuria 3 months before admission, which disappeared spontaneously. Chest roentgenography showed total haziness in left hemithorax with multiple nodular shadows in right lung. Abdominal ultrasonogram showed a single heterogeneous hyperechoic mass, measuring about $7.3{\times}7.1{\times}6.5cm$ in size in the upper and mid-pole of the right kidney, involving renal sinus. Computed tomography of the chest revealed highly enhanced multiple pulmonary and subpleural nodules with loculated pleurisy. In bronchoscopic finding, a fungating, hypervascular tumor mass was noticed at the orifice of anterior basal segment of left lower lung after removal of tenaceous mucus. Pleural and bronchoscopic biopsies showed findings of angiosarcoma confirmed by immunochemical stains with factor VIII related antigen(+), laminin(+) and vimentin(+), and by characteristic electronmicroscopic findings. Massive pleural effusion was controlled with several times of pleurodesis in both pleural spaces.

  • Synovial chondromatosis developes by metaplasia of synovial cell into chondroblast in synovium of joint. It most commonly involves large joints such as knee or hip, and rarely occurs in shoulder joint. It is often difficult to be diagnosed preoperatively and surgical synovectomy is known to be effective treatment. We encountered a case of synovial chondromatosis with hyaline cartilage forming loose body in subacromial and subdeltoid synovium on the shoulder of a 44-year old female suffered by pain and discomfort. The authors report this case with a review of the relevant literature.
