  • This study is designed to evaluate the effects of an oriental herbal medication namely Danggwihwahyeolsan-gamibang and Gami-Jeonssibaekchulsan, on ulcerative colitis. Subject: A patient diagnosed ulcerative colitis who're not improved through western medication. Methods: The Clinical data was analyzed on a pastient with ulcerative clitis due to Changpung(腸風) resulted from Hanyeolchakjab(寒熱錯雜), whose symptoms were combined diarrhea, abdominal pain et al. The patient visited at the internal medicine department of Daegu Haany University affiliated Daegu Oriental Hospital on May 18. 2007 to June 25. 2007. The improvement of symptoms were checked by Seo Colitis Activity Index(AI).

황련해독탕이 DSS로 유발된 흰쥐의 궤양성 대장염에 미치는 영향 (Effects of Hwangryunhaedoktang on DSS-induced Colitis)

  • Objectives : We examined the effect of Hwangryunhaedoktang(HH) on the experimental ulcerative colitis induced by dextran sulfate sodium (DSS). Methods : Experimental colitis was induced in rats by daily treatment with 5% DSS in the drinking water for 5 days. Afterward, the mts were divided into two groups: the control group was administered water and the sample group was administered HH for 7 days. Results : The sample group provided HH for 7 days demonstrated fast recovery of body weight compared with the control group. Histologic change showed fast regeneration of crypt and surface epithelial cells and decreased edema of the submucosa and decreased lymphatic follicle of mucosa compared with the control group. Immunohistochemical stain usingCOX-2 gene was decreased and there was localized Ki-67 positive reaction. Regeneration of surface epithelial cell and goblet cell in mucosa was observed by transmission electron microscope. These results indicate therapeutic effect of HH on DSS-induced colitis in rats.

  • Ulcerative colitis, a diffuse inflammatory disease of the mucosal lining of the colon and rectum, is characterized by bloody diarrhea that waxes and wanes without apparent cause. In oriental medicine, Jakyaktang has been used to treat the disease, diagnosed as Damp-Heat dysentery. We prescribed Jakyaktang-gamibang to two patients who had suffered from ulcerative colitis diagnosed as Damp-Heat dysentery. The two patients were treated with Jakyaktang-gamibang and improved in consciousness of symptoms. We suggest that oriental medicine therapy is effective for treatment of ulcerative colitis.

Dextran Sulfate Sodium 유도 흰쥐 궤양성 대장염에 대한 산초 추출물의 효과 (The Effects of Zanthoxylum schinifolium Extract in a Dextran Sulfate Sodium-induced Rat Colitis Model)

  • The aim of this study was to evaluate the anti-inlfammatory effect of the extract of Zanthoxylum schinifolium on ulcerative colitis induced by 3% dextran sulfate sodium in the rat. The experiment animals were divided into six groups: control (normal), DDS-induced colitis, 1 mg/kg, 15 mg/kg and 150 mg/kg of the extract of Z. schinifolium and 150 mg/kg of 5-aminosalicylic acid (5-ASA) as comparative drug. Anti-ulcerative colitis effect was evaluated pathologically by disease activity index (DAI), the change of weight and myeloperoxidase (MPO) in colon mucosa. Treatment with 15 mg/kg and 150 mg/kg of the extract of Z. schinifolium significantly improved the gain of body weight and DAI as clinical symptoms, and reduced MPO and $PGE_2$ level as biochemical index. Especially, 150 mg/kg of the extract of Z. schinifolium showed markedly more improvement than the same dose of 5-ASA in all kind of index such as MPO, $PGE_2$ and DAI. These results suggest that Z. schinifolium mediated anti-inflammatory action on colorectal sites may be a useful therapeutic approach to ulcerative colitis.

궤양성 대장염 유발 생쥐에서 두시추출물의 항염증 효과 (The Anti-Inflammatory Effect of Duchi extract on Ulcerative Colitis)

  • Objective The purpose of this study is to learn the anti-inflammatory effect of Douchi on ulcerative colitis (UC). Methods Three-month-old mice were divided into 4 groups as follows: control group (Ctrl), UC induced group (UCEG), Pentasa treated group after inducing UC (OPTG), and Douchi treated group after inducing UC (FGTG). NF-κB, p-IκB, iNOS, COX-2 were observed by immunohistochemistry and Masson trichrome, PAS, and Phloxine-tartrazine staining were used to observe histochemical changes. Results Inflammation indicators of the large intestine were significantly lower in FGTG than in the UCEG and OPTG. Also, indicators involved in pulmonary alveolar formation were significantly higher in the FGTG than in the UCEG and OPTG. Conclusions The result of this study suggests that Douchi extract was effective in ulcerative colitis and helped in the formation of alveolar. This result suggests that the lungs and colon are correlated.

작약, 감초 및 작약감초탕의 투여가 궤양성 대장염 동물 모델에 미치는 영향: 대사산물 변화를 포함하여 (Effects of Paeoniae Radix, Glycyrrhiza Uralensis and Jakyakgamcho-tang Treatment on Ulcerative Colitis Animal Model: Including Changes in Metabolites)

  • Objectives To investigate the effect of Jakyakgamcho-tang (JGT) on Dextran sulfate sodium (DSS) induced ulcerative colitis. Methods Experimental animals were divided into six groups; group 1, normal group; group 2, DSS-induced colitis group; group 3, 5-aminosalicylic acid (5-ASA)-treated group; group 4, Glycyrrhiza Uralensis (Gamcho, GC)-treated group; group 5, Paeoniae Radix (Jakyak, JY)-treated group; group 6, JGT-treated group. Inflammatory cytokines and their metabolites were detected. Result In the JGT group, the levels of tumor necrosis factor alpha (TNF-α), prostaglandin E2 (PGE2) were significantly decreased, whereas that of Interleukin-10 (IL-10) were significantly increased. In addition, the metabolite profile changed in the JGT group with most metabolites increasing. Conclusion This study demonstrates that the therapeutic potential of JGT in ulcerative colitis. Further studies should be conducted to confirm our findings.

궤양성 대장염에서 식이 인자와 장 마이크로비오타의 상호작용 (Interaction between Dietary Factors and Gut Microbiota in Ulcerative Colitis)

  • Ulcerative colitis (UC) exhibits chronic intestinal inflammatory conditions with cycles of relapse and remission. The incidence is rapidly growing in Asian countries including South Korea possibly due to changes in lifestyles. Although the etiology of inflammatory bowel disease is inconclusive, gut microbiota composition is considered a critical factor involved in the pathogenesis of UC. The overgrowth of pathogenic bacteria evokes hyper-immune responses in gut epithelium causing tissue inflammation and damage. Also, failure to regulate gut epithelium integrity due to chronic inflammation and mucus depletion accelerates bacterial translocation aggravating immune dysregulation. Gut microbiota composition responds to the diet in a very rapid manner. Epidemiological studies have indicated that the risk of UC is associated with low plant foods/high animal foods consumption. Several bacterial strains consistently found depleted in UC patients use plant food-originated dietary fiber producing short chain fatty acids to maintain epithelial integrity. These bacteria also use mucus layer mucin to keep gut microbiota diversity. These studies partly explain the association between dietary modification of gut microbiota in UC development. Further human intervention trials are required to allow the use of specific bacterial strains in the management of UC.

한약재 특성에 따른 궤양성 대장염 치료과 장내세균 상관관계 연구 (The Study about Relationship between Treatment of Ulcerative Colitis and Intestinal Bacteria According to Characteristic of Herbal Medicine)

  • Many modern people are exposed to chronic inflammatory diseases, such as inflammatory bowel disease (IBD), atopic dermatitis and immune disorder. Among those chronic diseases, the incidence ratio of IBD has been increased. IBD, including Crohn's disease and ulcerative colitis (UC), is known to cause abnormal inflammation in intestinal tissue. UC is accompanied by abdominal pain, bloody stool and diarrhea. Many therapeutic agents, such as sulfasalazine, corticosteroids, immunosuppressive agents, have been used for treating UC. However, those agents have side-effects and temporary effects on UC. The aim of this study was to investigate the effect of herbal medicine on UC and relationship between UC and intestinal bacteria according characteristic of herbal medicine. To determine the effect of herbal medicine on UC, various herbal medicine were chosen within oriental medicine category such as cheongyeol and onyeol medicine. In this study, we found carthami fructus, included in cheongyeol medicine, had stronger effect than onyeol medicine. Also, we determined influence of carthami fructus against lactic acid bacteria. Catthami fructus and lingon berry extracts affected the composition of mice intestinal bacteria in mice fecal. The symptoms of UC could be regulate by using herbal medicine, according to characteristic of herbal medicine. Also, herbal medicine might be change body condition to healthy by controlling intestinal bacteria composition. Herbal medicine characteristic could be a therapeutic agent by revealing relationship between intestinal bacteria and UC.

염증장병 환아의 혈액학적 소견 (Hematologic Features of Children with Inflammatory Bowel Disease)

  • 목 적: 최근 우리나라 소아에서 염증장병이 꾸준히 증가추세에 있으나 이들의 혈액학적 특징에 대해서는 알려진 바가 없다. 국내 염증장병 환아들의 장기간 관리에 도움이 되고자 혈액학적 소견을 분석하여 보았다. 방 법: 가천의대 길병원 소아청소년과에서 2002년 9월부터 2008년 9월 사이에 대장경과 생검으로 확진된 염증장병 환자 중 18세 이하인 25명의 염증장병 환아를 대상으로 하였다. 진단 당시의 혈색소, 적혈구용적률, 백혈구 수, 혈소판 수, 평균적혈구용적, 평균적혈구 혈색소량을 조사했다. 환아의 빈혈 빈도, 혈색소와 질병 이환기간과의 관계, 혈색소와 활동도지수와의 관계에 대하여 알아보았다. 결 과: 빈혈은 전체적으로 76% (19/25)에서 있었으며, 크론병 환아에서는 88% (14/16), 궤양결장염 환아에서는 56% (5/9)였다. 백혈구증가와 혈소판증가는 크론병 환아에서만 있었으며 빈도는 각각 56% (9/16), 25% (4/16)였다. 증상의 이환기간과 혈색소치는 크론병과 궤양결장염 모두에서 통계적인 유의성이 없었다. 활동도지수와 혈색소치도 크론병과 궤양결장염 모두에서 통계적인 유의성이 없었다. 결 론: 본 연구에서 염증장병 환아의 혈액학적 소견은 외국 문헌의 보고 내용과 큰 차이는 없었으나 빈혈의 빈도가 높았으며 질병의 이환기간 또는 활동도지수와 빈혈은 상관관계가 없었다. 이러한 사실은 염증장병환아를 장기간 관리하는 동안 빈혈에 대한 관리 역시 포함해야 하는 것을 시사하였다. 백혈구증가와 혈소판증가는 크론병에서만 관찰되어 궤양성장염과 감별진단하는데 도움이 될 수 있음을 제시하였다.

순창산 참두릅과 도라지의 항산화 활성 및 항염증 활성에 관한 연구 (Studies on Antioxidant and Anti-inflammatory Activities of Aralia elata Seem and Platycodon grandiflorum of Cultivated in Sunchang-gun)

  • 염증은 물리화학적 자극이나 세균 감염과 같은 외부 자극에 대응하기 위한 생체조직의 방어 반응의 하 나이며 조직이나 장기의 손상을 회복시키는 기전으로서 매우 중요한 역할을 하지만 염증반응이 과도해 질 경우 궤양성 대장염, 기관지염, 천식, 대장암 등을 유발하는 원인으로 작용하기 때문에 염증반응을 조 절하는 항염증제 개발은 매우 중요시되고 있다. 국내 염증개선 관련 시장은 현재 약 3조원 규모이며 지속 적인 성장세에 있지만 천연소재의 기능성 중심으로 출시된 제품 중 다수의 제품에서 주로 수입 원료를 사용하고 있어 국내 자생 및 재배하는 천연 소재들의 효능에 대한 검증을 통해 수입 원료 의존도를 낮추 기 위한 연구가 요구되고 있다. 따라서 본 연구에서는 순창에서 재배되는 참두릅 (Aralia elata Seem)과 도라지 (Platycodon grandiflorum)를 이용하여 천연 염증개선 소재화를 목적으로 기능성 증진을 위한 추 출조건 선정과정과 대식세포에 대한 세포독성 및 항염증 효능을 확인하여 천연소재를 이용한 염증개선 소재화를 시도하고자 한다. 본 연구는 전북 순창에서 재배되는 참두릅과 도라지의 추출조건을 선정하기 위해 용매, 온도 및 시간별 추출물의 total polyphenol 함량 평가를 통하여 최적 추출조건 선정을 진행하 였으며, 선정된 추출조건에서 추출된 추출물의 항산화 활성을 측정하기 위하여 DPPH & ABTS radical scavenging activity 및 total flavonoids 함량을 확인하여 항산화 효능을 평가하였다. 또한 대식세포인 Raw 264.7을 사용하여 MTT assay, Nitric oxide (NO) 생성 억제 효능을 확인하여 세포독성 및 항염증 활 성을 평가하였다. 실험결과, Total polyphenol 함량 분석을 통해 최적 추출조건이 선정된 두릅 (주정 40%, 25℃, 3 h), 도라지 (주정 60%, 25℃, 1 h)의 추출물을 이용하여 DPPH & ABTS radical scavenging activity 및 total flavonoids 함량을 분석한 결과, 도라지보다 두릅에서 더 높은 항산화 활성을 나타내었다. 대식세포를 활용한 두릅과 도라지의 세포독성을 측정한 결과, 100 ug/mL 이내의 농도에서 독성활성이 나타내지 않음을 확인되었으며, 항염증 활성을 측정한 결과 100 ug/mL에서 두릅 추출물이 도라지 추출 물보다 약 33% 이상 NO 생성억제 활성이 높게 나타내었다.
