Sclera Segmentation for the Measurement of Conjunctival Injection (결막 충혈도 측정을 위한 공막 영상 분할)

  • Conjunctival injection is the initial symptom of various eye diseases such as conjunctivitis, keratitis, or uveitis. The quantification of conjunctival injection may help the diagnosis and follow-up evaluation of various eye diseases. The size of the sclera is an important factor for the quantification of conjunctival injection. However, previous manual segmentation is time-consuming.Automatic segmentation is needed to extract the objective region of interest. This paper proposed a method based on the level set algorithm to segment the sclera from an anterior eye image. The initial model of the level set algorithm is calculated using the Lab color space, k-means algorithm and the geometric information. The level set algorithm was applied to the images in which the valley between the eyeball and skin was enhanced using the hessian analysis. This algorithm was tested with 52 images of the anterior eye segment. Results showed that the proposed method performs better than those with the level set algorithm using an arbitrary circle, or the region growing algorithm with color information. The proposed method for the segmentation of sclera may become an important component for the objective measurement of the conjunctival injection.

Acupuncture Treatment in a Case with Equine Conjunctivitis (말 결막염에 대한 침 치료 1례)

  • One male Arabian horse, 5-year-old, reared in Gort, Co Galway, Ireland was diagnosed as conjunctivitis. Acupuncture and aquapuncture with dexamethasone were done at BL01, BL02 and ST01, twice a week, and oculo-acupuncture was done into Shangjiao area using with oculo-acupuncture needle for human use. At session 3, there were no lacrimation, ocular discharge and hyperemia in the eye. The present patient was a case with equine conjunctivitis which showed favorable therapeutic response by acupuncture treatment.

An Outbreak of Epidemic Keratoconjunctivitis by Adenovirus Type 8 in a Neonatal Intensive Care Unit (신생아 중환자실에서의 아데노바이러스 8형에 의한 유행성 각결막염의 발생)

  • Purpose : Epidemic keratoconjunctivitis (EKC) caused by adenovirus is a highly contagious disease, which has been reported as outbreaks involving adults in the community. However, there has been no report on EKC outbreak by adenovirus in a neonatal intensive care unit (NICU) in Korea. Aims of this study were to investigate the EKC outbreak by adenovirus type 8 in NICU and to confirm an effectiveness of polymerase chain reaction (PCR) for diagnosis. Methods : Conjunctival swab or nasopharyngeal aspirate specimens were taken from all patients and tested by viral culture and PCR. Adenovirus serotype was determined by sequencing of PCR product of selected region of hexon gene using the virus isolates or specimens. Results : An outbreak of EKC occurred which was involving 12 preterm infants in the NICU of the Seoul National University Children's Hospital between July 12th and August 1st, 2005. Three hospital staffs and one family member of the neonate were also affected. Adenovirus was detected in 12/12 (100%), 6/11 (54.5%) by PCR and virus culture, respectively. Eleven PCR-positive neonates were identified as serotype 8 by sequencing. The first affected 4 babies have had routine ROP (retinopathy of prematurity) examinations one week ago. While previous outbreaks were sustained for a few months, the event in our unit was controlled without complications in 3 weeks. Conclusion : We analyzed the EKC outbreak by adenovirus type 8 in NICU. Adenovirus serotype was identified by PCR and sequencing with high sensitivity for the first time in Korea, so we suggest this method can be very useful for rapid diagnosis and infection control.

The Relationship between Habitual Patient-Reported Symptoms and Signs in the Soft Contact Lens Wearers (소프트 콘택트렌즈 착용자에서 평상시 나타난 증상에 관한 연구)

  • Purpose: To understand symptoms and signs in soft contact lens wearers the authors evaluate the attribution of the tear test to the contact lens related symptoms and signs. Methods: Sixty-two healthy soft contact lens wearers (23.95${\pm}$4.38 years old, 18.60${\pm}$16.92 months of prior lens wear) were participated in this study. 3 kinds of tear test and McMonnies' questionnaire test were performed for the soft contact lens wearers and subjective symptoms and objective signs were graded using CCLRU scales during the study period. Results: In this surveys, we found ocular dryness, redness and tiredness are the most common frequent and severe symptoms in soft contact lens wearers. Frequency of the ocular dryness, redness and tiredness are associated with grittiness, burning sensation and foreign body sensation respectively, and severity of the photophobia, foreign body sensation are associated with unstable vision and grittiness and which are associated with score of McMonnies' Questionnaire. Conjunctival redness and limbal redness are major signs but those are not need to be clinical care and rarely occurred corneal and conjunctival staining which are associated with tear break-up-time statistically. Conclusions: Dryness, redness and tiredness are primary common symptoms in Korean soft contact lens wearers. The frequency of those symptoms are increased with the period of soft contact lens wear and those are associated with other symptoms but have no relationship with signs.

Clinical Features of Atypical Kawasaki Disease (비정형 가와사키병의 임상 양상)

  • Purpose : To identify clinical and laboratory features of atypical Kawasaki disease(KD), and to develop criteria for early diagnosis of atypical KD patients. Methods : All patients with KD treated at our hospital from January 1998 to June 2000 were reviewed retrospectively. Results : Among a total of 167 patients, 28(16.8%) were atypical KD of which seven(25%) were infants. Among the five cardinal symptoms, oral mucosal change(96.4%) occurred most frequently, followed by conjuntivitis(57.1%) and rash(46.4%). Most notable laboratory findings were anemia, and increased erythrocyte sedementation rate(ESR) or C-reactive protein(CRP). Coronary artery abnormalities developed in seven(25.8%) atypical KD patients, compared with 14.4% in typical KD patients. We considered oral mucosal change as major criterion, and conjunctivitis, rash, hematocrit <35% and ESR >30 mm/hr or CRP >3.1 mg/dL as minor criteria. Proposed modification in diagnostic criteria for atypical KD include fever of ${\geq}5$ days+major criterion+${\geq}2$ minor criteria, or fever of ${\geq}5$ days+4 minor criteria. Conclusion : The modified diagnostic criteria has yielded a sensitivity 89.3%. Our diagnostic criteria may be used for early diagnosis of atypical KD.

The Status of Soft Contact Lens Wear in College Students in Korea (한국 대학생들의 소프트 콘택트렌즈 착용에 대한 실태)

  • This study investigated the status of soft contact lens wear in college students. The results were as follows: 1. The most prevalent age of initial contact lens wear was 19 years or 20 years, with an age distribution in females from as early as 10 years to 20 years. 2. For almost all subjects, the test clinic for the contact lens wear assessment and purchase of the contact lenses was an optical shop. 3. The main advantage for most male subjects was use for sports, whereas most female subjects used contact lenses for cosmetic reasons. 4. The most common side-effects experienced due to contact lens wear were red eye, foreign body sensation, and eye pain. 5. The most frequently reported eye disease caused by contact lens wear was conjunctivitis, with only few subjects experiencing keratitis. 6. Following treatment for the side effects of contact lens wear, most subjects hoped to re-use the previous contact lenses, some subjects wanted to change to new contact lenses, and others subjects refused further contact lens wear.

Comparison of the Clinical Effect after Wearing Conventional and 2-Weeks Cosmetic Tinted Contact Lens (일반 미용 칼라 콘택트렌즈와 2주 교체용 미용 칼라 콘택트렌즈 착용에 따른 임상성능 비교)

  • Purpose: To compare and analyze the ocular changes after wearing conventional and 2-weeks cosmetic tinted contact lens, which were made of different material and fabrication method. Methods: Thirty five university students, age of twenties, participated in this study. With daily wearing schedule, they had worn temporary cosmetic tinted lens for 2 weeks and conventional tinted lens for two month in their right and left eye, respectively. Subjective symptoms and objective signs were graded by using CCLRU scales and corneal thickness were measured after 1 day and 2 months wearing, and under or over 5 hours wearing in a day, respectively. Results: Major subjective symptoms found in the student wearing cosmetic tinted contact lens for a 1 day and 2 months were dryness, redness, foreign body sensation and itchiness. It was also found that those symptoms were more severe in left eye (conventional cosmetic tinted contact lens) than right eye (2-weeks cosmetic tinted contact lens). However, symptoms of redness in both eyes showed statistically significant difference in the case of 1 day and 2 months wearing and under or over 5 hours wearing in a day (p=0.000). After 2 months wearing, left eye with conventional cosmetic tinted contact lens showed limbal redness (p=0.000) and bulbar conjunctival redness (p=0.012) with statistically significance, whereas there was no significant difference in both eyes before lens wear. Left eye showed statistically significant changes in all objective signs, but right eye showed palpebral conjunctival redness, corneal staining and conjunctival staining. Therefore, it was concluded that conventional cosmetic tinted contact lens may be more effective on ocular changes after wearing of lens. A statistically significant changes in both eyes after wearing lens were found that corneal thickness increased about $5{\mu}m$ (p=0.001) in right eye whereas $7{\mu}m$ (p=0.004) in left eye. Conclusions: A statistically significant number of participants in this study preferred to wearing of high Dk/t temporary cosmetic tinted contact lens made by sandwich method which resulted in less ocular changes and may give more healthy and safe ocular conditions.

Changes in Objective and Subjective Responses in Soft Contact Lens Wearers Refitted to Daily-Wear Silicone Hydrogel Contact Lenses (매일착용 실리콘 하이드로겔렌즈로 바꿔 착용한 소프트 콘택트렌즈 착용자에서 나타난 증상 변화)

  • Silicone hydrogel contact lenses developed as a continuous-wear modality are now used as a daily-wear contact lenses. The purpose of this study was to investigate the clinical performance in a group of successful long-term wearers of conventional hydrogel contact lenses when refitted with daily wear $O_2OPTIX$ silicone hydrogel lenses. Sixty-two wearers ($30.7{\pm}19.9$ months of prior lens wear) among sixty-five soft lens wearers refitted with $O_2OPTIX$ silicone hydrogel lenses were participated in this study finally. Subjective symptoms were scored and objective signs were graded using CCLRU scales during the study period. 98% of subjects were successfully refitted with the one base curve of $O_2OPTIX$, and 33.9% of subjects were needed more or less lens powers compared with their habitual lenses. Subjects reported a concurrent reduction in dryness, redness and tiredness compared with their habitual lenses and both subjective and objective evaluations showed that dryness, tiredness, itchiness, bulbar and limbal hyperemia were decreased and significantly decreased particularly for those subjects with greater baseline symptoms (p<0.0001). But corneal staining was increased and was seen in many subjects during the study (p<0.0001). Refitting existing soft contact lens wearers with silicone hydrogel contact lens could result in a decreased in dryness, redness and tiredness, particularly more effective for those subjects with greater baseline of those symptoms. When patients using conventional hydrogel materials switch to a silicone hydrogel material, the potential for increased corneal staining should be considered and over-refraction should be needed to give a best vision.

  • Objective : This study was designed to investigate the effect of the Hwangryunhaedock-tang Herbal Acupuncture Therapy on Chronic Conjunctivitis or Xerophthalmia Methods : We injected distillation of the Hwangryunhaedock-tang(0.2cc) on both B-15(Shinsu-心兪), B-18(Kansu-肝兪) of patient and dropped it(0.6cc) in both eyes. We examined therapeutic value of Chronic Conjunctivitis or Xerophthalmia Results & Conclusions : The Hwangryunhaedock-tang Herbal Acupuncture Therapy was effective on reducing the symptoms. But further studies are required to concretely prove the effectiveness of the Hwangryunhaedock-tang Herbal Acupuncture Therapy for treating Chronic Conjunctivitis or Xerophthalmia.

Unilateral Parotitis and Kawasaki Disease in a Child (일측 이하선염으로 발현한 가와사키병 1례)

  • Kawasaki disease is generally diagnosed base on its clinical features. Sometimes unusual or atypical presentations make the diagnosis of Kawasaki disease difficult. We experienced an unusual case of Kawasaki disease presented with unilateral parotitis in a 23-month old girl. Despite of intravenous antibiotics treatment, fever and unilateral parotid swelling persisted. Skin rashes, conjunctival injections, and coronary abnormalities showed up on the 8th day of fever. After the intravenous immunoglobulin and salicylates treatment, all symptoms disappeared. Although unilateral parotitis is very unusual presentation of Kawasaki disease, in case of no response to antibiotics, Kawasaki disease should be included in the differential diagnosis.
