  • 말과 자동차는 서로 닮은 게 몇 가지 있는데, 순환기계 및 호흡기계는 말의 엔진을 구성하고 말이 소화 시키는 사료는 자동차의 연료와 같으며, 연료는 근육에 힘을 내는 영양적 에너지로 바뀐다. 호흡기계는 대사를 촉진하기 위해 산소를 제공하고 심장혈관계 또는 순환기계는 산소와 영양소를 조직에 운반할 뿐만 아니라 엔진이 작동하면서 생긴 노폐물도 제거한다.

Prevalence Rate of Lead Related Subjective Symptoms in Lead Workers (연취급 근로자의 연폭로 수준에 따른 주관적 자각증상 호소율)

  • The relationship between lead related subject symptoms and lead exposure indices was studied in 435 male lead workers in thirteen lead using industries. 212 male office workers who were not exposed to lead occupationally were also studied as a control group. Fourteen lead related symptoms were selected. They were further subdivied into 4 sub-symptom groups such as 1) gastrointestinal, 2) neuromuscular and joint 3) constitutional, and 4) psychological symptoms. Symptom questionnaires were provided to the workers and filled up by themselves and reconfirmed by interviewer(doctor). The test used fer the evaluation of lead exposure were blood lead(PbB), zinc protoporphyrin in whole blood(ZPP), hemoglobin(Hb), hematocrit (Hct), delta-aminolevulinic acid in urine(DALA). The results obtained were as follows; 1. The higher prevalence rate in the sub-group of neuromuscular and joint symptoms was observed in occupationally lead exposed subjects than non-exposed subjects. Among the sub-groups, the most frequent symptom was 'numbness of finger, hands or feet', and the prevalence of the symptom of 'arthralgia', 'weakness of fingers, hands or feet' and 'myalgia' were higher in order. 2. While the symptom which showed the biggest difference of prevalence rate among the 14 symptoms between exposed and non-exposed subjects was 'numbness of fingers, hands or feet', the symptom which showed the highest prevalence rate was 'feeling tired generally' in exposed and non-exposed subjects, but no statistical difference of symptom prevalence were observed. 3. In total study population, PbB and ZPP had dose-response relationship with 4 symtoms of neuromuscular and joint symptoms ('numbness of finger, hands or feet', 'arthralgia', 'weakness of fingers, hands or feet' and 'myalgia') and one symptom of gastrointestinal group('intermittent pains in lower abdomen'). 4. In lead exposed workers, only neuromuscular and joint symptoms group showed dose-response relationship with PbB and ZPP, 5. In lead exposed workers, the prevalance rate of overall symptoms of lead workers with age below 39 years was higher than that of lead workers with age above 40. While neuromuscular and joint symptoms group had a dose-response relationship with PbB in former group, it had a dose-response relationship with ZPP in latter group. 6. Age adjusted odds ratios of symptoms of non-exposed with exposed and odds ratios of low exposed with high exposed workers showed the dose-response relationship of lead exposure with neuromuscular and joint symptoms group('numbness of fingers, hands or feet', 'arthralgia', 'weakness of fingers, hands or feet' and 'myalgia') and gastrointestinal symptoms group('intermittent pains in lower abdoman').

The Effect of Hand Grip Force on the Activity of Shoulder Muscles in the Patterns of Arm Position (팔자세의 유형에서 손의 쥐기 강도가 어깨근육의 활성도에 미치는 영향)

  • This study is intended to provide a basic clinical data useful for preventing shoulder injuries related to occupation and curing them, by measuring the shoulder muscle activity of normal adults in an arm posture type of a wide shoulder joint angle, according to several differences of hand grip force. In order to examine the shoulder muscle activity during hand grips at a variety of intensity, according to the arm posture type, MVCs of all subjects were measured, and %MVCs of anterior deltoid, supraspinatus, the upper trapezius and infraspinatus at 0 degree, 90 degrees and 160 degrees of shoulder angle were yielded according to the grip force in an arm posture type, changing the maximum grip force into 30%, 50% and 90%, randomly. When measuring the hand grip at 30%, 50% and 70% of the maximum grip force to compare %MVC of each group depending on the arm posture type, there were no significant differences of muscle activity at 0 degree of shoulder angle among anterior deltoid, supraspinatus, the upper trapezius and infraspinatus, but there were significant differences of it among them at 90 degrees and 160 degrees of should angle(p<.001). According to the results of post-hoc test, also, anterior deltoid had the highest muscle activity, whereas the upper trapezius had the lowest muscle activity.

Development of Ankle Power Assistive Robot using Pneumatic Muscle (공압근육을 사용한 발목근력보조로봇의 개발)

  • This paper describes the development of a wearable robot to assist ankle power for the elderly. Previously developed wearable robots have generally used motors and gears to assist muscle power during walking. However, the combination of motor and reduction gear is heavy and has limitations on the simultaneous control of stiffness and torque due to the friction of the gear reducer unlike human muscles. Therefore, in this study, Mckibben pneumatic muscle, which is lighter, safer, and more powerful than an electric motor with gear, was used to assist ankle joint. Antagonistic actuation using a pair of pneumatic muscles assisted the power of the soleus muscles and tibialis anterior muscles used for the pitching motion of the ankle joint, and the model parameters of the antagonistic actuator were experimentally derived using a muscle test platform. To recognize the wearer's walking intention, foot load and ankle torque were calculated by measuring the pressure and the center of pressure of the foot using force and linear displacement sensors, and the stiffness and the torque of the pneumatic muscle joint were then controlled by the calculated ankle torque and foot load. Finally, the performance of the developed ankle power assistive robot was experimentally verified by measuring EMG signals during walking experiments on a treadmill.

Effects of Oral Parafunction on the Stiffness and Elasticity in the Muscles of the Mastication and Facial Expression (구강악습관이 저작근 및 안면표정근의 경직도 및 탄성도에 미치는 영향)

  • The purpose of this study was to evaluate the effects of oral habits on the muscles of mastication and facial expression by means of two parameters: muscle stiffness and elasticity. 10 healthy, fully-dentate male subjects in their twenties were selected for this study; all had normal Class I occlusal relationships. Muscle stiffness and elasticity were measured with a tactile sensor(Venustron, Axiom Co., JAPAN) while subjects were asked to relax and perform various parafunctional activities such unilateral clenching(biting the bite force recorder with a force of 50kg on each subject's preferred side), jaw thrusting and lip bracing. The following muscles were examined: temporalis anterior(Ta), masseter(Mm), frontalis(Fr), inferior orbicularis oculi(OOci), zygomaticus major(Zm), superior and inferior orbularis oris(OOrs and OOri) and mentalis(Mn). Paired t-test, Correlation Coefficients, ANOVA and Multiple Comparison t-tests were used for statistical analysis. Unilateral clenching was highly correlated with bilateral stiffness and elasticity of all the muscles tested. Mm was affected by all three oral habits; Ta was affected by unilateral clenching(p<0.05); Zm was affected by unilateral clenching and OOrs, OOri and Mn were most affected by lip bracing(p<0.05). This study indicates that not only the masticatory muscles but also the muscles of facial expression, mainly circumoral muscles, can be significantly influenced by parafunctional activities such as unilateral clenching and lip bracing.

Biomechanical Effect of Forearm Flexor Muscles depending on Handle Sizes (수공구 손잡이 형태에 따른 생체역학적 영향도 분석)

  • 수공구 사용시 과도한 힘은 작업성 근골격계 질환을 일으킬 수 있는 주요 원인중 하나이다. 이와 관련하여, 수공구 파지시 인체 내부에 부과되는 근력과 외적으로 작용된 힘 간의 비율을 이해하는 것이 중요하며, 이는 근육에 부과되는 과도한 힘을 최소화 시키고, 작업에 필요한 힘의 효율성을 극대화 시키는데 필수적이라 할 수 있다. 이러한 비율과 관련하여 많은 연구가 되어 왔으나, 대부분 수리적 인체역학적 모델과 같은 간접적 추정 방법에 의거하고 있는 실정이다. 이러한 인체역학적 모델을 검증하고 개선하기 위하여 해부용 팔 (cadaver)을 활용한 직접적인 근력과 악력 측정이 필수적이다. 본 연구에서는 이러한 해부용 팔을 이용하여 상지 굴근(hand flexor)을 자동으로 제어하고 근력과 함께 악력을 측정할 수 있는 Hand Motion Simulator를 개발하고, 이를 통하여 다양한 사이즈의 손잡이 파지시 요구되는 근력과 외적으로 적용된 악력을 비교함으로써 수공구 손잡이 사이즈에 따른 근력의 효율성에 대하여 측정을 해 보았다. 또한, 적용된 굴근 (FDP & FDS) 간의 힘 비율에 따른 파지법의 차이를 조사해 보았다. 내부에 주어진 근력은 외부로 작용된 악력보다 5.3배 높은 부하가 작용하였으며 이러한 수공구 손잡이 파지시 힘의 효율성 역시 FDP 와 FDS 간의 비율이 3:2 였을 때, 그리고 손잡이 크기가 작을수록 높은 결과를 보였다. 반대로 손잡이의 크기가 커질수록 힘의 효율성은 저하되었다. 또한, 손가락 관절 각도의 경우 FDP와 FDS간의 비율에 따라 상이한 자세를 나타내었다. FDP 굴근의 비율이 높을 경우 손가락 끝마디 관절 (DIP) 의 굴곡을 보였으며, FDS 굴근 비율이 높을 경우 손가락 두 번째 관절(PIP)의 굴곡을 보였다. 본 연구를 통한 결과는 추후 상지작업자에 대한 근골격계 질환 예방 기준안 마련 및 수공구 설계를 위한 기초자료로 활용이 가능할 것으로 보인다.

Muscle-Induced Accelerations of Body Segments (근육의 힘이 신체 각 부분의 가속도에 미치는 영향)

  • When the functional electrical stimulation is employed to recover mobility to the plegic, it is very important to understand functions of the selected muscles. I have investigated how a muscle acts to accelerate the body segments, since the body segements are connected by joints so that contraction of a muscle not only rotates the segments to which it is attached but also causes other segments to rotate by creation a reaction force at every joint, which is called the inertial coupling. I found that a single-joint muscle always acts to accelerate the spanned joint in the same direction as the joint torque produced by the muscle. However, a double-joint muscle can act to accelerate the spanned joint in the opposite direction to the joint torque produced by the muscle depending on (1) the body position, (2) the body-segmental parameters, and (3) the type of the movement. Investigating the condition number of the inertia matrix of the body-segmental model gave us some insights into how controllable the body-segmental system is for different values of the factors mentioned above. The results suggested that the upright position is the most undesirable position to independently control the three segments(trunk, thigh and shank) and that the controllability is the most sensitive to variation of the shank length and the trunk mass, which implies that accuracy is required particularly when we estimate these two body-segmental parameters before the paralyzed muscles are innervated by using electrical stimulation.

Neutral zone approach and external impression for rehabilitation of macroglossia: a case report (거대설을 가진 환자에서 중립대 개념과 연마면 인상을 이용한 총의치 수복 증례)

  • Macroglossia is an obstacle in regard to general prosthodontic restoration. Not only is it difficult to obtain support and stability from dentures manufactured from normal protocol, obtaining efficacy from basic mastication is also difficult. In such cases, realizing harmony between the occlusion and surrounding muscle structures may be important with regard to manufacture of stable full dentures, and it is necessary to form the appropriate polished surface for this case. The neutral zone is defined as the potential area resulting from the neuromuscular function that results in equilibrium between the outward force exerted by the tongue and the inward force exerted by the lips and cheeks. The artificial teeth of the full denture lies in this area, and if the polished zone follows the anatomic form of the dynamic muscles, the movement of the muscles simply acts as an element of stabilizing the dentures improving the esthetics through facial support.

Estimation of Knee Muscle Length and Moment Arm Using Knee Joint Angle (무릎 관절각을 이용한 무릎 근육 길이와 모멘트 암 추정)

  • Recently, lots of studies are performed in developing of active orthosis. Exact and simple muscle force estimation is important in developing orthosis which assists muscle force for disabled people or physical laborers. Hill-type muscle model dynamics is common method for estimation of muscle forces. In Hill-type muscle model, we must know muscle length and moment arm which largely affect muscle force. And several methods are proposed to estimate muscle length and moment arm using joint angle. In this study, we compared estimation results of those method with data from body model of opensim to find which method is exact for estimation of muscle length and moment arm.
