Adsorption of Salivary Proteins on Titanium Implants (타이태늄 임플란트 표면에 형성된 타액성 단백질에 관한 생체연구)

  • 치과용 임플란트 실패의 주요 원인은 임플란트 표면에 부착되는 세균의 침착의 결과로 생기는 임플란트 주위염이다. 구강 내에서 세균성 치태의 침착은 치태가 부착하는 기질 표면의 물리적 성상과 타액성 피막의 성분에 영향을 받으며 형성된 피막의 유기질 성분의 차이가 치태의 성분과 병원성에 영향을 미친다. 최근 연구에 의하면 생체재료의 표면에 침착되는 치태세균은 사용되는 재료에 따라 특이한 세균 침착을 보이며 이는 초기 타액성 피막의 차이에 의한 것으로 알려져 있다. 이 연구의 목적은 플라즈마분사법으로 표면 처리된 타이태늄 임플란트에 흡착되는 타액성 단백질 피막의 특성을 정성적인 방법으로 분석하는 데 있었다. 법랑질 조각과 플라즈마분사법으로 표면 처리된 타이태늄 임플란트를 스프린트에 치실을 이용하여 연결한 장치를 구강 내 장착하여 2시간 동안 피막이 침착되게 한 후 피막을 분리 추출하여 냉동 건조시켰다. 재수화 과정을 거치고 나서 전기 영동법과 Western transfer 분석을 통해 단백질 성분에 관한 분석을 시행하였다. 사람의 총 타액과 이하선 타액 및 악하선-설하선 타액을 수집기를 이용하여 채취하고 같은 방법으로 처리한 후 성분분석을 실시하였다. 피막 흡착 전후의 표면변화를 주사전자현미경을 이용하여 관찰하였다. 실험결과 타이태늄 임플란트에 흡착된 피막은 법랑질 표면의 피막과는 다른 단백질 성분을 가지고 있었으며, 주로 악하선-설하선 타액에서 유래하였다. 임플란트와 법랑질 표면 모두에서 흡착된 피막에는 아밀라제, 분비성 면역 글로불린A 및 락토페린이 존재함을 알 수 있었으나 법랑질의 경우는 blotting이 약하게 나타났다. 주사전자현미경 관찰결과 시편의 표면에 균질한 피막이 덮고 있었으며 세균의 부착은 거의 관찰되지 않았다. 이상의 실험 결과들을 통하여 플라즈마분사법으로 표면 처리된 타이태늄 임플란트 표면에 부착된 타액성 단백질 성분은 법랑질과는 차이가 있음을 알 수 있었으며, 이러한 차이는 치태세균의 종류 및 병원성에 영향을 미칠 것으로 생각된다. 법랑질과 타이태늄 임플란트는 기질과 표면구조가 다르므로 표면에 형성 되는 치태성분도 다르다는 사실과 본 연구 결과를 종합하여 볼 때, 타이태늄 임플란트 표면에 흡착되는 초기 타액성 단백질의 성불이 타이태늄 표면에 침착되는 미생물 군의 조절에 중요한 역할을 가지고 있으며, 임플란트 치료 시에 올바른 치태 관리법의 교육을 통하여 환자 스스로 적절한 관리를 하도록 함으로써 임플란트 치료의 성공률을 높일 수 있을 것으로 생각된다.

Preliminary study on the diversity and quantity analysis of oral bacteria according to the sampling methods (구강 세균 채취법에 따른 세균의 다양성과 양 분석을 위한 예비 연구)

  • Objectives: Oral bacterial samples included subgingival, supragingival, and saliva plaques. As the diversity and number of microorganisms deffer depending on the area of the oral cavity and the method used, an appropriate and reliable collection method is important. The present study investigated oral bacterial sampling methods. Methods: Supragingival dental plaque was collected from the buccal and lingual tooth surfaces of study participants using sterilized cotton swabs. Plaques were collected from the subgingival area using a sterilized curette. Bacterial genomic DNA was extracted using MagNA Pure 96 DNA and Viral NA low-volume kits. Real-time polymerase chain reaction (PCR) was performed using the PowerCheckTM Periodontitis Pathogens Multiplex Real-time PCR kit. Results: Aggregatibacter actinomycetemcomitans, Prevotella intermedia, and Fusobacterium nucleatum of the orange complex were not observed in the subgingival biofilms of all study participants. For Porphyromonas. gingivalis, a significant correlation was observed between supragingival, subgingival, and total tooth surface biofilms. Compared to the supragingival and subgingival biofilmss, total tooth surface biofilm exhibited the highest bacterial count when the inswabbing method was used. Conclusions: Based on these findings, the supragingival swab method is recommended for oral bacterial research.

The clinical and microbiological effects of non-surgical periodontal treatments in necrotizing periodontal disease: case report (괴사성 치주질환 환자에서 비외과적 치주치료의 임상적 미생물학적 효과: 증례 보고)

  • Necrotizing periodontal disease caused by plaque bacteria is showed clinical findings including pseudo-membrane, interproximal necrosis of the affected area, pain on palpation and gingiva bleeding. Microbiological examination is showed that patients have fusospirochetal bacteria. Two patients who were provisionally diagnosed as necrotizing periodontal disease received nonsurgical periodontal treatments in conjunction with dressing using 3% hydrogen peroxide and local antibiotic delivery. Before and 3 - 5 days after initial treatment, the levels of periodontal bacteria in gingival crevicular fluid obtained using quantitative PCR were compared. After treatment, patients recovered normal gingiva. The number of periodontal diseases related bacterial species decreased from seven or eight to one. As a result, periodontium of patients with necrotizing periodontal disease was recovered to normal periodontium by nonsurgical periodontal treatments.

A Study on the Converged Oral Environment of the Elderly with Dementia in the Nursing Home (요양시설 치매노인들의 융합적 구강환경 연구)

  • This study was conducted to identify the relationship among the cognitive function of the elderly with dementia in the nursing home and the oral health condition and pathogenic microorganisms in the mouth. Data was collected for a month in February 2019 for 30 people through a non-probable sampling method due to the special nature of dementia elderly people aged 70 years or older, living in nursing homes in Daegu. Technical statistics, ANOVA, and Pearson's correlation coefficient were analyzed using the SPSS window ver. 21.0 program. According to the study, the lower the rating of long - term care classification in nursing homes, the more the number of high-risk oral pathogens, and the more the number of existing permanent teeth, the more the number of microbes in the mouth. However, there was no significant difference in cognitive function. Therefore, it is suggested that oral health care, an elderly dementia people in nursing homes, should have different management methods depending on the oral environment, and also professional managers should be able to manage oral health condition for actual care.

Anti-microbial Activity of Bamboo Extract Against Oral Microbes (대나무추출액의 구강미생물에 대한 항균효과)

  • The main cause of oral disease is the formation of tooth surface bacteria, and a chemical mouthwash is used as a suppression method. However, chemical preparations have side effects, so we tried to verify the antibacterial effect of bamboo extract against oral microbes. Subjects were 15 college students as a control group and an experimental group, and the changes in oral microbes before and after the use of distilled water and bamboo extract were compared. Analysis of SPSS Windows ver. 20.0 was used (p<0.05). Results The total number of pathogenic microorganisms further decreased after using bamboo extract solution after using gargling solution between groups.

  • Oral diseases have been reported to affect approximately 3.5 billion people worldwide, and in Korea, gingivitis and periodontal disease ranked first in the most frequent diseases from 2019 to 2021. Microorganisms that cause oral diseases include not only some bacteria such as Streptococcus mutans, Porphyromonas gingivalis, Fusobacterium nucleatum, Streptococcus gordonii, Leptotrichia buccalis, Prevotella, and Treponema, but also fungi Candida albicans and archaea Methanobrevibacter oralis. In the process by which oral microorganisms cause periodontal disease, bacteria such as Streptococcus mutans first proliferate to form a biofilm, and then obligate anaerobes, opportunistic bacteria, and pathogens attach, proliferate and settles down, forming plaque in the subgingival area of the host with weakened immunity. In this way, various interactions within the community are important in causing oral disease. Furthermore, substances and inflammation resulting from oral microorganisms and oral diseases are closely related to the occurrence of digestive diseases, diabetes, cardiovascular diseases, cognitive function, rheumatoid arthritis, premature birth, and cancer, and vice versa.

Effects of smartphone app-based oral muscle strength training on functional improvement in the elderly (노인 대상 스마트폰 앱 콘텐츠를 이용한 구강 근력 강화 훈련의 기능 개선 효과)

  • Objectives: This study aimed to develop educational content for a smartphone app on oral muscle training and examine its effects on the elderly population. Methods: A total of twelve training sessions were delivered through the smartphone app over a six-week period, from late August to early October 2023. Each session lasted approximately 60 minutes. Participants were followed up after the program. Results: The experimental group showed significant improvements in oral health before and after using the smartphone app. These improvements included decreased dental plaque (p<0.05) by 0.69 units, decreased gingivitis (p<0.001) by 0.99 units, decreased tongue plaque (p<0.01) by 1.11 units, increased salivary secretion rate (p<0.001) by 0.73 units, increased hard palate strength (p<0.001) by 5.25 units, and increased soft palate strength (p<0.01) by 6.82 units. Compared to the control group, the experimental group showed significant improvements in dental plaque (p<0.001), gingivitis (p<0.001), and tongue coating (p<0.01). Conclusions: This study found that oral muscle strengthening training using the smartphone app effectively improved oral health in the elderly. The developed app content has the potential to be a valuable tool for promoting oral health in this population within their daily routines. However, further efforts are needed to ensure clear communication and effective utilization of the training program through user training or educational materials.

  • 황색 포도상구균은 급성 구강 감염에 있어서의 병원균이다. 그러나 그러한 황색 포도상구균의 병원성 기전은 완전히 이해되지 않았다. 이전 실험에서 황색 포도상구균의 단백질 A와 골격근의 액틴 필라멘트는 인체 상피 세포로의 황색 포도상구균의 침입에 관여한다. 구강 내 감염에 있어서의 황색 포도상구균의 병원성 기전을 조사하기 위해 인체의 치은 섬유모 세포에 대한 침입이 연구되고 있다. 급성 구강 감염을 가진 환자로부터 분리된 ATCC 25923 황색 포도상구균과 OPT 2 황색 포도상구균의 침입은 시간(0-120분)에 의존한다는 사실을 밝혀냈다. 60분을 초과하는 배양시간은 다시 배양된 균집락수 증가를 가져왔다. 배지에 접종한 세균의 숫자가 증가할 때 (100?10,000,000 cfu/ml/well), 직선적으로 증가한다. 단백질 A가 결핍된 Wood 46 황색 포도상구균의 침입은 단백질 A가 발현된 균주(ATCC 25923과 OPT 2)의 침입보다 훨씬 낮았다. 액틴 필라멘트의 합성을 방해하는 Cytochalasin D는 인체 치은 섬유모세포로 황색 포도상구균 (ATCC 25923과 OPT 2)이 침입하는 것을 방해한다. 이러한 결과는 구강내 감염을 일으키는 황색 포도상구균의 병원성 기전이 세포내 침입에 관여하고, 황색 포도상구균 단백질 A와 골격근의 액틴 필라멘트가 인체 치은 섬유모세포로의 황색 포도상구균의 침입 조절에 관여한다는 것을 보여준다.

Changes in oral health status of the elderly with visiting oral hygiene care intervention based on community health care: Case reports (커뮤니티케어에 기반한 방문 구강 관리 중재 노인의 구강 건강 상태 변화에 관한 증례 보고)

  • Objectives: This case - study was conducted to assess the oral health status change of the elderly through visiting oral care interventions based on a community health care project. Methods: Professional dental hygiene treatment and oral health education, including brushing using interdental toothbrushes and sponge brushes, were performed on three senior citizens who received home visiting oral health care benefit. Results: The subject's periodontal conditions improved including gingival inflammation and bleeding. The gingival color tured pink by controlling the dental plaque. Conclusions: Visiting oral health care contributed to the improvement of oral health of the elderly. Therefore, based on the characteristics of the elderly with various systemic diseases, it is necessary to discover various cases that can perform professional and customized visiting oral health care programs.

Evaluation on the oral health promotion program effect from some part of community child center (일부 지역아동센터 구강건강증진 프로그램 운영 효과 평가)

  • This study aims to examine the effect of the program by identifying changes in oral health behaviors, oral health knowledge, the Patient Hygiene Performance(PHP) index and others after performing oral health promotion program on children in community children's centers to develop persistent and more effective program. The final analysis subjects were 27 children. According to the examination results of dental plaque score, the PHP index score was reduced from 3.42 to 2.43 before and six month after the tooth brushing education, respectively, indicating the effect of oral health promotion program(p<0.001). The data of this paper can be used oral health promotion programs development based on the social ecological model.
