NIC 간호중재분류체계를 이용한 일개 지역 중소 종합병원의 간호중재 분석 (Analysis of the Nursing Interventions Performed by Nurses Working in Small-Medium Sized Hospitals Using NIC(Nursing Interventions Classification))

  • Purpose: The purpose of this study was to identify nursing interventions performed by nurses working in small-medium sized hospitals. Method: Using NIC(3rd revision), the data were collected from 241 staff nurses working in 6 general hospitals. Result: Fourteen interventions were performed at least once a day. The physiological: basic and complex domain was most frequently performed. The most frequently performed intervention was medication: IV, and followed by medication-related interventions such as medication: IM, analgesic administration, IV therapy, and IV insertion. Fifty-five interventions were rarely performed. Most of them were in the family domain. ICU nurses performed interventions most often, while OBGY-pediatric units nurses performed them least. The ICU nurses performed the physiological: basic and complex, and the safety domain more often than medical and OBGY-pediatric units nurses. For the OBGY-pediatric units, the nurses were performed rarely the family domain, especially child bearing and rearing care. Conclusion: This survey has helped to figure out the nursing care in small-medium sized hospitals. These findings will help in building and improving of the standardized interventions for small-medium sized hospitals in located local provinces.

  • Pursose: The purpose of this study was to evaluate the selected nursing interventions and to describe the most common nursing interventions used by neurosurgery unit nurses. Method: The data was collected from 65 nurses of 5 general hospitals from Jan. 8, 2001 to Feb. 28, 2001. The instrument for this study was the Korean translation of 486 nursing intervention classifications, developed by MacClosky & Bluecheck in 2000. In the 486 nursing interventions 310 nursing interventions were selected, 8 from among the 10 professional nurses group in the neurosurgery care unit. The 310 nursing interventions were used in a secondary questionnaire. In the secondary questionnaire, all 310 intervention lables and definitions were listed. The data was analysed with SPSS program. Result: The results of this study are as follows. 1. The most frequently used nursing intervention domains were "physiological: complex", "physiological: basic", "Health system", "Behavior", "Safety", "Family". 2. Neurosurgery care unit core nursing interventions were performed several times a day by 50% or more of the Neurosurgery care unit. Neurosurgery core nursing intervention, 5 domain ("physiological: complex", "physiological: basic", "Health system", "Safety", "Behavior"), 16 class, 48 core nursing intervention. The most frequently used Neurosurgery core nursing interventions were Intravenous Therapy, Pressure ulcer prevention, Documentation, Airway suctioning, Medication: intravenous, Pain management, Medication: intramuscular, Shift report, Intravenous insertion, Positioning, Aspiration precaution, Pressure management, Physician support, Pressure ulcer care. 3. Compared with carrier and age of nurses, the more effective nursing interventions were "Family", Compared with the nursing place and the use of nursing interventions of nurses the most effective nursing interventions were "Health system" performed by nurse in university hospital. Conclusion: The purpose of this study was to analysis the nursing intervention performed by neurosurgery unit nurses. This study analyses nursing intervention and core nursing interventions performed by neurosurgery unit nurses. Basis on this study result, neurosurgery nursing interventions will be systematized, and progression of qualitative nursing, data of computerized nusing information system will be utilized.

간호중재분류(NIC)에 근거한 정형외과 간호단위의 간호중재 수행분석 (Analysis of Nursing Interventions Performed by Orthopedic Surgery Nursing Unit Nurses Using NIC)

  • Purpose: The purpose of this study was to identify the frequency with which nursing interventions according to domains and classes, and core nursing interventions of the Nursing Intervention Classification (NIC) were performed by nurses on orthopedic surgery nursing units. Method: For this purpose, the third edition of NIC was used. Of the 486 nursing interventions, 424 were selected at 75% consent by experts. Data were collected from June, 2003 to July, 2003 5 hospitals and 69 nurses(return rate : 95.8%) in Gwang-ju and Chonnam region. 5 point Likert scale describing frequency was used. Results: The most frequently performed domain was 'physiological : basic'($2.97{\pm}.60$), followed by 'health system'($2.65{\pm}.65$) and 'physiological : complex'($2.55{\pm}.46$). The most frequently performed class was 'activity and exercise management'($3.82{\pm}.89$), followed by 'immobility management'($3.64{\pm}.62$), 'skin/wound management'($3.41 {\pm}.60$), 'physical comfort promotion'($3.23{\pm}.68$) and 'thermoregulation'($3.01{\pm}.91$). The most frequently performed nursing intervention was 'medication administration' ($4.96{\pm}.21$), followed by 'medication administration : intravenous'($4.93{\pm}.31$), 'analgesic administration'($4.91{\pm}.51$), 'pain management'($4.87{\pm}.34$) and 'medication administration : intramuscular'($4.78{\pm}.68$). Conclusion: In conclusion, the third edition of NIC was found to be a general and comprehensive classification system for application on orthopedic surgery nursing units. These findings will help in building of a standardized language for orthopedic surgery nursing units and enhance the quality of nursing care.

간호중재분류(NIC)에 근거한 간호중재수행분석 II -한방병동과 일반병동 간호사를 중심으로- (Nursing Interventions Classification(NIC) Use in Korea : Oriental Medicine Hospitals and General Hospitals)

  • The purposes of this research were to identify nursing interventions performed by Korean nurses and to compare the interventions performed by nurses working in the oriental medicine hospitals and with those performed by nurses working in the general hospitals. The samples consisted of 144 Korean nurses working in three hospitals, 70 nurses working in the oriental medicine hospitals and 74 nurses working in the general hospitals. The Nursing Interventions Classification (NIC) Use Questionnaire developed by the Iowa Intervention Project team was translated to Korean and verified using the method of back-translation. The questionnaire consists of 433 intervention labels and definition. Thirteen interventions were used at least daily by nurses working in the oriental medicine hospitals, while twenty-one interventions were used at least daily by nurses working in the general hospitals. The most frequently used interventions by nurses working in the oriental medicine hospitals were Documentation, Shift Report Vital Signs Monitoring, Pressure Ulcer Prevention, Positioning, Fall Prevention, Exercise Promotion, Intravenous (IV) Therapy, Pressure Ulcer care, and Bed Rest Care in that crder. For nurses working in the general hospitals the most frequent intervention was Analgesic Administration, followed by the interventions of Medication Administration : Parenteral and Intravenous Therapy (IV) Therapy, Documentation, Intravenous(IV) Insertion, Shift Report, Fall Prevention, Vital Signs Monitoring, Medication Adnninistraction : and, Fluid Monitoring, and Medication Maragement in that order. The interventions performed least often by nurses working in the oriental medicine hospitals were Hemodialysis Therapy and Bleeding Reduction : Antepartum Uterus, while the interventions performed least often by nurses working in the general hospitals were Rape Trauma Treatment and Contact Lens Care. The nurses working in the oriental medicine hospitals performed the interventions in the Physiological : Complex domain significantly more often than the nurses working in the general hospitals, while the nurses working in the general hospitals performed the intervention in the Behavior domain significantly more often than the nurses working in the oriental medicine hospitals. This study suggests that further study will be needed to developed and validate more interventions sensitive to Korean culture.

PC와 스마트 폰 기반 임상간호분류체계 교육 코스웨어 개발 (Development of Education Courseware for Clinical Care Classification System based PC and Smartphone)

  • 시간과 공간의 제약을 받지 않으면서 컴퓨터 또는 스마트 폰을 활용하여 간호사나 간호학생들의 평생교육을 지원하는 프로그램의 개발이 절실히 필요하다. 본 연구의 목적은 임상간호분류체계를 교육 프로그램으로 개발하여 간호학생의 임상실무를 위한 가이드라인으로 제공하는 것이다. 북 가이드라인과 개발된 PC 기반 가이드라인의 검색 시간을 비교한 결과 3.5배 이상 빠른 결과를 획득하였다. 오류율도 4배 이상 적게 나타났다. 이러한 결과는 간호 실무에서 빠르고 정확하게 중재 및 평가 가이드라인에 접근하므로 환자에게 정확한 중재를 할 수 있을 것이다.

가정간호를 위한 간호과정 정보시스템 개발 (Development of Nursing Process Information System for the Home Health Care)

  • 본 연구는 가정간호사의 표준간호중재를 개발하여 이를 근거로 각각의 진단과 연결된 중재를 분류하고 프로토콜을 전산화하여 사용함으로써 의료기관간, 또한 의료인간의 격차를 없애고 서비스의 질 개선에 도움을 주고자 시행하였다. 연구방법은 크게 가정간호 표준중재 개발과정과 개발된 가정간호 표준중재 프로토콜을 전산화하는 과정으로 나누었는데, 가정간호 표준중재를 개발하기 위하여 먼저, 국내외 가정간호사의 직무분석, 대상자 사정 프로토콜 개발, 대상자 진단 프로토콜 개발, 대상자 중재 프로토콜 개발의 4가지 연구내용으로 구성하였다. 개발된 가정간호 표준 중재 프로토콜을 전산화하는 과정으로, 크게 가정간호 정보시스템을 위한 전략수립과 가정간호 정보시스템의 분석, 설계, 운영 및 평가 등의 4개 연구내용으로 구성하였다. 개발된 프로그램은 P시에서 가정간호사업 실시 기관에 근무하는 가정간호사를 대상으로, 가정간호 정보시스템 매뉴얼을 사용하여 직접 시범, 교육하였다. 따라서 본 연구에서는 표준 가정간호중재에 속하는 구성요소를 가정간호정보조사지, 가정간호경과기록지, 가정간호경과요약서, 가정간호종결 요약서 양식에 포함될 수 있도록 개발하였으며, 표준 가정간호중재가 개발됨으로써 가정간호사업부서 간의 정보교환이 용이하고 기록의 누락 및 중복을 막을 수 있으며 EMR, HMR 개발시 병원용어의 표준화에 기여할 수 있을 것이다.

돌봄제공자 지원 : 수발발간호중재 목록 개발 - 뇌졸중을 중심으로 - (Development of Nursing Intervention List - Caregiver Support : Soobal -)

  • The purpose of this research was to develop a nursing intervention list for family caregivers. The specific steps were as follows : 1. Analyze the concept, Soobal, based on literature review and case observation. 2. Generate an initial list of defining activities for ‘Caregiver Support : Soobal’. 3. Validate the defining activities. 4. Complete the final list of defining activities. A two-round Delphi questionnaire with an adaptation of Fehring's methodology was used to establish the content validity of intervention, Caregiver Support : Soobal. The definition of ‘Caregiver Support : Soobal’ was provision of the necessary information, advocacy, and support to facilitate primary patient care by someone other than a health care professional in Korean traditional manners. Ten nurse experts participated in Round I and II of this study. They were asked to rate activities that examplified the interventions on a scale of 1 (activity is not at all characteristic) to 5 (activity is very characteristic). Round I contained 15 ‘critical’ activities and 10 ‘supporting’ activities, while round II contained 16 ‘critical’ activities and 6 ‘supporting’ activities. No activities were considered to be ‘nonsupporting’ in both round I and II. Finally, the definition and 25 defining activities were developed. Intervention, Caregiver Support : Soobal, attained an ICV score of .82. This study provides a protocol model to develop Korean nursing interventions.

간호중재분류(Nursing Intervention Classification)에 기반한 가정간호에서의 주요간호중재와 연계된 간호활동 분석 (Perceived Importance and Performance Frequency of Nursing Interventions and Nursing Activities in the Physiological Domains of Nursing Intervention Classification(NIC) in Home Health Care Nursing)

  • Purpose: To identify perceived importance and performance frequency of nursing interventions with nursing activities of 5 nursing interventions from the physiological domains of NIC used in the home health care nursing. Method: Five nursing interventions in the physiological domain of NIC were selected based on the previous finding. Data were collected from 85 nurses working in 54 home health care centers between August and October, 2004 using mailing survey(return rates : 41.5%). The questionnaire consisted of 96 nursing activities in 5 interventions with definitions, asking perceived importance and performance frequency of them. Result: Skin surveillance was perceived as the most important($3.52{\pm}0.36$) intervention and also performed most frequently($4.43{\pm}0.45$). All the nursing activities in skin surveillance appeared to be used frequently, which was rated over 4 out of 5 point Likert, while only 4 nursing activities in exercise therapy: joint mobility did. Correlations between perceived importance and performance frequency of 5 interventions were all significant (r=0.591-0.718, p=0.000). Conclusion: Identifying the use of particular interventions and nursing activities will help nurses simplify documentations and to deliver better care to the patients in home health care nursing.

연변조선족 간호사가 수행하는 간호중재 분석 (Analysis of Nursing Interventions Performed by Chosunjok Nurses in Yanbian Using NIC)

  • To identify nursing interventions performed by Chosunjok nurses in Yanbian using NIC. Methods: The sample consisted of 36 nurses working in 2 hospitals. The Nursing Interventions Use Questionnaire developed by the Iowa Intervention Project team was used for data collection. The instrument was translated to Korean using the method of back-translation. Results: Twenty-eight interventions were performed at least daily. Interventions in the Physiological: Basic domain were most frequently used at least daily. The most frequently used interventions was Positioning, followed by the interventions Pressure Ulcer Prevention, Intravenous (IV) Therapy, Hypothermia Treatment and Intravenous (IV) Insertion. The least frequently used interventions was Electronic Fetal Monitoring: Antepartum. Nurses working in special medical care units performed interventions most often, while nurses working in general surgical units performed them least. Nurses working in general medical, special medical and other care units performed interventions in the Physiological domain more often than the nurses working in general surgical units. Conclusion: Chosunjok nurses in Yanbian performed physiological interventions frequently. Further studies will be needed to compare interventions performed by nurses in two countries.

다문화가족 아동과 청소년에게 적용된 건강 관련 중재연구의 동향 : 간호교육에의 시사점 (Trends in Health-Related Intervention Research Applied to Children and Adolescents in Multicultural Families)

  • 본 연구는 다문화가족 아동 및 청소년에게 제공된 건강관련 중재 연구들의 동향과 효과를 분석함으로써 간호 중재 및 교육방안의 근거를 제공하고자 시행되었다. Scoping 방법에 따라 8개의 국내외 전자데이터베이스를 통하여 검색된 604개의 문헌 중 선정기준과 제외기준에 따라 최종 12편의 문헌을 분석하였다. 분석 결과, 초등학생 대상의 연구가 대부분이었고, 중재 프로그램의 유형은 미술 활동, 음악 활동, 심리 치료 및 상담, 신체 활동이며, 중재 효과는 신체적, 심리사회적, 발달상의 변수로 분류되었다. 본 연구를 통해 다문화가족 아동 및 청소년의 주요 건강 문제 예방 및 증진을 위한 중재 연구의 필요성이 제기되었으며, 더불어 신체적, 심리사회적, 발달적 영역을 포함하는 포괄적인 중재 전략 마련이 요구됨을 확인하였다.
