Detection of Viruses and Changes of Protein of Saliva in Patients with Recurrent Aphthous Ulcer (구내 재발성 아프타성 궤양 환자에서 타액내 바이러스 검출 및 단백질의 변화)

  • This study was performed to discover the underlining influences of Herpes Simplex virus (HSV) and Varicella Zoster virus (VZV), to detect the changes of whole protein and mucin level and to observe protein profiles in the saliva when recurrent aphthous ulcer (RAU) was present. Unstimulated whole saliva was collected from 23 patients who for over three years had a clinical history of RAU, in a group of 10 women and 13 men, ranging from 11 to 72 years of age, and 20 healthy subjects, in a group of 8 women and 12 men, who did not have the symptoms nor a past history of RAU. Through the means of Polymerization Chain Reaction, genomic DNA from the HSV and VZV was purified from the saliva samples for identifying precisely the two types of viruses, and the level of whole protein and sialic acids in the saliva and the ratio of sialic acid to whole protein were measured, and SDS-polyacrylamide gel electrophoresis was performed. The results obtained were as follows ; 1. 39.13% of patients showed 224 bp bands of VZV DNA, those were appeared more in patients than in control group (p<0.01), but there was no significant difference between patients and control group in HSV DNA (p>0.05). 2. The concentration of whole protein in men patients was lower than in men control group (p<0.05), but there were no significant differences between patients and control in other groups (p>0.05). 3. The concentration of sialic acids from patients was lower than control group in all groups (p<0.05). 4. The concentration of sialic acids in proportion to that of whole protein was lower in patients than in control group (p<0.05), and in the two women groups (p<0.01), but no noticeable difference was found between the two men groups (p>0.05). 5. There were no consistent differences observed in the protein profiles of patients with control group except that certain protein bands near 50 kDa was lower in patients than in control group. These results suggest that viruses such as HSV and VZV and reduction of salivary whole protein and mucin levels are related to development of RAU.

Phenotypic Characteristics of Pseudomonas syringae pv. actinidiae Strains from Different Geographic Origins (지리적 기원이 다른 Pseudomonas syringae pv. actinidiae 균주들의 표현형적 특성)

  • Pseudomonas syringae pv. actinidiae is the causal agent of bacterial canker in kiwifruit (genus Actinidia). Multilocus sequence analysis of seven housekeeping and 11 type III effector genes differentiated the virulent P. syringae pv. actinidiae isolates worldwide into three groups designated as Psa1-Psa3. In this work, a total of 12 P. syringae pv. Actinidiae strains, including three Psa1, three Psa2, three Psa3 strains isolated from Korea and three Psa3 strains from Italy, were compared based on their phenotypic properties. Strains with different geographic origins had unique growth patterns as demonstrated by growth rate at several temperatures; all tested strains exhibited maximum growth at temperatures below $22^{\circ}C$, while the growth of Psa3 strains was completely inhibited above $30^{\circ}C$. Psa3 strains isolated from Korea had longer lag phases than the Psa3 strains from Italy. The Psa2 strains were different from Psa1 and Psa3 strains in the API 20NE test, in which the Psa2 strains could not utilize potassium gluconate, capric acid and trisodium citrate. Psa3 strains isolated from Korea could hydrolyze esculin. The API ZYM test showed that ${\beta}$-glucosidase activity was detected only from Psa3 strains. The strains belonging to the three Psa groups differed with regard to their susceptibility to ampicillin, novobiocin, and oleandomycin.

Clinical Analysis of Intractable Diabetic Foot Ulcers: Accessing Risk Factors (난치성 당뇨 족부 궤양에 대한 임상적 고찰: 치료에 영향을 미치는 인자 분석)

  • Purpose: Diabetic foot ulcer is one of the most important diabetic complications because it increases the risk of amputations. Moreover, it lowers the quality of patients' life and increases the social medical expenses. Authors analyzed risk factors of intractable diabetic foot ulcer using retrospective study. Materials and Methods: From January 2007 to December 2010, 40 patients who could not achieve complete healing despite more than 12 weeks of proper management among who had been diagnosed and treated as diabetic foot ulcer at our hospital were included and evaluated retrospectively. We compared the risk factors between two groups who were finally treated by amputation and non-amputation. Results: The sample was composed of 31 male patients (77.5%) and 9 female patients (22.5%). Comorbidity including hypertension and hyperlipidemia were 77.5% and 80% each. By Wagner classification, 30 patients (80%) had ulcerative lesion over the grade 3. From bacteriology results, 29 patients (72.5%) had polybacteria infection. 35 patients (87.5%) had neuropathy and 26 patients (65%) had vascular stenosis at least one level. The mean initial ankle-brachial index and toe-brachial index were 0.982 and 0.439. In comparison between amputation group and non-amputation group, ulcer severity, number of stenotic vessel and initial ankle-brachial index/toe-brachial index had statistical significance. Conclusion: The most commonly risk factor of intractable diabetic foot ulcer was peripheral neuropathy reaching 87.5% of cases. In comparison with non-amputation group, ulcer severity according to Wagner classification, number of stenotic vessel and initial ankle-brachial index/toe-brachial index were demonstrated as a risk factor of amputation in intractable diabetic foot ulcer.

Simultaneous Infection of Duck Hepatitis Virus and Salmonella typhimurium in Ducklings (오리에서 Duck Hepatitis Virus와 Salmonella typhimurium의 혼합 감염 증례)

  • In January 2010, high mortalities of ducklings occurred in two adjacent farms. Ducklings in farm A showed neurologic signs including paddling, opisthotonus and lameness, but ducklings from farm B had no specific clinical signs. At post mortem, diffused hemorrhagic spots in the liver and hemorrhages in the small intestine were observed in ducklings from farm A, and ducklings from farm B had only proventricular ulceration. Microscopically, multiple necrosis of hepatocytes, hyperplasia of the bile ducts, hemorrhages, infiltration of lymphocytes and bacterial colonies were commonly observed in the liver of ducklings from both farms. Also, type A and C duck hepatitis virus (DHV) were isolated from farm A and farm B, respectively and Salmonella typhimurium was commonly isolated and identified serologically and biochemically. To our knowledge, this is the first report about the co-infection of DHV and Salmonella typhimurium in ducklings, and co-circulation of type A and C duck hepatitis viruses in Korea.

Occurrence of Atypical Aeromonas Salmonicida in Mandarin Fish (SSOGARI: Korean Name) Siniperca Scherzeri (양식 쏘가리에서 Atypical Aeromonas Salmonicida의 발병)

  • Mandarin fish (SSOGARI: Korean name) has emerged as new aquaculture and aquarium fish among Korean native fish species. Unfortunately, this fish could not exempt from infectious agent invasion. As an epidemiological survey, high mortality of mandarin fish (Siniperca scherzeri) observed from a fish farm in Gyeongnam province of South Korea. The major macroscopic sign of the diseased fish was severe multifocal dermal ulceration. Histological observation revealed inflammation, necrosis and colonization of bacteria in various tissues. Seven bacterial isolates from the diseased fish were identified as Aeromonas salmonicida subsp. salmonicida (typical A. salmonicida) using API 20E and 20NE system. However, the polymerase chain reaction (PCR) assay using two different primers (AP and MIY) revealed that the bacterial strains were not typical A. salmonicida, but atypical A. salmonicida. This study demonstrates that PCR assay is a reliable method for the confirmation of atypical A. salmonicida in mandarin fish as compared to biochemical tests. Furthermore, this is the first account of extensive dermatitis in mandarin fish due to atypical A. salmonicida infection, which has high potential in aquaculture and aquarium fish among native fish species.

Diagnosis and Treatment of Feline Inflammatory Polyp with Video Otoscopy : A Case Report (고양이에서 비디오 이경검사법을 이용한 염증성 폴립의 진단 및 치료 1례)

  • Otic mass removal was performed in a 19-month-old, castrated domestic shorthair cat. The patient had 1- year history of recurrent otitis externa, and then otic mass in the right ear canal was found. Under general anesthesia, 2.7 mm rigid endoscope was inserted to the right ear canal with the irrigation system. The ovoid-shaped, 4.9 mm in diameter red otic mass located in the right ear canal was removed via traction-avulsion. Then, rupture of the tympanic membrane was revealed and otic flushing was performed with sterile isotonic (0.9%) saline to remove exudates. Histologically, the removed polyp was diagnosed as granulation tissue with severe ulceration. The patient didn't reveal any remarkable abnormality after surgery, and no recurrence were found after 5 months follow up. The video otoscopy seems to offer a useful option for treatment of a feline inflammatory polyp.

Anti-inflammatory and apoptosis improving effects of sulfasalazine and Cinnamomi cortex and Bupleuri radix mixture in TNBS-induced colitis mouse model (TNBS로 유발된 마우스 대장염모델에서 설파살라진, 육계와 시호 복합 추출물의 항염증 및 세포 자멸 개선효과)

  • Inflammatory bowel disease (IBD) is including Crohn's disease and ulcerative colitis. Sulfasalazine commonly used in IBD, possibly has various side effects after high dosage and long term intake. The present study aimed to investigate the sulfasalazine and combination with herbal medicine on 2,4,6-trinitrobenzene sulfonic acid (TNBS)-induced in mice model. TNBS-induced mice were injected through a flexible catheter 4 cm in length 1.6 mg TNBS. Animals were divided into five groups (n=12): Normal group, TNBS control group, Sulfasalazine (30 mg/kg) group, Sulfasalazine (60 mg/kg) group, Sulfasalazine (30 mg/kg)+Cinnamomi cortex and Bupleuri radix mixture (30 mg/kg) (SCB) group. Administration groups were fed extract during 7 days. The inflammatory, and apoptotic protein levels were determined using western blotting. SCB treatment showed an outstanding effectiveness in counteracting the IBD, as assessed by reduction of body weight loss, down-regulation of pro-inflammatory proteins and cytokines, and by inhibition of proteins related to apoptosis. This is the first report that sulfasalazine and Cinnamomi cortex plus Bupleuri radix mixture improve the severity of experimental IBD through the inhibition of both inflammation and apoptosis. We confirm that the SCB treatment instead of sulfasalazine alone may be promising as an alternative therapeutic plan against IBD, without any evidence of adverse effects.

Gastropericardial Fistula as a Late Complication after Esophagectomy with Esophagogastrostomy, A Case report (식도 절제술 및 위-식도문합술 후 만기 합병증으로 발생한 위-심막루)

  • A 56 year-old man complaining of dry cough, dyspnea, chest pain, fever, and chills was admitted to the emergency room. The patient had a history of esophagectomy and esophagogastrostomy and subsequent radiotherapy because of an esophageal cancer. After the emergency echocardiography revealed a small amount of pericardial effusion and pneumopericardium. Upper GI contrast study showed a fistulous tract between the stomach and the pericardium, and an emergency operation was done under the diagnosis of gastropericardial fistula. The patient expired postoperative seven days later. Gastropericardial fistula caused by a peptic ulcer perforation after the esophagectomy and esophagogastrostomy operation is a very rare complication and brings forth a disastrous result. Early detection using the chest radiography, electrocardiogram, upper Gl study, echocardiography and a review of physical examination, and an immediate treatment are therefore mandatory.

Selection of Lactic Acid Bacteria Specifically Inhibiting the Growth of Helicobacter pylori (Helicobacter pylori의 생육을 특이적으로 억제하는 유산균 선발)

  • This study was conducted to select lactic acid bacteria which possess potential inhibitory effect on Helicobacter pylori, and to make feasibility test of fermented milk products using them. In order to select lactic acid bacteria specifically inhibiting the growth of H. pylori, antibacterial activity using paper disk method, adherence ability to Caco-2 cell inhibitory effect on urease activity of H. pylori, and milk fermentation feasibility were measured. Among 45 strains of lactic acid bacteria tested, 28 strains showed clear zone and Lactobacillus gasseri MK-03 showed the largest clear zone. Caco-2 cell adherence by lactic acid bacteria and inhibitory effect of them on H. pylori adherence were also evaluated. Of 28 strains tested, 18 strains appeared to be effective on adherence to Caco-2 cell, and especially Bifidobacterium longum MK-26 was found to be superior to others. When Bif. longum MK-26 and H. pylori were reacted with Caco-2 cell 2hrs before, adherence percentage of H. pylori decreased from 0.105% to 0.004%. To investigate inhibitory effect of lactic acid bacteria-derived supernatant on urease activity of H. pylori, pH-adjusted fermented supernatant(pH-4.4) was assessed by co-cultivation method. There of Lb. acidophilus MK-07-derived supernatant showed the most inhibitory effect on urease activity of H. pylori. Considering milk fermentation ability of selected 3 strains, they were comparably feasible to fermented milk products. Consequently, Lb. gasseri MK-03, Lb. acidophilus MK-07, and Bif. longum MK-26 were selected to specifically inhibit the growth of H. pylori, by antibacterial activity, inhibition of urease activity, and inhibition of Caco-2 cell adherence, respectively.

  • This study investigated external symptoms, behavior characteristics and the distribution pattern of the scuticociliates in the skin, gill and blood vessel of the tiger puffer, Takifugu rubripes, infected with scuticociliates among tiger puffer cultured. The fish infected with scuticociliates did not show any external symptoms, such as change in body color or swimming behavior, in the early infection stage. However, they showed congestion, erosion, and ulcer on the skin and fin at advanced stages. They showed a turning movement, mainly stayed on the bottom, and swam with their mouths open at the surface of water. Some fish showed a sudden swimming movement of zigzag type. The scuticociliates were ovoid in shape and $20\times40{\mu}m$ in size. The scuticociliates had a contractile vesicle and a caudal cilium that ranged 10-12 ${\mu}m$, and reproduced by binary fission. Therefore the scuticociliates were identified as ciliated, belonging to class Hymenostomatia, order Scuticociliatida in morphogenetic character. The gills infiltrated with scuticociliates showed clubbed gill filament due to hypertrophy of gill lamella. Within the blood vessels, scuticociliates were observed one to ten individuals, depending on diameter of the blood vessels. Some of the scuticociliates were observed to have ingested erythrocytes.
