Purpose : The purpose of this study is to compare the initial effect of nerve mobilization (NM), static stretching (SS), and contract-relax (CR) techniques to find the best method in improving hamstring flexibility and gait function in patients with hemiplegia. Methods : Eleven patients with hemiplegia were included in this study. Passive knee extension (PKE) range of motion and the sit and reach (SR) test were used to measure hamstring flexibility, while timed up and go (TUG) and the 10m walking (10MW) test were used to measure the subject's gait. Measurements on each test were assessed prior to the experiment, immediately following the experiment, and 30 minutes after the experiment. Analysis of the results utilized a repeated measures analysis of variance to examine hamstring flexibility and the difference in walking ability. Results : The results suggest significant increases in NM, SS, and CR techniques as they relate to hamstring flexibility (p<.05) following (both immediate and 30 minutes post experiment) PKE range of motion and the SR test, but post-hoc showed no significant difference between the three techniques (p>.05). Additionally, the results suggest significant increases in NM, SS, and CR techniques as they relate to gait function (p<.05) following the TUG test, but found no significant difference in the 10MW test (p>.05). Post-hoc analysis between the three techniques suggests that only the NM technique significantly changed gait function. The time of TUG and 10MW test showed no significant difference between the three techniques before an experiment, just after an experiment, and 30 minutes following the experiment according to the measurement points in time (p>.05). Conclusion : This study suggests NM, SS, and CR techniques immediately improve hamstring length and flexibility while improving gait function in patients with hemiplegia.
Objectives : This study aimed to evaluate the characteristics of patients with single infarction and multiple infarctions. Method : We studied inpatients who were admitted from 2005/10/1 to 2007/3/30 at the KyungHee University Oriental Medical Center (KOMC) Department of Cardiovascular & Neurology (stroke center). We sorted small vessel occlusion patients and evaluated general characteristics of the patients along with the characteristics of single and multiple infarction patients. Result : We evaluated 262 inpatients, and did not find any significant difference in age, hypertension, diabetes, hyperlipidemia, diet, exercise, homocysteine, or Sasang constitution between single infarction and multiple infarction. However, there were more significant associations with patients' smoking and stress with multiple infarctions than single infarction. Conclusion : From this study we could roughly grasp the characteristics of Small Vessel Occlusion patients and evaluated the characteristics of patients with multiple infarction. However, due to the special circumstance of the KOMC inpatients it is difficult to generalize our results; further multiple center research with a larger study group is needed.

Background and purpose : Stroke is one of the most deadly diseases and a leading cause of disability. Lipids confer an increased risk for cardiovascular morbidity and mortality. In spite of many studies on the relationship between stroke and lipids, their relationship is not clear. This study was undertaken to determine whether lipids were associated with stroke. Method : We compared the components of blood lipids between stroke patients group (n=217,Cases), non-stroke control group (n=160, Controls) and healthy control group without hypertension, diabetes mellitus, ischemic heart disease, or hyperlipidemia (n=140, Normals). These data were statically analyzed by general linear models and binary logistic regression analysis to get each adjusted odds ratio. Result : The results were as follows. The blood levels of total cholesterol (T-Chol), triglyceride (TG) and low density lipoprotein cholesterol (LDL-Chol) were significantly higher in patients of ischemic stroke, while the blood levels of T-Chol and LDL-Chol were significantly lower, and the blood levels of TG significantly higher in patients of hemorrhagic stroke. The blood level of high density lipoprotein cholesterol (HDL-Chol) was significantly lower in all cases. Conclusion : These results suggest that high TG and low HDL-Chol may be risk factors of hemorrhagic stroke and ischemic stroke, high T-Chol and LDL-Chol may be risk factors of ischemic stroke, and low T-Chol and LDL-Chol may be risk factors of hemorrhagic stroke in Koreans.
Resveratrol is a non-flavonoid polyphenol which belongs to the stilbenes group and is naturally generated in several plants in response to damage or fungal invasion. It has been shown in published studies that resveratrol has an anti-adipogenic effect. A good consensus regarding the involvement of a down-regulation of $C/EBP{\alpha}$ and $PPAR{\gamma}$ in this effect has been reached. In addition, different metabolic pathways involved in triacylglycerol metabolism in white adipose tissue have been shown to be regulated by resveratrol. Concerning lipolysis, though this compound in itself seems to be unable to cause lipolysis, it increases lipid mobilization stimulated by ${\beta}-adrenergic$ agents. The increase in brown adipose tissue thermogenesis, and accordingly the associated energy dissipation, can attribute to accounting for the body-fat reducing effect of resveratrol. Besides its effects on adipose tissue, resveratrol can also acts on other organs and tissues. Therefore, it increases mitochondrial biogenesis and accordingly fatty acid oxidation in skeletal muscle and liver. This effect can also attribute to the body-fat reducing effect of this molecule. The present review purposes to collect the evidence concerning the potential mechanisms of action which underlie the anti-obesity effects of resveratrol, acquired either in cultured cells lines and animal models.

This study was conducted to analyze the correlation between changes in white matter and homocysteine concentration through brain computed tomography of healthy 50-75 year old subjects without stroke or dementia history. We studied 722 out of 900 patients who underwent health screening at one hospital from 2016 to 2017. Based on the medical records, retrospective studies were conducted and analyzed using SPSS. A chi-square test, T-test and univariate logistic regression analysis were used for analysis. After the subjects were divided into the group with and without white matter changes, the population characteristics were analyzed. The mean age, homocysteine concentration and prevalence of hypertension and diabetes were higher and the duration of education was shorter in the group with white matter changes. In the group with white matter changes, the population increased as homocysteine concentration increased. When the odds ratio was compared based on the lowest group (Q1), age [p<0.001], hypertension [p<0.001] and hyperhomocysteinemia [p=0.021] were risk factors for white matter changes. We also identified modifiable risk factors such as hypertension and hyperhomocysteinemia to prevent complications of white matter changes. However, there has been no report of risk for the each causes of hyperhomocysteinemia and relationship between white matter changes and homocysteine concentration in Koreans. Therefore, large scale prospective studies are needed to better understand this topic.

There is a cause and effect relationship in that brain injury causes impairment of mental rotation and ultimately independent functional activities. The purpose of this study was to compare the effects of mental rotation on reaction time and precision between the normal adults and chronic hemiparetic stroke patients. Thirty-one patients with chronic hemiparetic stroke and twenty normal adults participated in this study. The participants conducted 2 types of tasks for mental rotation: a comparison task using mirror images, and a rotation task using angular disparity images for 2 different 3-dimensional objects. Each of the 3 possible angled shapes ($90^{\circ}$, $180^{\circ}$, and $270^{\circ}$) appeared in each pair. The test consisted of 6 mirror-reflected image pairs and 6 angular disparity image pairs visualized during angular rotation, and 12 test periods. The subjects were judged on how accurately and rapidly they could distinguish between the mirrored and non-mirrored pairs. The study measured reaction time and precision to compare the effect of mental rotation tasks. Reaction time during all 3 angular conditions were significantly longer in the stroke patients than that in the normal adult during the comparison tasks and the rotation tasks. In addition, precision during mental rotation tasks was not significantly different between normal adults and stroke patients. Our results suggest that rehabilitation therapists should keep in mind that mental rotation is more difficult in stroke population than in normal adults.
The purpose of this study was to determine the effects of knee flexor isokinetic training on the mean peak torque of knee muscles and hamstrings-to-quadriceps ratio(H/Q ratio) in hemiplegia able to walk independently for more than 10 meters, to analyze the effect of torque increasing on functional aspects; fatigability and ambulation times, also. Forty-one adult subjects with hemiplegia secondary to a stroke partipated in this study. All participants were in/out patients at the College of Medicine, Pocheon CHA University, Pundang CHA General Hospital. The patients were allocated to two groups: one group exclusively for isokinetic maximal voluntary knee flexor training at $150^{\circ}$/sec(n=20) and the other exclusively for isokinetic maximal voluntary knee flexor training from $30^{\circ}$/sec to $150^{\circ}$/sec (n=21) gradually. The allocation was performed according to patient age, sex, affected side to minimize imbalance between the two training groups. Training was carried out from February 14th, 2000 to April 15th, 2000. Analysis of the data was done by means of t-test, x2-test, paired t-test, ANOVA, and multiple regression analysis. The results of this study were as follows: 1. There were no significant differences between the two groups in mean peak torque of knee muscles and relative decreases in knee extensor mean peak torque with increased knee flexor velocities before training (P<.05). 2.There was no significant differences between the two groups in the H/Q ratio, and no relative increases with increased knee flexor velocities before training. 3. there were significant changes in mean peak torque in group A after training(P<.05), but no significant differences as the velocity increased 4.there were significant changes in mean peak torque in group B after training(P<.05), but no significant differences as the velocity increased 5.there were no significant differences between the two groups, and no significant differences in mean peak torque increase rate between the groups with increased knee flexor velocities after training 6.H/Q ratio increased with increased knee flexor velocities between the two groups, but not statistically And there was no significant differences between the groups with increased knee flexor velocities 7.After training, Ambulation time and its decreasing rate decreased significantly in group B (P<.05) 8Before and after training, there was no significant differences between the groups in the fatiguability 9. In the multiple regression analysis, mean peak torque increase rate of the knee extensor and flexor were higher in group B than A(P<.05), and significantly higher with increased knee flexor velocities (P<.05) Also, training method influenced on Ambulation times decreases significantly(P<.05). Results indicated that knee flexor isokinetic training was effective to knee extensor and flexor mean peak torque increase in the hemiplegia able to walk independently for more than 10 meters. Therefore, we were able to conclude that gradual training from low to high velocity was more effective in the increase of mean peak torque of knee joint and decrease of Ambulation times than training only at high velocity.

This study was a systematic review examining the effect of empowerment intervention studies among patients with chronic physical health conditions in Korea. The PICOT-SD (Participants, Intervention, Comparison, Outcomes, Timing of outcome measurement, Setting, Study design) strategy was applied. A total of 19 studies published between January 2000 and April 2020 were selected. Study participants were rheumatoid arthritis patients, hemodialysis patients, stroke patients, diabetes mellitus patients, chronic pulmonary disease patients, hypertensive patients, cancer patients, chronic kidney disease patients, and kidney transplantation patients, etc. A total of 17 studies were found to have presented a conceptual framework. Empowerment skills were participation, dialogue, experience, interaction, support, and problem solving, etc. The most frequently used dependent variables were self-care (n=13), empowerment (n=10), self-efficacy (n=7), and depression (n=5). Self-care, empowerment, and self-efficacy showed significant improvement among patients in at least 60% of the included studies. Based upon this review, there is evidence that empowerment intervention studies are an effective method to improve the health of patients with chronic physical health conditions. This study further suggests the development of an empowerment intervention program to verify the effects of the various variables identified in this study.

This study was performed to determine the levels of physical function (ADL, IADL) and to reveal its association with the related factors in the elderly people. The study subjects were 1,756 (male 872, female 884) people aged over 70 who received medical check-ups and long-term care services between 2009 and 2012 from the National Health Insurance Corporation. As a result, the distribution of impaired ADL and IADL increased significantly with age. Logistic regression showed that the risk ratio of impaired ADL was increased significantly in the following groups: female, urban, low weight, stroke history group, smoking, alcohol drinking, and not regular exercise group. The risk ratio of an impaired IADL were increased significantly in the group of females, low weight, smoking, alcohol drinking. On the other hand the risk ratio of an impaired ADL and IADL was similar in each age group. As above results, the levels of ADL and IADL in the study subjects are closely related to the socio-demographic characteristics and health related behaviors. In particular, they suggested that the levels of ADL and IADL were lower in the poor group of the health-related behaviors, such as smoking, alcohol drinking, and regular exercise.
This paper aims at developing 'gyro-mouse' which provides decent and comfortable human-computer interface that supports the usage of such software as an internet-browser in PC for the people paralyzed in upper limbs. This interface operates on information collected from head movement to get the cursor control. The interface is composed of two modules. One is hardware module in which the head horizontal and vertical angular velocities are detected and transmitted into PC. The other is a PC software that translates the received data into movement and click signals of the mouse. The ANN (artificial neural network) learns the quick nodding pattern of each user as click input so that it can provide user-friendly interface. The performance of the system was evaluated by three indices that are click recognition rate. error in cursor position control. and click rate of the moving target box. The performance result of the gyro-mouse was compared with that of the optical-mouse to assess the efficiency of the gyro-mouse. The average click recognition rate was 93%, average error in cursor position control was 1.4∼5 times of optical mouse. and the click rate with 50 pixels target box was 40%(30 clicks/min) to that of optical mouse. The click rate increased monotonously with the number of trial from 35% to 44%. The suggested system is expected to provide a new possibility to communicate with the society.
