  • 40두 규모의 농장에서 사육된 홀스타인 종, 4개월령의 수컷 송아지에서 거세 후 고열, 식욕부진, 기침 등의 증상과 함께 전신 체표 림프절의 종대가 관찰되어 백혈병으로 잠정진단 하고 도태를 권유하였으나 축주가 치료를 원해 항생제와 해열제 및 기타 대증요법을 실시한 후 치료반응이 없어 폐사하여 부검을 실시하였다. 혈액 검사상 이상핵과 다형핵을 가진 다수의 림프구 및 백혈구, 호중구, 림프구 및 단핵구의 증가가 관찰되었다. 또한 백혈병 바이러스에 대한 분리 및 PCR 검사는 음성이었다. 부검 소견으로 체표 림프절, 슬관절 부위 및 비장과 간의 종대가 관찰되었으며 비장 중심에 12x11 cm의 종괴와 폐의 전엽부 유착 및 폐문, 종격동 림프절의 심한 종대가 관찰되었다. 견갑전 림프절을 비롯하여 대퇴골 전, 서혜, 폐문, 이하림프절 등 전신 림프절의 종대 및 소성의 연한 황색의 매끄러운 절단면을 보였다. 전지 관절의 종대와 관절강 내부는 증가된 농성 활액을 보였으며 고관절 강 내에 농성 활액의 증가와 공기 노출 후 젤리양 응고를 보였다. 심장은 장액성 위축과 함께 섬외막성 점상출혈이 나타났다. 병리조직학적 소견으로 비장은 지주 주변에 미성숙형의 세포들의 침윤이 보이며 유사분열상이 다수 관찰되었고 백색 수질에도 유사분열상의 증가와 함께 림프아구성 세포들이 다수 나타났다. 이들 주요한 비정상 세포들은 다형성의 큰 핵을 가진 다양한 림프아구의 형태를 지녔으며 핵내 공포가 인정되었다. 비장의 종괴 주변에는 증식된 섬유조직으로 둘러싸여 있었으며 미세농양 형성되어 있고, 일부 석회화가 진행된 부위도 있었다. 간소엽성 중심성 울혈과 가벼운 간세포내 지방침윤, 혈관 내 림프아구 형태의 세포와 소수의 호중구가 관찰되었다. 간삼조 주변에는 가볍거나 중등도의 단핵세포의 침윤이 미만성으로 관찰되었다. 폐에서는 중등도의 기관지 폐렴과 함께 일부는 무기폐가 관찰되었으며 폐포강과 세기관지내에는 염증성 삼출물이 다량 들어 있었다. 다병소성 미세농양과 함께 괴사가 있었고 실질의 섬유화가 진행되어 있었다. 또한 중등도의 간질성 신장염과 림프절은 지주 주변에 간극 내 비정상 림프구 세포의 형태는 비장의 그것과 유사하였으며 적수와 백수의 구이 힘들며 림프소절이 증가되어 있었다. 한 시야에서 유사분열상이 6-8 개로 그 지수가 매우 높으며 이와 더불어 큰 림프구가 전반에 걸쳐 침윤되어 있었다. 주변부 동(sinus)에는 많은 물질들이 침윤되어 있으며 렴프소절내 미만성의 성상현상이 관찰되었다. 회장은 파이어판내 심한 림프구 소실이 나타났다. 이상의 소견을 바탕으로 본 증례는 산재성 송아지 백혈병으로 진단되었다

Clinical Effect of Arthrocentesis for Patients with Disc Displacement without Reduction of Temporomandibular Joint : Report of Cases (측두하악관절의 비정복성 관절원판변위 환자에 대한 관절강 세정술의 임상효과 : 증례보고)

  • The management of refractory pain and limitation of mouth opening in the temporomandibular joint (TMJ) is both challenging and controversial. Arthrocentesis is a simple technique and it can be used instead of more invasive surgical procedures for patients who failed to conventional conservative treatments. The patients in this case report had complained persistent severe pain and restricted mandibular movement of TMJ, although they had been treated with conservative therapy, e.g., stabilization splint therapy, behavioral therapy, physical therapy, pharmacological therapy. Therefore we employed arthrocentesis for these patients, and they showed significant reduction of pain and improvement of mandibular movement after repetitive arthrocentesis. We recommend arthrocentesis as an effective, minimally invasive technique for patients with continuing severe pain and restricted mobility in the TMJ that is unresponsive to conservative management.

Treatment of Distal Tibial Spiral Fractures Combined with Posterior Malleolar Fractures (후과 골절이 동반된 경골 원위부 나선상 골절의 치료)

  • Purpose: This study compared the functional and radiologic outcomes of intramedullary nailing (IMN) and minimally invasive plate osteosynthesis (MIPO) for tibia fractures in distal tibial spiral fractures combined with posterior malleolar fractures, as well as the functional and radiologic outcomes with and without fixation for posterior malleolar fractures. Materials and Methods: From January 2010 to December 2018 the radiological and clinical outcomes of 30 skeletally mature patients with tibial spiral fractures (AO Foundation/Orthopaedic Trauma Association classification 42-A1, B1, C1) combined with posterior malleolar fractures were analyzed. Sixteen patients were treated with IMN, and 14 patients were treated with MIPO. Depending on the surgical methods, the radiologic and clinical outcomes were compared by evaluating the bone union time, postoperative alignment, postoperative displacement of the posterior malleolar fragment, and American Orthopaedic Foot and Ankle Society (AOFAS) score. Moreover, the functional and clinical outcomes with and without fixation for posterior malleolar fractures were compared. Results: The mean bone union time was 21.8 weeks in the IMN group and 23.1 weeks in the MIPO group (p=0.500). At the final follow up, the mean alignment was coronal angulation of 1.8°, sagittal angulation of 1.6° in the IMN group and coronal angulation of 1.2° and sagittal angulation of 1.7° in the MIPO group (conoral angulation: p=0.131, sagittal angulation: p=0.850). The postoperative and final radiologic evaluation showed no displacement of the posterior malleolar fragment and excellent joint congruity in all cases. At the final follow-up, the mean AOFAS score was 88.0 on average in the IMN group and 87.6 on average in the MIPO group (p=0.905). The ankle range of motion and AOFAS score were similar in the fixation group and no fixation group for posterior malleolar fractures. Conclusion: Both IMN and MIPO for tibial spiral fractures combined with posterior malleolar fractures result in satisfactory radiological and clinical outcomes.

Clinical Outcomes of Ultrasound-Guided Transmeniscal Injection in Medial Compartment Knee Osteoarthritis (무릎 내측 구획 골관절염에서 초음파 유도하 경 반월상 연골 주사의 임상적 효과)

  • Purpose: The purpose of this study was to introduce the ultrasound-guided transmeniscal injection in medial compartment knee osteoarthritis and analyze the clinical outcomes. Materials and Methods: The electronic medical records of 36 patients with medial compartment knee osteoarthritis who were treated with an ultrasound-guided transmeniscal injection from March 2019 to July 2019 were accessed for this retrospective review. Using an ultrasound guided spinal needle, the patients received an intra-articular steroid injection at the medial compartment of the knee. A physical examination was conducted at the initial visit (pre-injection), and at one week, four weeks, and eight weeks after the injection. The numeric pain rating scale (NRS), Lequesne index, and Western Ontario and McMaster Universities Osteoarthritis (WOMAC) score were measured at each visit and analyzed over time. The percentage change of the patients who revealed substantial improvement was analyzed. The NRS, Lequesne index, and percentage of patients, who revealed substantial improvement over time classified by osteoarthritis grade, were analyzed. Results: The NRS and Lequesne index decreased at one week, four weeks, and eight weeks after the injection compared to the initial baseline, and the pain-relief effect continued without change until eight weeks. The percentage of patients who showed substantial improvement at one, four, and eight weeks was 50.0%, 47.2%, and 52.8%, respectively. The WOMAC scores decreased at one, four, and eight weeks compared to the initial baseline, and the decrease was continued without any difference until eight weeks. The percentage of patients with osteoarthritis stage 1 or 2 who revealed more than substantial improvement was significantly higher at one, four, and eight weeks than those with osteoarthritis stages 3 or 4 (p<0.05). Conclusion: In patients with medial compartment knee osteoarthritis, the pain reduction and functional improvement persisted for at least eight weeks after the ultrasound-guided transmeniscal injection at the medial compartment. In particular, patients with medial compartment osteoarthritis stage 1 or 2 showed more effective pain reduction.

  • A growing number of people are enjoying sports activity with a rise in national income. In this current, many patients complain of traumatic shoulder dislocation and chronic instability with bony Bankart lesion. Computed tomography arthrography is good diagnostic modality for bony Bankart lesion. It is important to consider the patients' factors such as occupation, sports activity, size of preoperative glenoid bone loss before decision of treatment. As development of arthroscopic treatment, there is no significant difference in the result of bony Bankart repair between arthroscopic surgery and open surgery. However, open surgery should be considered for patients with preoperative glenoid bone loss more than 25% or in need of collision sports activity.

A Case Report of Intra-articular Bee Venom Pharmacopuncture for Adhesive Capsulitis (견관절 유착성 관절낭염에 대한 관절강 내 봉약침 적용 1례)

  • Objectives : This study was performed to report a case of adhesive capsulitis treated by intra-articular bee venom pharmacopuncture. Methods : Patient was treated by intra-articular bee venom pharmacopuncture four times during two weeks, and after that the patient had intermission period for 2 weeks. The same treatment was administered four times during two weeks after the intermission. The effect of treatment was evaluated by visual analog scale(VAS), range of motion(ROM). Results : After treatment, VAS score declined from 7 to 4, ROM increased. Conclusions : Intra-articular bee venom pharmacopuncture was effective for adhesive capsulitis.

  • This study was intended to research into the possibility of corilagin (CRN) isolated from Euphorbia helioscopia as rheumatoid arthritis drug. For the study, CRN was medicated to the abdominal cavity of collagen induced arthritis (CIA) mice that was a animal model for rheumatoid arthritis and its effects on incidence and arthritis index (AI) were studied. The results were as folllows; Medicating corilagin that were isolated from Euphorbia heliscopia to CIA mice resulted in inhibiting an incidence of arthritis and reducing in arthritis index. It was found that inflammatory cells were remarkably decreased and joint cavity was well secured. Also it was displayed that the formation of pannus was not observed and a cartilage was preserved well by cell histological research. Performing fluorescence flow cytometry of CIA mice resulted in reducing in inflammatory cells infiltrated each tissue.

  • the primary diagnosis of synovial chondromatosis of temporomandibular joint is extremely rare. often many months, or even years pass before the diagnosis is confirmed. synovial chondromatosis is an uncommon disease of cartilage transformation of synovial membrane with formation of loose bodies within the joint space. a 28-year-old women had experienced pain of the left TMJ area at mastication. Radiographs of the left TMJ revealed calcified loose bodies in superior temporomandibular joint space. treatment consisted of removal of calcified loose bodies and synovail membrane. after surgery, pain of Lt temporomandibular area at mastication disappeared. until present after surgery there have been no recuurence of symptoms.

  • 저자등은 1996년 5월 부터 1999년 4월까지 인하대병원 치과에 악관절 질환을 주소로 내원한 958명의 환자 중 병력, 임상소견, 방사선학적 검사 결과 퇴행성 악관절 질환으로 진단된 68명에서 병록지 상에 기록된 환자의 성별, 나이, 주소, 병력, 현증상, 방사선학적 소견 및 치료방법 등에 관해 검사 후 다음과 같은 결론을 얻었다. 1. 남녀 발병 비율은 1:2로 여성에서 높았다. 2. 발병나이는 20대, 30대. 40대, 10대 순이었다. 3. 병력기간은 2년이상, 6개월이하, 1 - 2년 순이었다. 4. 전 환자에서 악관절 동통을, 44명의 환자에서 관절잡음을, 26명의 환자에서 개구제한을 호소하였다. 5. 그밖에 26명의 환자에서 관절부위에 민감성을, 5명의 환자에서 안면 비대칭을, 또한 개교합, 섬유성 악관절강직의 소견이 각각 2명의 환자에서, 양측성질환의 소견이 4명의 환자에서 관찰되었다. 6. 방사선학적 소견상, 15명 환자의 과두부에서 증식성 재성형소견(progressive remodeling)이, 51명의 환자에서 침식성 재성형소견(regressive or erosive remodeling)이 관찰되었으며, 침식성 재성형 환자 중 11명에서 과두주변부 증식성 재성형소견(peripheral remodeling or marginal lipping)이 관찰되었다. 7. 골증식체(osteophyte) 및 소성체(loose body)의 소견이 각각 2명의 환자에서 관찰되었다. 8. 약물에 의한 보존적 치료방법이 1차로 모든 환자에서 시행되었으며, 이중 12명의 환자는 교합안정장치를 병행하여 치료하였다. 9. 관절강내 스테로이드 주사요법에 의한 치료환자가 2명, 관절원판절제술 및 이개연골 이식술을 동반한 악관절성형술 환자가 각각 1명 및 2명이었다.
