Pain control using the Point-Inject Technique in dental local anesthesia (Point Inject Technique을 이용한 치과 국소마취의 통증 조절 및 진료 효율의 극대화)

  • Many approaches to local anesthesia have been studied in dentistry. In this study, we introduce a new local anesthetic method, "Point-Inject Technique (PIT)", and compare it with traditional injection techniques. The PIT method utilizes both the vasoconstrictive and antinociceptive properties of local anesthetics as well as the application of controlled pressure during injection, reducing the time to complete anesthesia. Fifty patients were selected as the experimental group who were anesthetized using PIT, and the other 50 patients were selected as the control group using the direct injection method with a carpool syringe. The PIT group received 0.25 cartridges of 2% lidocaine with 1:100,000 epinephrine. The control group received 1.5~2 cartridges of 2% lidocaine with 1:100,000 epinephrine. Both groups were asked to mark the intensity of the pain caused by anesthesia using the Numeric Pain Rating Scale. The average time to recover from anesthesia was 40 minutes in the experimental group and 90 minutes in the control group. Additionally, 96% of the experimental group reported feeling no pain, while 78% of the control group reported having some form of pain during injection. The PIT method reduced both the reported pain scores of patients as well as time to recover from local anesthesia than the widely-used syringe injection method.

Estimation of Internal Motion for Quantitative Improvement of Lung Tumor in Small Animal (소동물 폐종양의 정량적 개선을 위한 내부 움직임 평가)

  • The purpose of this study was to estimate internal motion using molecular sieve for quantitative improvement of lung tumor and to localize lung tumor in the small animal PET image by evaluated data. Internal motion has been demonstrated in small animal lung region by molecular sieve contained radioactive substance. Molecular sieve for internal lung motion target was contained approximately 37 kBq Cu-64. The small animal PET images were obtained from Siemens Inveon scanner using external trigger system (BioVet). SD-Rat PET images were obtained at 60 min post injection of FDG 37 MBq/0.2 mL via tail vein for 20 min. Each line of response in the list-mode data was converted to sinogram gated frames (2~16 bin) by trigger signal obtained from BioVet. The sinogram data was reconstructed using OSEM 2D with 4 iterations. PET images were evaluated with count, SNR, FWHM from ROI drawn in the target region for quantitative tumor analysis. The size of molecular sieve motion target was $1.59{\times}2.50mm$. The reference motion target FWHM of vertical and horizontal was 2.91 mm and 1.43 mm, respectively. The vertical FWHM of static, 4 bin and 8 bin was 3.90 mm, 3.74 mm, and 3.16 mm, respectively. The horizontal FWHM of static, 4 bin and 8 bin was 2.21 mm, 2.06 mm, and 1.60 mm, respectively. Count of static, 4 bin, 8 bin, 12 bin and 16 bin was 4.10, 4.83, 5.59, 5.38, and 5.31, respectively. The SNR of static, 4 bin, 8 bin, 12 bin and 16 bin was 4.18, 4.05, 4.22, 3.89, and 3.58, respectively. The FWHM were improved in accordance with gate number increase. The count and SNR were not proportionately improve with gate number, but shown the highest value in specific bin number. We measured the optimal gate number what minimize the SNR loss and gain improved count when imaging lung tumor in small animal. The internal motion estimation provide localized tumor image and will be a useful method for organ motion prediction modeling without external motion monitoring system.

The Results of Hyperfractionated Radiotherapy on Locally Advanced Non-Small Cell Lung Cancer (국소적으로 진행된 비소세포 폐암에 대한 과분할 방사선 치료의 성적)

  • Purpose : The effect of hyperfractionated radiotherapy on locally advanced non-small lung cancer was studied by a retrospective analysis. Materials & Methods : We analyzed sixty one patients of biopsy-confirmed, IIIA and IIIB non-small cell lung cancer. Using the ECOG performance scale, all the patients were scored less than 2. They were treated by curative hyperfractionated radiotherapy alone from Oct. 1992 to Oct. 1995 at the Department of Radiation Oncology. All the patients received 120cGy b.i.d with more than 6 hours interval between each fraction. The total dose of radiation was reached up to 6400-7080 cGy with a mean dose of 6934 cGy. The results were analyzed retrospectively. Results : The overall survival rate was 53 1$\%$ in 1 year, 9.9$\%$ in 2 years with a median survival time (MST) of 13.9 months. The progression free survival (PFS) rate was 37.0$\%$ in 1 year, 8.9$\%$ in 2 years. Twenty two Patients were classified as complete responders to this treatment and their MST was 19.5 months When this was compared with that of partial responders (MST: 11 7months), it was statistically significant (p=0.0003). Twenty nine patients of stage IIIA showed a better overall survival rate (1yr 63.3$\%$, 2yr 16.8$\%$) than IIIB patients (1yr 43.3$\%$, 2yr 3.6$\%$), which was also statistically significant (p=0.003). Patients with adenocarcinoma showed a better survival rate (1yr 64.3$\%$, 2yr 21.4$\%$) than that of squamous cell counterpart (1yr 49.4$\%$, 2yr 7.4$\%$), although this was not significant statistically (p=0.61). Two patients developed fatal radiation-induced pneumonia right after the completion of the treatment which progressed rapidly and they all died within 2 months. One patient developed radiation-induced fibrosis after 13 months. He refused further treatment and died soon after the development of fibrosis. Conclusion : Among locally advanced NSCLC, hyperfractionated radiotherapy was effective on stage IIIA patients by increasing MST with acceptable toxicities. Acute radiation-induced pneumonia should be carefully monitored and must be avoided during or after this treatment.

  • In this paper the Maximum Block Method is suggested for the Feature Extraction of stokes of off-line Chinese characters.The Maximum Block Method is a technique which enlarges the block from the first found pixel that wxtracts the skeleton and features of the input characters.The maximum Block mthod is an adequate technique for the correct extraction of the features since the exsting thining methods have shortcomings of making the feature extraction difficult from the distoritions generated from the effiects of the parial noises,inflection points and blemishes. The printed outputs and chinese books of the middle and high school students,and other materials are used for the test.It was found that the Maxthod is also an effective technique for the extraction of skeleton line and features,which is the preoprocessing of the pattern recognition,for the Korean chracters and English as well as chinese chracters.

Osteoblastoma of the Distal Fibula - A Case Report - (비골 원위부 골아세포종 - 증례보고 -)

  • We report an unusual case of osteoblastoma in the distal fibula in a 32-year-old man. Radiographs showed a ballooning lesion with internal trabeculation. MR images demonstrated a lobulated expanding lesion of central heterogeneous low signal intensity with enhancement. Curettage specimen was composed of abundant thick, pink osteoid without mineralization. The osteoid seams were lined by plump osteoblasts and a few giant cells. Focally, a fine unmineralized lace-like osteoid was seen.

  • As the semiconductor processing technology has been developing, multiple cores or NoC(network on chip) can be contained in recent chips. GALS(globally asychronous locally synchronous) clocking scheme that has multi-clock domains with different frequencies or phase differences is widely used to solve power consumption and clock skew in a large chip with a single clock. A synchronizer is needed to avoid a synchronization problem between sender and receiver in GALS. In this paper, the setup and hold time of DFF required to design the synchronizer are measured using 180nm CMOS processing parameters depending on temperature, supply voltage, and the size of inverter in DFF. The simulation results based on the bisection method in HSPICE show that the setup and hold time are proportional to temperature, however they are inversely proportional to supply voltage, and negative values are measured for the hold time.

Two-Dimensional Shape Description of Objects using The Contour Fluctuation Ratio (윤곽선 변동율을 이용한 물체의 2차원 형태 기술)

  • In this paper, we proposed a contour shape description method which use the CFR(contour fluctuation ratio) feature. The CFR is the ratio of the line length to the curve length of a contour segment. The line length means the distance of two end points on a contour segment, and the curve length means the sum of distance of all adjacent two points on a contour segment. We should acquire rotation and scale invariant contour segments because each CFR is computed from contour segments. By using the interleaved contour segment of which length is proportion to the entire contour length and which is generated from all the points on contour, we could acquire rotation and scale invariant contour segments. The CFR can describes the local or global feature of contour shape according to the unit length of contour segment. Therefore we describe the shape of objects with the feature vector which represents the distribution of CFRs, and calculate the similarity by comparing the feature vector of corresponding unit length segments. We implemented the proposed method and experimented with rotated and scaled 165 fish images of fifteen types. The experimental result shows that the proposed method is not only invariant to rotation and scale but also superior to NCCH and TRP method in the clustering power.

  • The normal tissue in radiation therapy, to minimize radiation, it is most important to maximize local tumor control rates in intensive research the exact dose to the tumor sites. Therefore, the initial, therapist accuracy of detecting movement of the patient fatigue therapist has been a problem that is weighted down directly. Also, by using a web camera, a difference value between the image to be updated to the reference image is calculated, if the result exceeds the reference value, using the system for determining the motion has occurred. However, this system, it is not possible to quantitatively analyze the movement of the patient, the background is changed when moving the treatment bed in the co-therapeutic device was not able to sift the patient. In this paper, using a alpah(${\alpha}$) filter index is an attempt to solve these limitations points, quantifies the movement of the patient, by separating a background image of the patient and treatment environment, and movement of the patient during treatment It senses only, it was possible to reduce the problems due to patient movement.

Experimental Study on the Wall-Wetting Formation and Spray Characteristics of Gasoline Engine Injector (가솔린엔진 인젝터의 벽류 및 분무특성에 관한 실험적 연구)

  • Fuel spray characteristics of the gasoline engine injector has been studied experimentally. Wall wetting fuel stream of the 4-hole and 12-hole injectors has been tested and measured with various installation angle and port masking shapes. Spray visualization has been performed to analyze spray formation, spray angle, and penetration length. Test result shows that wall wetting is greatly influenced by the induction air flow and injector installation angle. Wall wetting amount decreased as injector installation angle decreased. Masking decreased wall wetting amount by increasing local intake-air flow velocity due to the decreased section area. Spray visualization showed that the 12-hole injector has robust performance characteristics compared with the 4-hole injector.

  • Item response theory(IRT) estimates latent ability of a subject based on the property of item and item parameters using item characteristics curve(ICC) of each item case. The initial value and another problems occurs when we try to estimate item parameters of IRT(e.g. the maximum likelihood estimate). Thus, we propose the asymptotic approximation method(AAM) to solve the above mentioned problems. We notice that the proposed method can be thought as an alternative to estimate item parameters when we have small size of data or need to estimate items with local fluctuations. We developed 'Any Assess' and tested reliability of the system result by simulating a practical use possibility.
