A Study for the Institutionalization of Alternative Medicine (대체의학의 제도화를 위한 연구 - 법률정보와 공인화 중심으로 -)

  • 사회 전반에 의료의 다원화 혹은 다변화를 요구하는 열망이 높아져 가고 있다. 이는 '대체의학'의 도입과 직결된 문제라 할 수 있다. 본 연구에서는, 최근 법률정보에 따르면 헌법재판소의 의료법에 대한 합헌결정으로 불거진 대체의학의 제도화 움직임을 시작으로 향후 대체의학의 제도화 모델을 결론으로 그 내용을 담았다. 이는 대체의학을 '왜' 도입하여야 하는 가의 논의단계를 지나 '어떻게' 대체의학을 도입할 것인가의 문제로 논의의 방향성을 제시함과 동시에 선행연구들을 면밀히 분석하여 재조명하므로 써 지금까지 축적되어온 연구 자료들을 충분히 고찰 하고자 하였다. 헌법재판소 판결 및 대법원 판례를 바탕으로 대체의학으로 야기되는 법적 쟁점 사항을 분석하고 대체의료행위가 제도화 되기 위한 선결요건을 도출하였다. 또한 무분별하게 사용되고 있는 대체의학에 관한 용어 사용을 재정립하고 향후 대체의학을 공인화 한다면 그 방법은 어떻게 되어야 하는지 그 방안을 제시, 방안 별 장단점을 분석하였다.

  • 본 논문은 근거중심 대체의학의 현황과 범위를 알아보기 위한 것이다. 연구방법: 2000년부터 2010년 3월까지 Pubmed에 게재된 논문 중 alternative medicine으로 검색하여 체계적 고찰을 하였다. 2000년부터 2010년 현재까지 근거중심 대체의학에 대한 다양한 연구결과을 통합하기 위해 체계적 문헌고찰을 실시하였다. 682편의 연구 중 대체의학에 관한 임상연구는 19개였으며 연구전체 대상자는 2,573명이었다. 논문분석 결과 RCT(Randomized Controlled Trial) 실험 설계연구가 44%였으며 CCT(Controlled Clinical Trials)가 16%, OD(efficacy studies with either a controlled or an Other than controlled Design) 40%였다. 가장 많은 임상실험이 시행된 중재 방법은 Acupuncture과 Massage 였다. 대상 질병군 분포는 근골격계 및 결합조직의 질환과 관련된 '통증 완화'에 대한 임상연구가 20%, 다음으로 특정 감염성 및 기생충성 질환, 신생물이 각각 15% 순이었다. 근거중심 대체의학 현황을 파악하고자 체계적으로 고찰한 결과 대체의학을 통한 중재가 질병치료에 효과가 있는지에 대한 과학적 근거를 제시한 임상연구가 부족하다. 대체의학의 품질, 안정성, 유효성 확보를 위해 대체의학에 대한 질 높은 연구필요하며 근거중심 대체의학의 활성화가 필요할 것으로 생각된다.

  • Recently, desires for diversification of medical treatment throughout our society have been enhanced. It is thought that such a trend may be directly related to the introduction of 'alternative medicine'. This study is to establish the foundation of legalization of alternative medicine, starting with the movement for legalization of alternative medicine from constitutionality decision of medical law by the legal Information Constitutional Court. It also suggested the direction of discussion with issue of how to introduce alternative medicine beyond the stage of basic discussion, 'why' we must introduce alternative medicine, through profound investigation of preceding studies. In addition, the present study analyzed legal controversies from the appearance of alternative medicine based on the decisions of the Constitutional Court and the precedents of the Supreme Court and drew the prerequisites for the institutionalization of alternative medical treatments. It also reestablished terms of alternative medicine which have been indiscreetly used, presented methods for officialization of alternative medicine and compared and analyzed advantages and disadvantages of the methods.

  • 고령화 시대에 있어서 노인복지시설을 이용하는 많은 노인들이 본인의 의지와 관계없이 일반적으로 경험하게 되는 건강관리 관련 프로그램은 대체의학과 밀접한 관계를 가지고 있다. 따라서 대체의학을 노인관련 의료복지에 적용함으로써 노인성질환에 대한 예방과 치료에 활용하고 치료를 통하여 노인의료복지의 수준을 제고하는데 노력을 할 필요성이 있다. 본 논문은 노인들의 일상생활에서의 대체의학의 요법을 통해서 포괄적인 건강증진을 위한 실천적 방안을 제시하고자 하였다.

A Literature Review of Clinical Trials on Alternative Medicine (대체의학 임상실험 현황에 대한 문헌고찰)

  • The aim of this study is to know about condition of alternative medicine. Methods which An literature search in Pubmed January 2000 to March, 2010 was performed. RCT(Randomized Controlled Trial), CCT(Controled Clinical Trials), and OD(efficacy studies with either a controlled or an Other than controlled Design) were included. PubMed databases were searched using key terms relating to alternative medicine. 19 clinical trials were included among 682 studies searched. RCT was 44%, CCT was 16% and OD was 40% in the review. In the all of included studies, the acupuncture and the massage was the biggest part. The using rate was 20% of pain control about Disease of the Musculoskeletal system and Connective tissue, 15% of Certain infectious and parasitic diseases, and 15% of Neoplasms. There was lack of evidence that alternative medicine was effective for progress. Future well-designed clinical trials is needed.

  • 여기서는 보완대체의학의 임상효능을 평가하기 위한 일반적인 연구원칙들을 제시하였다. 어떻게 우리가 보완대체의학을 평가할 것인가? 그것이 정말 통상의학과 다른가? 우리는 보완대체의학을 통상의학과 비교하여 우리의 연구 질문들을 다르게 제시해야만 하는가? 보완대체의학은 안전하고 경제적인가? 그리고 우리가 이러한 전문적인 이슈들을 어떻게 제시할 것인가? 보완대체의학 연구는 관심을 가진 임상가들에게 지적인 도전을 제시 한다. 과학적으로 평가하는 것은 가능하긴 하지만 어렵다. 특정 약물치료를 통한 무작위대조 임상시험들은 침술, 동종요법, 수기요법과 같은 유사한 분야의 학문들보다 계획하고 투여하는 것이 훨씬 쉽다. 보완대체의학 분야에서 임상연구의 전술과 전략들은 개념상의 연속되는 도전들을 제시할 것이다.

Undergraduate Medical Students' Perception of Complementary and Alternative Medicine and Korean Medicine (의학전문대학원생의 보완대체의학과 한의학에 대한 인식)

  • The purpose of this study was to analyse undergraduate medical students' perception of the complementary and alternative medicine(CAM) and Korean medicine. A total of 409 undergraduate medical students participated in the survey. Methodologically, descriptive statistics, MANOVA and logistic regression analysis were conducted. First grade students had more positive perception of CAM and Korean medicine than others. There were no significant differences in the perception of CAM in terms of students' grade and gender(affective attitude, compatability, effectiveness, intention to adoption). However, there was gender difference in perception of Korean medicine. Also, the result showed that students' perception of effectiveness had influence whether they recognize CAM and Korean medicine as medicine or not. Based on results, this study suggest that medical school should develop the curriculum that improve students decision-making ability and critical capacity, especially for lower grades, so that stdents can give reasonable advice for patients' needs.

  • The purpose of this study was to identify the perception of alternative medicine and factors affecting the intention to use for nurses and caregiver. In order to achieve the purpose of the study, this study was conducted to examine the awareness of alternative medicine, use experience, and intention to use for 400 workers in hospitals and nursing homes. The results of the analysis are summarized as follows. Recognition of alternative medicine effected on reliability of alternative medicine, awareness and use experience. In addition, experience with alternative medicine has a negative impact on the intention to use alternative medicine. In conclusion, this study suggested that the development of alternative medicine requires efforts to improve the therapeutic effect of alternative medicine.

Recognition and Application Factors of College Students on Complementary and Alternative Medicine (보완대체의학에 대한 대학생의 인식 및 적용요인에 관한 연구)

  • The purpose of this study was to find out how to apply CAM to college students by studying the perceptions and application factors of CAM, the actual condition of application, satisfaction according to sex, and usefulness. The subjects of this study are narrative research on H university students in Chungnam. The results of this study showed that 150(50.3%) respondents recognized CAM as an auxiliary means of modern medicine, 61(20.3%) mental and psychological stability (33.7%), and the satisfaction rate of CAM according to gender was higher in women than in men. The usefulness of CAM was 2.60, and the improvement and characteristics were 2.32 and 2.80, respectively. As a result of this study, it is necessary for university students who are the foundation of society to correctly recognize and promote CAM to prevent disease and maintain health. In addition, the analysis of the recognition and application factors of CAM, and the satisfaction according to gender, could provide information for program development of CAM in the future.

A study on preference for complementary and alternative therapy using Q method (Q방법론을 활용한 보완대체요법 선호에 관한 연구)

  • Today modern people have been searching on managing themselves in easy ways without side-effects to lead a healthy and quality life, accordingly concerns about complementary and alternative medicine have been increasing. The scientific term' complementary and alternative medicine' has been originally used in the US and has become the general term for natural treatment of medical methods, which can be suggested as an alternative solution to modern medical problems. There should be a study based on users' subjectivity to reinvigorate complementary and alternative medicine more than it does now. The purpose of this study is to offer basic materials when assessing practical applications on complementary and alternative medicine using the Q method which is subjective pattern analysis, help understand features of complementary and alternative therapy according to patterns, and find out preferences for patterns of complementary and alternative therapy. As a result patterns of complementary and alternative medicine were classified into three types. Firstly, type I prefers to biological/herbal treatment. Secondly, type II prefers to mixed treatments. Thirdly, type III prefers to manual healing.
