Aortic Valvuloplasty in Pediatric Age (소아연령군에서의 대동맥판막성형술)

  • Background: In this study, we retrospectively analyzed the outcomes of aortic valvuloplasty in pediatric age. Material and Method: Between January 1993 and March 2004, 35 patients underwent aortic valvuloplasty for aortic stenosis (AS) or aortic regurgitation (AR). The mean age was 81.1$\pm$61.5 (1∼223) months. The mean follow up was 50.8$\pm$30.2 (3∼121) months. Nine patients had AS, 21 had AR, and 6 had AS and AR. Valve morphology was tricuspid in 24 patients, bicuspid in 9, quadricuspid in 1, and unicuspid in 1. The mean peak pressure gradients of AS were 72.0$\pm$33.0 mmHg, and the mean grades of AR were 3.1$\pm$0.9. Result: There was one late mortality without early mortality. After operation, AS improved with mean peak pressure gradients of 23.5$\pm$21.0 mmHg (p < 0.05), and AR improved with mean grades of 1.9$\pm$0.8 (p < 0.05). At mean follow up of 35.0$\pm$23.0 months, AS maintained with mean peak pressure gradients of 31.5$\pm$24.0 mmHg, but AR progressed with mean grades of 2.8$\pm$1.3 (p < 0.05). Reoperation was required in 6 patients 38.3$\pm$21.8 months after the original operation. The actuarial figures for freedom from reoperation at 2, 5 and 8 years were 96.9$\pm$3.1%, 79.5$\pm$5.5%, and 56.8$\pm$11.4%, respectively. Age at operation, presence of AS, preoperative severity of AS or AR, and morphology of aortic valve were not significant risk factors for reoperation, and improvement of AS or AR. Conclusion: Aortic valvuloplasty showed good immediate postoperative valve function. Aortic valvuloplasty offers children many years with tolerable valve function and allows to postpone aortic valve replacement or Ross procedure in pediatric patients.

Effect of starch extraction solutions on extraction and physicochemical property of Chinese yam (Dioscorea batatas) starch (마(Dioscorea batatas) 전분의 추출 및 물리화학적 특성에 대한 전분추출용액의 영향)

  • This study investigated the influence of starch extraction (ST-EX) solutions on the extraction yield and characteristics of Chinese yam (CY) starches from domestic Dioscorea batatas. Ascorbic acid (VitC), $Na_2S_2O_3$, $NaHCO_3$, and $Na_2CO_3$ were used as ST-EX solutions (0.4%, w/v). The extracted CY starches were examined for ST-EX yield, chemical composition, size distribution, X-ray diffraction, solubility, swelling power, gelatinization, and pasting viscosity. The highest ST-EX yield was obtained from $NaHCO_3$, followed by VitC. Lower protein content, relative crystallinity, and gelatinization enthalpy were found in CY starches from alkaline ST-EX solutions ($NaHCO_3$ and $Na_2CO_3$). Size distribution and gelatinization temperature did not generally differ for CY starches from the different ST-EX solutions. Pasting viscosities increased in the order from $Na_2CO_3$ > $Na_2S_2O_3$ > $NaHCO_3$ > VitC ST-EX solutions. Thus, VitC may be most appropriate to extract CY starch from Dioscorea rhizomes, considering its ST-EX yield, total starch content, and variation in pasting viscosity.

A Convenient Radiolabeling of [$^{11}$C](R)-PK11195 Using Loop Method in Automatic Synthesis Module ($^{11}$C 표지 자동합성장치에서 루프법을 이용한 ($^{11}$C)(R)-PK11195의 간편한 합성법)

  • Purpose: ((R)-1-(2-chlorophenyl)-N-1-[$^{11}$C]methyl-N(1-propyl)-3-isoquinoline carboxamide ((R)-PK11195) is a specific ligand for the peripheral type benzodiazepine receptor and a marker of activated microglia, used to measure inflammation in neurologic disorders. We report here that a direct and simple radiosynthesis of [$^{11}$C](R)-PK11195 in mild condition using NaH suspension in DMF and one-step loop method. Materials and Methods: (R)-N-Desmethyl-PK11195 (1 mg) in DMSO (0.1 mL) and NaH suspension in DMF (0.1 mL) were injected into a semi-prep HPLC loop. [$^{11}$C]methyl iodide was passed through HPLC loop at room temperature. Purification was performed using semi-preparative HPLC. Aliquots eluted at 11.3 min were collected and analyzed by analytical HPLC and mass spectrometer. Results: The labeling efficiency of [$^{11}$C](R)-PK11195 was 71.8$\pm$8.5%. The specific activity was 11.8:$\pm$6.4 GBq/$\mu$mol and radiochemical purity was higher than 99.2%. The mass spectrum of the product eluted at 11.3 min showed m/z peaks at 353.1 (M+1), indicating the mass and structure of (R)-PK11195. Conclusion: By the one-step loop method with the [$^{11}$C]CH3l automated synthesis module, [$^{11}C$](R)-PK11195 could be easily prepared in high radiochemical yield using NaH suspension in DMF.

Analysis of Flood Control Effect by Applying the Connecting Channel in Estuary Area Including the Confluence of Two Rivers (2개의 하천이 합류하는 하구역에서의 연결수로 통수능에 따른 홍수위저감효과 분석)

  • In the estuary where the structure such as river-mouth weir has been installed, the flow is developed very complicatedly due to river water from upstream, tide of the sea and floodgate operation. Especially, if basin outlets more than one exists in one estuary, the boundary conditions will be significantly more complex form. Saemangeum(SMG) project area in Korea is the most typical example. There are Mankyung river and Dongjin river in upstream. The water of them inflows into SMG project area. In the downstream, river flow was drained from inland to sea over the SMG sea dike through the sluice. The connecting channel was located between Mankyung and Dongjin basins. It functions not only as transportation by ship in ordinary period but also as flood sharing by sending flood flow to each other in flood period. Therefore, in order to secure the safety against flood, it is very important to understand the flood sharing capacity for connecting channel. In this study, the flood control effect was analyzed using numerical simulation. Delft3D was used to numerical simulation and simulated period was set up with neap tide, in which the maximum flood stage occurred due to poor drainage. Actually, three connecting channels were designed in land use plan of the SMG Master Plan, but they were simplified to a single channel for conciseness of analysis in this study. According to the results of numerical analysis, the water level difference between two basins was increased and the maximum flood stage at dike sluice was also upraised depending on decrease of conveyance. And the velocity induced by same water level difference was decreased when the conveyance became smaller. In certain conveyance above, there was almost no flood control effect. Therefore, if the results of this study are considered for design of connecting channel, it will be expected to draw the optimal conveyance for minimizing dredging construction cost while maximizing the flood control effect.

Construction of Retrovirus Vector System for the Regulation of Recombinant hTPO Gene Expression (재조합 hTPO 유전자의 발현 조절을 위한 Retrovirus Vector System의 구축)

  • In this study, we constructed and tested retrovirus vectors designed to express the human thrombopoietin gene under the control of the tetracycline-inducible promoters. To increase the hTPO gene expression at him-on state, WPRE sequence was also introduced into retrovirus vector at downstream region of either the hTPO gene or the sequence encoding reverse tetracycline-controlled transactivator (rtTA). Primary culture cells (PFF, porcine fetal fibroblast; CEF, chicken embryonic fibroblast) infected with the recombinant retrovirus were cultured in the medium supplemented with or without doxycycline for 48hr, and induction efficiency was measured by comparing the hTPO gene expression level using RT-PCR, western blot and ELISA. Higher hPTO expression and tighter expression control were observed from the vector in which the WPRE sequence was placed at downstream of the hTPO (in CEF) or rtTA(in PFF) gene. This resulting tetracycline inducible vector system may be helpful in solving serious physiological disturbance problems which have been a major obstacle in successful production of transgenic animals.

Experimental Study on N2 Impurity Effect in the Pressure Drop During CO2 Mixture Transportation (CO2 파이프라인 수송에서의 N2 불순물이 압력강하에 미치는 영향에 대한 실험적 연구)

  • Carbon-dioxide capture and storage (CCS) process is consisted by capturing carbon-dioxide from large point source such as power plant and steel works, transporting and sequestrating captured $CO_2$ in a stable geological structure. During CCS process, it is inevitable of introducing impurities from combustion, capture and purification process into $CO_2$ stream. Impurities such as $SO_2$, $H_2O$, CO, $N_2$, Ar, $O_2$, $H_2$, can influence on process efficiency, capital expenditure, operation expense of CCS process. In this study, experimental apparatus is built to simulate the behavior of $CO_2$ transport under various impurity composition and process pressure condition. With this apparatus, $N_2$ impurity effect on $CO_2$ mixture transportation was experimentally evaluated. The result showed that as $N_2$ ratio increased pressure drop per mass flow and specific volume of $CO_2-N_2$ mixture also increased. In 120 and 100 bar condition the mixture was in single phase supercritical condition, and as $N_2$ ratio increased gradient of specific volume change and pressure drop per mass flow did not change largely compared to low pressure condition. In 70 bar condition the mixture phase changed from single phase liquid to single phase vapor through liquid-vapor two phase region, and it showed that the gradient of specific volume change and pressure drop per mass flow varied in each phase.

Determination and Monitoring of Grayanotoxins in Honey Using LC-MS/MS (LC-MS/MS를 이용한 벌꿀 중 grayanotoxin 분석법 연구 및 실태조사)

  • This study was performed to establish analysis methods, and evaluated for grayanotoxin in domestic/foreign honey and wild honey. The molecular weight of grayanotoxins I, II and III, excluding grayanotoxin III that has been commercialized, were analyzed by LC-MS/MS. Then, the molecular structure of grayanotoxins I and II were analyzed by NMR. A total 111 samples (25 Korean honey, 21 Korean wild honey, 13 Korean honeycomb honey, 44 foreign honey, 8 foreign wild honey) were examined to determined whether or not each sample contained grayanotoxins I, II, and III. The honey samples were mixed with methanol and loaded into a tC18 cartridge, the filtrate was diluted with water, and the mixture was then analyzed by ESI triple-quadrupole LC-MS/MS. Grayanotoxins were only found in the foreign wild honey and were not detected in Korean honey, Korean honeycomb honey, or Korean wild honey. Three of the samples contained grayanotoxin I, II, and III, and one sample contained only grayanotoxins I and III. The lowest level for grayanotoxin I was 3.13 ${\pm}$ 0.00 mg/kg, and the highest level was 12.93 ${\pm}$ 0.01 mg/kg. The levels of grayanotoxin II were 0.84 ${\pm}$ 0.01 mg/kg, 0.92 ${\pm}$ 0.00 mg/kg and 1.08 ${\pm}$ 0.01 mg/kg, respectively. The lowest level of grayanotoxin III was 0.25 ${\pm}$ 0.01 mg/kg and the highest level was 3.29 ${\pm}$ 0.74 mg/kg. Through this study, safety management for foreign wild honey has been enabled.

The Development of Quality Assurance Program for CyberKnife (사이버나이프의 품질관리 절차서 개발)

  • [ $\underline{Purpose}$ ]: Standardization quality assurance (QA) program of CyberKnife for suitable circumstances in Korea has not been established. In this research, we investigated the development of QA program for CyberKnife and evaluation of the feasibility under applications. $\underline{Materials\;and\;Methods}$: Considering the feature of constitution for systems and the therapeutic methodology of CyberKnife, the list of quality control (QC) was established and divided dependent on the each period of operations. And then all these developed QC lists were categorized into three groups such as basic QC, delivery specific QC, and patient specific QC based on the each purpose of QA. In order to verify the validity of the established QA program, this QC lists was applied to two CyberKnife centers. The acceptable tolerance was based on the undertaking inspection list from the CyberKnife manufacturer and the QC results during last three years of two CyberKnife centers in Korea. The acquired measurement results were evaluated for the analysis of the current QA status and the verification of the propriety for the developed QA program. $\underline{Results}$: The current QA status of two CyberKnife centers was evaluated from the accuracy of all measurements in relation with application of the established QA program. Each measurement result was verified having a good agreement within the acceptable tolerance limit of the developed QA program. $\underline{Conclusion}$: It is considered that the developed QA program in this research could be established the standardization of QC methods for CyberKnife and confirmed the accuracy and stability for the image-guided stereotactic radiotherapy.

Acute Kidney Injury Models: Focus on the Therapeutic Effects of Stem Cell in Preclinical Approach (줄기세포 연구를 위한 급성신장손상 모델)

  • Stem cell-based therapy is under intensive investigation to treat acute renal failure (ARF). The purpose of this study was to evaluate available ARF models, and suggest a model appropriate to therapeutic evaluation of the stem cells in preclinical approach by determining the optimum concentration of nephrotoxic agents and duration of ischemia induction. Three different types of available acute kidney injury (AKI) animal models were analyzed using rats: Cisplatin (saline, 5 and 7.5 mg/kg, IP) or glycerol (saline, 8 and 10 ml/kg, IM)-induced nephrotoxicity as toxic models and ischemia-induced (sham, 35 and 45 minutes) nephropathy as an ischemic model. The relevance and applicability to investigate especially the regenerative ability of stem cells were evaluated regarding morphology, renal function and survival at this time point. In the point of renal function, 10 ml glycerol/kg and 7.5 mg cisplatin/kg model in toxic models and 45 min model in ischemia models showed significant decrease for the longer observation time compared to 8 ml glycerol/kg, 5 mg cisplatin/kg and the 35 min ischemia models, respectively. All groups were observed no mortality except 45 min-ischemia model with 50% survival. Histological significant alterations including cast formation in the tubular lumen, tubular necrosis and apoptosis were revealed on the second day in either ischemiaor glycerol-induced models, and on day 5 in cisplatin-induced models. The results indicate that ischemia 35 min-, cisplatin 7.5 mg/kg- and glycerol 10 ml/kg-induced AKI would be ideal animal models to monitor a outcome parameter related to the therapeutic effects on renal function with noninvasive techniques in the same animal at multiple time points. Our findings also suggest that the best time points for the functional or histological interpretation of renal will be on day 2 in both glycerol- and ischemia-induced AKI models and on day 5 in cisplatin-induced AKI.

Effects of Various Ventilation Systems on the Carbon Dioxide Concentration and Fruiting Body Formation of King Oyster Mushroom (Pleurotus eryngii) Grown in Culture Bottles (새송이버섯 병재배에서 환기방법이 이산화탄소 농도 및 자실체형성에 미치는 영향)

  • In an attempt to establish the appropriate ventilation device for the bottle culture of king oyster mushroom (Pleurotus eryngii), we investigated carbon dioxide concentration and fruiting body formation according to the various ventilation systems within the mushroom house. In addition to, the efficiency of air circulation and growth rate as well as the appearance of physiologically abnormal phenotypes during their growth stage were also evaluated. four different ventilation devices, parallel-pressure type, positive-pressure type, negative-pressure type, and positive- and negative-pressure type were applied in this study. The positive-and negative-pressure type showed the highest efficiency of air circulation as $CO_2$ concentration was 800 ppm and the level of air current was relatively low compared to the other types (the $CO_2$ concentration of parallel-pressure type was 1,400 ppm). Moreover, the stipe length, the cap diameter, yield, and general quality grown in positive- and negative type ventilation device were also better than in the other three devices though it took slightly longer period for harvesting (18.4 days) than the others (17.6, 17.9 and 18.3 days). The appearance of physiologically abnormal phenotypes such as fruiting body lump, soft rot, and brown rot were significantly decreased in positive-and negative type compared to other types, while the appearance rates were not much different for other symptoms of bacterial ooze, stipe limb and stipe bumpy. In summary, we propose that the optimal ventilation system for the bottle culture of king oyster mushroom is positive- and negative type, and this device is expected to increase the total quality as well as yield all year around.
