Three Dimensional Comparative Study On The Accuracy Of Impression Technique Using New Impression Coping (새로운 인상용 코핑을 이용한 임플란트 인상법의 정확성에 대한 3차원적 비교 연구)

  • The objective of this study was to examine the availability of the new impression coping by comparing with conventional coping in implant pick-up impression technique. Five implant fixtures were installed on #14, 21, 23, 25, 27 in acrylic resin model. That model with 5 fixtures was standard model, which was divided 3 groups; using new flag type impression coping, conventional impression coping splinted with self-curing resin, conventional impression coping without splinting. We made metal circular cones for calculation 3-dimensional coordinates by attaching to implant fixtures or analogs. Three-dimensional relationships of each model were calculated. Data was analyzed by multiple ANOVA and Bonferroni. The accuracy of impression between using new flag type impression coping and conventional impression coping did not show differences in 3 - dimensional analysis.Within limitations of this study, the new flag type impression coping is available in implant pick-up impression technique.

TRANSPOSITION OF THE IMPACTED PRIMARY SECOND MOLAR AND THE TOOTH GERM OF SECOND PREMOLAR (매복된 제 2유구치와 제 2소구치 치배의 전위에 관한 증례 보고)

  • Most of impacted teeth reported are permanent teeth. Impaction of primary tooth caused by primary failure of eruption is rare and the most of impacted primary tooth are second primary molars. The etiology of a primary molar impaction would appear to be a physical barrier, early ankylosis of the tooth, abnormal development of the primary molar germ or malposition of the successor bud. Transposition has been described as an interchange in the position of two teeth within the same quadrant of the dental arch. It is most commonly seen with canine and lateral incisor teeth, but is rarely associated with the primary dentition. The two cases show the transposition of impacted primary molar and the tooth germ of second premolar in their clinical and radiographic examinations. In case 1, lower second primary molar was deeply impacted inferiorly to the tooth germ suspected to be a second premolar. In case 2, impacted all second primary molars were positioned inferiorly to the tooth germs suspected to be the successors. We extracted all second primary molars under general anesthesia and the extracted tooth germs appeared to be second premolars.

  • Impaction is generally defined as the lack of eruption of a tooth after the normal age for the eruption. An impacted tooth may appear blocked by another tooth, bone, or soft tissue, but cause of tooth impaction is often unknown. The clinician should consider the various treatment options available : (a) No treatment and observation, (b) surgical exposure and orthodontic traction (c) auto transplantation (d) extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. Especially, in case 1, #21 was ectopic impacted state with root dilaceration. It is required to examine further root development and alignment of dentition serially.

  • Infections involved with the oral and maxillofacial area are associated with various anatomical structures. If the proper treatment is not done in an immediate period, the infections will be quite fatal. The causes of the infections are numerous, but the most common cause of odontogenic infections in children is a dental caries. It is known to lead to some kinds of diseases such as periapical abscess, cellulitis, osteomyelitis, Ludwig's angina, toxic shock syndrome and so on. The common pathogenic sequence of fascial abscess is a necrotic pulpal inflammation in the form of dentoalvelor abscess which spreads over and gradually penetrates into the fascial membranes through the cortical bones and finally contracts the potential fascial spaces. If the infections of oral maxillofacial area were penetrated into the surrounding soft tissues, then they would diffuse into the directions of the least tissue resistance along with the connective tissues and the fascial spaces. These infections can be properly cured by tooth extraction, endodontic therapy, surgical treatment including Incision & drainage and antibiotics. The purpose of the cases is to report the satisfactory treatment results in the patients derived from the canine fascial space abscesss or buccal fascial space ones of the odontogenic origin.

Evaluation of Characteristics of the Dental Maturity in Girls with Precocious Puberty (성조숙증으로 진단된 여자 어린이의 치아 성숙도 특성 평가)

  • Precocious puberty (PP) is defined as the early development of secondary sexual characteristics (before the ages of 8 years in girls and 9 years in boys). The aim of this study is to identify characteristics of the dental maturity in girls with PP that discriminate them from normal healthy girls. This study included 99 girls aged 6 - 8 years with PP and 99 girls without past medical history of same chronological age (control group). The study was performed on 198 panoramic radiographs (99 PP girls, 99 control group girls). Demirjian method was used to evaluate the panoramic radiographs and determine dental maturity. Difference in dental maturity score and tooth formation stages between the two groups were analyzed. The PP group showed significant higher maturity score than control group. Among mandibular teeth, mandibular 2nd premolar and 2nd molar were significantly more mature in the PP group than control group. Logistic regression analysis showed that mandibular 2nd molar was only significant predictor for PP girls.

  • Down syndrome (DS) is a genetic disease known as trisomy 21. Congenital cardiac anomalies are present in about 40% of DS patients. Dental anomalies are also common among DS patients. In DS patients, canine impaction is 10 times more frequent and transposition of maxillary canine and first premolar is 50 times more common than in general population. A female DS patient with congenital heart disease was diagnosed as having a transposed impacted maxillary canine. Sectional fixed appliance with Nance holding arch was used for the orthodontic treatment. After space was regained for the eruption of the canine, orthodontic button was attached using flap operation with closed technique. Traction and alignment of the tooth followed. To prevent endocarditis, prophylactic antibiotics were prescribed for the recommended dental procedures. Total treatment time was 25 months and no complication was found.

  • The appropriate treatment for eruption guide of impacted teeth necessitates the formulation of a comprehensive treatment plan, which is dependent upon a number of factors such as the condition of the deciduous teeth, dental and skeletal relationship, dental age of the patient, willingness of the patient to undergo extensive dental treatment and financial considerations. If the etiology of the eruption disturbance has been identified, the elimination of the causes and various procedures can be used for eruption guide. Particularly the transplantation is a valuable alternative to extraction of impacted teeth, where surgical exposure and subsequent orthodontic realignment are difficult or impossible. This report present three cases of autotransplantation of impacted maxillary canine. As the result in these cases, atraumatic removal of donor tooth during operation is prerequisite to an optimal clinical result. Due to a high possibility of pulp necrosis, endodontic treatment of fully developed transplanted teeth should be undertaken. In complex case, autotransplantation can save time and less expensive than orthodontic forced eruption. Recipient socket should be prepared to a size that is slightly larger than the root of the donor tooth, and can be prepared with open or close procedure depends on root size of donor tooth and buccolingual width at transplantion site.

Full mouth Rehabilitation in a Patient with Occlusal Collapse with Vertical Dimension Increase (교합 붕괴 환자에서 수직 고경을 증가한 보철 수복 : 증례 보고)

  • In a case of multiple posterior teeth loss, antagonistic teeth extrude to the edentulous space and compensatory occlusion on the remained anterior teeth leads to occlusal trauma. Extrusion of antagonistic teeth breaks down occlusion plane and loss of posterior support bring about severe wear of remained teeth. In this situation, it is needed to restore remained teeth and edentulous space by increasing vertical dimension to obtain prosthodontic rehabilitation space and to correct occlusion plane. In this case report, the patient had a masticatory problem with loss of posterior teeth support and an esthetic problem of shortened anterior teeth. Before the tooth preparation for the prosthodontic restoration, the patient used removable device for 2 months to increase vertical dimension reversibly. After that, he got provisional fixed restoration with irreversible tooth reduction and used it for 3 months. It had spent 5 month to evaluate the adaptation state on final restoration with incresed vertical dimension. The increasing amount was 3 mm, which was relatively in less degree and masticatory system adapted to the increased vertical dimension without any pathologic changes. Final restoration was made to have equal-intensity contacts on all teeth in a verifiable centric relations and immediate disclusion of all posterior contacts the moment the mandible moves in any direction from centric relation. In addition, metal occlusion surface on posterior teeth was applied to prevent excessive muscle activation, occlusal trauma and the porcelain fracture.

A STUDY OF MANDIBULAR DENTAL ARCH FORM OF THE KOREAN WITH NORMAL OCCLUSION (한국인 정상교합자의 하악치열궁 형태에 관한 연구)

  • The purpose of this study was to classify mandibular dental arch forms based on Raberin's method, and to compare Raberin's arch forms with that of the Korean's, and to designate arch form of bracket level according to distance between cusp tip and buccal surface of bracket level. The sample consisted of 159 mandibular dental casts showing normal occlusion which was taken from 62 males and 97 females of the Korean, aging from 13 to 25 years. The model was taken by X-ray. The landmarks were cusp points which expressed the mandibular dental arch line of cusp tips and buccal points which were measured from cusp tips to buccal surfaces of bracket level. The landmarks on the film were digitized, and measurements and statistics were performed. The results were as follows; 1. The models were classified as type 1, type 2, type 3, type 4 and type 5 by the author, and polynomial functions of the six degree and R-square values were calculated using statistical method, and each calculated equations explained each group with the least R-square value of 0.97, and each arch forms' were plotted. 2. The distribution of type 1 was $17.6\%$, type 2 $20.8\%$, type 3 $20.8\%$, type 4 $16.3\%$ and type 5 $24.5\%$. 3. The Korean arch form was characterized by larger width, smaller height compared to the French arch form. 4. The designated arch form of bracket level, viz the distance between cusp point and buccal point was calculated. The distance between cusp point and buccal point of incisor was 1mm, canine 1.9mm, first premolar 2.5mm, second premolar 2.6mm, first molar 2.7mm and second molar 2.7mm.

The effect of NaF on bone and tooth resorption around an anchor tooth during a rapid maxillary expansion procedure (급속상악확대술 시행 후 지대치와 지지골 표면에 나타나는 치근흡수의 불화나트륨 단독투여를 통한 예방에 대하여)

  • This study was undertaken to determine the effect of a 2.2mg/Kg/day intraoral administration of NaF on the amount of root resorption and osteoclastic activity during or after a rapid maxillary expansion procedure. Ten puerile female dogs were divided into two groups: a control group and a NaF-treated group. A fixed type maxillary expansion device was delivered to all dogs. The appliance was activated twice daily throughout a 20-day period, causing a 5-mm expansion of maxillary bone. After the expansion procedure, the animals were sacrificed at days 0, 15, 30, 45, and 60 of the retention period. The buccal surface of the root of each maxillary canine was examined by means of a surface electron microscope (SEM). Using SEM, web-like resorption lacunae were observed on the bone or the tooth surface at the site of osteoclastic activity; these observations were verified by histological methods. No peculiar resorption lacunae were found in the apical tip of the roots of either the control group or the NaF-treated group animals. The NaF-treated retention group was found to have less resorption lacunae formation on day 45 and day 60. The preventative effect of NaF on resorption lacunae formation on the surface of the bone covering the anchor tooth was confirmed. Larger areas of resorption lacunae were found on the surface of the bone covering the canines in the control group animals, as compared to those of the NaF-treated group, especially on day 30 and day 60. Using SEM, the present study revealed a difference between the control group and the NaF-treated group in the prevalence and the size of the resorption lacunae formation on the cemental root surface. The preventative effect of NaF on bone resorption was confirmed. Further studies concerned with the optimum concentration of NaF that has an effect in vivo are necessary.
