Multiple Xanthomatosis in Familiar Hypercholesterolemia Patient - A case report - (가족성 고콜레스테롤혈증 환자에서의 다발성 황색종 - 1례 보고 -)

  • We experienced the case of familiar hypercholesterolemia with multiple xanthomas which was treated by combined surgical and medical therapy. He was 26-year-old male patient of familiar hypercholesterolemia with multiple xanthomas in 22 sites on whole body, and was treated by 17 surgical excisions of the xanthomas and by medical therapy of the hypercholesterolemia. There was a normal healing process of the surgical wounds. Continual postoperative medical therapy of the hypercholesterolemia was done. There was no recurrence of the symptoms during more than 13 months of follow-up. But the serum level of the cholesterol was not lowered significantly, so we are treating him with drug therapy. Familial hypercholesterolemia is caused by a specific disorder of lipid metabolism, and is characterized by increased LDL cholesterol, tendon xanthomas, coronary disease associated with autosomal dominant transmission. Xanthomas usually appear in the second decade of life with familiar hypercholesterolemia which may have high risk for premature coronary atherosclerosis, which might be prevented with early diagnosis and medical treatment. So, orthopedic surgeons do not only excise the xanthomatosis surgically but also can diagnose the underlying hypercholesterolemia.

Effects of Ethylacetate Fraction of Plantago asiatica L. on Hypercholesterolemia Induced by High Cholesterol Diet in Rats (질경이(Plantago asiatica L.) 에틸아세테이트분획이 흰쥐의 고콜레스테롤혈증에 미치는 효과)

  • Plantain was extracted with ethanol and fractionated systemically with n-hexane, chloroform, ethylacetate, n-butanol and water to study inhibitory effect on cholesterol synthesis in vitro. To screen the effect, inhibitory activities on 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase obtained from Saccharomyces cerevisiae were examined using the five fractions of Plantain. The HMG-CoA reductase activity was inhibited most by ehylacetate fraction among the fractions, although the all five fractions had the effect To see the hypocholesterolemic effect of the ethylacetate fraction of Plantanin (PAE) in vivo, male Sprague-Dawley rats were received 5 types of diets for 6 weeks: normal diet group (NOR), high cholesterol diet group($1\%$ cholesterol and $0.25\%$ sodium cholate, CON), normal diet and PAE 70 mg/kg administered group(S1), high cholesterol diet and PAE 140 mg/kg administrated group(S2), and high cholesterol diet and PAE 140 mg/kg administered group(S3). Body weight gains of the CON were significantly increased compared to those of S1, S2 and S3. Activities of serum AST and ALT were tended to be increased in CON compared with NOR and reduced by the PAE administration. Concentrations of serum total cholesterol, free cholesterol, LDL-cholesterol, triglyceride and the atherogenic index were tended to be decreased in the PAE administered groups compared with the CON. HDL-cholesterol and phospholipid concentrations were significantly decreased in the CON and markedly increased by the PAE administered groups. Taten together, it is suggested that the ethylacetate fraction of Plantanin exerts antiatherosclerotic effect by reducing serum cholesterol concentrations in rats fed high cholesterol diets.

Effects of the Chitosan Oligosaccharide Intake on the Improvement of Serum Lipid Level in Hypercholesterolemic Rats (키토산 올리고당이 고콜레스테롤혈증 흰쥐의 혈청 지질농도 개선에 미치는 영향)

  • This study was performed to investigate the effects of the chitosan oligosaccharide administration on the improvement of the lipid compositions, blood glucose and enzyme activities in the serum of hypercholesterolemic rats fed the experimental diets for 5 weeks. Concentrations of total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free cholesterol, cholesteryl ester ratio, triglyceride (TG), phospholipid (PL) and blood glucose in serum were higher in the hypercholesterolemic group (group CW) and cholesterol+chitosan oligosaccharide administration group (group CCW) than those in the control group (group BW, basal diet+water). But the concentrations of total cholesterol, atherosclerotic index, LDL, LDL-cholesterol, free cholesterol, cholesteryl ester ratio, TG, PL and blood glucose in serum were lower in the group CCW than those in the CW, whereas the ratio of HDL-cholesterol concentration to total cholesterol and HDL-cholesterol concentration in the group CCW were higher than in the group CW. The activities of alkaline phosphatase (ALP) and aminotransferase (AST, ALT) in serum were lower in the group CCW than in the hypercholesterolemic group CW. From the above results, it was suggested that chitosan oligosaccharide administration was effective in the prevention and improvement of the lipid level, blood glucose and enzyme activities in serum of hypercholesterolemic rats.

Prediction model of hypercholesterolemia using body fat mass based on machine learning (머신러닝 기반 체지방 측정정보를 이용한 고콜레스테롤혈증 예측모델)

  • The purpose of the present study is to develop a model for predicting hypercholesterolemia using an integrated set of body fat mass variables based on machine learning techniques, beyond the study of the association between body fat mass and hypercholesterolemia. For this study, a total of six models were created using two variable subset selection methods and machine learning algorithms based on the Korea National Health and Nutrition Examination Survey (KNHANES) data. Among the various body fat mass variables, we found that trunk fat mass was the best variable for predicting hypercholesterolemia. Furthermore, we obtained the area under the receiver operating characteristic curve value of 0.739 and the Matthews correlation coefficient value of 0.36 in the model using the correlation-based feature subset selection and naive Bayes algorithm. Our findings are expected to be used as important information in the field of disease prediction in large-scale screening and public health research.

Effect of Prunus persica Batsch var. davidiana Max. Extract on the Free Fatty Acid, Creatine Phosphokinase and LCAT Activities in Hypercholesterolemic Rats (고콜레스테롤혈증 흰쥐의 유리지방산, Creatine Phosphokinase 및 LCAT 활성에 돌복숭아(Prunus persica Batsch var. davidiana Max.)추출액이 미치는 영향)

  • This study was performed to observe the effects of the feeding Prunus persica Batsch var davidiana Max. extract on the improvement of the free fatty acid, creatine phosphokinase and lecithin cholesterol acyltransferase(LCAT) activities in the serum of dietary hypercholesterolemic rats(S. D. strain, male) fed the experimental diets for. 5 weeks. Concentration of electrolyte(Na, K, Cl) in serum were fairly reduced in the group BCP(basal diet + cholesterol + Prunus persica, $5.0\;g\%$ extract) than in the group BCW(basal diet + cholesterol + water). However, no significance was found in the effect of an electrolyte concentration among the groups. Concentrations of free fatty acid, lipid peroxide and creatine phosphokinase activity in serum were significantly higher in the cholesterol administration groups(group BCW, BCP) than those in the control group(group BW, basal diet + water). However, concentrations of free fatty acid, lipid peroxide and creatine phosphokinase activity in serum were remarkably lower in the group BCP than those in the group BCW The LCAT activity in serum was increased in the Prunus persica $5.0g\%$ extract administration group(group BCP) than in the cholesterol of diet group(group BCW). From these results, the Prunus persica Batsch var davidiana Max. extracts were found to be effective on the improvement of the lipid compositions in serum of dietary hypercholesterolemic rats.

Effects of Unripened Cheese Supplements on Lipid and Antioxidant Status in Hypercholesterolemic SD Rats (고콜레스테롤혈증 흰쥐에서 비숙성치즈의 보충섭취가 지질 및 항산화 체계에 미치는 영향)

  • The aim of this study was to evaluate the effects of unripened cheese supplements on lipid metabolism and antioxidant status in hypercholesterolemic SD rats. Rats were induced to have hypercholesterolemia by feeding them high cholesterol diet (0.5% cholesterol and 0.2% sodium cholate) for 4 weeks and then divided into 2 groups. One group was fed a high cholesterol diet with 5% unripened cheese (URC) daily for 6 weeks, and the other one was fed a high cholesterol diet without 5% unripened cheese (URC) daily for 6 weeks. Significantly-increased plasma total cholesterol (TC), triglycerides (TG), and AST activity because of the high-cholesterol diet were reduced 18.8%, 40.5%, and 33%, respectively, by URC supplementation. Also, URC lowered hepatic total lipids, TCs, and TGs, whereas fecal lipid profiles were not changed by URC. The supplementation of URC resulted in an increase of plasma retinol and ${\alpha}$-tocopherol by 40.5% and 39.2% and leukoytic DNA resistance to oxidative stress by 52.3% compared to hypercholesterolemic control. These results suggest that unripened cheese supplements could exert significant health benefits to those with hypercholesterolemia through ameliorating lipid profiles and antioxidant effects.

Study on relationship between milk intake and prevalence rates of chronic diseases in adults based on 5th and 6th Korea National Health and Nutrition Examination Survey data (제 5기, 6기 국민건강영양조사 자료를 활용하여 성인의 우유 섭취와 만성질환 유병률 사이의 관련성 연구)

  • Purpose: The purpose of this study was to explore the relationship between milk intake and prevalence rates of chronic diseases using KNHANES data, and the significance of the relationship was tested based on meditators, socioeconomic status (income, education), dietary behaviors (smoking, alcohol drinking, breakfast, and eating out), and physical activity (walking, medium, and high). Methods: Using the 5th and 6th survey data of KNHANES, milk intake rates and presence of seven chronic diseases were summarized and analyzed by ANOVA for two groups of adult men and women as follows: hypertension, hypertriglyceridemia, low HDL-cholesterol, diabetes, abdominal obesity, obesity, and metabolic syndrome. The dependent variables for the presence of seven chronic diseases regressed with socioeconomic, dietary behavior, and physical activity variables according to Logistic models. The dependent variables for milk intake using predictor variables of socioeconomic, dietary behaviors and physical activity were analyzed according to Logistic models. Finally, the significant socioeconomic, dietary behavior, and physical activity variables in the above model along with milk intake as a control variable or mediator variable regressed with significant chronic diseases according to Logistic models. Results: Milk intake, socioeconomic status, dietary behaviors, and physical activity were significantly different among the two groups of adult men and women, which were also critical factors to the prevalence of chronic diseases. The dependent variable for prevalence of chronic diseases regressed with significant factors of socioeconomic status, dietary behavior, and physical activity variables according to chronic diseases using the control or mediator variable of milk intake and summarized as follows: For adult men, milk intake controlled the education effect on diabetes partly, alcohol on hypertension and hypertriglyceridemia, low HDL-cholesterol, metabolic syndrome, breakfast on metabolic syndrome, eating out on obesity, and medium physical activity on hypertriglyceridemia. For adult women, household income on hypertriglyceridemia, diabetes, abdominal obesity, education level on hypertension, alcohol drinking, eating out, and walking activity on abdominal obesity, alcohol, breakfast, eating out, walking activity on low HDL-cholesterol, and medium physical activity on hypertriglyceridemia and low HDL-cholesterol were partly controlled by milk intake. Other significant socioeconomic status, dietary behavior, and physical activity variables related to prevalence of chronic diseases were fully controlled or mediated by milk intake. Conclusion: This study shows that milk intake (daily more than 200 g) prevents chronic diseases such as hypertension, hypertriglyceridemia, low HDL-cholesterol, diabetes, obesity, abdominal obesity, and metabolic syndrome.
