  • 모바일 환경의 발전 따라 어플리케이션의 수요 증가로 데이터 보안의 관심도가 높아지고 있다. 본 논문에서는 모바일 환경에서 시나리오에 따른 암호 알고리즘을 비교 분석하였다. 시나리오는 단말저장형 데이터, 단방향전송형 데이터, 실시간전송형 데이터로 정의하고 각각의 시나리오에 적합한 암호알고리즘을 분석하였다.

  • Objectives : This study was designed to evaluate the effects of traditional oriental therapies(TOT) on advanced gastric cancer(AGC) patient with metastasis to liver. Methods : We retrospectively analyzed the medical record of a case of AGC patient in stage IV with metastasis to liver who had been treated with TOT for 5 months continuously in East-West Cancer Center of Oriental Hospital of Daejeon University from 5 March 2003 through 22 July 2003. Results : After treated with TOT for the 5 months, most of symptoms were disappeared or improved and tumor size was decreased on abdominal CT compared with the previous result. The hematological results and tumor markers were not increased. Conclusions : From the above results, it could be suggested that TOT have effects on improving symptoms and quality of life as a supportive or curative therapy for AGC patients.

Effects of Orostachys Japhonicus Herbal-Acupuncture on Transferred Hepatic Cancer of Mouse Induced by Colon26-L5 Human Colon Cancer Cells (와송(瓦松) 약침(藥鍼)이 mouse의 간전이 암모델에 미치는 영향)

  • To study the effects of anti-cancer, anti-metastasis and immune response improvement effects of herbal-accupunture with Orostachys japhonicus A.Berger, infusion solution put into Kansu(BL18) of mouse induced by Colon26-L5 human colon cancer cells, which are corresponding to humanbody. We observed the change of body weight, surviving number, median surviving time, increase of life span, changes in amount of leukocyte, erythrocyte, platelet, total protein, creatinine, glucose and LDH, weight of spleen and kidney, histological analysis on tissue metastasis of liver, splenic cell proliferation, the expression of cytokine gene, the number of CD4+, CD8+, CD9+ and NK cell, and concluded like this. The results were obtained as follows ; 1. In acute and sub-acute cytotoxicity experiment, significantly signs were not appeared in all groups. 2. Antimetastatic experiment in vitro and in vivo showed that Orostachys Japhonicus A.Berger Herbal-acupuncture at Kansu(BL18) has antimetastatic effects. 3. The spleen cells proliferation of the experimental groups treated with Orostachys Japhonicus A.Berger infusion solution extract has increased significantly compared with that of the control group. 4. As compared with control, the population of total T cell, helper T cell, cytotoxic T cell and macrophage were increased. 5. The production of Th 1 type cytokines from splenocyte and cytokines which is associated with anti-tumor activity form macrophage were increased significantly. Above the results revealed that herbal-accupunture with Orostachys Japhonicus A.Berger infusion solution has effects of anti-cancer, anti-metastasis and immune response improvement.

Ascites-decreasing Effect of SB Intraperitoneal Injection to a Refractory Ascites Patient with Synchronous Colorectal Liver Metastasis and Metachronous Peritoneal Carcinomatosis : A Case Report (동시 간전이 직장 구불결장암 환자의 재발성 복수에 대한 SB 복강 주입의 복수 감소 효과 증례)

  • Objectives : To report and demonstrate the effect of decreasing ascites volume by SB intraperitoneal injection to a refractory ascites patient with synchronous colorectal liver metastasis and metachronous peritoneal carcinomatosis. Methods : Two cycles of intraperitoneal and intravenous SB injection were conducted. Each injection cycle was made up of 4 days. Nine vials of SB were injected to the patient every day. To compare the volume of ascites between pret- and post-treatment, follow-up computed tomography was done on June 3, 2013. To observe other therapeutic effects of SB injection, laboratory tests were conducted periodically. Results : On the follow-up computed tomography images, the amount of ascites and pleural effusion had decreased compared to the April 30, 2013 computed tomography images. The levels of aspartate transaminase, alanine aminotransferase and lactate dehydrogenase decreased significantly from May 9, to May 30, 2013. The amount of oral intake increased constantly during hospitalization. The patient's symptoms such as abdominal distension, abdominal pain and dyspnea were improving until discharge. Conclusions : Even if thiese results cannot be applied to every synchronous colorectal cancer liver metastasis patient, we demonstrated that SB intraperitoneal injection has ascites-decreasing effect to refractory ascites patients with synchronous colorectal liver metastasis and metachronous peritoneal carcinomatosis.

A Case Report of Partial Remission of End-stage Pancreatic Cancer Patient with Liver Metastasis Treated with Chemotherapy and Integrated Medicine Therapy (항암화학요법과 통합의학치료를 병행하여 부분 관해 된 간전이 동반 말기 췌장암 1례)

  • Objectives: This study reports on the effect of Integrative Medicine Therapy (IMT) on a patient with pancreatic cancer with liver metastasis. Methods: One pancreatic cancer with liver metastasis patient was treated using IMT in conjunction with Gemcitabine/Abraxane since September 2019. The cycle was repeated every four weeks for a total of 11 times. At the same time, the patient was treated with IMT. Tumor size was measured by scanning with Computed Tomography (CT). Adverse events were evaluated using the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0. Results: After treatment with Gemcitabine/Abraxane and IMT for eight months, the size of the body and tail of the cancer tumor and several hepatic metastatic regions decreased (partial response, [PR]), size, and number of multiple nodules in both lungs decreased. No evidence of newly developed metastatic lesions was found. The patient has maintained a good treatment outcome and has shown prolonged overall survival. Conclusions: This case demonstrates that treatment with IMT may have substantial benefits for patients with end-stage pancreatic cancer.

  • Metastases to the liver presents a common clinical problem in the management of patients with cole-rectal cancer, and are responsible for a high degree of morbidity and mortality associated with this malignancy. Unfortunately, attempts at preventing the development of liver metastases in "high risk" patients has so far been unsuccessful. Ongoing studies of adjuvant chemotherapy have not yet illustrated a significant increase in survival in patients receiving such therapy. The purpose of the study is to investigate the value of adjuvant radiotherapy given in the form of colloidal chromic phosphate P-32 suspension administered via portal vein after radical resection of the primary cancer, in preventing the growth of occult metastases in the liver. Twenty one patients (10 patients of treated group with 11 controls) were followed 18 months after operation. There was no significant change in the CBC and liver functions after administration of P-32 labeled colloidal chromic phosphate. The number of patients who showed local metastases at 18 months were 2 in the treated group and 3 in the control group. While liver metastases occurred in one patient at 6 months and in three at 12 months in the control group, there was no development of liver metastases by 12 months in the treated group. At 18 month follow-up CT scan one patient in the treated group showed a single nodule in the liver. In conclusion liver metastasis rate was lower in the patients who received colloidal P-32 chromic phosphate via portal vein after radical resection of the primary cole-rectal cancer.

Herbal Medicine Therapeutic Attempt for Diarrhea Induced by Carcinoid Symptoms of Rectosigmoid Neuroendocrine Tumor with Liver Metastasis Patient : a Case Report (간전이가 있는 직장구불결장의 신경내분비 종양 환자의 카르시노이드 증후에 의해 발생한 설사에 대한 한약 치료 시도 : 증례 보고)

  • This study aims to report a case of rectosigmoid neuroendocrine tumor patient with liver metastasis whose outstanding carcinoid symptom was mucosal diarrhea for more than 10 times a day. Three different herbal decoctions were administered orally for the treatment of diarrhea which was among carcinoid symptoms from diffuse metastatic liver lesion. The herbal decoctions given were modified Lijung-tang, modified Sambaek-tang and modified Jeokseokjiuyeoryang-tang. The administration period was 5, 14 and 9 days respectively. Frequency of patient's mucosal diarrhea had increased consistently during hospitalization. It had almost doubled compared to pre-hospitalization up to 27 times/24 hrs. But during the period of modified Sambaek-tang administration, frequency of diarrhea was maintained between 9 to 15 times/24hrs. There was no difference of liver tumors between Nov 16th 2013 and Dec 9th 2013 in computed tomography images. Three conventional established herbal prescriptions administered in this case didn't show any favorable effect on improving mucosal diarrhea induced by carcinoid symptoms of a rectosigmoid neuroendocrine tumor with liver metastasis. Even if modified Sambaek-tang showed partial effect on frequency of diarrhea, we concluded that it was not sufficient to be a therapeutic method of the diarrhea as carcinoid symptom. Furthermore, it is necessary to conduct additional studies about the diarrhea of neuroendocrine tumor with liver metastasis.

A Case Report of Pancreatic Cancer with Liver Metastasis Patient Treated with Integrative Cancer Treatment (췌장암 간전이 환자의 통합 암 치료에 대한 증례보고)

  • Objective: The purpose of this study is to report effects and potential of Integrative Cancer Treatment (ICT) on metastatic pancreatic cancer patient. Methods: A 79-year-old pancreatic cancer patient diagnosed with metastasis on liver visited the Daejeon Korean medicine hospital of Daejeon university East West Cancer Center (EWCC) on May 2021. The patient has been received chemotherapy (gemcitabine plus abraxane) and concurrently treated with ICT since May 2021. The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA19-9) and numeric rating scales (NRS). Laboratory analysis and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety of ICT. Results: After treatment, constipation was relieved from NRS 5-6 to 2, both leg numbness was improved from NRS 9 to 2. Tumor size was generally decreased accompanying by reducing the levels of tumor markers. There were no severe adverse events induced by ICT based on NCI CTCAE version 5.0. Conclusion: This case study suggests that ICT in combination with chemotherapy may help in the treatment of patients with metastatic pancreatic cancer.

CT Findings of Hepatic Metastasis from Hepatoid Adenocarcinoma of the Rectum Mimicking Hepatocellular Carcinoma: A Case Report (간세포암으로 오인된 직장에 발생한 간양 선암종의 간전이 CT 소견: 증례 보고)

  • Hepatoid adenocarcinoma (HAC) is a rare form of adenocarcinoma that is diagnosed based on immuno-histochemical findings reminiscent of hepatocellular carcinoma (HCC). The clinical characteristics of HAC include increased levels of serum alpha-fetoprotein and a poor prognosis due to early liver metastasis. In particular, diagnosing liver metastasis of HAC can be challenging owing to radiological findings similar to those of HCC. Although HAC can occur in various organs, the stomach is the most common site. We present the case of a 64-year-old femalewho presented with multiple tumors in the liver. During subsequent examination, rectal cancer was identified and diagnosed as HAC through a biopsy. Herein, we report this case along with a literature review.

The Results of Palliative Radiation Therapy in Patients with Unresectable Advanced Pancreatic Cancer (절제 불가능한 췌장암의 고식적 방사선치료 결과)

  • $\underline{Purpose}$: To evaluate the treatment results and prognostic factors of palliative radiation therapy in the patients with unresectable advanced pancreatic cancer. $\underline{Materials\;&\;Methods$: Thirty-seven evaluable patients with unresectable advanced pancreatic cancer who were treated by palliative radiation therapy for pain relief at the Department of Radiation Oncology, Kangnam St. Mary's hospital, the Catholic University of Korea between March 1984 and February 2005 were analysed retrospectively. There were 22 men and 15 women. Age at diagnosis ranged from 30 to 80 (median 57) years. Twelve patients (32.4%) had liver metastases and 22 patients (59.5%) had lymph node metastases. Radiation therapy was delivered to primary tumor and regional lymph nodes with $1{\sim}2\;cm$ margin, and total dose was $3,240{\sim}5,580\;cGy$ (median 5,040 cGy). Chemotherapy with radiotherapy was delivered in 30 patients (81%) with 5-FU alone (21 patients) or gemcitabine (9 patients). The follow-up period ranged from 1 to 44 months. Survival and prognostic factors were analysed using Kaplan-Meier method and log-rank test respectively. $\underline{Results}$: Overall mean and median survival were 11 and 8 months and 1-year survival rate was 20%. Among 33 patients who were amenable for response evaluation, 7 patients had good response and 22 patients had fair response with overall response rate of 87.9%. Mild to moderate toxicity were observed in 14 patients with nausea, vomiting, and indigestion, but severe toxicity requiring interruption of treatment were not observed. Chemotherapy didn't influence the survival and symptomatic palliation, but the group containing gemcitabine showed a tendency of longer survival (median 12 months) than 5-FU alone group (median 5.5 months) without statistical significance (p>0.05). The significant prognostic factors were Karnofsky performance status and liver metastasis (p<0.05). Age, sex, tumor location, lymph node metastasis, and CA 19-9 level did not show any prognostic significance (p>0.05). $\underline{Conclusion}$: Radiation therapy was effective for symptomatic palliation in the patients with unresectable advanced pancreatic cancer and would play an important part in the survival benefit with gemcitabine or other targeted agents.
