The Cervical Spinal Fractures: Comparison of the Sites and Incidences According to the Causes and the Types of the Injuries (경추 골절의 원인과 형태에 따른 위치와 빈도의 비교)

  • The fractures of the cervical spine are relatively uncommon, but they may cause serious neurologic deficits temporarily or permanently. So, it is very important to treat the patients early by way of exact evaluation for the sites and the mechanisms of the injuries. The authors reviewed retrospectively 188 cervical spinal fractures in 100 patients from Sep. 1984 to Aug. 1990. Commonly involved levels were $C_5$ and $C_6$ in lower cervical level and $C_2$ in upper cervical level and the sites in each spine were body, lamina and odontoid process. The hyperflexion injury was the most common type of the cervical spinal fractures occupying 53% of all cervical fractures and cause more multiple fractures(2.26 fractures/patient) than in hyperextension(1.68 fractures/patient). In hyperflexion injuries, body, transverse and spinous process were commonly involved but lamina fracture was relatively common in hyperextension injury. The dislocations associated with fractures were developed most commonly in hyperflexion injury and 70% of these were anterior dislocation and the most commonly involved levels were $C_{5-6}$ and $C_{6-7}$. In conclusion, hyperflexion injury needs more close examination for the entire spinal levels than injuries of other mechanisms because it results in more severe fractures with or without dislocation and relatively frequent multiple fractures in different levels.

  • Fractures of isolated spinous processes of cervical and thoracic vertebrae are called as Clay shoveler's fracture. Such fractures related to sports activities are quite rare. In this report, a case of 27-year-old male with isolated spinous process fracture of C7 during golf swing is reported. The patient was prescribed on muscle relaxant and analgesic therapy. Cervical immobilization was maintained for four weeks with a cervical collar. The patient gradually recovered and was asymptomatic. He started playing golf twice a week after 5 months from the onset of pain.

  • It is necessary to obtain images of high diagnosis worth based on ability and skill of radiological technologist in case of acquisition of medical radiologaphy information. In addition, we need the various kinds of education considering individual radiological technologist variation and the organization of clinical education according to the large amount of learning and more expensive knowledge of radiology. In this paper, we implement CAI system for cervical vertebrae fracture radiograph with multimedia authoring tools on web environments. The CAI system can train a new radiological technologist and study a lot of radiological technologist on the cervical vertebrae fracture radiography acquisition based on more objective and systematical data in radiography room. The proposed CAI system is also expected as a useful program which can help to cure patients based on accurate diagnosis as well as obtain radiography more quickly using a technology of radiography acquisition for cervical vertebrae fracture.

  • 사회가 발달함에 따라 교통사고 등에 의한 외상 환자는 20 -30 년전에 비해 기하급수적으로 증가 하고 있으며 그러한 외상 중에 경추부 상해의 빈도는 매우 높다. 경추부 상해 원인을 규명하기 위한 임상학적 시도는 한계성을 지니고 있다. 본 논문에서는 실제 경추부 형상을 바탕으로 근육과 인대 등 을 비선형 스프링, 댐퍼 등으로 수치 모델링하여 후면 충돌시 경추부의 거동과 경추부에 미치는 상해를 구현하였다. 이를 이용하여 편타성 상해와 에어백으로 인한 경추부 골절의 생리학적 원인을 분석하고 자 한다.

Stabilization using Screws, Wire, and PMMA for Traumatic Cervical Fracture in a Maltese Dog (말티즈 견에서 Screw, Wire와 PMMA를 사용한 외상성 경추 골절의 안정화)

  • A 2-year-old, 2 kg Maltese dog was evaluated for progressive tetraparesis and neck pain. The dog had been treated with steroids for the preceding 2 weeks after unknown trauma but was deteriorating progressively and had become tetraparetic. The dog was presented with a non-ambulatory tetraparesis. Radiographic and computed tomographic examinations revealed a transverse C2 fracture with subluxation of the atlantoaxial joint and C2-C3. In addition, hydrocephalus was observed on magnetic resonance imaging. Stabilization of C1-C3 using screws, wires, and polymethyl methacrylate (PMMA) was performed. Application of ventral screws, wires, and PMMA resulted in improvement of the clinical signs after 4 weeks, and the dog could walk as before the tetraparesis 6 weeks after the operation. This stabilization method is an effective surgical treatment for management of cervical instability.

The influence of wearing helmet and cervical spine injury in skiers and snowboarders (스키와 스노우 보드에서 헬멧의 착용이 경추부 손상에 미치는 영향)

  • Purpose: As the number of people enjoying skiing and snowboarding which are two popular winter sports has been increasing, wearing helmet during doing these sports has been needed for safety. The rates of head or face injury have decreased after using helmet. However the effect that wearing helmet has on cervical damage is not yet to be known. So through this research we intend to be helpful in developing effective program and safety equipment. Materials and Methods: During two seasons from December 2009 to march 2011, cased 658 cervical injuries within 14538 admittance in medical center of major resort due to skiing and snow-boarding injuries. For survey and research model, one year before the research year conducted a pilot study. Admittance were 432 male and 226 female, advanced 273 and 385 novice. We divided them into two groups depending on wearing helmet, measured cervical damage ratio and injury mechanism, and researched the severity of damage and diagnosed injury. Each group used SPSS 12.0 (SPSS Inc., Chicago, IL, USA) to process data statistically. Results: The number of patients was 312 in skier and 346 in snow boarder. Patients wearing helmet were 146 in skier and 127 in snow boarder. Classification of each injuries were confirmed as 292 cases of simple sprain, 359 bruising, 6 cervical fractures and 1 case of dislocation. Classification of injury mechanisms were 287 of human collision, 212 material collision, 108 of slip down by oneself, 39 of falling and 12 cases of etc. In cases wearing helmet ski 78/ snow board 70 were simple sprain, ski 64/ snowboard 68 were shown as bruising, ski 1/ snow board 2 had cervical fracture or dislocation. The ratio of cervical sprain increased in cases of wearing helmet compared to non-wearing cases and there was a statistical significance (p<0.001). The ratio of cervical contusion increased significantly in non-wearing helmet user (p<0.05). However, there was no significant increase in fracture and dislocation compared between helmet user and non-user (p> 0.05). Conclusion: In this study, wearing helmet had no relation to additional cervical injury occurrence or severity among skiers and snow boarders. The ratio of cervical sprain increased significantly in helmet user with person to person accident. However, the cervical contusion decreased. On this ground, further biomechanical studies are required and modified helmet will be necessary.

  • Fractures of the dens (odontoid process) in children's cervical spine are rare, with very few case reports dealing with these injuries. In particular, a treatment modality has not been established for severe displaced fractures. Surgical fixation can theoretically afford early reduction and better maintenance, but it requires a very delicate and cautious procedure and results in permanent fusion of 1-2 cervical articulation. The authors chose conservative management for a displaced dens fracture in a two-year-old toddler and achieved a promising result. We report this case with a review of the relevant literature.

Correction of Atlanto-axial Instability by Avulsion Fracture of the Axis in a Dog (중쇠뼈 찢김골절로 인한 고리중쇠 관절 불안정의 교정)

  • An eight-year-old, castrated male, Poodle dog was presented with a bite wound in the neck region. Clinical examination revealed non-ambulatory signs of tetraparesis with deep pain, and upper motor neuron changes in all limbs. Fractures of the transverse process of C1 and the dorsal spinous process of C2 were observed on the radiographs and the computed tomography (CT). Conservative therapy was performed for 1 week. However the neurological problems were not alleviated until 7 days after injury. The owners agreed to perform surgical stabilization. The surgical stabilization was performed 7 days after presentation. Ventral stabilization was performed with screws, K-wires and bone cement. The dog stood up independently after 4 weeks and was able to walk 6 weeks postoperatively. In this report we suggest that using combined stabilization techniques are effective for this type of cervical fracture in which the dorsal and ventral structures of the vertebra are severely disrupted.

A Case Report of Prescribing Yanghyeolgeopung-tang(yangxuequfeng-tang) to Two Patients with Cervical Disc Herniation and Headache (양혈거풍탕(養血風湯)을 투여한 두통을 동반한 경추 추간판 탈출증 환자 치험 2례)

  • Objectives : The purpose of this study is to investigate the clinical application of Yanghyeolgeopung-tang(yangxuequfeng-tang) to two patients with cervical disc herniation and headache. Methods : The patients were hopitalized at the Dept. of Oriental Rehabilitation Medicine, Jaseng Oriental Medicine Hospital, and diagnosed as cervical disc herniation and treated mainly with herbal medicine; Yanghyeolgeopung-tang(yangxuequfeng-tang). Additional acupuncture and pharmaco-puncture was provided. NRS(Numeric Rating Scale) and NDI(Neck Disability Index) scores were used as outcome measures. Results : After taking Yanghyeolgeopung-tang(yangxuequfeng-tang), the patients' pain was controlled and the patients slept well after treatment. NRS and NDI scores both decreased. Conclusions : As seen in these two cases of cervical disc herniation with headache, Yanghyeolgeopung-tang(yangxuequfeng-tang) has a positive effect in controlling neck pain and headahce due to cervical disc herniation.
