간호대학생의 간호전문직관과 윤리적 의사결정 자신감의 관계에서 도덕적 민감성의 매개효과 (Mediating effect of moral sensitivity on the relationship between nursing professionalism and ethical decision-making confidence in nursing students)

  • Purpose: The purpose of this study is to confirm the relationship between nursing students' nursing professionalism and ethical decision-making confidence, and to clarify the mediating effect of moral sensitivity which has a mutual influence on nursing professionalism and ethical decision-making confidence. Methods: The data collection for this study was conducted from January 23 to February 1, 2021 with junior and senior nursing students enrolled in five nursing departments in two regions. The data collected in this study were analyzed using the IBM SPSS statistics version 23.0 program. Results: Through this study, it was found that nursing professionalism (β=.47) and moral sensitivity (β=.32) acted as important influencing factors on the ethical decision-making confidence of nursing students, with an explanatory power of 31.2%. In addition, it was confirmed that there was a partial mediating effect of moral sensitivity in the relationship between the nursing professionalism and ethical decision-making confidence of nursing students, and the mediating effect coefficient of moral sensitivity was .16. Conclusion: Based on the results of the study, it is suggested to prepare a plan to increase ethical decision-making confidence and moral sensitivity when planning interventions to improve nursing professionalism in nursing students.

간호사와 의사 간의 윤리적 딜레마 사례개발과 간호사의 의사결정 양상 (Case Development on Nurses' Ethical Dilemmas with Physicians' and Nurses' Decision Making)

  • Purpose: This study was done to develop a realistic clinical case and investigate nurses' decision-making about nurses' ethical dilemmas with physicians in the fields of nursing practice. Methods: Case development and a hypothetical case study were used. Participants were 52 nurses. Data were collected in 2012 and 2013 using an open-ended questionnaire and interviews and analyzed using content analysis and descriptive statistics. Results: Various dilemma situations between nurses and physicians, such as violence, deathbed, medication prescription, and physicians' incapacity unfairness, were suggested. A clinical dilemma case about medication prescription was developed based on nurses' experiences. Nurses' responses to the developed case situation and responses were classified into five types. Various reasons were given for making the decisions and about 56% of the nurses decided to notify their supervisor without deleting nursing records. Conclusion: In this study, a realistic clinical dilemma case was developed, and nurses' ethical decision making was identified. These findings can be used in developing effective strategies for nurses to solve ethical dilemmas and to improve ethical decision-making abilities.

간호대학 1학년 학생을 위한 생명의료윤리 교육 프로그램의 효과 (The Effect of Biomedical Ethics Education Program for Nursing Students Freshman)

  • Purpose: This study was conducted to exam the effect of a biomedical ethics education program on the consciousness of biomedical ethics, moral sensitivity and critical thinking disposition in nursing students. Method: A quasi-experimental research design and repeated measured ANOVA were used. The participants were divided into an experimental group (n=148) and a control group (n=169). Each group underwent freshman university nursing training in two major cities. The mean age was 21.3 years and the time of providing the biomedical ethics education program was 15 hours for 15 weeks. The experimental group was provided biomedical ethics education during the semester; the control group was not provided any biomedical ethics education. Results: The consciousness of biomedical ethics was increased significantly in the experimental group (t=-4.724, p<.001). Moral sensitivity was increased significantly in the experimental group (t=-2.021, p<.004). Critical thinking disposition was increased significantly in the experimental group (t=-5.775, p<.001). Conclusion: The results of the study show that providing biomedical ethics education to nursing students is an effective method to increase the consciousness of biomedical ethics, moral sensitivity and critical thinking disposition.

간호대학생의 팬데믹에 대한 윤리인식과 표준주의 건강신념이 표준주의 수행도에 미치는 영향 (Effect of Ethical Awareness about Pandemic and Health Beliefs of Standard Precaution on the Performance Standard Precaution of Nursing Students)

  • 본 연구는 간호대학생의 팬데믹에 대한 윤리인식과 표준주의 건강신념 요인을 조사하고, 이들이 표준주의 수행도에 미치는 영향을 파악하기 위해 시도되었다. 연구대상자는 3개의 대학에 재학 중인 163명이었고, 구조화된 설문지를 통해 수집된 자료는 SPSS 25.0의 t-test, ANOVA, Pearson correlation 및 multiple regression을 사용하여 분석하였다. 연구결과, 대상자의 표준주의 수행도는 윤리인식(r=.17, p=.033), 지각된 민감성(r=.33, p<.001), 지각된 심각성(r=.24, p=.003) 및 지각된 유익성(r=.35, p<.001)과 양의 상관관계가, 지각된 장애성(r=-.31, p<.001)과 음의 상관관계가 있었다. 표준주의 수행도에 영향을 미치는 요인에 대한 회귀모형은 유의한 것으로 나타났고(F=8.09, p<.001), 지각된 유익성(β=.20, p=.014)과 지각된 장애성(β=-.20, p=.008)이 표준주의 수행도에 영향을 미치는 주요 요인으로 나타났으며, 회귀모형의 설명력은 18.1%이었다. 본 연구를 통해 간호대학생의 표준주의 수행도를 높이기 위해 윤리인식과 함께 표준주의 건강신념 요인 중 유익성 및 장애성을 고려한 체계적인 감염관리 교육프로그램 마련이 필요하다.

국내 간호학생 대상 윤리관련 연구 동향 (Trends of the researches related to ethical topic in Korean nursing students)

  • Purpose: The purpose of this study is to explore the trends of research related to ethical topics in Korean nursing students. Methods: A total of 131 articles that were published from 2000 to 2020 were analyzed and summarized according to publishing type, research design, subject, data analysis method, main research topic, research variables and instrument. Results: Most studies were journal articles (93.9%) and their most frequent research design was survey (75.7%). The research subjects covered all grades (35.1%) or they were divided between clinical nursing practicum (29.8%) or not (21.4%). The main research topics were biomedical ethics, ethical values, moral judgment and ethics education. Recently, ethical decision making and practical ability in nursing practice were reported. The instruments for measuring variables were limited and the same tools were used several times. Conclusion: Based on the findings of this study, it is suggested that the selection of various research topics and the application of research methods related to ethics for nursing students will continue in response to rapidly changing social phenomena in the future. In particular, it is necessary that research related to ethical and practical ability as well as ethical attitudes and perceptions of nursing students be actively carried out.

간호대학생과 비 간호대학생의 생명의료윤리 의식 (Consciousness of Biomedical Ethics in Nursing Students and Non-Nursing Students)

  • Purpose: The purpose of this study was to provide basic data necessary for educating nursing students by establishing a desirable sense of ethics values. Method: In this descriptive research, 101 nursing students and 191 non-nursing students (in other health related fields) from Daejeon City were selected. Tools used for data analysis were descriptive statistics, t-test, one-way ANOVA, Scheffe test and step wise multiple regression with SPSS/WIN 12.0 version. Results: Average scores for consciousness of biomedical ethics in nursing students was 2.94, and in non-nursing students, 2.77. When differences in the consciousness of biomedical ethics of nursing and non-nursing students were compared, nursing students had significantly higher scores for artificial abortion, artificial insemination, prenatal diagnosis of fetus, right to life of newborn, euthanasia, organ transplantation and human biotechnology, but significantly lower for brain death. Variables which influenced consciousness of biomedical ethics were religion and economic status in the nursing students and intent to attend a class in biomedical ethics, quantity and quality of biomedical ethics in the current curriculum and religion in the non-nursing students. Conclusion: Continuing educational programs need to be considered and attention given to the significant variables that can promote consciousness of biomedical ethics in nursing students.

간호대학생의 생명의료윤리 의식에 영향을 미치는 요인 (Factors Affecting the Consciousness of Biomedical Ethics of the Nursing Students)

  • Objectives: This study sought to provide basic data for the establishment of a desirable sense of ethics in medical service by analyzing the consciousness of biomedical ethics of nursing students. Methods: Subjects were nursing students selected from four-year and three-year nursing colleges (n=355 and 360, respectively) in D city and G buk-Do region. The Data was analyzed using t-test, one-way ANOVA, and multiple regression. Results: The average point of the consciousness of biomedical ethics was 2.99. As result of analyzing general and specific aspects of the consciousness of biomedical ethics, statistically significant differences were found in age, religion, participation in religious activity, kinds of student group activity in subjects, ethical values, experience of hearing for biomedical ethics, quantity and quality of biomedical ethics in the current curriculum, the experience of conflict for biomedical ethical problem in subjects. The predicting variables that influence the consciousness of biomedical ethics, ethical values, quantity and quality of biomedical ethics in the current curriculum, religion, age, and experience of hearing for biomedical ethics. Conclusions: An arbitration program that could promote either changeable or controllable ethical values must be considered with attention to the significant variables that can promote the consciousness of biomedical ethics of nursing students.

중환자실 간호사의 생애말 간호수행에 미치는 영향요인 (Factors Affecting Performance of End-of-Life Care among ICU Nurses)

  • 본 연구는 중환자실 간호사의 도덕적 고뇌, 윤리적 환경, 생애말 간호 어려움 및 긍정심리자본이 생애말 간호수행에 미치는 영향을 파악하기 위한 서술적 상관관계 연구이다. 대상자는 G도의 J시와 C시에 위치한 G대학병원 중환자실에서 근무하는 간호사 144명으로, 2020년 10월부터 11월까지 설문조사를 통해 자료수집하였다. 자료분석은 SPSS/WIN 21.0 프로그램을 이용하여 통계 분석하였다. 본 연구결과 대상자의 생애말 간호수행은 결혼상태, 생애말 간호 교육경험에 따라 유의한 차이가 있는 것으로 나타났다. 중환자실 간호사의 생애말 간호수행에 유의한 영향을 미치는 변인은 긍정심리자본, 생애말 간호 어려움으로 설명력은 24.8%였다. 이러한 결과를 바탕으로 중환자실 간호사의 생애말 간호수행을 향상시키기 위해서는 생애말 간호 어려움을 감소시키고, 긍정심리자본을 향상시킬 수 있는 교육과 중재 프로그램 개발이 요구된다.

요양병원 간호사의 윤리적 환경과 환자안전문화 (Ethical Climate and Patient Safety Competencies between Nurses in Long-term care Hospital)

  • 본 연구는 요양병원 간호사를 대상으로 윤리적 풍토, 환자안전문화를 확인하고, 환자안전문화에 영향을 미치는 요인을 확인하기 위해 시도되었다. 연구참여자는 D시의 요양병원 간호사 120명을 대상으로 하였다. 자료분석은 SPSS 25.0 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, t-test, ANOVA, 피어슨 상관계수, 다중회귀분석으로 분석하였다. 대상자의 윤리적 의사결정 자신감은 3.45± .41점이었고, 일반적 특성에 따른 환자안전문화는 차이를 보이지 않았다. 연구대상자의 환자안전문화는 윤리적 풍토의 동료(r=.123, p<.001), 관리자(r=.241, p<.001), 병원(r=.241, p<.001)과 정적상관관계를 나타냈다. 대상자의 환자안전문화에 영향을 미치는 요인은 동료(β=.23, p=.003), 관리자(β=.55, p=.004), 병원(β=.43, p=.031)이었고, 설명력은 33.5%로 확인되었다. 본 연구결과를 통하여 요양병원 간호사의 환자안전문화에 영향을 미치는 요인을 확인하기 위한 반복연구가 필요하다.
