  • The purpose of this study was to develop an integrated-simulation practice program using standardized patients and to identify the effects of such program. The present study used a pretest-posttest design with a single group applied to 30 fourth-year nursing students in a university and developed a case scenario for alcoholism patients with diabetes mellitus in a community. As results showed, communication skills were significantly improved (t = 4.24, p < .001), but the learning of self-efficacy and motivation of transfer were insignificantly improved compared with the pretest. Moreover, motivation of transfer showed a positive correlation with the learning of self-efficacy (r = .758, p < .01). The purpose of utilizing an appropriate case development based on practical experience and hands-grade students was to improve the motivation of transfer and increase self-efficacy through problem solving. Therefore, we identified that an integrated-simulation practice program using standard patients was useful in the improvement of client centered nursing competence, such as communication skills. In addition, further studies would help develop various scenarios for the integrated-simulation practice program to improve not only communication skills but also increase self-efficacy and motivation of transfer.

  • Background: Falls are among the most common and serious health problems of older people. The psychological symptoms of falling have received relatively little attention compared to physical problems. Objective: The purpose of this study is to test a model to explain the factors that influence fear of falling among older adults living in a continuing care retirement community (CCRC) in Baltimore city, United States. Methods: A secondary analysis was conducted using data obtained from a Health Promotion Survey done on 149 older adults living in a CCRC. Data was originally obtained during face to face interviews with each participant. Descriptive statistics and bivariate correlations were used to describe the sample and evaluate simple correlations. A path analysis was done using the AMOS 4.0 statistical program. Results: Of the 49 hypothesized paths, 13 were statistically significant, and the model accounted for 22% of the variance in fear of falling among the elderly. There was support for the fit of the model to the data with a nonsignificant chi square at 0.478 (df=2, p=0.79), and the ratio of chi-square to degrees of freedom was 0.24, a CFI of 0.99 and RMSEA of 0.00. In particular, gender, a history of falling, and exercise were significant predictors of fear of falling. Conclusions/Implications: As anticipated, exercise is an important factor to prevent fear of falling. As a modifiable variable, self-efficacy and outcome expectation indirectly influence fear of falling through exercise.

  • The purpose of this study was to develop a wandering management guideline for family caregivers of community-dwelling elders with dementia (EWD). In the first stage of the study, focus group and individual interviews (n=10) were performed to analyze family caregivers' experiences about wandering of the EWD under their care. In the second stage, preliminary contents for the guideline were made based on the interview results of family caregivers and reviews of literature. Final version of the guideline with 86 items was established after experts' review using the Content Validity Index analysis. In the final stage, a pilot test was conducted to evaluate the guideline using a sample of 13 family caregivers of EWD. Family caregivers were educated about how to use the guideline and asked to complete a set of questionnaire to examine their knowledge about wandering, degree of application of the guideline, and satisfaction with the guideline. Family caregivers' knowledge score was significantly improved (p=0.014) and the average scores in the process evaluation (9 items) and user satisfaction (7 items) about the guideline were 2.69-3.46 (range: 1-4), and 2.85-3.38 (range: 1-4), respectively. Further study with a large random sample is necessary to confirm the results of this study.
