Development of Education Program for Nursing Process based on Mobile Application (모바일 응용 기반 간호과정 교육 프로그램 개발)

  • The purpose of this research paper is to develop Mobile application-based Nursing Process Programs on 'Nursing Diagnosis', 'Nursing Interventions' and 'Nursing Outcomes Classification' targeting nurses and nurse students. To achieve it, this paper uses 'standard classification-focused research data' on the basis of Nursing Diagnosis Classification established by NANDA (North American Nursing Diagnosis Association), NIC (Nursing Interventions Classification) and NOC (Nursing Outcomes Classification) mainly developed by Iowa State University. The existing research methods are difficult to be applied to patients, since such methods put a restriction on choosing, developing, and generalizing 'Nursing Process Programs' in clinical spheres. But, this research thesis focuses on developing guidelines applicable to any clinical experiences, with the use of the framework in mutual links with all the nursing diagnosisoutcomes- interventions. In this regard, the Korean version programs were developed and registered in App store in March. Thus, it is expected that these programs would be wildly-available as tools for nursing education.

  • The paper presents a review of three data sets(Uniform Hospital Discharge Data Set, Nursing Minimum Data Set, and Nursing Management Minimum Data Set) and six major nursing classifications(the North American Nursing Diagnoses Association Taxonomy I, Omaha System, Nursing Interventions Classification, Nursing Intervention Lexicon and Taxonomy, Nursing Outcome Classification, Nursing Outcomes Classification, and Classification of Patient Outcome). The reviewed data sets and nursing classifications were different from each other in the purpose, structure, and user. Nursing Interventions Classification and Nursing Outcomes Classification were linked to North American Nursing Diagnosis Association, but others not. The data set and nursing classifications need to be linked to other data sets and classifications.

Analysis of the Nursing Interventions performed by orthopedic surgery care unit using NIC (NIC 간호중재 분류체계를 이용한 간호중재분석;정형외과 간호단위를 중심으로)

  • Purpose : This study analysis of nursing interventions and core nursing interventions performed orthopedic surgery care unit. Method : The data were collected from 55 nurses of 4 general hospital from July. 10, 2002 to Aug. 30, 2002. The instrument for this study was the Korean translation of 486 nursing intervention classifications developed by McColskey & Bulechek in 2000. In the 486 nursing interventions 350 nursing interventions were selected 8 among the 10 more than 5 years working group in orthopedic surgery care unit. Self frequency checking performed by 350 nursing interventions were used secondary questionnaire In the orthopedic surgery care unit nurses. Results : 1. The most frequently used nursing interventions domains were "Physiological : basic" and then "Physiological: complex", "Health system" "Behavior", "Family", "Safety". 2. Orthopedic surgery care unit core nursing interventions were performed several times a day by 50% or more of orthopedic surgery care unit and this result were 4 of domain, 11 of class, 26 core nursing interventions. This core nursing interventions were 5 of "Physiological : basic", 17 of "Physiological: complex", 1 of "Safety", 3 of "Health system" and have no "Behavior", "Family". Conclusion : This study analyzed nursing intervention of orthopedics and reestablished nursing intervention concept and I hope this study will be helpful for the nurse to be a professional by applying this to actual clinic, for development in qualified nursing and for establishment nursing information system.

  • It is urgently needed to develop programs supporting lifelong education for nurses and students of nursing, which are not restricted by time or space and use personal computers or smartphone. The purpose of this study is to develop CCC(Clinical Care Classification) System into a education program and provides guideline to support clinical tasks for students of nursing. Comparing the search times of the book guideline and the web guideline developed, this study found that it was over 3.5 times faster. And its error rate was over four times lower. This result shows that it can provide accurate intervention for patients since it approaches to intervention and evaluation guideline fast and precisely in the actual tasks of nursing.

Analysis of the Nursing Interventions done by MICU and SICU nurses using NIC (간호중재분류체계 (NIC)를 이용한 내외과계 중환자실 간호중재 분석)

  • The purpose of this research was to identify nursing interventions performed by MICU and SICU nurses. For data collection this study used the taxonomy of the Nursing Interventions Classification (NIC : 433 nursing interventions) which was modified by McCloskey and Bulecheck(1996). Each of the 433 interventions were identified as used by MICU and SICU nurses. More than 50% of the ICU nurses performed 280 nursing interventions at least monthly. Rarely used interventions included 26 nursing interventions in the childbearing care class. Overall, both MICU and SICU nurses used interventions in the Physiological : Complex domain most often on a daily basis and the interventions in the Family domain least often. The most frequently reported interventions as being used daily in the MICU were chest physiotherapy, airway suctioning and coughing enhancement and, in the SICU, documentation and airway suctioning. There were significant differences between MICU and SICU nurses in 17 nursing interventions childbearing care, cognitive therapy, communication enhancement, coping assistance, elimination management, lifespan care, health system mediation, immobility management, medication management, neurologic management, patient education, psychological comfort promotion, physical comfort promotion, respiratory management, risk management and information management. The SICU nurses performed there interventions more frequently than the MICU nurses. These findings will help in building of a standardized language for the MICU and SICU and enhance the quality of nursing care.

Analysis of Core Interventions of Operating Room using Nursing Intervention Classification (NIC간호중재분류체계를 이용한 수술실 핵심간호중재분석)

  • Purpose : The purpose of study was to identify to analysis of core nursing interventions performed by Operating Room nurses. Method : The subjects of the study were arbitrarily selected nurses(n=104) working in Operating Room. The period for data collection was 15 days from July, 15, 2002 to July, 30. 2002. The instrument for study was 486 Nursing Interventions Classification developed by McClosky & Bulechek(2000) and was translated into Korean. In 486 nursing interventions, 57 nursing interventions were selected by more than half of 47 professional nurses group of Operating Room. 57 nursing interventions were used as a secondary questionnaire. In the secondary questionnaire, labels and definitions of all 57 interventions were listed. The collected data were self reported by Operating Room nurses. The data were analysed with SPSS program. Result : In 57 nursing interventions, the 'Behavior' domain was the most frequently used. Core interventions of Operating Room were performed several times a day by more than 50% of Operating Room nurses. Core interventions of Operating Room were 16 Core interventions, 7 classes, 5 domains. In the core interventions, the 'Physiological:Complex' domain was the most frequently used. Core interventions of Operating Room were Surgical Preperation, Infection Control:Intraoperative, Surgical Precautions, Fall Prevention, Documentation, Surgical Assistance, Environmental Management:Safety, Skin Surveillance, Physical Restraint, Pressure Ulcer Prevention, Environmental Management:Comfort, Infection Protection, Presence, Emotional Support, Specimen Management, Shift Report. Conclusion : Core interventions of Operating Room have implications for nursing care practice, nursing education, nursing research, and nursing information system in Operating Room.

Nursing Interventions In Korea (한국의 간호사들이 행하는 간호중재)

  • 본 연구의 목적은 한국의 간호사들이 행하는 간호중재를 규명하는 것이다. 연구대상은 한국의 2 병원에서 일하는 수 간호사와 평 간호사를 포함한 443면의 간호사이며, 연구도구는 Iowa Intervention Project Team 이 개발한 336 문항의 5점 척도로 이루어진 "간호중재"설문지를 사용하였다. 5점 척도는 다음과 같다 : (a) 거의 행하지 않는다 : (b) 거의 한달에 한번 행한다 : (c) 거의 일주일에 한 번 행한다 : (d) 거의 하루에 한 번 행한다 : (e) 하루에 여러 번 행한다. 한국자료를 위해 설문지는 본 연구자에 의해 한글로 번역되었고, Back-translation을 이용하여 한글로 번역한 설문지는 다시 bilingual nurse에 의해 영어로 옮겨졌고, 마지막으로 영어로 옮겨진 설문지를 원래의 영문으로 된 설문지와 비교한 후에(Equivalence Test) 수정${\cdot}$보완하여 사용하였다. 한국 간호사들은 336 간호중재 모두를 행하고 있으며, 21개의 간호중재가 적어도 하루에 한 번 정도는 이루어지고 있었다. 간호사들은 "생리적 : 복합영역"에 속하는 간호중재를 가장 빈번하게 행하였고, 이 중에서 "비경구적 투약"은 일주일에 거의 11번 정도로 가장 많이 행하여졌다. 가장 적게 행하는 간호중재는 "동물이용요법"이었다. 간호사들은 교육을 많이 받을수록, 임상경험이 많을수록 더 많은 간호중재를 빈번하게 행했다. 특히, 석사학위를 받은 간호사는 "행위 영역"에 속한 간호중재를 더 많이 행했는 데, 예를 들면 "적극적 경청", "적응 증진", "의사결정지지"등이다. 또한 4년제 대학을 졸업한 간호사들은 "생리적 : 복합영역"에 속한 간호중재들을 전문대학을 졸업한 간호사보다 더 많이 행했는 데, 예를 들면, "뇌부종 관리", "반사 장애 관리", "기구를 사용한 호흡"등이다. 10년 이상의 경험을 가진 간호사들은 "행위 영역"에 속한 간호중재를 임상 경험이 1년 미만이거나 또는 1-5년 사이의 경험을 가진 간호사보다 더 많이 행했는 데, 예를 들면, "죄책감 극복 증진", "진실 말하기"등이다. 또한 6-10년 사이의 임상경험을 가진 간호사들은 "자가 간호영역"에 속한 "옷 입히기", "두발 간호", "구강건강 유지" 등을 임상경험이 1년 미만인 간호사들보다 더 빈번하게 행했다. 이 연구는 간호중재 규명이 간호원가 산출을 위한 연구와, 간호의 질 향상을 위한 연구, 간호지식 탐구를 위한 연구에 공헌할 것을 기대한다.

Analysis of the Nursing Interventions Performed by Nurses Working in Small-Medium Sized Hospitals Using NIC(Nursing Interventions Classification) (NIC 간호중재분류체계를 이용한 일개 지역 중소 종합병원의 간호중재 분석)

  • Purpose: The purpose of this study was to identify nursing interventions performed by nurses working in small-medium sized hospitals. Method: Using NIC(3rd revision), the data were collected from 241 staff nurses working in 6 general hospitals. Result: Fourteen interventions were performed at least once a day. The physiological: basic and complex domain was most frequently performed. The most frequently performed intervention was medication: IV, and followed by medication-related interventions such as medication: IM, analgesic administration, IV therapy, and IV insertion. Fifty-five interventions were rarely performed. Most of them were in the family domain. ICU nurses performed interventions most often, while OBGY-pediatric units nurses performed them least. The ICU nurses performed the physiological: basic and complex, and the safety domain more often than medical and OBGY-pediatric units nurses. For the OBGY-pediatric units, the nurses were performed rarely the family domain, especially child bearing and rearing care. Conclusion: This survey has helped to figure out the nursing care in small-medium sized hospitals. These findings will help in building and improving of the standardized interventions for small-medium sized hospitals in located local provinces.

  • Pursose: The purpose of this study was to evaluate the selected nursing interventions and to describe the most common nursing interventions used by neurosurgery unit nurses. Method: The data was collected from 65 nurses of 5 general hospitals from Jan. 8, 2001 to Feb. 28, 2001. The instrument for this study was the Korean translation of 486 nursing intervention classifications, developed by MacClosky & Bluecheck in 2000. In the 486 nursing interventions 310 nursing interventions were selected, 8 from among the 10 professional nurses group in the neurosurgery care unit. The 310 nursing interventions were used in a secondary questionnaire. In the secondary questionnaire, all 310 intervention lables and definitions were listed. The data was analysed with SPSS program. Result: The results of this study are as follows. 1. The most frequently used nursing intervention domains were "physiological: complex", "physiological: basic", "Health system", "Behavior", "Safety", "Family". 2. Neurosurgery care unit core nursing interventions were performed several times a day by 50% or more of the Neurosurgery care unit. Neurosurgery core nursing intervention, 5 domain ("physiological: complex", "physiological: basic", "Health system", "Safety", "Behavior"), 16 class, 48 core nursing intervention. The most frequently used Neurosurgery core nursing interventions were Intravenous Therapy, Pressure ulcer prevention, Documentation, Airway suctioning, Medication: intravenous, Pain management, Medication: intramuscular, Shift report, Intravenous insertion, Positioning, Aspiration precaution, Pressure management, Physician support, Pressure ulcer care. 3. Compared with carrier and age of nurses, the more effective nursing interventions were "Family", Compared with the nursing place and the use of nursing interventions of nurses the most effective nursing interventions were "Health system" performed by nurse in university hospital. Conclusion: The purpose of this study was to analysis the nursing intervention performed by neurosurgery unit nurses. This study analyses nursing intervention and core nursing interventions performed by neurosurgery unit nurses. Basis on this study result, neurosurgery nursing interventions will be systematized, and progression of qualitative nursing, data of computerized nusing information system will be utilized.

Analysis of the Nursing Interventions Performed in the Medical & Surgical Units and the Health Insurance Cost Items Based on the NIC (간호중재분류체계(NIC)에 근거한 내${\cdot}$외과계 간호단위의 간호중재 수행 및 건강보험 수가 항목 분석)

  • Purpose: This study aims to offer the fundamental data in order to cost the nursing service on the basis of the NIC and a close examination of the interventions that are contained in the health insurance cost list under the system of the current health insurance. Methods: The data is handled with the SPSS 10.0 program. The participants' general peculiarity is calculated in terms of the real number and the percentage, and the performing frequency of the nursing interventions is calculated in terms of the mean and the standard deviation. the correlation between the participants' general peculiarity and the performing frequency of the nursing interventions is analysed with t-test or one way ANOVA of SPSS. Results: In the performing frequency of the nursing interventions, the domain of "the physiological: basic" was the highest as 2.69${\pm}$1.21, the domain of "the behavioral" was the lowest as 2.11${\pm}$1.12. There were 50 core interventions in the medical unit, 48 in the surgical unit, 24 in the MICU and 33 in the SICU. The health insurance cost items contained commonly in the core interventions of each unit were 12, and the health insurance cost items except 12 items contained commonly in the core interventions of each unit were appeared 14 items in the medical unit, 6 in the surgical unit, 7 in the MICU and 2 in the SICU. The core interventions contained commonly in four units of the medical unit, the surgical unit, the MICU & the SICU are 18. And among these, the core interventions contained in the health insurance cost items are 10; pain management, hyperglycemia management, analgegic administration, medication administration: intravenous, oxygen therapy, pressure ulcer prevention, fluid management, fluide monitoring, intravenous(IV) insertion, intravenous(IV) therapy. As the result of the comparison & analysis between the core interventions of the NIC and the health insurance cost items, the core interventions contained in the health insurance cost list are 21(29 as the health insurance cost items). Conclusion: In the performing frequency of the nursing interventions, the domain of "the physiological: basic" is being performed most frequently, and in the performing frequency of the core interventions, the interventions of the domain of "the physiological: complex" is being performed most frequently. On the basis of these results, the writer hopes that the attempts to interlink the nursing interventions into the nursing cost by using of standard terms and the efforts to cost the nursing services would also be made in the future constantly.
