  • 국내의 질환별 사망률에 대한 보고를 참조하면, 뇌졸중은 암에 이어서 높은 사망률을 보이는 질환이며, 단일 질환으로는 가장 높은 사망률을 보인다. 뇌졸중은 높은 사망률을 나타내는 문제 외에도 후유증으로 심한 장애를 남길 수 있다. 그러므로 고령화 사회로 접어들고 있는 우리나라의 상황을 고려할때 뇌졸중은 치매와 더불어서 주요한 노인 질환일 뿐만 아니라, 뇌졸중이 발생하게 되면 후유증으로 장애가 남는 경우가 많으므로 지속적으로 환자의 가족과 주변에 대한 사회적 부담을 가중 시키는 문제를 불러온다.

A Effect of Peer Mentor Program on Recovery after Stroke (동료멘토(peer mentor) 프로그램이 뇌졸중 환자의 회복에 미치는 영향: 연구 프로토콜)

  • The purpose of this study was introduce peer mentoring which is well used in various rehabilitation areas in overseas country to Korea academically and clinically and help psychological and physical recovery for stroke survivor. It was two group experimental design study, this protocol is designed for stroke survivor, 3-6 months after the onset. Peermentors were consisted of outpatients who is stroke survivor in different recovery levels, at least 2 year after the onset. Peermentors received education about how to support and mentor participants of the program emotionally, appraisally, and informationally and how to run the program. Geriatric Depression Scale Short Form-Korea Version(GDSSF-K) is used to measure psychological factor, Stroke Impact Scale(SIS) is used to measure recovery level of patient, and for physical factor Upper Extremity Function Test for the Elderly(TEMPA) was used. A researcher who is interested in the peer mentor program needs to find more effective applying method based on be offered method in this study for helping recovery after stroke.

Quality of life of Elderly Chronic Stroke according to types of leisure (노인 뇌졸중 환자의 여가형태에 따른 삶의 질의 차이)

  • The purpose of this study was to identity the types of leisure and to examine the quality of life according to them and suggest the basic resources about leisure on elderly chronic stroke. The subject of this study was elderly chronic strokes(n=80) in Gwang Ju. A total of 80 questionnaires from subjects were analyzed. The social leisure activity shows more clear and positive purpose and more higher quality of life than private leisure. But it also has limited time and require more money. Elderly chronic stroke presents passive and limited leisure activity and shows low satisfaction about leisure. But the leisures that needed more physical movements show higher quality of life. So leisure activity program which is able to support and assist the limited body function will be helpful to increase the quality of life of elderly chronic stroke.

Discharge Assessment Tool in Stroke and Elderly Patients: A Systematic Review (뇌졸중 및 노인 환자의 퇴원 평가도구에 대한 체계적 고찰)

  • Objective : The aim of this systematic review is to analyze the assessment items and tools used for the discharge of stroke and elderly patients and to help select appropriate assessment tools for discharge planning. Methods : The studies included in this review were collected from the PubMed, Medline Complete (EBSCOhost), and Scopus databases from January 2009 to December 2018. A total 22 studies were selected. The research types, the areas and sub-areas of assessment, as well as the assessment tools according to the areas were analyzed. Results : Descriptive research (59.2%) was the most frequent type of research. The 12 main areas of assessment were medical condition, activities of daily living, swallowing function, sensation, mobility, cognition and perception, communication, emotion, home environment, patient knowledge and readiness for discharge, social support, and well-being. Among the assessment tools, the most frequently used tools were Bathel Index; Functional Independence Measure, to assess activities of daily living; Short-Form Health Survey, to assess well-being, and Timed Up and Go test, to assess mobility. Conclusion : This study will help select the assessment areas and tools to be considered at discharge of stroke patients and serve a basis for the development of comprehensive assessment tools for discharge planning.

Effects of Fluid Therapy Education Program for Aged Stroke Patients (노인 뇌졸중환자의 수분섭취교육 프로그램의 적용 효과)

  • Purpose: This study was to verify the effects of a fluid therapy education program for aged stroke patients for preventing dehydration, pyuria and bacteriuria. Methods: The study employed a non-equivalent control group pretest-posttest design in quasi experimental basis. Subjects were 38 stroke patients (20 in the experimental group and 18 in the control group) older than 65, hospitalized in a senior care center in S city, Korea. Results: The amount of daily average fluid intake, normal ratio of blood urea nitrogen-creatinine ratio, serum $Na^+$ and urine white blood cells were increased significantly in the experimental group. Normal ratio of bacteriuria increased in the experimental group, but not significantly. Conclusion: The education program is considered to be an effective nursing intervention tool for preventing dehydration and urinary tract infections which related to the secondary wellness of aged stroke patients. However, longer term study is necessary for better quality of nursing and developing more specific education programs for aged stroke patients usually hospitalized for a long period of time.

  • Despite intensive therapy, a large proportion of stroke survivors are left with significant disabilities. To improve the disable condition, a major component of stroke management is aimed at facilitating functional independence and community reintegration. There are many evidences that community-based rehabilitation after stroke is most effective when provided by a well-organized multidisciplinary team. Key to the implementation of community-based rehabilitation(CBR) is that the disabled person, their family and their community decide on what the priorities of the service should be. The stroke patients in the community need a wide range of services, including the traditional rehabilitation services of physiotherapy, occupational therapy, speech therapy, social workers, orthotists, nurses and so on. The purpose of this research was to investigate necessity and methods of community-based rehabilitation system in Korea.

Comparison of Chin Tuck Against Resistance and Shaker Exercise on Suprahyoid and Sternocleidomastoid Muscle Activity in Stroke Older Patients with Dysphagia (삼킴 장애가 있는 뇌졸중 노인 환자에서 저항성 고개 숙이기 운동과 Shaker 운동의 목뿔위 및 목빗근 활성 비교)

  • Chin Tuck Against Resistance Exercise(CTARE) and Shaker exercise is used to common for strengthening of suprahyoid muscle. No previous studies was compare of two exercises for patients with dyaphagia. This study investigated the effects of CTARE and Shaker exercise on suprahyoid, sternocleidomastoid muscle activity in stroke older patients with dysphagia. Ten stroke patients with dyaphagia voluntarily participated in this study. All subjects was performed in the order in the CTARE (isometric, isokinetic), Shaker (isometric, isokinetic), and repeated each ten trials. After CTARE was performed, subjects took a 5 min wash out period to minimize muscle fatigue. Activity of suprahyoid and sternocleidomastoid muscle during two training was analyzed using surface electromyography(sEMG). Wilcoxon signed rank test was used to assess differecences for muscles activity between the effects of the CTAR and Shaker exercise within group. CTARE and Shaker exercise showed no significant difference activity in suprahyoid muscle(p > .05). CTARE showed significantly lower muscle activity in sternocleidomastoid muscle than Shaker exercise(p < .05). CTARE in stroke older patients with dysphagia may be a effective intervention to improve swallowing function than Shaker exercise.

Effects of Song Discussion on Depression and Rehabilitation Motivation in Stroke Patients (노래 가사 토의가 뇌졸중 환자의 우울 및 재활동기에 미치는 효과)

  • This study investigated the effects of song discussion on depression and rehabilitation motivation in stroke patients. Older adults with chronic stroke participated in this study: nine for the experimental group and eight for the control group. The experimental group was divided into three subgroups and participated in 12 sessions over 6 weeks. Target lyrics were selected by the investigator among popular songs from the participants' young adulthood. The song-based discussion was facilitated to address issues targeted at supportive, insight-focused, or reconstructive stage. The control group was provided with delayed intervention. At pre and posttest, the short form of Geriatric Depression Scale and the Rehabilitation Motivation Scale were measured. The experimental group showed significantly decreased depression and significantly increased rehabilitation motivation (p < .01), while the control group showed no significant changes. Positive changes were also observed in all subcategories of rehabilitation motivation in the experimental group, particularly in significantly increased task-oriented motivation and decreased amotivation. This study suggests that song discussion will be effectively applied in rehabilitative settings to address psychological issues of older adults with stroke.

A Study on Health Status' Factors Affecting HRQOL in the Elderly Stroke Patients With or Without Sequela (노인 뇌졸중 환자의 후유증 유무에 따른 HRQOL 요인별 중요도 분석)

  • Objective : The main purpose of this study was to analyze factors affecting a health related quality of life(HRQOL) in the elderly stroke patients with(Group 1) or without(Group 2) sequela. Methods : This survey performed secondary analysis with 'the fifth korea national health and nutrition examination survey', and investigated 91 elderly stroke patients over 65 years. Results : The results were as followed: HRQOL and self rated health had a significantly lower in Group 1(p<.05). In all groups, HRQOL was affected by self rated health(${\beta}=0.39$), mental health, and medical health significantly(p<.05). In group 1, HRQOL was affected by mental health(${\beta}=0.45$), self rated health significantly(p<.05). In group 2, HRQOL was affected by medical health(${\beta}=0.45$), functional health, mental health significantly(p<.05). Conclusion : In order to improve the HRQOL of stroke patients during rehabilitative therapy, it is important that early detection and early intervention of depression and suicide, which affect mental health status, are important in the case of severe aftereffects. If the patient continues to support his/her current status regardless of aftereffects, he/she can improve the HRQOL with functional recovery of the stroke patient, and finally have a positive effect on the independence of the stroke patients.
