IDIOPATHIC RETARDED ERUPTION : A CASE REPORT (Idiopathic retarded eruption의 1 증례)

  • The strict meaning of the term retarded eruption is that tooth eruption is delayed compared to its developmental stage. The etiological factors for it may be local, systemic or genetic. Its pathogenic mechanism is unknown in most cases. In cases of generalized retarded eruption without any clear reason, the general growth and development seems normal in all aspects and the teeth usually keep the normal eruption sequence. Usually the primary teeth may often be extracted hoping to provoke the eruption of the retarded teeth, only to be proved a failure. Consequently, the patient will be without teeth for many years, with loss of esthetics, loss of occlusal height and resorption of alveolar process. The primary goal in the treatment may therefore be to keep and maintain the primary teeth in good condition until they are naturally shed. This observation was done on 10 year 1 month old patient who were diagnosed as idiopathic retarded eruption through both radiographic and systemic examination and follow-up observation was done for 11 month.

An electron microscopic study on endocrine cells in the cardiac and fundic regions of the Korean hedgehog(Erinaceus koreanus) (한국산 고슴도치 분문부와 위저부의 내분비세포에 관한 전자현미경적 연구)

  • Endocrine cells in the cardiac and fundic regions of the Korean hedgehog were studied ultrastructurally. Five types of endocrine cells classified as EC, ECL, $D_1$. G and A-like cells were identified in these regions. EC cells contained pleomorphic granules, 170~500nm in diameter, with high electron density and highly dense bodies in a dense matrix. ECL cells were characterized by the presence of round or oval granules, 220~450nm in diameter, with high electron density. Some granules of ECL cells showed a small amounts of contents or empty. $D_1$ cells contained round and relatively small granules, 140~400nm in diameter, with low to moderate electron density. G cells were characterized by the presence of round or oval granules, 200~400nm in diameter, with moderate electron density. Some granules of these cells showed a narrow halo between the limiting membrane and the granular matrix. A-like cells contained round granules, 170~260nm in diameter, With high electron density. The granules of these cells showed homogeneous matrix surrounded by the tight membrane.

The regional distribution and relative frequency of gastrointestinal endocrine cells of the ICR mice: An immunohistochemical study (ICR 마우스 위장관 내분비 세포의 부위별 분포 및 출현 빈도 : 면역조직화학적 연구)

  • The regional distributions and relative frequencies of some gastrointestinal endocrine cells in the 8 portions (fundus, pylorus, duodenum, jejunum, ileum, cecum, colon and rectum) of the gastrointestinal tract of ICR mouse (ICR) with immunohistochemical method using 7 types of specific antisera against somatostatin, serotonin, glucagon, cholecystokinin (CCK)-8, secretin, pancreatic polypeptide (PP) and gastrin. In this study, somatostatin-, serotonin-, glucagon-, CCK-8-, secretin- and gastrin-immunoreactive (IR) cells were identified. Most of these IR cells in the intestinal portion were generally spherical or spindle in shape (open-typed cell) while cells showing round in shape (close-typed cell) were found in the stomach regions occasionally. Their relative frequencies were varied according to each portion of gastrointestinal tract. Somatostatin-IR cells were demonstrated throughout whole gastrointestinal tract except for large intestine. Serotonin-IR cells were detected throughout whole gastrointestinal tract and they were most predominant endocrine cell types in this species of mouse. Glucagon-IR cells were restricted to the fundus and rectum with moderate and a few frequencies, respectively. CCK-8-IR cells were observed in the pylorus, duodenum and ileum with numerous, moderate and rare frequencies, respectively. Secretin-IR cells were restricted to the duodenum and ileum with a few and rare frequencies, respectively. Gastrin-IR cells were restricted to the pylorus with numerous frequency. However, no PP-IR cells were found in this study. In conclusion, some peculiar distributional patterns of gastrointestinal endocrine cells were found in the ICR mouse compared to those of other mammals.

Immunoelectron Microscopic Study on the Endocrine Pancreas of the Native Korean Goat (한국재래산양 췌장내분비세포의 면역전자현미경적 연구)

  • Pancreases obtained from native Korean goats were used, and examined by immunoelectron microscopy using several antisera. Five types cells, glucagon (A), insulin (B), somatostatin (D), and pancreatic polypeptide (PP-I and PP-II) cells, were identified in the pancreatic islets. The morphologies of A, B, and D cells corresponded to the typical charateristics described in previous reports on other mammals. Serotonin immunoreactivity was observed in the D cells on the basis of the granular profiles. Two types of PP cells could be distinguished on the basis of the granular profile: the first type was formed by round, homogeneous secretory granules ($220{\sim}400nm$) having a narrow halo between the dense core and limiting membrane, while the other type consisted of cells whose secretory granules ($240{\sim}440\;nm$ in the major axis, $150{\sim}200nm$ in the minor axis) were pleomorphic, having a dense core and a closely fitting limiting membrane. From these results, we suggest that the pancreatic islets of the native Korean goat consist of five types of endocrine cells, A, B, D, PP-I and PP-II cells. Among these, PP-I type cells may correspond to the classical PP of other mammalian pancreases, while PP-II type cells may correspond to the enterochromaffin cells in other species.

  • The regional distribution and relative frequency of the pancreatic endocrine cells in the ICR mouse were studied by immunohistochemical (PAP) method using four types of specific antisera against insulin, glucagon, somatostatin and human pancreatic polypeptide (PP). The pancreas of mice could be divided into three portions; pancreatic islets, exocrine and pancreatic ducts. Pancreatic islets, furthermore, were subdivided into three regions (central, mantle and peripheral region) according to their located types of immunoreactive cells. In the pancreatic islet portions, insulin-immunoreactive cells were located in the central and mantle regions but most of somatostatin-, glucagon- and PP-immunoreactive cells were detected in the mantle and peripheral regions with various frequencies. In addition, PP-immunoreactive cells were also found in the central regions of pancreatic islets of ICR mouse. In the exocrine portions, all four types of immunoreactive cells were demonstrated in the ICR mouse. In the pancreatic duct portions, insulin- and glucagon-immunoreactive cells were situated in the epithelial lining of ICR mouse with a few and rare frequencies, respectively. In addition, rare PP-immunoreactive cells were also demonstrated in the subepithelial regions of the pancreatic duct. However, no somatostatin-immunoreactive cells were demonstrated.

  • As dipeptidyl peptidase-4(DPP-4) inhibitors are used widely as a secondary treatment for type 2 diabetes because they tend to be well tolerated with minimal side effects, the human DPP-4(hDPP-4) gene was injected into a pig zygote through micro-injection, and 1-cell stage fertilized embryos were then transplanted surgically into the oviduct. Three pigs were fertilized with hDPP-4 genes and produced sixteen piglets, in which one male piglet was identified to be transgenic. Finally, transgenic pigs showing hDPP-4 gene expression in the tail were produced. Western blot and RT-PCR analysis confirmed that the hDPP-4 is expressed strongly in the membrane cells of the transgenic pig, and that the hDPP-4 gene appears in various tissues and tails. This suggests that the expression vector is normally expressed in transgenic pigs. These results are anticipated to be a model animal to check the endocrine function for insulin resistance that occurs in a hDPP-4 transgenic pig and to increase its value for use as a material in newly developed medicines.

Effects of Repetitive High Frequency Motor Cortex Transcranial Magnetic Stimulation and Cortical Disinhibition in Diabetic Patients with Neuropathic Pain: A Case Control Study (신경병성 통증이 있는 당뇨 환자에서 반복 경두개 자기자극치료의 효과 및 피질 탈억제 현상: 환자 대조군 연구)

  • Objective: To investigate the cortical disinhibition in diabetic patients with neuropathic pain and without pain. In addition, we assessed the cortical disinhibition and pain relief after repetitive transcranial magnetic stimulation (rTMS). Method: We recruited diabetic patients with neuropathic pain (n = 15) and without pain (n = 15). We compared the TMS parameters such as motor evoked potential (MEP) amplitude, cortical silent period (CSP), intracortical inhibition (ICI %) and intracortical facilitation (ICF %) between two groups. Moreover, we evaluated the changes of pain and TMS parameters after five consecutive high frequency (10 Hz) rTMS sessions in diabetic patients with neuropathic pain. The neuropathic pain intensity (visual analog scale) and TMS parameters were assessed on pre-rTMS, post-rTMS 1day, and post-rTMS 5 day. Results: The comparison of the CSP, ICI % revealed significant differences between two groups (p<0.01). After rTMS sessions, the decrease in pain intensity across the three time points revealed a pattern of significant differences (p<0.01). The change of CSP and ICI % across the three test points revealed a pattern of significant differences (p<0.01). The ICI % revealed immediate increase after first rTMS application and significant increase after five rTMS application (p<0.01) in diabetic patients with neuropathic pain. The MEP amplitude and ICF % did not reveal any significant changes. Conclusion: Our findings demonstrate that cortical inhibition was decreased in diabetic patients with neuropathic pain compared with patients without pain. Furthermore, we also identified that five daily rTMS sessions restored the defective intracortical inhibition which related to improvement of neuropathic pain in diabetic patients.

A Retrospective Study of the Safety and Effect of Co-administration of Glucose-lowering Medication and Bojungikgi-tang on Blood Glucose Level in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자에서 혈당강하제와 보중익기탕 병용 투여가 혈당 변화에 미치는 영향 및 안전성 연구)

  • Objective: This study aimed to assess the safety and effect on glucose level of Bojungikgi-tang in patients with type 2 diabetes mellitus. Methods: To review patients' clinical characteristics and laboratory tests retrospectively, we investigated 15 hospitalized patients with type 2 diabetes mellitus who took Bojungikgi-tang at Kyung Hee University Korean Medical Hospital for at least one day between January 2012 and December 2022. The blood glucose levels, including fasting blood sugar (FBS), 2-hour postprandial glucose (PP2) levels, and glycated hemoglobin level, were collected to determine the effect of the Bojungikgi-tang on blood sugar changes. Furthermore, to evaluate the safety of Bojungikgi-tang, hepatic function and renal function tests were implemented, including aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, γ-glutamyltransferase, blood urea nitrogen, creatinine, and Modification of Diet in Renal Disease estimated glomerular filtration rate. Results: When Bojungikgi-tang and a standard treatment for diabetes were administered for patients with type 2 diabetes, it was confirmed that there were no statistically significant changes in FBS and PP2 levels in the analysis of each medication taken. There was no significant difference in the safety profile after taking Bojungikgi-tang. Conclusions: The combined administration of Bojungikgi-tang with standard hypoglycemic medication for patients with type 2 diabetes may not affect blood glucose levels and safety.

Interleukin-1β -511C/T Gene Polymorphism and Depression Related to Breast Cancer (Interleukin-1β 유전자 내 -511C/T 단일염기다형성과 유방암 관련 우울증)

  • Objectives : Pro-inflammatory cytokines are related to the pathophysiology of both cancer and depression, and their secretion is controlled by the transcriptional activity of particular gene polymorphisms. This study aimed to investigate whether interleukin (IL)-1β -511C/T gene polymorphism is associated with depression following mastectomy for breast cancer. Methods : A total of 309 patients with breast cancer were evaluated one week after mastectomy, and 244 (79%) were followed one year later. Depression (major+minor depressive disorders) was diagnosed according to DSM-IV criteria using the Mini International Neuropsychiatric Interview, and classified into prevalent, persistent, and incident depression. Associations of IL-1β -511C/T polymorphism with the three depressive status were estimated using logistic regression models. Results : At baseline, 74 (24%) patients were classified with prevalent depression ; and at follow up, 19 (8%) and 25 (10%) patients were classified with persistent and incident depression, respectively. The IL-1β -511T/T genotype was independently associated with prevalent and persistent depression, but not with incident depression. Conclusion : IL-1β -511T/T genotype may involve in the etiology of depression occurring in women with breast cancer who receive a mastectomy.

Composite Tumor of Adenocarcinoma and Small Cell Neuroendocrine Carcinoma of the Uterine Cervix -A Case Report- (자궁 경부의 선암과 혼합된 신경내분비 소세포 암종 - 1 증례 보고 -)

  • Small cell neuroendocrine carcinoma of the uterine cervix is a distinct subtype of cervical cancer that appears analogous to oat cell carcinoma and carcinoid tumors of the lung. It has been assumed to be derived from the neural crest via argyrophilic cells in the normal endocervix. We have recently encountered a case of small cell neuroendocrine carcinoma of the uterine cervix coexisting with adenocarcinoma which was argyrophil negative. A 66-year-old multiparous woman was admitted because of vaginal bleeding for 2 months. Cervicovaginal smear revealed several scattered clusters and sheets of monotonous small cells with some peripheral palisading in the background of hemorrhage and necrosis. Radical hysterectomy specimen revealed an ulcerofungating tumor on endocervical canal which was composed of two components. Major component of the tumor was made up of monomorphic population of small oval-shaped tumor cells arranged in sheets and partly in acinar structures or trabecular fashion. Other component was adenocarcinoma, endocervical well-differentiated type. Argyrophilia was present on the Grimelius stain and immunohistochemical studies revealed diffuse positivity to neuron-specific enolase and carcinoembryonic antigen. Electron microscopic examination showed clusters of small round to oval cells, which had a few well-formed desmosomes and several membrane-bound, dense-core neurosectetory granules.
