Evaluation of periopathogenic bacteria of peri-implantitis using real-time PCR in Koreans: pilot study (한국인의 임플란트 주위 질환에서 real time PCR법을 이용한 치주세균 평가: pilot study)

  • Purpose: We performed quantitative and qualitative analysis of typical periodontal bacteria using real time PCR method to investigate the microbiological difference according to the severity of peri-implant disease in Koreans. Materials and Methods: Total of 60 implants were divided into three groups (healthy group, peri-implant mucositis group, peri-implantitis group) through periapical radiographs and clinical indices. The evaluated clinical parameters were pocket depth, plaque index, suppuration and bleeding on probing. Using a sterilized curette instrument, microbial samples were collected from the subgingival plaque and real-time PCR was performed on five periodontal bacteria. The relative expression levels of microorganisms were compared by comparative delta-CT method. Results: The relative expression levels of E. corrodens and T. denticola were significantly higher in the peri-implantitis group (P < 0.017). On the other hand, the relative expression level of F. nucleatum and P. gingivalis was relatively high in the healthy implant group regardless of the severity of disease. P. intermedia was significantly lower in the healthy implant group (P < 0.017). Conclusion: Periodontal bacteria were detected in Koreans with peri-implant diseases, but there was no microbiological distribution similar to periodontitis.

Fabrication of closed hollow obturator for hard palate defect patient undergone maxillectomy (상악절제술로 인한 경구개 결손 환자에서의 closed hollow obturator 제작 증례)

  • Maxillectomy is performed to remove the tumor in the palate, maxillary sinus, buccal mucosa or nasal cavity. The resection range depends on the size and the extent of the tumor and it affects speech production or cause nasal regurgitation during feeding. Obturator can occlude an opening such as an oro-nasal fistula and protect the defect area. Successful reconstrucion of the patient's oral cavity who have gone over the maxillectomy is a difficult task. The condition and number of teeth, the remaining support area, and the extent of the defect area have a great influence on manufacturing the obturator. If these factors are disadvantageous, the prognosis of the prosthesis is uncertain. The final obturator must have a sufficient retention in the patient's oral cavity and must not irritate the surrounding tissue and support area where the resection was performed.In this case, a 55 year old female went through the maxillectomy and the only 3 teeth remained. And the retention of the maxillary prosthesis seems to be poor. So that, we fabricated the closed hollow obturator which has reduced weight compared to the conventional obturator. Consequently the closed hollow obturator can give better sealing and the adaptation.

ORTHODONTIC TRACTION OF AN IMPACTED MAXILLARY CENTRAL INCISOR (변위 매복된 상악 중절치의 교정적 맹출 유도)

  • Impaction is generally defined as the lack of eruption of a tooth after the normal age for the eruption. An impacted tooth may appear blocked by another tooth, bone, or soft tissue, but cause of tooth impaction is often unknown. The clinician should consider the various treatment options available : (a) No treatment and observation, (b) surgical exposure and orthodontic traction (c) auto transplantation (d) extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. Especially, in case 1, #21 was ectopic impacted state with root dilaceration. It is required to examine further root development and alignment of dentition serially.

Retrospective Study of Mortality Rates and Prognostic Indicators of Equine Colic in Korea (한국에서 산통마의 치명률과 예후인자의 회귀적 분석)

  • Equine colic is a major concern in equine industry due to high morbidity and mortality rates. The purpose of this study was to establish mortality rates during medically and surgically treated colic and to identify potential prognostic indicators for mortality of equine colic in Korea. The overall mortality rate was 18/119 (15%). The 93 cases (78%) were treated medically and 26 cases (22%) surgically out of the 119 records reviewed. The mortality rate in small intestinal lesion was 7/9 (78%) and strangulating lesion was 10/11 (91%). The significance between individual factors and prognosis of equine colic was not valid but high frequency rate was found in ${\leq}3$ years age (59%) and male (50%). In dead group showed higher, rectal temperature and heart rate than those of survived group. The mortality rate was significantly high in the pale mucous membrane color and severe pain and decreased intestinal motility. In laboratory factors, dead group showed lower level of platelet than survived group, and had higher values of RBC, hemoglobin, PCV with significance (P < 0.05). Group with increased enzyme activity of CK, LDH, glucose showed poor prognosis.

The effect of denture adhesives on the retention of complete denture (의치 접착제가 총의치의 유지력에 미치는 영향에 관한 연구)

  • Purpose: The purpose of this study was to compare the retention of the two denture adhesives with a silicone edentulous model and a resin denture base in vitro study. Material and methods: The experimental groups were used two denture adhesives and classified into 5 subgroups each. Subgroups were divided by the number of times the saliva sprayed. The control group was used synthetic saliva only. Dislodging tensile strengths were applied to the resin denture base using Instron in 3 directions ; vertical, lateral, and anterior-posterior. Results: The retention of two denture adhesives was increased significantly than saliva alone (P <.05). In each denture adhesives, the retention of saliva sprayed first was decreased than denture adhesive alone, but it didn't have significant differences. Whenever saliva sprayed repeated, the retention was decreased significantly than saliva sprayed first (P <.05). In each denture adhesives, vertical retentive force was highest than lateral and anterior-posterior retentive forces, and anterior-posterior retentive force was higher than lateral retentive force. This results were significantly different (P < .05). Significant differences of the retentive ability among two denture adhesives were not observed. Conclusion: From the results, use of the denture adhesives resulted in improved retentive ability of denture. Especially retentive force was highest in vertical direction.

CYTOTOXICITY OF DENIAL CAST BASE METAL ALLOYS ON HUMAN ORAL KERATINOCYTES (구강점막 상피세포에 대한 치과 주조용 비귀금속 합금의 세포독성)

  • Although many studies on the cytotoxicity of the dental cast base metal alloys and their components have been carried out, the results are rather conflicting because of the different type of cells used and the various experimental procedures taken. Recently a number of scientists have claimed that it would be preferable to focus on the use of cells from relevant specific location of the human bodies. Consequently, the primary cultured oral keratinocyte derived from oral mucous along with nickel chloride and several of widely used dental cast base metal alloys(two Ni-Cr alloys and one Co-Cr alloy)in domestic were selected for this study, from which 1) The amounts of released metal ions were determined using atomic absorption spectrometry, 2) The cytotoxicity of nickel chloride and dental cast base metal alloys was evaluated via MTT assay, and finally, 3) The amounts of released metal ions and the cytotoxicity of nickel chloride were correlated with the cytotoxicity of dental cast base metal alloys And, the results were summarized as follows; 1. Nickel ion from Ni-Cr alloys and Cobalt ion from Co-Cr alloys resulted in maximum releasing rate during first 2h hours, followed by a decrease in releasing rate with time. Chromium ion were found to be minimal in all alloys. 2. In cytotoxic test. with $40{\mu}M,\;80{\mu}M$ of nickel chloride, there were observed an increase in the relative cell number compared to control samples after 24 hours. With $160{\mu}M$, there was found to be no difference in the relative cell number with control, except that 48 hour showed a increase in relative cell number. With $320{\mu}M$, the relative cell number remained constant and decreased after 48 hours, and with $640{\mu}M$, a continuing decrease in relative cell number was observed throughout test period. 3 The sensitivity of primary cultured oral epithelium to nickel was lower compared to the cells used in other studies. 4. CB-80 Soft and Regalloy showed no cytotoxicity to primary cultured oral epithelium and New crown resulted in a slight cytotoxicity. In conclusion, it was shown that the primary cultured oral keratinocytes could be applied successfully as testing cells in cytotoxicity test. Futhermore, the dental cast base metal alloys used in this study were found to be biocompatible.

  • The term 'impaction teeth' is used to designate a tooth which remains unerupted in jaw beyond the time at which it should normally be erupted. Most cases of impacted teeth reported in the literature are permanent teeth. The impaction of primary teeth occur rarely whereas impaction of second primary molars is more numerous than all other impactions. Failure of eruption of primary teeth may cause a number of complications, such as interference with development and eruption of successive permanent teeth, malocclusion, cystic change of tooth follicle. The clinican should consider the various treatment option available (a) No treatment and observation, (b) surgical extraction (c) space regainer. Proper treatment plan should be established after thought consideration of impacted tooth and it's relation with successive permanent tooth.

LOCALIZED SCLERODERMA IN A CHILD : CASE REPORT (소아 피부 경화증 환자의 치험례)

  • Scleroderma is a connective tissue disease of unknown etiology, but known as a kind of auto-immune disease. It is most common in women especially in $30{\sim}50$ years, and very rare in childhood. It can be classified into two main classes : localized scleroderma & systemic sclerosis. Localized type has better prognosis, and usually involves skin only, or in some cases, the muscle below, except inner organs. Systemic type involves skin, oral mucosa and major internal organs. Involving facial skin, we can see small and sharp nose, expressionless stare and narrow oral aperture. Usually they have Raynaud's phenomenon, and in progress, show mouth opening limitation and sclerosis of tongue and gingiva. It is called CREST syndrome showing calcinosis cutis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangioectasia. Treatment of scleroderma is systemic and localized steroid therapy, use of collagen-link inhibitor (D-penicillamine), immune depressor and etc. Mouth opening limitation can be improved by mouth stretching exercise. We report a 6 years old boy, diagnosed with localized scleroderma who had mouth opening limitation. We could get additional mouth opening, and have done successful restorative treatment of mandibular and maxillary 1st and 2nd deciduous molar under deep sedation with nitrous oxide and enflurane.

GARRE'S OSTEOMYELITIS IN CHILDREN (소아에서의 Garre 골수염)

  • Garre's osteomyelitis is associated with bacterial infection and bone necrosis resulting from obstruction of blood supply. The most common cause for Garre's osteomyelitis is odontogenic infection that originates from periodontal tissue or dental pulp. Subperiosteal abscess may also cause Garre's osteomyelitis in the progress of the infection. Mandible is more often affected than maxilla, most commonly in the permanent first molar region of mandible. Clinically, it results in a hard swelling over the jaw, producing facial asymmetry. Meanwhile, radiograph shows a characteristic feature of irregular pulpal cavity, showing new periosteal proliferation located in successive layers to the condensed cortical bone on stimulated site. The treatment method for Garre's osteomyelitis are removal of the infection source, root canal treatment, antibiotic medication, and incision and drainage. This report presents a case of Garre's osteomyelitis under 15 years old. The patient was successfully treated by antibiotic medication accompanied with root canal treatment. Since the symptom of pediatric patients is less severe than adult, careful diagnosis with history taking and clinical examination is necessary. Furthermore long-term follow-up examination is needed to prevent recurrence even after the symptom disapears.

Immediate Connection of Customized Zirconia Abutment Using Flapless Guided Surgery: A Clinical Report (무절개 수술을 이용한 맞춤형 지르코니아 지대주의 즉시 장착 증례)

  • There are some similar aspects at histological and morphological characteristics between the peri-implant tissue and periodontal tissue and the direct attachment between the titanium and soft tissue around the implant called as "Functional ankylosis" can prevent the apical infiltration of inflammatory and bone resorption around implant. But, the repeated connection and disconnection of the abutment can destroy the mucosal barrier of soft tissue around the implant and can cause the marginal bone resorption. The amount of marginal bone resorption may reduced if the prosthetic abutment is placed at that time of surgery. Connection of the prosthetic abutment at surgery was limited because the low accuracy of conventional method, but by using of Cone Beam Computed Tomography(CBCT) and guide surgery, the 3-dimensional accuracy of implant placement became much higher than before and it became possible. This is a clinical case of immediate connection of prosthetic abutment and provisional restoration by using of precise CBCT diagnosis and pre-fabricated zirconia customized abutment at surgery and the alternative method is described in this article because of the clinically contentable results.
