구강점막의 창상치유에 대한 식나무(Aucuba japonica) 추출물의 효과 (Effects of Aucuba japonica Extract on Oral Wound Healing)

  • Aucuba japonica has variable pharmacological effects such as hepatoprotective, choleretic, hemodynamic, antimicrobial, and anti-inflammatory activities. This study was performed to investigate the effects of Aucuba japonica extract on oral wound healing. Aucubin was extracted from Aucuba japonica, and injected on either side of buccal mucosa of male mice. Artificial full thickness wounds were made on the site with 1.5 mm biopsy punch under sterile technique. The specimens had taken on day 1, 3, and 5 with 4 mm biopsy punch. Light microscopic examination and quantitative histologic analysis were performed for reepithelization, inflammatory cell infiltration. Reepithelization of the aucubin (0.1%) group was earlier than the control group. And the number of inflammatory cells of the aucubin group was lesser than the control group. In view of the results so far achieved, the aucubin extracted from Aucuba japonica may be useful for oral wound healing and it can be applied as a topical agent on the oral wound. Further research should be performed on the mechanism of aucubin on oral wound healing and proper formulation for effective topical agents.

국소치료용 구강점막패취의 제제설계 및 약제학적 특성 (Formulation and Pharmaceutical Properties of Local Mucoadhesive-Patch Preparation)

  • In order to ameliorate disadvantages of buccal ointments and mucoadhesive tablets used for the treatment of aphthosis, a thin mucoadhesive patch containing triamcinolone acetoni de was designed and evaluated for the pharmaceutical properties. The adhesive gel layer consisting of Noveon AA-1, hydroxypropylcellulose-M and ethylcellulose N 100, and the protective gel layer of ethylcellulose N 100, Eudragit RSPO and castor oil have been formulated and various properties such as viscosity of drug gel layer, thickness, in vitro adhesion time, adhesive strength, surface pH, content uniformity and drug release are tested. The mean viscosity of drug-containing gel layer was found to increase with increasing amount of Noveon OAA-1 or hydroxypropylcellulose-M. The optimum formulation showed the thickness of 171 ${\mu}$m, surface pH of 4.6, in vitro adhesion time of 8 hours and adhesive strength of 272.7g/sheet. The drug content of each patch was relatively homogeneous with the value of 273${\pm}$6.77g. Drug release study showed that compared to mucoadhesive tablet, the patch showed a faster drug release. Drug release was delayed by hydroxypropylcellulose-M, but not by ethylcellulose N 100. The patches prepared were nonirritant and the muco adhesion was better than the commercial product (AftachR) on the market. Based on these results, this mucoadhesive patch is expected to be an effective dosage form for the treatment of aphthosis.

구강 점막 하 주입 Midazolam과 경구 투여한 Chloral Hydrate의 용량에 따른 산소 포화도 및 생징후 변화에 대한 비교 연구 (Effect of Submucosal Midazolam on Percutaneous Saturation Percentage of Oxygen ($SpO_2$), End-tidal Carbon Dioxide ($EtCO_2$) and Physiologic Response When Combined with Chloral Hydrate, Hydroxyzine and Nitrous Oxide Sedation)

  • Background: The aim of this study was to examine the difference of $SpO_2$, PR, $EtCO_2$, RR with submucosal injection of midazolam to oral chloral hydrate and hydroxyzine for pediatric patients Methods: Thirty two sedation cases were performed in this study. Patients were randomly classified into one group taking oral CH (60 mg/kg). hydroxyzine (1 mg/kg) and submucosal injection of midazolam (0.1 mg/kg) and the other group recieving oral CH (50 mg/kg), hydroxyzine (1 mg/kg) and submucosal injection of midazolam (0.2 mg/kg). For evaluating the depth of sedation. data including saturation percentage of oxygen ($SpO_2$), pulse rate (PR), end-tidal carbon dioxide ($EtCO_2$), respiratory rate (RR) and the behavior scale were checked every 2 minutes and were collected for only 40 minutes from the beginning of treatment and were analyzed using Two independent sample T-test. Results: Analysis showed no significant difference in the mean $SpO_2$, PR, $EtCO_2$, RR during sedation between two groups (P > 0.05). The values of $SpO_2$, PR, $EtCO_2$ and RR for both groups remained within the normal values. Conclusions: The results of this present study indicate that combination of oral CH, hydroxyzine, nitrous oxide gas inhalation and submucosal injection of midazolam improved the sedation quality without compromising safety.

수평 매복된 상악 견치의 교정적 견인 (ORTHODONTIC TRACTION OF HORIZONTALLY IMPACTED MAXILLARY CANINE)

  • 치아 매복은 구강점막이나 악골내에서 치아의 맹출이 중지된 상태를 의미하며, 제 3대구치와 상악 견치에서 호발한다. 상악 견치의 매복은 주로 측절치가 상실되거나 왜소한 경우와 연관성이 높지만 치배의 위치 이상과 같이 유전적인 요소와도 관련될 수 있다. 매복치는 심미적, 기능적으로 여러 가지 문제점을 야기하므로 조기에 진단하여 치료하는 것이 중요하며, 매복치의 상태에 따라서 치료 방법을 적절하게 선택하여야 한다. 본 증례는 11세 여아에서 구개측으로 수평 매복된 상악 우측 견치의 치료를 보고한 것으로, 매복치는 외과적 수술을 동반한 교정적 견인 방법을 사용하여 기능적, 심미적으로 회복되었다. 견인 치료는 매복 견치의 맹출 공간을 확보하면서 이루어졌으며, 치료 종료 후 매복 견치는 치근 흡수와 같은 합병증 없이 적절한 부착치은을 확보하여 양호하게 위치되었다.

  • Mucosa-associated lymphoid tissue(MALT) lymphoma is thought to originate from marginal zone B-cells. In the WHO classification, Extranodal marginal zone lymphoma of MALT is classified B-cell Non-Hodgkin lymphoma. Common sites of MALT lymphoma include stomach, lung and the ocular-adnexa. Although less common in other sites, it is the most common low-grade lymphoma of the breast, thyroid, bowel skin and soft tissue. No strong age or gender predominance exists in MALT lymphoma. Dissemination to other sites can occur. In the oral cavity, MALT lymphoma is rare. Herein, we present a case of intra-oral MALT lymphoma. 66 year-old woman without any background of immunodeficiency or autoimmune disease admitted department of oral & maxillofacial surgery in Ulsan university hospital for evaluation of long-standing mild upper lip swelling. The lesion was completely resected and biopsied. Histological and immunohistochemical stains(CD3, CD5, CD20, CD21, CK) findings were used to confirm the lesion. Bone marrow biopsy was done and no bone marrow involvement was found. She did not receive chemotherapy and radiotherapy after surgery. No recurrence has been noted in the 22 months to date.

Pulsed Nd:YAG 레이저로 유발된 백서 설점막 창상의 조직학적 변화-수술칼에 의해 형성된 창상과의 비교 실험 (Histological Changes of the Wound in the Tongue Mucosa of White Rats by Pulsed Nd:YAG Laser - A Comparative Experiment with the Scalpel Incision)

  • The Author examined the clinical and histological changes on the dorsal tongue mucosa of the adult rats after lasing by pulsed Nd:YAG laser and incising with scalpel. The dorsal tongue was lased through 320$\mu\textrm{m}$optic fiber moving 2.5mm/second to make linear incision 5mm. The five conditions of lasing were three application with 1.0W, 1.75W, 3.0W and 3.0W under saline cooling, and single application with 3.0W at 20Hz. With scalpel, linear incisions through the surface epithelium were performed to 5mm in length. After observing the clinical changes of the incised wounds, the animals were sacrified and the tissues were excised to make the tissue specimens. The stained microscopic tissue slide were observed histologically under the microscope. The following results were obtained : 1. While incision with scalpel causes severe bleeding, lasing does not cause bleeding. 2. In three applications with 1.75W and 20Hz, tissue ablation was limited to areas contacted with optic fiber. 3. In three applications with 3.0W and 20Hz, deep incised wound, extensive destruction of the adjacent epithelium and charring were observed. 4. In three applications with 3.0W and 20Hz under saline cooling, tissue ablation was limite to the superficial mucosa and charring was not observed. 5. In single application with 3.0W and 20Hz, the shape of the incised wound was similar to that of the scalpel incision.

미성숙 영구치의 탈구성 외상 이후 계속된 치근 성장 (CONTINUED ROOT DEVELOPMENT AFTER AVULSION OF IMMATURE TEETH)

  • 외상에 의한 미성숙 영구치의 손상은 유치열에서 영구치열로 이환되는 8~10세경에 가장 빈발하며 전체 외상 환자의 높은 빈도를 차지한다. 외상에 대한 결과는 치아의 파절, 전위, 함입, 정출, 탈구 등의 경조직 손상 뿐 아니라 치수, 치주인대, Hertwig 상피 근초, 치조골, 치은 및 구강점막 등의 치아 인접조직의 손상도 포함한다. 일반적으로 Hertwig 상피 근초는 외상성 손상에 취약하지만, 때때로 감염이나 외상에 의한 손상을 견디고 생활력을 유지하여 치근성장에 대한 정상적인 기능을 수행하는 것이 보고된 바 있다. 본 증례에서는 외상에 의해 완전 탈구된 미성숙 영구치를 가진 두 명의 환자에 대해 보고하고자 한다. 첫 번째 환자의 경우 탈구된 상악 중절치를 재식하였고 두 번째 환자의 경우 탈구된 하악 중절치를 재식하지 않았다. 하지만 두 환자 모두에서 탈구된 치아의 치조와 부위에 분리된 치근의 계속적인 성장을 보이는 바 이를 보고하고자 하며, 나아가 계속적인 치근형성에 있어서 미성숙 치수 조직과 Hertwig 상피 근초의 생활력 보존이 결정적임을 알리고자 한다.

구강점막질환환자에서 치과재료를 이용한 첩포시험 결과에 대한 고찰 (Dental materials in patients with oral mucosal disease based on the results of patch test study)

  • The aim of this study was to investigate the frequency of positive patch test reaction to dental materials in patients with oral mucosal diseases. Epicutaneous patch test was performed in 110 patients with oral mucosal diseases; 41 patients with oral lichen planus(OLP), 44 patients with burning mouth syndrome(BMS), 25 patients with other oral mucosal diseases including recurrent aphthous ulcer and mucous membrane pemphigoid. The obtained results were as follows: Oral gold restorations were most common in patents with oral mucosal diseases and porcelain fused metal crown, implant appeared in the order. 33 of 110 patients did not appear skin reactions (negative, 30%) and 77 patients (positive, 70%) had skin reactions including redness, rash, blisters. Dental materials causing positive reaction to patch test were mainly as gold-sodium-thiosulfate (26.7%), nickel sulfate(Ni) (22.7%), cobalt chloride(Co) (14.7%), palladium chloride(Pd) (11.9%), potassium dichromate (10.7%) in order, respectively. In conclusion, old metal restorations could be the cause of oral mucosal diseases and epicutaneous patch test could be used as a tool to improve the oral conditions.

치과용 폴리머 기구의 세포독성(MTT) 평가 (Cytotoxicity(MTT) evaluation of dental instruments made of polymers)

  • 최근 치과에서 사용되는 10종의 폴리머 기구에 대한 세포독성을 평가하기 위하여 L-929 세포를 이용한 MTT 시험을 시행하였다. 검체를 4g 당 20mL의 비율로 제조한 37℃의 RPMI 1640 용액에서 24시간 동안 용출한 후 식품의약품안전처 고시 제2020-12호 의료기기 생물학적 안전에 관한 공통기준규격에 따라 검체 용출액과 공시험액, 음성 및 양성대조를 사용하여, 37℃, 5% CO2 Incubator에서 24시간 배양하여 ELISA reader로 판정한 결과, Intraoral camera는 용출물 농도 약 50%에서 약 70% 이상의 세포 생존율을 나타냈나 Plastic impression tray, 3D printing tweezer, Impression disposable syringe, Dental floss holder, Hand implant scaler, Surgical retractor, Oral scanner tip, Dental mirror, Water pick tip은 모두 용출물 농도 100%에서 70% 이상의 세포 생존율로 세포독성을 나타내지 않아 구강점막에 직접 접촉하여 사용이 가능한 기구로 평가되었다.

세륨옥사이드나노입자(Cerium Oxide Nano Particles; CNPs)를 첨가한 치면열구전색재의 물리적 특성 및 세포독성 (Physical Properties and Cytotoxicity of Dental Pit and Fissure Sealants Containing Cerium Oxide Nano Particles(CNPs))

  • 본 연구는 기존의 치면열구전색재 ConciseTM에 세륨옥사이드 나노입자(Cerium oxide nano particles; CNPs)를 0-4.0 wt%를 첨가하여 새로운 치면열구전색재를 제조하여 물리적 성질과 세포독성을 평가하였다. 물리적 성질은 중합깊이, 물흡수도와 용해도를 측정하였고 세포독성평가는 불멸화된 구강점막세포(Immortalized human oral keratinocyte (IHOK))를 이용하여 MTT assay법으로 평가하였다. 실험 결과 중합 깊이는 CNPs 첨가량이 증가할수록 감소하였고, 용해도는 CNPs를 2.0 wt% 첨가된 실험군에서 가장 낮은 값을 보였으며 물흡수도는 각 실험군별 유의한 차이는 나타나지 않았다(p>0.05). 세포독성 실험 결과는 모든 실험군에서 높은 세포 생존율을 보였다. 이는 CNPs가 물리적 성질을 크게 약화 시키지 않으며 세포독성을 나타내지 않았으므로 생체 적합성을 입증하였다. CNPs의 특성을 고려하여 향후 CNPs의 효율적인 분산 기술에 대한 추가 연구가 필요할 것으로 사료된다.
