A Rationale for Instrumental Music Playing for Upper Extremity Rehabilitation in Subacute Stroke (아급성 뇌졸중 환자의 상지재활을 위한 악기 연주의 임상적 활용 근거 연구)

  • Upper extremity dysfunction is a common consequence following stroke. Spontaneous recovery during the first six months post-stroke is rigorous and considered as a significant indicator of potential long-term progress. Various approaches have been utilized to regain functional upper limb movement necessary for independent living; however, conventional therapy approaches have failed to prove consistency, especially for subacute stroke patients. There is, thus, a need for innovative therapeutic strategies that motivate stroke survivors to facilitate neural and functional recovery during the critical window immediately following stroke. The effect of music on physical enhancement has been frequently reported in the field of medicine as well as neurorehabilitation. The efficacy of rhythm on lower extremity deficits has been well established. Yet, the rationale for using instrumental music making enhancing subacute upper extremities rehabilitation is not clearly described to date. Based on the key mechanism of music as sensori-motor movement facilitator, this paper reviews previous empirical research that utilized music-based interventions for upper extremity rehabilitation for stroke patients, either in the form of receptive or expressive activity. This paper, further, focuses on the current research trends in subacute stroke upper limb rehabilitation and provides applicable rationale of using instrumental music playing.

Phenomenological Research on Recovery Lived Experience of Stroke Inpatients (뇌졸중 입원 환자들의 회복체험에 관한 현상학적 연구)

  • The components that influence recovery were investigated to understand the recovery lived experience of stroke inpatients. Using the phenomenological research methodology reported by Giorgi, 3 conversation sessions were performed on 12 stroke inpatients. The conversations were recorded through agreement, in which the data were analyzed according to the scientific phenomenological methods. Sixteen summarized meaning units were integrated to deduct 10 main meanings and 6 themes. The themes of the restoration experience included the burden of help, performance of independent roles, self-overcoming, return to society, psychological support, and enhanced motivation for recovery. This can be used as data to predict the difficulty that stroke patients experience during hospitalization and propose a direction of intervention for restoration. Rehabilitation experts must provide intervention for the restoration of stroke patients based on the formation of a therapeutic relationship to strengthen the psychological support and motivation, and make strategies for self-overcoming and a proper cooperation relationship with their family.

  • The purpose of this review was to investigate feasibility of intensive virtual reality training to improve upper extremity function with brain plasticity of individuals with stroke through the literature. The recovery of the paretic upper extremity depends on regularity and intensity of training as use-dependent plasticity. In resent, virtual reality program has been widely used in the occupational therapy field of augmented stroke rehabilitation. There is a growing body of evidence that virtual reality training of the paretic extremity induces brain plasticity associated with motor improvement. In terms of therapeutic feasibility to improve paretic upper extremity, recent research has explored several important factors of virtual reality training for recovery of upper extremity motor function. These factors include high repetition intensity, high motivation like type of game, enhanced multisensory feedback regarding performance, and interactive task-oriented training. Therefore, occupational therapy combined with intensive and repetitive virtual reality training will enhance recovery of upper extremity motor function after stroke.

The Usefulness of Video Fluoroscopic Swallowing Study in Post-Stroke Dysphagia Patients (뇌졸중 후 연하장애 환자에서 비디오 투시 연하 조영검사의 유용성)

  • Dysphagia is common and serious problems in post-stroke patients. The post-stroke dysphagia with aspiration is associated with dehydration, malnutrition, pneumonia, sepsis and death. Up to date, gag reflex and choking history used to decide the aspiration in clinical. The purpose of this study was to evaluate the aspiration and to choice the proper meal formation using by video fluoroscpic swallowing study(VFSS) with post-stroke dysphagia patients. 58 post-stroke patients and ten normal person participated to perform the VFSS with liquid type, puding, yoplait, rice mixed with barium(Ba). Two rehabilitation medicine doctors and a radiological technologist analysed the phase(oral, pharyngeal, esophageal phase)with video film, and checked the pharyngeal transition time(PTT). 38 patients showed abnormality in pharyngeal phase, 13 patients in oral and pharyngeal phase, 3 patients in oral phase, and 2 patients in pharyngeal and esophageal phase. 43 patients(65.2%) occured the aspiration, but 23 of 43 patients improved by the chin tuck position. Aspiration occured 34 patients in liquid type, 2patients in rice gruel and nothing in boiled rice. After VFSS, 13 of 23 patients change the intaking pathway from nasogastric tube(NGT) to oral, On the contrary with 3 of 42 patients from oral to NGT. Consequently VFSS is clearly effective to evaluate the aspiration with post-stroke dysphagia patients.

Kinematics of Bimanual Complementary Movement in Stroke Patients (뇌졸중 환자에서 양손 보완운동의 운동형상학)

  • The objective of this study was to compare the unimanual and bimanual complementary movements of the affected upper extremity. Thirty participants living in Busan area with post-stroke hemiparesis were involved in this study. They were selected according to twelve criteria. We used the Fitmeter accelerometer to measure Signal Vector Magnitude, peak acceleration and peak deceleration. The movement time and Signal Vector Magnitude of bimanual complementary movement were less than those of unimanual movement(p<0.05). Therefore, we suggest that bimanual complementary movement is more useful, as for the kinematic aspect, than unimanual movement when a person with stroke perform activities of daily living.

The Effects of Neck Extensor Vibration Stimulation on Neglect and Balance of Stroke Patients (목 폄근 진동 자극이 뇌졸중 환자의 편측무시와 균형에 미치는 효과)

  • This study was aimed to investigate the effects of neck extensor vibration stimulation on neglect and balance of stroke patients. Patients diagnosed with stroke were selected according to select the study target, 10 patients with experimental group with traditional occupational therapy and neck extensor vibration, and 10 patients with control group with traditional occupational therapy and neglect therapy for 30 minutes per each session 5 times a week for total 8 weeks. As a result of the study, comparison of two groups after the intervention showed that the experimental group had a more significant on the neglect and balance than the control group(p<.05). These result demonstrate that neck extensor vibration stimulation for the effective of neglect and balance in stroke patients, therefore is expected to be useful in rehabilitative therapy.

The Effects of Art and Craft Activities on Anxiety and Stress of Stroke Patients (수공예 활동이 뇌졸중 환자의 불안과 스트레스에 미치는 효과)

  • This study was aimed to evaluate the effects of art and craft activities on anxiety and stress of stroke patients. Patients diagnosed with stroke were selected according to select the study target, 9 patients with experimental group receive art and craft activities, 9 patients with control group receive traditional occupational therapy for 30 minutes per each session 3 times a week for total 8 weeks. Art and craft activities were structured through a group of occupational therapy practitioners interview. A comparison of two groups after the intervention showed that the experimental group had a more significant on the anxiety and stress(p<.05). These result demonstrate that art and craft activities decrease anxiety and stress for stroke patients, therefore is expected to be useful in clinics.

Predicting Functional Outcomes of Patients With Stroke Using Machine Learning: A Systematic Review (머신러닝을 활용한 뇌졸중 환자의 기능적 결과 예측: 체계적 고찰)

  • Objective : To summarize clinical and demographic variables and machine learning uses for predicting functional outcomes of patients with stroke. Methods : We searched PubMed, CINAHL and Web of Science to identify published articles from 2010 to 2021. The search terms were "machine learning OR data mining AND stroke AND function OR prediction OR/AND rehabilitation". Articles exclusively using brain imaging techniques, deep learning method and articles without available full text were excluded in this study. Results : Nine articles were selected for this study. Support vector machines (19.05%) and random forests (19.05%) were two most frequently used machine learning models. Five articles (55.56%) demonstrated that the impact of patient initial and/or discharge assessment scores such as modified ranking scale (mRS) or functional independence measure (FIM) on stroke patients' functional outcomes was higher than their clinical characteristics. Conclusions : This study showed that patient initial and/or discharge assessment scores such as mRS or FIM could influence their functional outcomes more than their clinical characteristics. Evaluating and reviewing initial and or discharge functional outcomes of patients with stroke might be required to develop the optimal therapeutic interventions to enhance functional outcomes of patients with stroke.

Factors Influencing Caregiver Burden During Rehabilitation of Stroke Patients (뇌졸중 환자 재활 시 간병인 부담에 영향을 미치는 요인)

  • In this study, we aimed to determine which medical conditions of the stroke patients admitted for rehabilitation more than four weeks after onset affect caregiver burden. Participants diagnosed with stroke and their respective primary caregivers were enrolled and assessed after an 8-week follow-up period. The areas of evaluation for stroke patients included neurologic state, cognition, performance in daily life movements, gait, and balance, and caregivers were evaluated in the area of burden. The evaluation was conducted at the start of the rehabilitation course and eight weeks later. Patient caregivers were found to be under mild to moderate burden while providing care throughout the hospitalization period. The patient's neurologic state and cognition were correlated with caregiver burden. In the all patient and the subacute stroke patient group, multiple regression analysis confirmed that the neurologic state and balance stability were factors that influenced caregiver burden. Hence, we suggest that improvement in the patient's balance stability be emphasized during the course of rehabilitation to mitigate caregiver burden.

Correlation between ACLT and FIM, MMSE-K, and MFT in Stroke Patients (뇌졸중 환각에서 알렌인지수준과 일상생활활동, 인지기능 및 상지기능의 상관관계)

  • The purpose of this research is to test the relationship between ACLT(Allen Cognitive Level Test) and FIM(Functional Independence Measure), MMSE-K(Mini-Mental State Examination), and MFT(Manual Function Test). 38 elderly persons with stroke were recruited. Pearson Correlation Coefficients is used for correlation test. There is statistically significant correlation between ACLT and FIM, MMSE-K, and MFT(p<.05). This results identified the relationship between cognitive function and activities of daily living, indicated the usability of ACLT as a cognition assessment tool and the need of research for it's application in stroke patients.
